How health care reform affects your Medicare enrollment Health care legislation passed earlier this year may make Medicare annual enrollment season particularly challenging for the millions of baby boomers aging into Medicare and for seniors already enrolled who are considering different coverage for 2011. Annual enrollment runs from Nov. 15 to Dec. 31, allowing people to select their Medicare coverage for 2011.

"Choosing a Medicare plan can be overwhelming in any year," says Adrienne Muralidharan, senior Medicare specialist for the Allsup Medicare Advisor, a Medicare plan selection service for people with disabilities and those 65 and older. "However, this year there are not only the usual changes, such as premium and coverage changes, you also need to understand how the Patient Protection and Affordable Care Act may affect your 2011 Medicare coverage." According to Muralidharan, the most important ways the new health care legislation will affect Medicare beneficiaries for 2011 include: Prescription drug costs should be lower in the "doughnut hole." For 2011, Medicare beneficiaries will receive a 50 percent discount for the cost of brand-name prescription drugs and a 7 percent discount for generic drugs they are taking while in the prescription drug doughnut hole. Medicare Part B (medical insurance) will fully cover preventive care. Beginning in 2011, Medicare beneficiaries will no longer have to pay deductibles or co-pays for preventive services that fall under U.S. Preventive Service Task Force guidelines, such as an annual wellness exam. More beneficiaries may have higher costs for Medicare Part B and Part D (prescription drug) coverage. The Part B income threshold freezes at the 2010 levels through 2019. As a result, individuals with modified adjusted gross income (income) exceeding $85,000 and married couples with income exceeding $170,000 will have to pay higher premiums. Because this will not adjust with inflation, it's likely more people over time will be subject to higher premiums. In addition, the law reduces the Medicare Part D premium subsidy for indi-

viduals with incomes above these levels. Opportunities to change coverage after the annual enrollment period will be more limited. Historically, an open enrollment period ran from Jan. 1 through March 31, immediately after annual enrollment ended. During open enrollment, people were able to switch from their existing Medicare plans to similar Medicare coverage, choosing from "like to like" options. Now, open enrollment has been replaced with a shorter annual disenrollment period, which runs from Jan. 1 through Feb. 14. During this period, the only change that can be made is to disenroll from a Medicare Advantage plan in order to enroll in traditional Medicare and join a Medicare Part D plan. Other selections will not be available to consumers. "It's easy to get confused over the various Medicare enrollment periods," Muralidharan says. "However, it's now more important than ever to look at annual enrollment as your main opportunity to choose your coverage."

Reasons to review your medicare coverage Changes brought on by health care reform are just one reason people should evaluate their Medicare coverage. Each year, individuals with Medicare should consider different health care coverage if they experience any of the following:

• Your health situation has changed in the past year. • Your provider situation has changed (for example, you hospital or physician left your plan). • Your coverage has changed (for example, certain drugs, procedures or conditions are no longer covered). • Your plan premiums and/or co-payments have increased. • You have moved to a new location. • Your current plan no longer will be available.

Additionally, people now turning 65 and becoming Medicare-eligible for the first time should carefully review their options and make certain they follow the enrollment guidelines. Failing to do so can trigger costly penalties and may mean certain coverage is unavailable in the future. "The choices you make about your Medicare coverage can have a significant effect on your health care and your finances," says Muralidharan. "If you are uncertain about which Medicare plans are available to you, or which would best meet your needs, seek help before enrolling." Communication is important for patients with lymphoma Seventy year-old Jane Fretz is a loving wife, a dedicated mother of five, and the rock of her family. She is always the first one to offer support when someone is going through a tough time, and the first one to receive good news from relatives. So when she was diagnosed with indolent non-Hodgkin’s lymphoma five and a half years ago, roles reversed and her family became her rock. Non-Hodgkin lymphoma (also known as NHL) is a cancer that starts in cells called lymphocytes, which are part of the body’s immune system. The American Cancer Society (ACS) estimates that in 2010 alone, more than 65,500 Americans will be diagnosed with NHL, and four out of 10 lymphoma cases will be of the indolent, or slow growing, form. Throughout her cancer experience, Fretz’s family was not the only support network that helped her get through this time in her life. She tapped into her local Cancer Support Community (CSC) affiliate for support groups, educational workshops and mind/body classes. The CSC, uniting The Wellness Community and Gilda’s Club Worldwide, provides high-quality psychological and social support through a network of nearly 50 local affiliates, more than 100 satellite locations and online. Introduced to the CSC by her husband’s friend, Fretz felt immediate ease when she went to her first meeting. There were people there going through the same experience as her, and they were bonding over their condition. Fretz soon learned firsthand that one of the CSC’s goals was to provide people dealing with cancer resources and programs designed to help patients and their families navigate through this moment in time. One of CSC’s newest educational initiatives, Framing Life With Lymphoma, was

