Health Hydrocephalus

Background The brain and spinal cord is surrounded by and encased in a thin membrane (akin to wrapping cling film around food). The membrane protects the brain, & bathes it in fluid (called cerebrospinal fluid or CSF) which provides nutrition for the brain. The brain constantly makes CSF & CSF is constantly draining away. Normally, the fluid production & drainage occurs at a constant rate, such that excess CSF can never accumulate. Because there is no “spare room” inside the skull, if the CSF did accumulate, it would put pressure on the brain itself. This pressure causes brain cells to die. Unfortunately, sometimes problems occur with this simple “fluid going in must equal the fluid going out” principle. An example of this is a condition called Hydrocephalus. What is hydrocephalus? The literal translation for hydrocephalus is “water on the brain”. In dogs, it most commonly affects small breeds. Fortunately it’s not a common bulldog affliction, however I have seen it in 1 French bulldog puppy.

What causes it? There are 2 main “causes”: Congenital or acquired. Congenital hydrocephalus: this refers to puppies that are born with it. It can be hereditary, or caused by exposure to drugs that cause birth defects, in-uterine infections and trauma from a difficult birthing process. Acquired hydrocephalus: Occurs later in life and is generally caused by tumours, inflammation and trauma of the brain/spinal cord. Congenital hydrocephalus is much more common and this is what will be discussed here. Symptoms of hydrocephalus Newborn pups bones are very soft & the skull isn’t fused. As they grow over their first few weeks of life, affected pups start to show head enlargement. This is because the pressure from the CSF can “push” the skull bones outward. However once the skull fuses (at 8-12 weeks of age), excess CSF pressure can no longer push the skull bones outward. So, this excess pressure pushes inward on the brain. This causes death of brain cells. This may lead to a variety of symptoms, mostly neurological including: seizures. head pressing. Flickering eyes. Blindness. A drunken-like walk.
Each individual’s symptoms may vary, in terms of which symptom(s) they actually display, and how severely they are affected by each one. Some may start with no symptoms. As a general rule, symptoms become more severe over time. Affected pups are often the runt of the litter & are often slower to learn than their littermates. Diagnosis This is generally easy, based on the symptoms. However, an ultrasound (before the Skullbones fuse) or a CT scan can confirm hydrocephalus. Treatment In humans, a specialist brain surgeon can redirect the flow of CSF to prevent pressure on the brain. For the vast expense involved in this, and in the setting of newborn puppies being affected; most owners of affected pups elect euthanasia. Supportive treatment for the mildest of cases my suffice. Prevention Dogs that produce a pup with hydrocephalus shouldn’t be bred with again; unless it can be confirmed to be caused by a something “external” such as trauma or drug administration.
