Herrin Hospital confronts chronic disease at the Cardiac Management Center

C

hronic diseases, such as congestive heart failure (CHF), play havoc with a person’s life. In these cases, much responsibility is laid on the patient and they often become frequent visitors to local emergency rooms and doctor’s offices. However, Herrin Hospital is home to a new type of clinic designed to ease the burden on patients and help them have better quality of life. People with CHF, particularly those with trouble managing the disease, find themselves in the hospital repeatedly. CHF affects so many areas of the body, patients show up to the emergency room with pneumonia, intestinal pain, etc. The emergency room treats what’s happening at that moment,

operating procedure. Starting with Herrin Hospital, 10 other local hospitals in seven counties have now been trained in the same fashion. The first year, staff at Herrin averaged 88 minutes from the time the patient arrived for stabilization at the first hospital to when the treatment occurred in the cardiac cath lab in Carbondale. Today, Herrin Hospital’s STAT Heart program has a median time of 66 minutes, which is well below the goal of 90-minutes to open the blockages. This far surpasses the national average for such care, which is 180 minutes. For this work, Herrin Hospital is a fully accredited Chest Pain Center and recognized by the American Heart Association as a Mission Lifeline: Gold recipient. Herrin was the third referring hospital in the entire nation to receive such commendation. For Herrin Hospital, the process is so much more than meeting a number. What it really comes down to is that when a Prairie STAT Heart is executed with such precision, lives are being saved. That is why they do what they do.

“Nobody’s role is unimportant,” said Shadowens. “I’ll never forget the first time we did a re-enactment with one of our patients. He came up and hugged me. I cry every time I see the patients. And I don’t do the hands-on patient care. I’m the traffic director. It’s this team; that one call comes in and they save lives.”

WHY NOT DRIVE MY LOVED ONE? WOULDN’T THAT SAVE TIME? ‌A heart attack event is traumatic. Driving in a state of panic is not safe. Today, coordinators in the field work feverishly to continue to reduce treatment times. For example, ambulances are now equipped with EKG capabilities to officially diagnose a severe heart attack on-site. This activates the rest of the team earlier, which allows the care team and cardiologists to be prepared and waiting for that patient to arrive at the hospital door. Calling 9-1-1 is the best and safest route to take.

Carla went from a coma to quilting In the not — too — distant past, Carla Shasteen found herself in a situation that none of us ever hopes to be in. It was an icy day in January when Shasteen was driving her car on an Indiana highway finishing up a day’s work selling quilting fabric in Indianapolis. Suddenly, she hit a patch of ice, lost control of her vehicle and ended up in a lane of oncoming traffic. The driver in the other vehicle also lost control and ran directly into Shasteen’s car. It took workers nearly an hour to cut Shasteen out of her car. She was taken to a hospital in Indianapolis, where she fell into a coma. Despite having several strokes, a collapsed lung, six broken ribs and a broken arm, Shasteen’s mother decided to give her daughter 30 days to

wake up from her coma. Against all odds, Shasteen did wake up on day 27. As Shasteen explains, “That’s when the real work began.” She was transferred to the Acute Rehabilitation Center at Herrin Hospital where she began her recovery journey. In those first few weeks, Dr. Terrence Glennon, a physiatrist of the Rehabilitation Institute of Chicago at Herrin Hospital asked Shasteen what was her goal and what did she want to

get back to doing? She answered — “Quilting!” In the course of learning to walk, talk and eat again, one nurse told her, “It’s like God came down and pressed the reset button on your head.”

Hope inspired success

It was a long journey and at one point an occupational therapist offered to bring in a sewing machine for Shasteen to practice her newly relearned skills. “It meant the world to

me! Offering to do that just gave my family that I am where I am today!” me so much hope!”

At the end of this remarkable journey and after nine months of outpatient therapy, Shasteen was talking, walking, writing and even quilting again. world. Today, an estimated two-thirds of American adults are overweight or obese — a number that has doubled since 1980. Research shows that once obesity is established, diet and exercise alone have a limited capacity to provide a longterm solution.

