PURPOSE
This document describes the requirements for agency membership and participation in United Way of Central Ohio’s (UWCO) program investment process. UWCO will consider successful program investment applicants to be member agencies. The benefits and responsibilities of membership are identified in this document, along with specific program investment requirements. This document identifies the overall requirements of membership. Specific program investment agreements will also be developed for each successful program investment application.

OVERVIEW
The intent of the program investment process is to support high quality human service programs that address specific UWCO-defined strategies and performance measures and ultimately contribute to the achievement of UWCO’s identified community results. The program investment process is guided by several key assumptions: • • • • • • • • • • •
UWCO invests in programs, rather than providing general operating support for agencies; Community results, supporting strategies and related performance measures serve as the framework for all program investment; Program investments are made on a multi-year (three-year) investment cycle; The process is open to a broad group of local human service agencies; Collaboration among human service agencies and systems, both public and private, is strongly encouraged; Investment decisions are based on consistent investment criteria; UWCO does not guarantee future investment or any specific amount of investment for currently funded programs; The process incorporates UWCO’s commitment to diversity and inclusion and its emphasis on reducing racial disparities; Emphasis is placed on leveraging UWCO investments to achieve greater impact; UWCO focuses on accountability to donors and the broader community for funds invested in local human service programs; and The process includes meaningful roles for UWCO volunteers, staff and service providers.

GENERAL ELIGIBILITY REQUIREMENTS 3.10
Agency Membership/Program Investment Applicants
To apply for UWCO membership and program investment, an applicant organization must provide proof of its tax exempt status, demonstrate at least three years of operating history, and meet the following general eligibility criteria: 1. The applicant organization supports UWCO’s overall mission, values and strategic direction and will actively assist in the accomplishment of that work; (Attachment A) 2. The applicant organization offers programs directly responsive to UWCO’s identified community results, supporting strategies and related performance measures (Attachment B); 3. The applicant organization addresses other UWCO eligibility requirements referenced later in this document; and 4. The applicant has a current “PowerPhilanthropy Portrait” registered with The Columbus Foundation. 3.20
Collaborative Partners
UWCO values and supports collaborative efforts that help us achieve the common good and contribute to the accomplishment of our identified community results. Non 501(c)3 non-profit organizations may receive UWCO program support but only in partnership with a qualified 501(c)3 organization as the lead applicant. 3.30
PowerPhilanthropy Portraits
PowerPhilanthropy is an online resource which provides consistent information about local nonprofit organizations in Central Ohio to donors in order to inform their charitable giving. United Way of Central Ohio, in partnership with the Columbus Foundation, requires all applicants for UWCO program investment to complete or update a PowerPhilanthropy Portrait with the foundation as a first step in applying for UWCO funding. Applications without a complete current portrait will NOT be considered. As part of this process UWCO and The Columbus Foundation agree to share certain information. The Columbus Foundation will share information contained in the portraits with UWCO staff and volunteers, and UWCO will share program outcome information for UWCO funded programs with The Columbus Foundation, although not for inclusion in an organization’s portrait. USA Patriot Act Anti-Terrorism Certification Organizations funded by UWCO must certify that they are in compliance with the requirements of the USA Patriot Act. (Attachment C)
4.12
UWCO Diversity & Inclusion Policy Compliance Programs funded by UWCO must comply with UWCO’s Diversity and Inclusion Policy and must provide written verification of compliance with UWCO’s Statement of Non-Discrimination. (Attachment D)
4.13
Proof of Liability Insurance
Member agencies must provide annual proof of appropriate liability insurance coverage. It is the responsibility of the agency board to ensure that adequate insurance coverage is in place to protect the organization. Types and levels of coverage will vary with the size and needs of the organization. Policies will differ by plan and provider, so the organization needs to secure the most appropriate insurance coverage for its needs. Coverage may include, but is not limited to, general liability insurance, directors’ and officers’ insurance (D & O), and fidelity coverage. 4.14
Certification of Compliance with Legal and Regulatory Requirements Agencies receiving UWCO program support must comply with all applicable local, state and federal legal and regulatory requirements. (Attachment E)

Financial Reporting Requirements Several specific financial reports are required annually from each member agency.