recently launched with support from Cephalon Inc. Created through input from an expert Steering Committee, a patient focus group, and a national survey fielded by Harris Interactive of 150 hematologists/oncologists and 133 indolent lymphoma patients, this program is research-based and provides lymphoma patients with easy-to-use tip sheets outlining ways to approach each conversation with their healthcare team across the course of their cancer journey — from diagnosis through treatment. The patient tip sheets, survey results and other program information can be found at www.FramingLifeWithLymphoma.org. “We are pleased to provide patients with the Framing Life With Lymphoma program, and hope it will become an important resource for the lymphoma community,” says Kim Thiboldeaux, president and CEO of the Cancer Support Community and advisor for Framing Life With Lymphoma. “A clear majority of patients and physicians reported that discussion materials would improve their conversations. That is exactly what this program aims to do — provide patients with valuable resources — such as our patient tip sheets — to ensure that areas of concerns are covered during appointments.” Throughout her years of battling, and ultimately achieving remission from, indolent NHL, the CSC became a staple in Fretz’s life. She became a dedicated board member, and through that position was able to participate in a patient focus group that helped the CSC develop the program’s tip sheets. “I am proud to have been involved in the focus group to help develop the tip sheets, and I am really excited to be able to share this program with everyone,” Fretz says. “Some of the tips that I really stress to my family and friends that are going through a similar diagnosis are 1. establish your network; 2. be informed on your disease; and 3. you’re not alone — there are support systems out there for yourself, and your family.” Some of the other tips featured on the Framing Life With Lymphoma tip sheets include: • Bring questions and write everything down • Engage your support network • Speak up • Have a 24/7 plan • Know your cancer - it’s unique • Understand your team; know your “teamcaptain” To learn more about Framing Life With Lymphoma, and to contribute feedback that will help inform future materials, please visit www.FramingLifeWithLymphoma.org.

Shortness of breath could be a sign of

COPD Do you sometimes wheeze or stop to catch your breath when walking up stairs? Perhaps you get tired while grocery shopping or when walking around the mall. Maybe you have a cough or a cold that just won't go away. These seemingly simple interruptions to your daily life could be a sign of a serious disease called chronic obstructive pulmonary disease, or COPD. COPD - which includes emphysema and chronic bronchitis - is a serious lung disease that over time makes it hard to breathe. In COPD, people's airways - the tubes that carry air in and out of the lungs-- are partially blocked. People often ignore their symptoms, passing them off as normal signs of aging. In fact, COPD is the fourth leading cause of death in the United States. Twelve million Americans are currently diagnosed with COPD, while an additional 12 million Americans may have the disease and remain undiagnosed. COPD affects one in five people older than 45 and occurs most frequently in people age 40 and older with a history of smoking. Current and former smokers are at risk. Smoking is the most common cause of COPD, accounting for as many as nine out of 10 COPD-related deaths - but smoking is not the only cause. As many as one out of six people with COPD have never smoked. Heavy or long-term exposure to secondhand smoke or other air pollutants also may contribute. Environmental exposure also can be a factor. People who have had long-term exposure

to substances that can irritate the lungs, such as certain chemicals, dust, or fumes in the workplace, can develop COPD. In some people, COPD is caused by a genetic condition known as alpha-1 antitrypsin, or AAT, deficiency. "Recognizing the signs and symptoms of COPD and your own risk factors is an important first step. But it must not stop there. COPD can be treated,'' says James P. Kiley, Ph.D., director of the Division of Lung Diseases at the National Heart, Lung and Blood Institute (NHLBI), part of the National Institutes of Health. "It's critical that people talk to a doctor if they are experiencing symptoms. The earlier a person receives proper treatment, the better their chances to improve quality of life." COPD is diagnosed with a simple breathing test called spirometry. It's quick and painless - and worth it. With proper diagnosis and treatment, COPD patients can breathe better.