Herrin Hospital’s New Life Weight Loss Center has changed the lives of nearly a thousand people through bariatric and metabolic surgery since the program began in 2009. The program consists of two surgeons, one physician assistant, a program

coordinator, five Certified Bariatric Nurses plus a dedicated medical/surgical floor rooms and staff and OR staff. During a restrictive procedure, the surgeon creates a small stomach pouch that limits the amount of food that can be eaten. During a malabsorptive

Jeralds noted, “Joint Camp was helpful. When you’re going through major surgery like this, you don’t know what to expect. With Joint Camp, Stephanie goes in there and she explains everything that you are going to go through,

helps you prepare and explains the exercises to be done before surgery. She was very informative and I think that if I hadn’t gone through it ... it would have been very difficult. It would be difficult for anyone to have this surgery without the knowledge that I gained from her.” On Friday, October 5, 2012, Jeralds had his total knee replacement with Dr. Davis through the Joint Camp program at Herrin Hospital.

What did Jeralds think after his total knee replacement surgery? “I put it off for far too long and really, I don’t know why! In my opinion, it’s been a blessing.” When looking back on the experience, Jeralds felt that the nurses and doctor were very caring. “My nurses were fantastic! They were concerned about managing my pain and worked to keep the doctor informed. Dr. Davis

really seemed to care. He seemed to really have my best interest at heart. They were expedient, efficient and I would recommend Herrin Hospital to anybody!” Jeralds added, “For those that are having any hesitation about getting their joint replacement done, listen to what your Joint Camp surgeon tells you. My surgery went extremely well! So get it done and get on with life!”

Herrin Hospital: Orthopaedic Care of the 21st Century

uly 2013 ushered in yet another new era of excellence at Herrin Hospital, with the christening of The Orthopaedic Bone & Joint Center. The Orthopaedic Bone & Joint Center is host to Joint Camp, the only Joint Commissionaccredited program for hip and knee replacement within a 100-mile radius. Since the opening of the Orthopaedic Bone & Joint Center, over 290 hip and knee surgeries have been performed. The 13-bed unit also houses a gym for recovering patients to work with Rehabilitation Institute of Chicago-trained therapists.

Phillip had trouble walking, but now he can ride Phillip Jeralds had reached a point

where he could not walk long distances anymore or spend long periods of time doing something that he loved to do — horseback riding. Jeralds and his wife, Cheryl, loved to ride horses for five to seven hours at a time near their home in Creal Springs in the Shawnee National Forest. Jeralds, a retired Illinois state trooper, suffered three tears to his knee during his tenure.

Making the big decision After the most recent meniscus tear, Jeralds visited an orthopedic doctor, who simply informed him that all of his cartilage was gone. n 2008, a 911 emergency STAT Heart was called in from EMS crews in Williamson County. An acute heart attack patient was on the way to Herrin Hospital. However, for this patient, an improved process would forever change the way regional hospitals, EMS and physicians would approach this lifethreatening emergency. Across the nation, doctors were understanding that in the event of an acute heart attack (the worst of the heart attacks, caused by a blocked coronary artery), they had a 90-minute window before heart muscle would die, and there was no getting it back. For southern Illinois, this was a

challenge. Ninety minutes to save a heart muscle seemed an impossibility given our rural status. Opening a blocked artery could only happen at Prairie Heart Institute Memorial Hospital of Carbondale. How do you reduce the time from a call in Williamson County to treatment? Prairie cardiologist, Dr. Raed AlDallow and Pam Shadowens, a cardiac care coordinator at the time, took on the challenge and soon realized that a multidisciplinary/multi-hospital approach was essential. If a patient in an outlying community had a heart attack, stabilization for safe travel had to start at the nearest hospital. A pilot program, Prairie STAT Heart, developed at Herrin Hospital.

