Say It Pays To Be an Educated Consumer

When it comes to skin care products that can help us look younger, it is difficult to know what to use. There are so many choices, and promises of instant results of looking 10 years younger can be alluring. So alluring, in fact, that Americans have made skin care products a $7 billion a year industry. While promises of outstanding results sell products, the products do not always deliver. To select effective age-fighting topicals, dermatologists say, you need to be an educated consumer. The following tips from dermatologists can help you become an educated consumer: 1. Do your homework . Understanding what products can and cannot do before you buy can save you a lot of money and minimize disappointment. 2. Pick your #1 concern. While it can be tempting to go shopping and bring home many products to sample, this approach generally will not help you find an effective product. To start, dermatologists recommend that you pick your #1 concern such as adult acne, age spots, fine lines, or a blotchy complexion. Then look for a product that addresses this concern. 3. Use sunscreen and moisturizer. Protect by day and repair by night is what dermatologists recommend. Two products that science has shown can protect the skin are: Sunscreen and moisturizer. 4. A claim that sounds too good to be true gener ally is. The saying, “If it sounds too good, it probably is,” applies to skin care products that promise youngerlooking skin. It is not possible to get the results of a facelift from a jar. 5. Expensive does not mean better. Consumers often equate more expensive with more effective, but this is not always the case with age-fighting products. There are some very effective, affordable over-the-counter products. 6. No one product works for everyone. While a friend may swear by a product, it does not mean you will be as pleased. People have different skin types and different skin concerns. Some people’s skin is more sensitive. As such, one product cannot effectively treat everyone’s mature skin. 7. Product should contain a proven active ingredient. Only a handful of active ingredients have the scientific studies to prove that they can effectively treat signs of aging. This does not mean that other ingredients do not work. It just means that the studies have not been done to back up the claims. Active ingredients that have some evidence of effectively treating signs of aging include retinoids (may appear as retinol, retinyl or retinoic acid on the label), alpha hydroxy acids, 16 | WISCONSIN WOMAN • PHYSICIANS & WELLNESS GUIDE JULY 2010

azelaic acid, growth factors and salicylic acid. 8. “Clinically proven” does not necessarily mean the U.S. Food and Drug Administration (FDA) found the product effective. 9. “Preservative free” is a misleading term. All products must contain preservatives or they could not sit on store shelves without spoiling. 10. “All natur al” is another misleading term. People often interpret “all natural” to mean that a product contains pure plant extracts. Most plant extracts cannot be added to a skin care product in their natural form. If these guidelines still leave you yearning for specific product recommendations, talk with your dermatologist. No one product is right for everyone.

Shoulder and Upper Extremity Injuries are Common BY CYNDI STRAYER According to Non-Surgical Orthopaedics, P.C., shoulder and upper extremity injuries are extremely common. Many of these are the result of years of cumulative injuries brought on by trauma such as a motor vehicle accident, falls or sports injuries. These cumulative and trauma injuries can result in rotator cuff tears, fractures, bursitis (inflammation of the fluid sac, or bursa, that lies between a tendon and skin, or between a tendon and bone), tendonitis, nerve injuries and even degenerative joint disease. Some symptoms to look for include: · Loss of range of motion. Many people lose some range of motion as they age; however, these injuries can begin to affect day-to-day activities in very painful ways. · Pain in the shoulder or upper extremity. One clue may be the number of pain pills you take each day. · Weakness. This can be sudden or gradual. · Ecchymosis. This discoloration of the skin caused by the escape of blood into the tissues from ruptured blood vessels is largely found in trauma injuries. · Muscle aches, swelling and numbness or tingling are also key symptoms. In order to diagnose and understand your injuries, a doctor would conduct a thorough examination and recommend treatment, which may include medications, physical therapy and possible injections. Only a small percentage of patients may actually require surgery.

Help Prevent Diabetes With According to a recent study, nine out of 10 new type 2 diabetes cases in older adults could be prevented through healthy lifestyle changes. The study confirmed that a combination of five lifestyle factors -physical activity, diet, smoking habits, alcohol use and body fat -- accounted for 90 percent of all new diabetes cases in men and women 65 and older. Researchers said that even modest lifestyle changes later in life can make a big difference in reducing the risk of developing type 2 diabetes. For example, a single change such as to quit smoking or to become more physically active could have a significant impact on your chances of developing type 2 diabetes. Overall, the study showed people in the low-risk category for each lifestyle factor had a 35 percent lower risk of developing the disease. Researchers at the Harvard School of Public Health, said the study’s findings highlight that type 2 diabetes really is a lifestyle disease and is largely preventable. While previous studies have linked these

