Pre and post-operative care for your knee injury
W
ith the warmer weather approaching, so too do dormant knee niggles that may have plagued you last year. Knee injuries can occur both acutely (immediate injury) or over time. If you have a reccurring knee injury and are anticipating surgery, the following information is provided to assist in achieving the best possible preparation & outcome of your surgery. Damage to the structures of your knee can result in tissue disruption (tears), bony damage, swelling or bleeding within the knee as a response or reaction to an injury. This can occur from a single event or accumulate over time from knee loads/stress resulting from the way we walk, run or twist, or from the inherited shape of our knees. Muscle wasting – especially of the quadriceps muscle group at the front of your thigh - plays a significant role in the rate of recovery from knee surgery and your ability to return to your activities post injury or surgery. The quadriceps muscle group can begin

wasting immediately following an injury or surgery & requires a concerted program to bring muscles back. Prior to surgery, TSIC Physiotherapists aim to prevent muscle wasting by teaching you the correct exercises to perform, as this will both assist during
recovery and reduce the load through your knee post operatively. Good muscle strength, particularly your quadriceps, equals less stress through your knee long term. Other muscles such as your calf and gluteals (bottom) also play a huge role on reducing
Physio knee stress and these should not be forgotten with rehabilitation at any age. Strengthening these muscles will help with balance, steps and walking at a good pace post operatively.
ACTIVITIES THAT CAN STRESS YOUR KNEE • Squatting for long periods • Twisting • Stairs • Running incorrectly • Incorrect footwear
Preparing you for the best outcome Once you know the knee is injured, focus on the things that can improve recovery time: • Following your knee injury it is imperative you follow the R.I.C.E (Rest/Ice/Compression/Elevation) regime for at least 72 hours and maintain circulation in the calf by moving your foot and ankle • You can keep icing your knee if it remains swollen post operatively • Anti-inflammatories and pain killers are often used to reduce the degree of swelling and assist with muscle activation. Muscles do not return if pain is present • Tensing of the quadriceps should commence within a few hours of your injury/or post surgery and maintained every waking hour at 10-20 reps per hour. Do not lift your leg - just tense your thigh with your knee out straight and hold for 5 seconds. A good muscle contraction also assists in keeping the knee swelling reduced. At TSIC we use a Real-Time Ultrasound machine to measure the size of your inner quadriceps muscle (VMO). This is a great tool for you to learn how to perform the correct quadriceps exercise, as you can see your muscles move on the screen! Whilst a knee operation can repair the damage in your knee, Physiotherapists at TSIC will assist in managing the contributing factors and give your knee the best possible chance for recovery. fter travelling to Asia several times and to LA and Miami already this year it’s exciting to see that hair in Australia is still at the forefront of our ever changing industry.

It’s really inspiring to see that the UK and Australia share a passionate bond for creativity and beautiful hair. Whilst presenting on stage to our friends in Kuala Lumpur earlier this year, I experienced street vendors doing hair to hordes of onlookers; it was amazing to see these unique craftsmen showing their techniques to adults, children and anyone who wondered what all the fuss was about. They were even selling DVD’s on how to create the same looks at home. The beautiful people of LA and Miami were a little more conservative but they love the way we approach hair with ease and confidence, especially our colouring.
The coming months will see more emphasis on fringes - some straight and blunt, others soft with wispy edges. With the new seasons ahead why not try something new! If you have long hair for instance a new fringe can make a beautiful change without losing length. Perhaps even a big change is in stall, so why not try a short and sassy new do! Hair colouring is moving into a very creative era, with soft tone-on-tone techniques. Blending clean blondes into pastels with pale pink, copper, caramels or even blues, lilacs and greys, we are here to help stretch your imagination. Consultations using iPads & photos are a great way to excite you into the next stage of creative colouring. Whatever you choose this season, remember that simplicity is the ultimate sophistication! Well, with a little guidance from a fashion ‘guardian angel’ anyone can put their best foot forward and take the steps to rediscover their confidence. When author Nicky Johnston reached out to an old friend she could not have anticipated her decision would have such a transformational effect. Nicky is an author, illustrator, educator and speaker, but before anything else she is a mum of four. In 2013 she released her third book and sold-out workshops all over Melbourne. But this sell-out success was no coincidence according

... Styling is really about empowering women to be the best version of themselves all the time... When she first reached out to Beckie, Nicky faced an issue that is familiar to a lot of mums. After dedicating 15 years to raising her sons she had lost her sense of who she was. With the launch of her third book fast-approaching Nicky met with Beckie in-store in search of a new outfit, but what she left with Nicky found much more valuable. “We basically sat down and talked; and I walked out of there a different person” she says. It was in that consultation that Nicky realised it was about more than just clothes, it was about how she viewed herself and Beckie picked up on this almost instantly. “Too often we underestimate ourselves. HEATH MELDRUM (Pro National Stand-up Paddleboarder) on the benefits of Myotherapy... Heath is a regular at The Sports Injury Clinic, often incorporating myotherapy into his gruelling fitness and recovery regime. As well as a stand-up paddleboard champion, he’s a Mornington Peninsula local and proud family man. We were very privileged to get an interview with Heath who was more than happy to discuss the direct benefits he receives from myotherapy treatments. I&O: What Activities are involved in your training regime? MELDRUM: I’m on the water 5 to 6 days a week paddling and X-training

