The Orb is coming to Torbay Torbay’s new online information and advice directory, The Orb will officially launch this November, allowing people across the Bay to be better connected and informed about services in the area. The brand new directory, which officially launches on the 2nd November will be free of charge to use and available for everyone, giving people across the Bay advice and access to information, including a ‘Directory of Services,’ a ‘Rate and Review’ service and a community ‘What’s On’ guide. The site can be accessed online via www.torbayorb.co.uk The Orb directory has been created and developed following extensive collaboration with individuals and organisations from across Torbay including the public, NHS, Torbay Council and the Torbay Community Development Trust who worked extensively with the wider voluntary sector community. The name, logo and colour scheme for the Orb were all chosen by the people of Torbay, following a mass call out at the beginning of the summer. The Orb has been developed to meet requirements put forward by the Care Act (2014) and will also help both professionals and the public alike access information on Social Prescribing and Personalisation. The organisations involved envisage that the Orb will become the go to place for all community information across the bay. Billy Hartstein, from the Torbay Community Development Trust says “The Orb will streamline the process of accessing services and information for everyone. It is fantastic the system has been created with the help and guidance of the people who will be using it on a daily basis, including the public who took the lead on naming and branding the directory.”

The CCG have released the following Stakeholder Briefing outlining how it will keep key stakeholders up to date with current thinking on the future of commnity health services across South Devon and Torbay and their plans for consultation later in the year.
Into the Future Re-shaping Community Health Services in South Devon and Torbay Purpose This is the first of a series of briefings aimed at keeping key stakeholders up to date with current thinking on the future of community health services across South Devon and Torbay and our plans for consultation later in the year. These briefings will help ensure that issues arising from engagement discussions being held across the area are shared. This should help build maximum understanding in identifying the best way of delivering safe, high-quality care within the resources available. Context In late 2013, South Devon and Torbay Clinical Commissioning Group (CCG) – in partnership with our acute and community providers, and Devon County Council and Torbay Council – carried out extensive engagement about our community health and social care services. People told us the most important things to them were: •Accessibility of services – convenient opening hours, transport and accessible buildings. •Better communication – between clinician and patient, and between clinicians themselves. •Continuity of care – to allow relationship-building with clinicians and carers. •Coordination of care – including joined-up information systems. •Support to stay at home – with a wide range of services and support. On 1 October this year, South Devon Healthcare NHS Foundation Trust (Torbay Hospital) and Torbay and Southern Devon Health and Care NHS Trust merged to form a single, integrated organisation. This means that the resulting Torbay and South Devon NHS Foundation Trust (TSDFT) will deliver the majority of our health and care services, from district nursing, community therapy, complex care and multi-agency teams, to highly specialist acute care. With organisational barriers removed, this is a huge opportunity for acute and community services to be integrated, with much more care – including specialist care – available outside the district general hospital. Challenges faced by health services We face significant challenges in providing health and care services, and these will increase.
In our communities we have: •Increasing number of older people. •Increasing number of people with long-term conditions, many of which are complex. •Flat or reducing finances, especially when health and social care resources are combined •Changes to professional NHS standards. For example, current guidance states that a qualified nurse should look after eight hospital beds and that there should be a minimum of 16 beds, so there will always be two nurses working at any one time. A new model of care Responses from the 2013 public engagement, the challenges faced by health services and the opportunities presented by a more integrated healthcare system mean we need to consider whether there are better ways of delivering services. We have already started to develop and test a new model of care in Teignmouth and Dawlish, and in Dartmouth. In these towns, input from the League of Friends, town councils, Patient Participation Groups, the voluntary sector and others has helped to shape an emerging model of care. We have also had meetings with stakeholders in Bovey Tracey, Ashburton and Newton Abbot, with further meetings planned in Paignton, Brixham and Torquay in October to discuss the principles of this new approach. The emerging model sees GPs, community health and social care teams and the voluntary sector working together to provide for the vast majority of people’s health and wellbeing needs. It is founded on joined-up care across the whole community. We want to be able to provide care as close to home as possible, supporting people to remain independent and in their own homes, reducing reliance on bed-based services. We want to see local communities actively helping to support the wellbeing needs of the local population We recognise that one size will not fit all. From locality to locality, and from town to town, there will be differences in health, demography and geography, as well as variation in the availability of other services such as residential and nursing care. The proposed model of care will need to reflect these differences while being able to deliver consistent, high-quality care. Consultation The emerging model is evolving in the light of comments received. Our intention is to consult formally across the whole area (except the Coastal locality, which covers Teignmouth, Dawlish and the surrounding villages where changes have already been agreed) so that people from Torquay, Newton Abbot, Moor to Sea and Paignton & Brixham localities all have the opportunity to have their say on any proposed model of care, reflecting on what it will mean for healthcare in their own area. No changes will be made until this has taken place. We expect this consultation to take place over the winter months, with details being provided in these briefings when timescales are finalised. We would like to talk to as many groups as possible. Public meetings will be arranged in each locality; however, we are also keen to speak to any local meetings/ groups/clubs. If you would like further information and/or to request a talk, please email us at sdtccg.consultation@nhs.net.