4.21
Proposed Budget Reflecting Annual Revenue & Expenses
Member agencies must submit a proposed agency budget detailing anticipated expenses and revenues by program on an annual basis for each year in which UWCO support is received. Beginning in 2010, UWCO will operate on a calendar year funding period. The budget must show the revenue and expenses for all agency programs (not just United Way funded programs). It must also provide additional information on government grants and on staff salary and position costs spread by program. 4.22
Report of Actual Revenue & Expenses
At the conclusion of each funding year agencies must also submit a report of actual expenses and revenues by program for the program year just ended. A complete agency report, showing revenue and expenses for all agency programs (not just United Way funded programs), as well as detail information on government grants and on staff salary and position cost spread by program should be included. 4.23
IRS Form 990
On an annual basis, each member agency must forward to UWCO a copy of its completed IRS Form 990 along with all attachments. This form is a requirement for participation in the various public sector campaigns that UWCO manages. 4.24
Annual Audit and Management Letter
On an annual basis, each member agency is required to submit a copy of its audited financial statements and accompanying management letter. NOTE: If a management letter is not provided during the course of the agency’s audit by the auditing firm, written verification must be provided by the auditor, stating that a management letter was not issued. 4.30
Organizational Accountability Requirements 4.31
UWCO Agency and Management Standards of Accountability
Member agencies must complete certification of compliance with UWCO Agency and Management Standards of Accountability within three years of the initiation of UWCO program investment. Portions of these standards may be substituted with a UWCO-approved alternative (e.g., Ohio Association of Nonprofit Organizations’ Standards of Excellence, Commission on Accreditation of Rehabilitation Facilities accreditation requirements, etc.) (Attachment F)

UWCO Cultural Competence Standards United Way of Central Ohio has developed cultural competence standards to be used by UWCO and its member agencies to enhance their ability to more effectively serve an increasingly diverse Central Ohio community. Member agencies must agree to work toward congruence with UWCO Cultural Competence Standards. (Attachment G)

UWCO Fundraising Requirements 4.41
Participation in UWCO Annual Campaign
Member agencies must be willing to conduct an annual UWCO fundraising campaign among agency staff. All campaign materials and any needed assistance is available through the UWCO Resource Development Department. Employee payroll deductions for UWCO pledges should be submitted to UWCO at least quarterly. 4.42
Agency Fundraising Restrictions
Member agencies may not conduct a work-place based solicitation of donors using payroll deduction. (Note: This is the only fundraising prohibition required by UWCO) 4.43
Participation in Ambassador Program
Member agencies must be willing to participate in UWCO’s Ambassador Program at levels commensurate with overall UWCO investment support. The Ambassador Program provides agency speakers and tours to donor companies and organizations during the annual UWCO campaign. 4.44
Participation in Public Sector Combined Campaigns
All member agencies must submit a completed application for all public sector federated campaigns in which UWCO participates, where such applications are required. Materials will be provided via email, and any specific requirements and deadlines will be communicated as needed. 4.45
Compliance with UWCO Donor Choice Policies 4.45(a) Non-Solicitation Policy Member agencies may not under any circumstances solicit donor designations as part of UWCO’s annual fundraising campaign. An agency found to be soliciting donor designations jeopardizes its participation in this donor choice program as well as any future investment from UWCO.

4.45(b) First Dollars In Any donor designations received through the UWCO campaign by the applicant organization will be treated as “first dollars in” toward the organization’s UWCO program support. 4.45(c) Designations in Excess of Allocation Any agency receiving donor designations in excess of its approved annual allocation will generally receive payment of the excess amount included in the last monthly payment of the allocation cycle, unless special payment arrangements are requested and agreed to by UWCO. Total designations are charged a 10% administrative processing fee and the 7.8% uncollectible allowance before computing any excess amounts due. These are the same fees as charged to non-member agencies for designation payouts. 4.50
UWCO Marketing Support Requirements
The United Way of Central Ohio logo and identification should be present or UWCO should be mentioned in as many of the following ways as possible: • Operational sites visited by customers/clients where funded programs take place; • Collateral/printed materials for the funded program (e.g., newsletters, brochures, pamphlets, etc.); • Identification on the agency’s website and link to www.liveunitedcentralohio.org; • Listings of organization and program funders including the annual report; • Press releases about the funded program; • Special events (e.g., annual meetings, fundraising events, etc.)