lifestyle factors individually to diabetes prevention in certain people, this study quantifies the effect of several lifestyle factors on diabetes prevention in a large group of older men and women. The study, published in the Archives of Internal Medicine, followed 4,883 men and women 65 and older for 10 years. The participants were divided into low and high risk groups within each of the five lifestyle factors, which were physical activity, diet, alcohol use, smoking habits and body fat. During the follow-up period, 337 new cases of type 2 diabetes were diagnosed. What the researchers discovered was that each of the lifestyle factors examined was independently associated with diabetes prevention. For example: Physically active people had a 46 percent lower risk of developing type 2 diabetes. Being physically active was defined as simply walking regularly and engaging in leisure.

Concerning the study, Dr. Haley cautioned that the group of high-strained African American males was small (only 15). He also pointed out that the study only looked at stroke risk, not stroke incidence. A future analysis will investigate caregiver stress as a factor in the number of strokes, heart disease diagnoses and mortality rates linked to these outcomes, he said. As for the preliminary findings, Dr. Gary Abrams, MD, chief of rehabilitation, San Francisco VA Medical Center, California, said that as the number of caregivers increases with an aging population, trying to reduce their strain will become increasingly important. He also said that judging from the results of this study it looks as if African American male caregivers may need special attention.

BY CHARLYN FARGO Here’s another reason to keep blood sugar in check and maintain a healthy weight. A new study suggests that having high blood sugar levels may increase the risk of cancer, independently of body weight. Scientists at Umea University in Sweden looked at the levels of sugar in the blood of more than 500,000 people, and then later they revisited the same people to determine if there was a development of cancer. The results found that people with high blood sugar levels may be at increased risk of both developing cancer and also of dying from the disease. This association was stronger for women than for men. The strongest blood sugar-cancer links observed were for pancreatic cancer, particularly in women, and liver cancer in men. The American Institute for Cancer Research (AICR), which reported on the study, already recommends limiting consumption of energy-dense foods (such as cookies and cakes) and avoiding sugary drinks to maintain a healthy weight. Scientists now say that, after not smoking, maintaining a healthy weight is the most important thing people can do for cancer prevention. High blood sugar levels—and weight—can be reduced by eating plenty of fruits and vegetables and whole grains. Being overweight increases the risk of six types of cancer, according to the AICR.

The study, part of the ME-Can project looking at risk factor for metabolic syndrome, looked at 274,126 men and 275,818 women from Norway, Austria and Sweden who had an average age of 44.8. When they were followed up after 10.4 years, 18,621 men and 11,664 women had been diagnosed with cancer, and 6,973 men and 3,088 women had died of the disease. For men, the study found there were significant increases in risk of developing and dying from cancers of the liver, gallbladder and respiratory tract. There was also increased risk of developing thyroid cancer and multiple melanomas and of dying from rectal cancer. For women, there was increased risk of both developing and dying from pancreatic cancer. There was also increased risk of developing urinary bladder cancer and of dying from cancers of the uterine corpus, cervix uteri and stomach. Americans Fall Short on Heart-Healthy

Tooth Erosion BY CYNDI STRAYER Dental experts all agree that good dental care is not only important to your dental health, but also your physical health. This is why they are so concerned with the number of cases of tooth decay and tooth erosion. While the causes of tooth decay are well-documented, dentists are concerned about the prevalence of dental erosion in recent years. The most common causes of tooth erosion, sometimes called tooth-surface loss are dietary acids and physical abrasion from grinding teeth or vigorous brushing. While tooth erosion can do severe damage to your teeth, it is preventable. Here are some preventive measures that can be taken to control tooth enamel loss and prevent dental erosion: 1. Decrease the consumption of acidic foods and drinks. One way to do this is to replace carbonated drinks with water, milk or un-sweetened coffee and tea. 2. Drink carbonated beverages or fruit juices with a straw to limit the contact of acids with the front teeth. 3. After eating or drinking acidic foods or beverages, rinse your mouth with fresh water so that the acid is diluted and easier neutralized. 4. You should never brush your teeth immediately after eating, drinking something acidic or vomiting because acids temporarily soften the tooth surface. If you brush immediately the softened enamel will be further damaged by the brushing, speeding up the process of erosion. Wait at least an hour to allow calcium in saliva to repair tooth enamel after the acid exposure. 5. You can chew sugar-free gum to help reduce dry mouth and increase the saliva flow. This actually neutralizes acids and helps teeth remineralize. 6. Be sure to brush your teeth twice a day with a soft toothbrush using a fluoride toothpaste. The fluoride in toothpaste helps the remineralization of the tooth enamel and prevents tooth erosion. 7. Also, be sure to see your dentist regularly and follow his or her instructions in order to prevent further tooth enamel loss.

