EXECUTIVE SUMMARY In 2013, the Centre of Research Excellence in Suicide Prevention (CRESP) was launched. In its first year, it moved quickly to bring together academics from 25 Australian institutions, five collaborating international organisations, and many industry partners to tackle suicide rates in Australia through high quality research.

In the first year of operation, CRESP employed three post-doctoral fellows and two research assistants. It attracted seven grants to the Black Dog Institute for suicide research plus an additional 20 grants from partners. CRESP activities were reported in the media 80 times. Four public events were held, including a workshop at the The MHS conference, which attracted 70 participants. In 2013/14, the acting Federal Minister for Health and Ageing, the Federal Minister for Mental Health, the NSW Minister for Mental Health and the NSW Mental Health Commissioner supported events hosted by the Centre. More than 300 research papers were published by Chief Investigators and affiliates. Research projects commenced across the four streams of the program, the results of which will emerge in the next 12 months.
CHAIR’S REPORT I am delighted to present the first Annual Report of the Centre of Research Excellence . in Suicide Prevention (CRESP). These are exciting times for suicide prevention. Evidence is accumulating about the effectiveness of various components of a suicide prevention response. A new idea has emerged – that the best suicide prevention response may be gained from a multilevel, multifactorial, systems-based approach. This systems approach must include both the healthcare and the public health system. What is required now is government support across federal, state and local jurisdictions, and the design of a systematic and simultaneous systems response. We need to act urgently, as suicide rates are not diminishing. In 2012, compared to the previous year, three hundred additional people in Australia took their lives. Suicide rates are currently at the highest rate for the last 10 years and rising rapidly in the youngest age groups. We are only a year old, but we are keen to present our research programs, our staff, and to affirm our intention to pursue our agenda to lower the prevalence of suicide in Australia over the next five years through research and advocacy. What is CRESP? Funded by the Australian National Health and Medical Research Council (NHMRC), the Centre of Research Excellence in Suicide Prevention (CRESP) brings together, for the first time, leading Australian and international experts in suicide prevention to share expertise and resources and undertake the necessary research to effect rapid advances in suicide prevention and lower suicide rates in Australia. In addition, advice and input are provided from those at the coalface of suicide prevention including crisis support services,
Our partnerships will not only improve outcomes for individuals, they will greatly improve international understanding of suicide prevention and appropriate public health and healthcare strategy.
OUR OBJECTIVES CRESP aims to lower suicide rates in Australia by focusing on four research streams. These are – • Better delivery of interventions • Better knowledge of causes and risks • Improved help-seeking; and

By undertaking quality research, translation, education and collaboration, we aim to generate new knowledge, inform policy and practice, train further researchers and educate the public, policy makers and clinicians. We link the expertise of researchers, clinicians, suicide prevention organisations, consumers, carers and government in order to bring rapid gains in suicide prevention. CURRENT PROJECTS BETTER DELIVERY OF INTERVENTIONS In 2013 we focused on a range of trials to determine whether suicide risk would be lowered in response to eHealth interventions. Because of their reach, eHealth interventions enable us to target high risk groups through mobile phones, tablets or computers. Six research studies are in progress and an additional project is in the planning stages. The iChill project aims to prevent generalised anxiety disorder in young adults and includes a measure of suicide ideation to determine the effects on suicide risk. The GoodNight trial examines whether treatment for insomnia will prevent the development of depression and includes measures of suicide risk. The Healthy Thinking Pilot Trial, which aimed to lower suicide risk via specific suicide therapies on the internet, such as mindfulness, concluded, and the larger randomised controlled trial (RCT) commenced. A suicide prevention app (iBobbly) was developed and the pilot trial in the Kimberley commenced. Planning also commenced for the larger RCT of this app, which draws on Acceptance Commitment Therapy. In conjunction with UNSW’s National Drug and Alcohol Research Centre, two trials treating or prevention depression or substance abuse were included in CRESP in order to determine suicide outcomes, and thereby leverage effort. Within this stream, CRESP produced a review of suicide prevention interventions, and published a database of all suicide prevention trials using RCTs. This database is available to all researchers by approaching us through our website. CRESP also worked in parallel with the Young and Well Cooperative Research Centre (Young and Well CRC) to assist in the development of early intervention and online professional clinics.
BETTER KNOWLEDGE OF CAUSES AND RISK In 2013 we focused on understanding the causes of suicide through a range of projects. A number of survey and focus group projects were commenced. We partnered with the National Mental Health Commission to survey peoples’ experiences following a suicide attempt, both those making the attempt and those who support them. With beyondblue, we examined men’s experiences of suicide and depression. We undertook reviews of