Program Outcome Reports
Agencies receiving UWCO program support must submit an annual report of program outcome achievements using prescribed UWCO performance measures and measurement methodology. This report will include individual level data, including demographics and outcome achievement information, for all clients served. A limited number of programs may link to UWCO strategies not focused on individual level change. These programs will be required to submit annual reports outlining the services provided in accordance with the UWCO funding agreements governing the program in question.

Units of Service
As part of the required program outcome reports, agencies must also provide an annual count of key program outputs as defined in the agency’s program plan submittal. 5.13
“Success Story” Example
Member agencies must annually provide at least one “success story” for each program receiving UWCO funding support. A success story should highlight an example of a successful client/customer program outcome which can be used for UWCO marketing purposes. 5.20
Program Change Notification Policy
Agencies are required to notify UWCO in writing of significant changes that are expected to occur in a UWCO-funded program and/or any expected interruption in service delivery. Significant changes might include changes in service locations/sites, target population, scope and type of program activities, performance measure, evaluation plan, and program capacity. The interruption of service delivery could include discontinuing services for some or all clients for a period of time. The written notification should include the program name and UWCO allocation amount. The notification should include detailed information about the changes that will be taking place within the program and/or the time period for which program services are being suspended (for example, by loss of a staff person). The notification should also include an explanation of the reason for the service delivery changes and how the program has or will respond to the circumstances. An agency that does not notify UWCO of significant program changes or interruption of services in a timely manner may jeopardize all investment for that specific program. This may include terminating investment completely as well as requiring the agency to return investment already received for the program in question. All program change notifications should be forwarded to the Director of Capacity Building in Community Impact at patrick.powell@uwcentralohio.org.

UWCO MEMBER AGENCY BENEFITS
UWCO member agencies will have access to certain benefits not otherwise available to other local human service organizations. Organizations will be considered provisional members for the first three years in which they receive UWCO program investment. Organizations will be considered full member agencies at the beginning of a second, three year program investment cycle. Additional explanation of each of the member agency benefits may be found on Attachment I.

UWCO Community Results, Strategies and Performance Measures Community Results
UWCO’s community results are important, specific, long-term community-level outcomes necessary to support the achievement of UWCO’s four impact areas and vision. Community results defined UWCO priorities for all of its work related to community impact. What is a strategy? A strategy represents a specific approach that will directly contribute to the achievement of the community result. The strategy represents the “how to” -- the most effective or preferred means of achieving a community result. Strategies are grounded in research and evidence-based practice and the accumulated knowledge and experience of our volunteers, agencies and community partners. How they will be used Strategies will guide the development of the following: • Initiatives • Community partnerships and other collaborations • Program investment • Public policy agendas and other advocacy efforts • Marketing and community awareness efforts What is a Performance Measure? Performance measures are the results that we will track to quantify impact for each strategy, using instruments from a short list of identified measurement tools. What is a Measurement Tool? Measurement tools are self-administered questionnaires, or other procedures designed to accurately and efficiently identify status relative to a performance measure. Three types of tools identified by UWCO as appropriate for program-level accountability include: • Tracking measures • Pre / post comparisons – benchmark specifies improvement over time • Comparisons to a standard – benchmark specifies improvement relative to others Regardless of the type of tool used, collection of data for each performance measure will be at the individual level and will include demographic information such as race, gender, age, zip code, household income, etc. What is a Benchmark? A benchmark represents some threshold that must be obtained on a performance measure to assume that an individual has benefitted or is in relative good standing. Programs will be asked to identify a benchmark as part of the investment process.

No agency will be allowed to transfer UW funds from a program not in compliance with this policy to a program that is in compliance simply to circumvent this policy.

Targeted Support We recognize that our member agencies and community partners have unique missions and long term strategies for achieving those missions. We further recognize that some agencies have specific charters and missions to provide services to a particular constituency or target population. This may be particularly true for agencies with strong religious traditions, single-gender organizations and those that serve a particular racial or ethnic group. Programs supported by United Way may appropriately target specific populations for service delivery as long as they do not engage in discrimination. Discrimination is defined as denying service, volunteer opportunity or employment to a particular qualified individual or group when the denial is unrelated to the expressed intent 2 of the service or program in question or the stated mission and core values of the agency itself.