How to recognize and treat TMJ

McDaniel, DMD, MS agree the first signs of temporomandibular joint disorder (TMD) or orofacial pain are often clicking or popping noises in the jaw joint, limited or abnormal jaw opening, pain when chewing or talking, jaw pain or discomfort, pain in the facial muscles, earaches and headaches. The most common group of individuals who manifest symptoms of TMD are women over men, usually in the 20-50 year old age group. However, TMD is also common in children, as well as senior citizens, and crosses all socioeconomic boundaries. Because TMD symptoms can be so varied and mimic so many other conditions, the most important step in evaluating a possible TMD patient is to make a proper diagnosis. Once diagnosed, treatment for TMD is relatively simple. Again, however, because of the vast and varied number of possible symptoms, a dentist specially trained in the diagnosis and treatment of TMD is necessary to prescribe the appropriate type of treatment for that particular patient's needs. Most patients require an oral orthopedic appliance (or splint), designed specifically for each patient's individually diagnosed symptoms. This splint is made of clear acrylic and

usually fits over all the upper teeth. Is it ok to use a store bought appliance? Store bought nightguards or pre-made appliances supplied by dentists are not usually appropriate for TMD treatment. As stated previously, oral orthopedic appliances need to be designed and custom made for each individual patient's diagnosis. For the muscular component of the TMD, physical therapy (i.e. ultrasound, massage, exercise therapy) is usually required. Patient education on how to protect the temporomandibular joint and how to prevent future occurrences should also always be provided. Patients usually wear the splint for six to 12 months full-time (during which time the splint is adjusted approximately once every three weeks), followed by being weaned from the splint for two to three months. Once the symptoms are alleviated and the jaw and muscles are stabilized (healed to their best potential), the patient is usually provided with a nightguard. In only a very small percentage of instances, and only after conservative treatment (splint and physical therapy) has been performed, will crowns, orthodontics or surgery be required to treat the TMD.

Approximately two-thirds of all adults nationwide are overweight, and almost one-third of them are obese. In the past, there have been several studies on the role of fast food and soft drinks in obesity. Now, a new study suggests that the “built environment” may also play a role--that is, your access to stores that sell healthy foods and to resources and green spaces that support physical activity in the area you live in. Researchers at a number of east coast universities set out to explore how the food environment relates to weight in New York City. The researchers tapped the New York Cancer Project for data on height, weight, age, race/ethnicity, gender, income and education for over 13,000 residents of New York City. The researchers defined a person’s neighborhood as a half-mile around the person’s home. To help form a picture of these areas, the researchers compiled data from several sources, incorporating population information, bus and subway stops, land use and neighborhood food establishments. The researchers then analyzed any possible associations between a person’s food environment and his BMI (the ratio of weight to height used to determine body fat). They only found one significant association between food environment and BMI. It came as a bit of a surprise that there was no association with unhealthy food outlets such as fast-food restaurants, convenience stores, pizzerias, bakeries and candy stores; however, the researchers noted that the lack of association reflected the fact that virtually all New York City neighborhoods provide many opportunities for people to eat poorly. Almost 99 percent of the participants in the study lived within a half-mile of an unhealthy food outlet. By contrast, only 82 percent lived within a half-mile of a healthy food outlet. Basically, what the study revealed was there were more unhealthy food choices available to people over the healthy choices (an average of over seven unhealthy food outlets for every one healthy outlet). Researchers know this doesn’t prove healthy food choices are responsible for lowering a person’s BMI. People who prefer healthy foods may tend to move to areas that offer more healthy food outlets. In turn, healthy food retailers tend to open stores where they think their products will be best received. These results will have to be confirmed by future studies, but the study does show increasing people’s access to healthy food outlets could help combat the obesity epidemic more effectively than limiting unhealthy food. Many communities are now promoting access to supermarkets, farmers’ markets and fruit and vegetable stands.

