ip injuries can strike anyone by surprise and often be due to activities you performed when you were younger. They can go for years undiagnosed but thankfully technology and assessment procedures have improved. In younger patients, an underlying hip deformity or joint problem: such as a tear in the hip labrum, or femoroacetabular impingement (FAI: the hip that pinches!), can cause one or more of the following symptoms: • Activity-related hip pain (especially in the front of the hip or groin)
• Discomfort with walking, running, pivoting, sitting, twisting
Pain in the back or buttock can be related to the hip, but as the muscles that control the hip become stressed, they too can start hurting. Soft tissue injuries around the hip typically respond to specific Physiotherapy treatment in 3-6 weeks. If symptoms last longer, a hip joint problem may be the cause and require further investigation.
Diagnosing Hip Problems
Advances in technology means earlier detection of hip problems. At TSIC, our Physiotherapists perform a thorough patient evaluation & assessment of symptoms to formulate a treatment plan for you. Evaluation of a hip problem may include the following components: • History & physical examination • Radiographic series (x-rays/CT scan) of the hip • MRI (magnetic resonance imaging) scan • Referral to a Sports Physician
Why Is Early Diagnosis So Important? Younger patients with hip symptoms often have a hip deformity or hip joint disorder that are not easily recognised resulting in ‘silent symptoms’ that may go for an extended period of time without treatment. Most hip conditions will cause limitation of activity & can lead to the development of early arthritis. Early diagnosis and intervention is extremely important to minimize symptoms & prevent premature joint damage. Osteoarthritis, for example, is caused by the breakdown and eventual loss of cartilage in the joint, ultimately leading to the need for joint surgery. If caught early, many hip problems may be corrected, and the natural hip joint may be preserved to avoid a hip replacement procedure at a young age.

Physio How Do I Know If I Have A Hip Problem? • Trouble with twisting, putting shoes & socks on, squatting • Walking with a limp (though you may not notice that you have one: but others will notice!) • Trouble getting in and out of a car (groin or pain on the outside of the hip) • Pain laying on a side of the hip
Treatment
A majority of hip problems commence with the muscles, tendons, bursa or inside cartilage being the main source of pain. These can respond well to physiotherapy management and may need some co ordination with your GP and /or Sports Physician. Learning correct loads and activities you can do for your hip are vital as well as walking correctly can prevent the downhill slide of your hip and ending up with gross ‘wearing out’ or arthritis. DWYER: I had become bored with road running and too old for racing a sailboard so when my daughter asked me to watch her in a BRW event I was hooked. It looked easy and there were other older people competing. I&O: When was your first season? DWYER: The first was 1997 at age 55. This was quite a challenge as I hadn’t really ridden a bike since my teens. I have competed in the last two world triathlon sprint championships and placed 4th both times. I am undecided yet as to whether I will go to Edmonton this year to defend my hold on fourth place. I&O: So competing runs in the family? DWYER: Yes, my son will attempt his first Ironman Melbourne this year as well. We didn’t set out to make a dynasty it just turned out that way. I&O: What’s your typical training week? DWYER: 11-13 hours per week – swim, bike & run.

SET A GOAL:
How do you know if you’ve kicked a goal if there are no goal posts on the field? Whether it be a goal weight, an event to complete or a PB: Name it, write it down and go get it!
MAKE A PLAN:
When busy at work, school or at home, things don’t “just get done”. Set a plan for all training and dietary intake as you’d plan out a busy period in the office, school assignment or major house renovation.
BE ACCOUNTABLE:
Regular progress updates are crucial to keep you on track, you won’t wake up deadline day and magically have achieved your goal without ticking off smaller goals along the way, being accountable so something/someone can ensure smaller goals are met, and ultimately your big goal!
MATCH YOUR DIET AND YOUR EXERCISE:
Basic physiology tells us that a massive increase in exercise expenditure and decrease in energy intake simultaneously is not sustainable and will only end in decreased energy levels, increased fatigue and ultimately a missed goal. Why is this still a big part of common weight loss and fitness programming? Don’t eat less, eat smarter to achieve your goal.
UNDERSTAND WHAT TO EXPECT:
There are going to be times of rapid weight loss, big gains in fitness and overwhelming positivity regarding your progress. There will also be plateaus and times when things just don’t seem to be going anywhere. Understanding the ups & the downs and what you can do to maximize results is a big advantage to have.