The process started with emergency crews in Williamson County. A one-call system from the field would activate a multitude of individuals: local hospital emergency personnel, switchboard operators, security officers, ambulance or helicopter crews, emergency staff at Memorial Hospital of Carbondale, the Prairie physicians, the cardiac catheterization staff and more — approximately 100 people taking different pieces of the action, carefully synchronized. Herrin Hospital staff learned to run tests, accurately diagnose the severity of the heart attack, give appropriate medications, and prepare the patient for travel, all in record time. Meanwhile, the emergency staff in Carbondale would

be organizing themselves to receive the patient. In the cardiac catheterization lab, the Prairie cardiologist would be called to duty, along with the appropriate technologists, prep the catheterization unit, and be ready and scrubbed to treat the patient as soon as he/she arrived. Their first patient under the new protocol arrived at Herrin Hospital and was stabilized, transferred and treated in Carbondale in 84 minutes. It worked! “Our teamwork is the hallmark of our program,” said Dr. Al-Dallow. “We customized our program to overcome the geography.” This amazing coordination of so many individuals has now become standard

Today, however, Herrin Hospital plays a critical role in Southern Illinois Healthcare’s stroke network. In fact, Herrin is on the Target Stroke Honor Roll©, a prestigious recognition by the American Heart Association/American Stroke Association and has been since 2012. At the time of the initial award, Herrin Hospital was one of only three hospitals in the entire state of Illinois to receive this coveted distinction. “From the beginning, we’ve said if you are having a heart attack, the sooner you get treatment the better your outcomes,” said Dr. Joshua Miksanek, medical director of the Emergency Department. “Early on at Herrin Hospital, we applied the same principles to our stroke treatment and took a team-approach to providing comprehensive stroke care, especially at the start when time is of the essence.” hospitals take time to do it right. Thanks to programs like the Southern Illinois Healthcare (SIH) Stroke Network, this gap is now closed. Herrin Hospital: The Early Years

‌I

n the early years of Herrin Hospital, a registered nurse made $125 a month, and key personnel made up to $100 a month after 20 years of service. Employees worked 12-hour shifts, and nurses prepared food for the patients. In fact, the first culture of penicillin was grown in the lab in sterile beef broth in 1944. A sparse operating room served as the lone locale for the complex surgeries of the day. From its infancy as the Baker-Gardiner Hospital on Harrison Street in 1914, to the sophisticated provider of care where complex bariatric, orthopedic, urological, gynecological and neurosurgical cases are performed every day, the hospital is the economic anchor and heart of this Williamson County community. Paying it forward: The Herrin Hospital Auxiliary

n integral part of Herrin Hospital’s success comes from a group of individuals who do not offer medical care. The women and men of the Herrin Hospital Auxiliary offer their time to assist staff, patients and visitors and raise money for the facility, which to date has provided over $1,000,000 to the hospital. Included in their contributions: $100,000 to the new emergency room expansion, $100,000 to the chest pain center and $25,000 to the new cancer center under construction on Illinois 13 in Carterville. Donned in their salmon-colored smocks, the volunteers operate the hospital’s gift shop, “The Gift Garden,” located near the main entrance to the hospital. The shop is open Monday through Friday, 9 a.m. until 6 p.m. and Saturday and Sunday, 1 p.m. until 4 p.m. The Gift Garden carries snack items/drinks, balloons, books, fresh flowers and a wide variety of

014 is a watershed for Southern Illinois Healthcare as Herrin Hospital celebrates 100 years of continuous service to its communities. No other hospital throughout all of southern Illinois can equal this duration of service. From a very modest beginning, Herrin Hospital has kept pace with the rapid growth of Herrin and has grown into a modern, fully equipped 114 bed progressive facility. Herrin Hospital was built in 1914 by the late Dr. Griff Baker. His son, the late Carl Baker, and Dr. William R. Gardiner, aided in the beginning of the Herrin Hospital. It was originally named the BakerGardiner Hospital. In 1918, the name was changed to Herrin Hospital. In 1934, Dr. Illinois Hospital Corporation purchased Columbus Brown and Dr. Frank C. Perhaps no other building in all of southern Illinois has experienced the contrast of machine gun fire of the warring gangland factions; yet also known the compassion exhibited by doctors, nurses and healthcare professionals for a century; has known the joys and sorrows of birth and death to such a degree; has seen firsthand the willingness to serve others for twenty four hours a day, seven days a week, 365 days a year. At no time in its storied history has the hospital slowed down or stood still, it has expanded over the years and continued to add services to improve the health and well being of the people in the communities we serve. In addition to the many physical and facility improvements to expand the structure, Herrin Hospital has developed services of national distinction.