Hope for Breast Cancer In the near future, there may be a vaccine to prevent breast cancer. This vaccine was developed by researchers at the Cleveland Clinic in Ohio. Currently, in the United States, 40,000 women are killed by breast cancer each year. Also, one in eight women will be diagnosed with the disease sometimes during her lifetime. The scientist involved in this 10 month trial injected six mice that were genetically engineered to be prone to breast cancer with the new vaccine. They also injected six other mice prone to breast cancer with a placebo. They found that none of the six mice that were injected with the vaccine developed any tumors, while all of the placebo-injected mice did. Although many more trials need to be completed, the results of this particular trial have made the scientists optimistic that some day this vaccine will provide protection against breast cancer, much like the vaccine for the human papillomavirus (or HPV).

STRESS Produces Emotional Responses People who are emotionally healthy are in control of their thoughts, feelings and behaviors. They feel good about themselves and have good relationships. They can keep problems in perspective. It's important to remember that even people who have good emotional health can sometimes have emotional problems or mental illness. According to the Department of Health and Human Services, mental illness often has a physical cause, such as a chemical imbalance in the brain. Stress and problems with family, work or school can sometimes trigger mental illness or make it worse. However, people who are emotionally healthy have learned ways to cope with stress and problems. In our day-to-day lives, some degree of stress is unavoidable. The body responds to stress by making stress hormones. These hormones help your body respond to situations of extreme need. But when your body makes too many of these hormones for a long period of time, the hormones wear down your body -- and your emotions. People who are under stress a lot are often emotional, anxious, irritable and even depressed. If possible, try to change the situation that is causing your stress. Relaxation methods, such as deep breathing, meditation and exercise are also useful ways to cope with stress. The Department of Health and Human Services lists the following tips to help deal with emotions: • Learn to express your feelings in appropriate ways. It’s important to let people close to you know when something is bothering you. Keeping feelings of anger or sadness takes extra energy. Let them out. • Think before you act. Emotions can be powerful. But before you get carried away and say or do something you may regret, give yourself time to think. • Strive for balance in your life. Make time for things you enjoy. Focus on positive things in life. • Take care of your physical health. Your physical health can affect your emotional health. Take care of your body by exercising regularly, eating healthy meals and getting enough sleep. • Don’t abuse drugs or alcohol.

Exercise, diet help ovarian cancer survival A new study finds that a healthy lifestyle of regular exercise and eating right may increase the survival rates of those diagnosed with ovarian cancer. The 2009 study estimates projected that 21,550 new cases of ovarian cancer would be diagnosed in the United States alone, and 14,600 would be given a relatively low five-year survival rate of about 45 percent. Published in the March 2010 issue of the Journal of the American Dietetic Association, this study evaluated the possible diet associations with ovarian cancer survival. Researchers from the University of Illinois at Chicago determined that there is a strong relationship between healthy eating and prolonged survival. The subjects included 351 women diagnosed with incident epithelial ovarian cancer; they had participated in a previous casecontrol study. The original study collected demographic, clinic pathologic and lifestyle-related variables, including diet. Each subject completed a food frequency questionnaire, where they were asked to report their usual dietary intake over the three to five years prior to their diagnosis. To translate the diet estimates in a meaningful way, the Food Frequency Questionnaire items were assigned to the major food groups reflected in the Dietary Guidelines for American 2005 (DGA), which include fruits, vegetables, grains, meats, dairy, fats and oils, sweets and alcohol. Grains, meats and dairy were further subdivided to “suggested” subdivisions as healthier food choices, whereas the “other” subdivisions contained less desirable selections. The authors of the study found that higher total fruit and vegetable consumption and higher vegetable consumption alone led to a survival advantage. Likewise, a statistically significant improvement in survival was observed for the healthier grains. Higher intakes of less-healthy meats were associated with a survival time disadvantage. “The study findings suggest that food patterns three to five years prior to a diagnosis of epithelial ovarian cancer have the potential to influence survival time,” wrote Therese Dolecek-research associate professor of epidemiology, Division of Epidemiology and Biostatistics and Institute for Health Research and Policy, School of Public Health and Member, Cancer Control and Population Science Research Program, UIC Cancer Center, University of Illinois at Chicago. “The pre-diagnosis food patterns observed to afford a survival advantage after an epithelial ovarian cancer diagnosis reflect characteristics commonly found in plant-based or low-fat diets. These diets generally contain high levels of constituents that would be

expected to protect against cancer and minimize ingestion of known carcinogens found in foods.” The study provides new evidence that dietary factors, particularly total fruit and vegetable, red and processed meat and milk intakes, may influence ovarian cancer survival. They may become hostile and agitated for seemingly no reason, and may begin coming up with an endless series of excuses. While there is no easy way to tell if your teen is using drugs, knowing some of the warning signs can help you to make proactive parenting decisions.