with a combination of strength training, running, swimming and stretching on the same days. I&O: What are the main issues you have as a Paddleboarder? MELDRUM: The main areas I get tight and sore are Hammies, Quads, Glutes especially, lower back, Lats, Traps and pecs! Tightness and soreness does vary week to week between these areas which is why I find it very important to keep it as balanced as possible so I can continue to train hard each day. I&O: What are your goals in your sport? MELDRUM: My main goal each season Issue #06 - Spring/Summer ‘13/14
is to continue to love the sport and have lots of fun while finishing toward the front of the field as much as possible. This year I qualified to represent Victoria at the Stand-up Paddleboard Nationals which was a proud achievement. I also hope to achieve a high level of fitness this year and qualify to compete in the 50km/32mile inter island Stand-up Paddleboard world championships in Hawaii. I&O: Why do you get Myotherapy? A question we often hear is “when should I get my child’s eyes tested”? We always advise that before starting prep is the ideal time for a first examination. ver since Chris McDougall published his book “Born to Run,” the trend of barefoot and minimalist running has received a lot of interest between sportsmen and medical practitioners alike. As any fan of barefoot running will tell you, the idea behind this movement is to take human beings back to how we “naturally” run. Indeed, barefoot running does promote forefoot landing, a reduction in heavy heel striking and more intrinsic muscle activation. However, other claims by the barefoot community cannot be supported with any reputable evidence. For example; running shoes cause medial knee osteoarthritis, therefore barefoot running is better. Or the claim that barefoot running decreases overall injury rates in runners willing to make the transition across. Both of these statements are completely false. In a study by Salzler et al (2012), ten experienced, traditional runners switched to minimalist footwear. In the following ten months a total of eight metatarsal stress fractures, one calcaneal stress fracture and one plantar fascia rupture occurred in this group.

... Exercise & running is truly beneficial to both physical and mental health... If you are thinking of making the move to minimalist runners such as the Vibram FiveFingers, Nike Free Runs or New Balance Minimus, please do so with caution. It would be wise to organize a “wear in” plan with a podiatrist to ensure a smooth transition is achieved. This is important as the tissue in the foot and lower limb needs time to adapt to the different footwear. Running barefoot or in minimalist runners is contra-indicated in those with a history of osteoporosis, previous stress fractures or Achilles pain. Exercise and running is truly beneficial to both physical and mental health. We just implore you to do it safely and with the best level of evidence behind it. Good luck! What is it & how do we fix it?
tress Incontinence refers to bladder leakage when pressure in the tummy above the bladder rises. Coughing, sneezing, blowing your nose, raising your voice, etc requires you to produce a huge upwards ‘expulsive’ force which also moves downwards onto the bladder. If the pelvic floor muscles are weak, then you may leak urine. It can be mild or in severe cases require the use of daily pads. Who does it affect? It is fairly common for women to notice a leakage problem after having a baby and if not managed properly can lead to secondary problems like urinary urgency, frequency and urge urinary incontinence (leakage associated with a strong desire to pass urine). For men it is most common after surgery to remove the prostate. Most surgeons will recommend Physiotherapy before and after the surgery, but we know there are still men suffering in silence. Pelvic

floor muscles can be weakened in men and women by being overweight, straining over the years to empty the bowel and by repeated heavy lifting. How can we fix it? The great news is that no one has to put up with this challenging condition. The answer to stress incontinence is pelvic floor exercise. However the most important thing is to learn how to use your muscles
correctly to control the problem. Sadly current statistics show that 40% of people who have been told how to do pelvic floor exercises end up doing their exercises INCORRECTLY, which can lead to further problems. Only the correct muscle action will strengthen the pelvic floor.
• S o are you 100% sure that you are doing exercises correctly? • W hen you switch on your ‘core muscles’ are you sure you’re switching on your pelvic floor? • H ave you trained yourself to switch on the pelvic floor when you need it most in daily activities such as lifting a shopping bag, pushing a heavy door open, exercising, coughing, sneezing, doing housework, gardening? Pelvic floor should be on and off all day long. • C an you feel the muscles working, squeezing, tightening and lifting in the right place? • C an you maintain a normal breathing pattern as you switch on and hold a pelvic floor contraction? If you answered yes to all the questions above - WELL DONE! If in doubt, seek help from a trained Pelvic Floor Physiotherapist & regain your confidence and control. tress is not necessarily bad - it’s how we react to it that matters. In times of crisis and trauma following the death of a loved one, a major life change, a physical or psychological workplace injury or a car/motorbike accident, it is understandable that you may suffer from symptoms of stress. Some of us are well supported with good networks of friends and family, whilst others
often feel they’re battling on their own. As much as physical and emotional support can help you through a difficult time, it does not always prevent depression and anxiety from occurring.
