This directory is designed to provide an easy access list of helpful resources in our community, the kind of thing you can keep by the phone for quick reference to connect to the people and programs you need most. But it is also designed to keep you connected to the rest of the Tri-Cities, with opportunities throughout these pages to reach out to fellow 50 plus seniors. If you’re a senior citizen in the Tri-Cities, you’ll find no shortage of recreational activities and opportunities to make friends. In addition to a variety of local groups whose members sing, dance, play softball or bridge, the cities of Coquitlam, Port Coquitlam and Port Moody offer recreation facilities geared for seniors — and we urge you to check out the one most convenient for you:

PORT COQUITLAM

CENTRAL COQUITLAM

Wilson Centre, an accessible one-level facility attached to the PoCo rec complex, is an activity and recreational hub for those aged 50 years or older. There are a number of drop-in activities, from snooker, table tennis, darts and carpet bowling to bingo and a variety of card games. Wilson also offers registered programs, including those to help you boost your computer and online skills, plus accessible programs to meet the varying needs of the community, such as monitoring blood pressure and health and wellness talks. Dogwood Pavilion is located at the corner of Poirier Street and Winslow Avenue. Offering free parking and a cafe serving hot lunches on weekdays, Dogwood presents a variety of activities, including: fitness and yoga classes, art classes, snooker room, woodworking room, dance classes, talks and lectures, computer classes, language classes, seminars and clubs. FINANCES Compassionate Care Benefit 1-800-206-7218 www.servicecanada.gc.ca This is an Employment Insurance (EI) special benefit that will pay up to six weeks to claimants who provide care or support to a gravely ill family member who has a significant risk of death. The claimant must have sufficient hours of insurable employment to qualify. Up to six weeks of leave may be taken, with job protection, to provide compassionate care to a family member. Disability Tax Credit 1-800-959-8281 www.cra.gc.ca This tax credit is available to people with a severe and prolonged mental or physical impairment. All or part of this credit can be transferred to a spouse or supporting individual. Fill out form T2201-E and submit it with the income tax of the person. The doctor must sign the form under “Particulars of Disability”. People with a permanent disability (e.g. Alzheimer Disease) only need to submit this form once to get the tax credit in future.

HEALTH & SUPPORT SERVICES PROVINCIAL HEALTH SERVICES HealthLink BC 811 A trained health service representative will answer 8-1-1 telephone calls, and where appropriate, calls will be transferred to a registered nurse, pharmacist or dietitian for further information and advice. The 8-1-1 service can provide assistance with answers to questions about a variety of health topics, help in identifying symptoms and advice on when to seek medical treatment. 8-1-1 has translation services available in over 130 languages. New residents to the province are eligible for coverage after a waiting period of the remainder of the month of arrival plus two full months. MSP pays all medically required services of physicians and surgeons (and dental surgery if performed in a hospital). Routine eye exams are a benefit for people under the age of 18 years and for those 65 years and older.

For people who receive Premium Assistance, MSP also pays the cost of a combined annual limit of 10 visits for chiropractic, massage therapy, naturopathy, physical therapy, acupuncture and non-surgical podiatry when performed by a practitioner enrolled with MSP. (MSP pays $23 for each visit, patient is responsible for any charge over and above.) Among the services not covered by MSP are dental services (other than dental surgery performed in a hospital), eyeglasses, hearing aids and other equipment, annual or other examinations where there is no medical requirement e.g., for driving a motor vehicle / life insurance purposes, or the services of counselors or psychologists. To be covered by MSP you must apply and pay monthly premiums. If your income is low and you are a Canadian citizen or landed immigrant and have lived in Canada for the last 12 months you may be eligible to receive premium assistance. Gold cards are automatically issued prior to your 65th birthday. If you lose your Care Card, call Seniors One-Stop (604- 983-3303 or 604-925-7474) for an application for replacement. HEALTH & SUPPORT SERVICES Home Health Services Home Health offers a variety of at-home and community programs and services depending upon your care needs and eligibility. To identify your most appropriate care options, Home Health professionals consider the needs related not only to your physical symptoms, but also to other dimensions of your life such as your emotional well-being and the support available through your family, friends and close community ties. While some Home Health clients require only specific short-term medical treatments, others have more complex and ongoing health needs requiring the careful coordination of a number of integrated services. Short-term support helps you recover from a hospital procedure or condition, while longer term programs provide ongoing care to enable you to maintain your independence and remain at home comfortably. No matter what the circumstances are, Home Health professionals take the time to understand your unique needs and situation so they can plan the best care solutions for you. 1-877-REACT-08 (1-877-732-2808) or email react@fraserhealth.ca or call the Home Health Service line(number above) Fraser Health Adult Day Programs: Adult day programs assist seniors and adults with disabilities to continue to live in their own homes by providing supportive group programs and activities that assist with daily activities or give clients a chance to be more involved in their community. Activities vary with each centre. They may include: • personal care services, such as bathing programs and administering medications; • therapeutic recreation and social activities; • caregiver respite, education and support; and • in some centres, meals and transportation may also be provided or arranged. Clients may attend an adult day program in addition to receiving other services, such as Meals-on-Wheels and home support. Many programs are connected with residential care facilities, while others operate independently. An assessment and referral from a Home Health professional are required prior to participating in any programs. You, or any person, family member, hospital, family doctor, friend or neighbour, community group (such as a seniors centre) or government agency is welcome to make a referral to Home Health on your behalf.

