Healthcare is in transformation. Length of stay in hospitals has decreased. What is contributing to this phenomenon? Efficiencies in operations, high-tech equipment, and innovative medical procedures have all played a part. It would be impossible to list all the contributing factors, but some specific examples highlight what is happening in healthcare. Hospitals are now utilizing more efficient methods of triage, admitting, patient monitoring and care, and record keeping. Computer technology has simplified many of these areas, making it easier to access critical information both past and present. Hospitals are designed around the needs of the patient, emphasizing bed-side monitoring, private rooms, and expanded visitation. Surgical suites and processes have been designed for better patient outcomes and safety. Secure medical information is readily available on-line to patients and family members. Surgical procedures are being assisted with the use of high-tech equipment. Blood vessels have been reconnected in microsurgery using a Synovis coupler which saves valuable time versus hand suturing. Vioptix tissue oximeters can detect compromises in tissue viability before obvious clinical signs using near-infrared light sensors. The Impella® heart pump can help patients with severe heart conditions to receive lifesaving interventions to open obstructed heart arteries. Telestroke video conferencing enables stroke patients and emergency physicians to connect to neurology specialists for immediate evaluation and treatment. Virtually every area of the hospital has new equipment to make their jobs easier, more efficient, and safer for the patient.

Innovative procedures are making intervention and surgical techniques less invasive, which makes recovery times shorter and more comfortable. Transradial catheterization utilizes the wrist instead of the groin for stent insertion. “Off-pump” open-heart surgery allows a surgeon to operate on a beating heart. Post cardiac arrest therapeutic hypothermia is an innovative body-cooling procedure that can significantly improve a patient’s chances for long-term recovery after sudden cardiac arrest. These are just a few of the many ways healthcare is being transformed. As technology changes and improvements are made, the healthcare industry is actively pursuing ways to make the patient experience less stressful and more successful. BREAST CANCER TODAY Breast cancer is the second most common cancer among American women, with about 1 in 8 (12%) women developing invasive breast cancer at some point during their lifetime. According to the American Cancer Society, in the year 2015, more than 231,000 cases of invasive cancer and 60,000 cases of carcinoma in situ will be diagnosed in women. Additionally over 40,000 women will die from breast cancer each year, making it the second leading cause of cancer death in women. That being said, death rates from breast cancer have been declining since 1989, likely due to earlier detection through screening and increased awareness, as well as improved treatment. At Breastlink Women’s Imaging center of Temecula Valley in 2014 there were 26,951 mammograms and 1,761 biopsies performed. Out of every 1000 exams performed, it is estimated that two to ten cancers will be detected. This was mirrored in our experience, as 232 cancer cases were diagnosed last year. The majority of cases are detected through mammography, where the goal is diagnosis prior to clinically palpable or symptomatic masses. At this point, the cancer is less likely to have metastasized. Although mammography is less sensitive in younger women and in those with dense breast tissue, the use of digital mammography improves the rate
of detection in this population of women, and is the most cost-effective and accurate screening tool for early detection. Death rates have been shown to be impacted as much as 30% in trials such as the Two County Trial in Sweden. As such, the American College of Radiology (ACR) and the American Society of Breast Surgeons (ASBS) recommend annual screening mammogram for women age 40 and older. In order to improve breast cancer screening accuracy, newer technologies are being investigated and are showing promise. Breast tomosynthesis, for example, has demonstrated higher detection rates and fewer patient recalls for additional testing. This improved sensitivity will most likely save numerous lives. Overall, the objective is detection as early as possible. Detecting cancer at an earlier stage not only allows for more conservative surgery, but may obviate the need for chemotherapy altogether. Additionally, in the current health care climate, it’s impossible to ignore costs. Early detection assists in driving these down, allowing for more efficient allocation of resources. In the end, and most importantly, the early detection of breast cancer increases survival, allowing us to better accomplish our ultimate goal. An additional 350+ jobs were created in 2014 from company relocations/expansions. With nearly 43,000 jobs and a growing diverse economic base, Temecula is the job center of Southwest Riverside County. The City of Temecula will close out 2014 with a flurry of more than a half dozen new company relocation/expansions bringing over 350 jobs to the community. Temecula continues to create a positive economic current that draws company relocations and spurs expansions. Moreover, the City continues to work with additional prospective company relocations through the successful Economic Development Ombudsman service. Industries from manufacturing to research and development have either relocated to Temecula or have expanded over the past year. Some of these include: EMD Millipore: The City of Temecula is pleased to welcome the business expansion of one of its largest life science companies, EMD Millipore. EMD Millipore has finalized plans to lease close to 38,000 square feet of additional building space in Temecula’s Business Park. In total, the organization will have nearly 150,000 square feet, spread across three buildings, dedicated to the development, manufacturing and distribution of biologics & chemicals. Over 100 additional jobs were created by this expansion, bringing EMD Millipore’s job count to over 350 people. Iron Grip: The largest provider of commercial free-weight equipment worldwide and the only manufacturer with a line of exclusively American made free weight equipment has recently acquired a 44,100 square foot building in Temecula.