UWCO Agency and Management Standards of Accountability The provision of high quality, human service programs by United Way of Central Ohio (UWCO) funded agencies is the primary means through which the UWCO system achieves meaningful and measurable impact in areas of critical community need. UWCO recognizes that non-profit agencies need to be well-managed and effectively governed in order to appropriately respond to critical community needs and to improve the quality of life in central Ohio. United Way of Central Ohio values its relationship with funded agencies and is committed to supporting agencies in their efforts to achieve and maintain excellence in management and governance practices. To this end, UWCO provides training and technical assistance and access to other resources that will enhance the capacity of affiliated agencies to operate effectively and efficiently. United Way of Central Ohio and its funded agencies share a joint obligation to donors, to persons served by UWCO funded programs, and to the community in general to demonstrate the highest standards of professionalism and accountability. Before the start of each three-year investment cycle, member agencies and community partners are required to evaluate their management and governance practices against a set of specific standards and indicators. The results of these self-evaluations are then reviewed by UWCO volunteers in order to certify that the agency meets UWCO standards of accountability. United Way of Central Ohio’s Agency Management and Governance Standards of Accountability and the process for the agency self-evaluation are implemented during the first three-year investment cycle that an agency receives UWCO program investment. This self-evaluation focuses on eleven standards in the following areas of agency operations: • • • • • • • • • •

The self-evaluation process is designed to assist the agency leadership in determining future goals and needs of the organization. It also assists UWCO in identifying the resources and support that agencies need to fully meet the standards of accountability.

Directions for the Completion of the Self-Evaluation Process The agency should identify a self-evaluation review team that should include representatives from senior staff, other agency staff, and the board. The review team should determine the agency’s compliance with each standard and compliance indicator by selecting the appropriate response. If improvement is needed, the review team should provide information about how the agency will bring itself into compliance, including timelines and specific action steps to be taken. This information will be reviewed in the follow up site visit that volunteers conduct with the selfevaluation process. United Way of Central Ohio Cultural Competence Standards The United Way of Central Ohio has developed cultural competence standards to be used by United Way of Central Ohio and its member agencies and programs to enhance their ability to: ƒ ƒ
Serve an increasingly diverse Central Ohio community; and Work more effectively on cross cultural situations.
Cultural Competence Standards gives United Way of Central Ohio and its member agencies and programs another opportunity to demonstrate their added value. The standards will assist UWCO and its member agencies and programs in planning and decision making, establishing priorities, building capacity, maintaining accountability and allocating resources. During the first three-year investment cycle, an agency receiving UWCO program funds will work with UWCO staff to incorporate the UWCO cultural competence standards into its work. Attachment H Program Performance Reporting Requirements Program evaluation is a key component of UWCO’s accountability to donors and the community. Program evaluation processes and procedures are currently being revised to support the identified performance measures and measurement tools developed for each UWCO community result. Training is provided at no cost to member agencies. Access to the Non-Profit Management Assistance Program (MAP) Organizations may access the management assistance program. This program matches volunteer consultants with agencies seeking one-time assistance about management or operations issues. This can include legal assistance. System Enhancements UWCO will provide all UWCO member agencies with memberships to the Ohio Association of Nonprofit Organizations (OANO).

UWCO believes strongly that agency staff benefit from training and workshop participation. UWCO covers the cost for an annual membership to OANO for this opportunity. Ancillary UWCO Grant Funds Capacity Building Grants Grants available to assist in capacity building efforts, including strategic planning, board development, mergers, strategic restructuring, etc. See full grant application for process and investment areas. Emergency Grants UWCO provides limited funds to support one-time annual grants to member agencies to assist them in responding to unforeseen financial emergencies that threaten agencies’’ ability to deliver client services. Input to UWCO Decision-Making Formal representation on key UWCO committees UWCO believes in partnership and inclusion. Full member agencies may be invited to participate on key UWCO committees, decision making processes and special work groups and ad hoc committees. UWCO Board representation UWCO believes in partnership and inclusion. CEOs of full member agencies may also be invited to serve as members of the UWCO Board of Trustees and its various committees.