If you feel you need services such as home support or access to an adult day program, or you wish to be considered for assisted living or residential care, a Home Health Case Manager will assess your situation. The Case Manager will review your needs, eligibility for services, and costs. The assessment will take place in your home, in the hospital or in another care setting. The Case Manager will work closely with you, your family and friends to: • evaluate your needs • determine what kind of services are needed • determine eligibility for services, including residency • determine whether there will be any costs involved for certain services • plan, coordinate and monitor multiple care services • arrange relief for family caregivers • assess clients who may need to move to an assisted living or residential care facility or to a hospice residence If you require nursing or physical/ occupational therapy services, a nurse or therapist may visit you in your home to assess your specific additional needs. Following the assessment, an individualized care plan is developed and appropriate Home Health services are arranged as required. The goal is to help people with multiple needs live at home safely and to avoid unnecessary or premature admission to a hospital or care facility.

Case Management Helps develop a care plan and arrange for health care services. An annual membership is $18.75 and provides access to subsidized drop-in and registered program fees, services, and special events. There are a number of drop-in activities including snooker, table tennis, darts, carpet bowling, bingo, and a variety of card games. Members pay $1 to drop in and non-members pay $3. Wilson Centre offers a wide variety of registered programs to help you keep active, be creative. Learn a variety of arts and crafts; refine your computer, iPad and tablet skills, and much more. Crossroads Hospice Society – Walking Groups and Hike for Hospice 604-945-0606 www.crossroadshospice.bc.ca Crossroads Hospice Society’s walking group Grief’s Journey, is open to bereaved individuals or those grieving the imminent death of a significant person in their life. Monthly full moon walks at our Crossroads Hospice Labyrinth Healing Garden introduce the healing powers of nature and the labyrinth to our community. The first Sunday of May Crossroads Hospice Society hosts the annual Hike for Hospice, which helps raise money and awareness for hospice care. All proceeds from this family-friendly walk stays in our community. Coquitlam Public Library – Home Bound Services The library provides free delivery and pick-up of books, audiobooks and magazines for individual Coquitlam residents who are temporarily or permanently homebound in private homes or in care centres. Patrons on the Homebound Services Program may place reserves on materials of their choice or allow the Community Services Librarian to select materials for them. A

profile of reading interests and a list of materials read or desired is kept for each Homebound patron. This information is confidential and available only to Homebound Services staff. Low Vision Resources The library offers the following resources for people with impaired vision: Large Print Materials Many popular fiction and non-fiction titles are available in large print. We attempt to purchase new and bestseller large print books published simultaneously with the regular print. Audiobook Collection The Poirier Branch has a collection of audiobooks on cassette tapes, Daisy Book format and MP3, available to users who are visually or physically challenged and cannot read regular print material. This collection is funded in conjunction with Public Library InterLINK, and users must sign an eligibility form to use this collection. Daisy Books and MP3 format books can be played on Daisy Players, personal computers and DVD players. A second collection of books on cassette tapes and CDs is available to the general public at both branches.