Rough Brothers Inc.: Since 1932, Rough Brothers Inc. designs, engineers, manufactures and installs custom greenhouses and conservatories that help commercial growers, retail garden center, research facilities, universities and schools throughout the country. Rough Brothers recently purchased a 60,000 square foot facility in Temecula. Temecula has not only successfully attracted large industry employers, but is also home to a growing number of new high-tech companies that are located in the City’s business incubator, the Temecula Valley Entrepreneur’s Exchange (TVE2). Currently, 11 start-up technology companies with 16 company founders and an additional 12 employees reside in the TVE2. The Office of Economic Development is focused on attracting, retaining and creating jobs in Temecula. A BUSY LEGISLATIVE YEAR AHEAD S O U T H W E S T

The California Legislature has been in session for just a few weeks now and they’ve already introduced several hundred bills that will tax us, regulate us or otherwise impact some aspect of our lives and businesses. By February 27, the final date to submit new bills, in all likelihood between 2,200 and as many as 3,000 new pieces of legislation will be launched down the alimentary canal that is the Sacramento Legislative Process. And you already know what type of effluvium will greet us at the end of the journey. The Southwest California Legislative Council is already on the job evaluating these new bills to see what impact they will have on your business. We’ve already seen some of that at work with re-introduced bills seeking to: • escalate the pace of minimum wage increase, • to expand the scope and reach of AB 32, the Global Solutions Warming Act of 2006, and • another shot at the Retail Bill of Rights. You might remember that one, if you are successful and buy another company, even an underperforming company, you will be required by law to retain ALL those employees for at least 90 days and you better have a darn good reason to fire them after that. We’re also likely to see: • another proposal to ‘reduce’ taxes by expanding taxes to include all services, • more attacks on Prop 13, • at least two efforts to implement a split-roll tax system,

• introduction of a test program to study the feasibility of a vehicle miles traveled tax. That’s particularly welcome in our region with so many commuters, and • 5 bills already introduced expanding or otherwise seeking to legalize medical and/or recreational marijuana use. There will undoubtedly be a few beneficial bills come through as well - whether they make it or not will be anybody’s guess, but Senator Roth has again introduced: • SB 25, another attempt at retrieving some of the funds pilfered from our newest cities in 2011. His bill passed last session with broad bipartisan support but was vetoed by the Governor. • re-introduce the document dumping bill which would clarify and reduce the cost of property transfers within an HOA. LEE COHEN WITH LEGAL SHIELD Lee Cohen is from Spartanburg, S.C. Upon graduating from high school he decided to join the United States Marine Corps instead of going to college. What was supposed to only be 4 years turned into 12. He loved serving his country but after his son was born it was time to focus on his family. In 2005 Lee, his wife Sheri and son Jakory moved to Temecula California. While attending college he decided to become a sole proprietor with LegalShield in 2009. While learning about the