e all know that podiatry features as a major medical field in the sporting arena, with most professional sporting teams having their own dedicated podiatrist. However, anyone can experience foot pain. They may not have sporting based injuries, but that does not mean a podiatrist cannot help you! The purpose of this article is to introduce and identify everyday foot related pathologies that the general public face, and how these are managed from a podiatric standpoint.
• NAILS:
Most of the public will suffer from a nail related problem at some point in their lives. Whether this is an ingrown nail, fungal infection, bruised nail bed or they are simply difficult to cut and maintain. A podiatrist can aid in all of the above pathologies and are willing to be faced with uncommon or unsightly conditions.

• CALLUS:
Callus (hyperkeratosis) will form on any surface of the foot that is under direct plantar pressure. This can cause pain and discomfort leading to altered gait and associated lower limb problems. A podiatrist will debride this callus away, and in appropriate situations can offer padding to offload the symptomatic area.
• CORNS:
Like callus, corns will form under direct pressure. Given a corn looks a lot like an inverted “blackhead”, the discomfort associated with a corn is the nucleus giving the feeling of a pebble stuck under the skin. The role of a podiatrist is to remove the nucleus and debride the surrounding callus, resulting in immediate relief.

ometimes life doesn’t go to plan. Even with the best intentions, no matter how well relationships start, many don’t last. The surprising thing is, the average time from the low of separation to the high of feeling good again is 3 years! PAIN MANAGEMENT A PSYCHOLOGICAL JOURNEY
Living with pain:
Pain will affect just about everything you do. It interferes with work, relationships, looking after the family, socializing, sleep, travel, exercise, and your general emotional well-being. Wow, that’s a lot! It is common to experience feelings of depression and anxiety. You may feel angry and frustrated a lot of the time, and become snappy and moody, and generally feel as though you are not coping.
The pain message:
Pain can choose anyone, it isn’t picky, and likes to hang around. It is important that we stop the pain message as soon as possible once an injury occurs so the neural pathways that carry the message to our brain don’t get too well worn. Imagine a goat track going up the side of a mountain, the more often the goats walk the track, the more obvious the track becomes. Pain likes to do the same thing. It uses the same path to get from the injury site to our brain, over and over, so that it gets stuck on a loop, even when we no longer need to know about it. The pain message can be stopped by using a number of techniques, including prescribed medication, relaxation, hypnosis, keeping busy and being distracted, massage, exercise (as appropriate), and heat. The more we stop it, the less we notice it. The less we notice it, the less the message gets sent.

Being on one of these compensable bodies has its own set of challenges. There are many people with their “fingers in the pie” and you may feel as though you have lost control of your own life. It is important to develop, or keep a hold of, the skills that allow you to be the boss of your situation. Assertiveness training, being clear of your goals for treatment, therapy and returning to work, and developing a healthy support network, are all important skills and strategies.
What you can do:
Learning how to manage your situation is very important, and is unique to each individual. Seeking treatment in a multidisciplinary setting can really assist with this as all therapists can easily communicate with each other and develop a program for the best possible outcome. Sometimes you need to change your expectations, and this can be daunting, especially if you are very driven and determined. Putting your energy into taking care of yourself, rather than your usual rushing around becomes a priority. Letting others help you out and do things for you also improves your chances of a good outcome. Remember, if you can change your usual way of doing things for a little while now, you reduce your chances of having to do it long term.
