HEALTH FORMS AND IMMUNIZATIONS

CURRENT TOWER HILL FAMILIES

Required forms include a current physical and immunization record. If forms are still current from the school year, you do not need to do anything further. Please log-in to Magnus Health now and make sure all information is up to date for your camper.

NEW FAMILIES STARTING IN 23-23 SCHOOL YEAR

• Log-in to Tower Net and locate the Magnus Health Tile in the resources section. • Answer all “Conditional Questions” • If your child has a current physical from the past 12 months, upload it to the section titled “Delaware Student

Health Form TT - 6” ◉ If they do not have a current physical, please download and print the required forms and make an appointment with their physician to complete them. • Upload “Immunization Form” (no required template - use any form from your physician) to the appropriate section. • If you answered “yes” to the severe allergy/asthma conditional questioning: ◉ Submit a current asthma or allergy action plan to the correct requirement ◉ Complete the “Prescription Medication Consent” requirement ◉ Contact the school nurse (aherbein@towerhill.org) to discuss health concerns and treatment protocols ◉ Bring epi-pens/inhalers on the Monday of camp to be left on-site all week; all medications will be returned on Fridays

All other requirements need to be completed before the start of the school year, but are not required for camp.

NON TOWER HILL FAMILIES

The Vital Health Record is extensive because of state requirements for our students. This cannot be adjusted for summer camp. It is your choice to leave it blank or fill out only relevant sections.