suicide stigma and literacy using Facebook survey techniques, and began the task of understanding how suicide might be best screened. We also partnered with the National Drug and Alcohol Research Centre and Queensland Medical Research Institute to examine predictors and causes of suicide risk in samples of twins and those with heroin dependence. A number of analyses were undertaken examining Joiner’s Interpersonal Theory, using a range of datasets, and generally provided support for his model.
IMPROVED HELP SEEKING This stream investigates the effectiveness of potentially useful methods to increase help seeking, building on developments in mental health help seeking. Self-screening, suicide literacy and lowering stigma will be investigated as potential methods of increasing the rate at which individuals will seek help. CRESP completed a trial of the Black Dog Institute youth program HeadStrong to determine whether this resource would lead to reduced stigma and thus increase help seeking. Initial results show that the program lowered stigma and increased mental health literacy, but no
significant improvements in help seeking were recorded. Via a beyondblue grant we also began to collect information to determine the strategies that men normally use to cope with anxiety and depression. In addition, we commenced a study in conjunction with the NSW Mental Health Commission and CSIRO to determine whether the context of suicide tweets could be determined. The latter study examines whether it is possible to determine the meaning of suicide tweets from the context in which they are tweeted, i.e. to determine whether they reliably indicate suicide risk.
BETTER PRIORITISATION OF RESOURCES This stream aims to develop guidelines to assist policy makers in determining priorities for funding suicide prevention programs. We aim to model the effectiveness of particular interventions, their costs and impacts. We also plan to identify the most appropriate measures to determine cost effectiveness for suicide interventions, and have already begun to include cost effectiveness measures into our prevention and intervention studies.

Snapshot: iBOBBLY Suicide rates in Aboriginal and Torres Strait Islander communities are amongst the highest in the world. Despite increased funding and implementation of new prevention programs, very few Indigenous people will seek help before acting on suicidal thoughts. Black Dog Institute, in partnership with WA-based suicide prevention group Alive and Kicking Goals, is undertaking a trial of the world’s first suicide prevention app designed especially for use by Indigenous people on mobile phones or tablet devices. Called iBobbly (a name derived from a Kimberley greeting), the app delivers treatment-based therapy in a culturally relevant way. Based on psychological therapies proven to reduce suicidal thoughts, it draws heavily on indigenous metaphors, images and stories drawn from local Aboriginal artists and performers. According to Black Dog Institute researcher Professor Helen Christensen, the app format leaps two of the major hurdles to help seeking – perceived stigma and geographical isolation. “We know that Indigenous Australians are not seeking face to face mental health care with more than 70% of Indigenous suicides occurring in people who are not previously known to health services.”

“Indigenous youth have a high rate of mobile phone usage so it makes sense that we engage them on technology they are comfortable with and able to use in their own private time.” “Once the app is downloaded they don’t need ongoing internet access and the program is password protected, thus remaining confidential if the technology is shared amongst the community.” “The initial pilot trial being run in WA will allow us to test and refine the program. We hope to expand iBobbly access to Indigenous people living in other States later in 2014.” iBobbly is funded by the Australian Government and NHMRC Centre for Clinical Excellence in Suicide Prevention. The program was developed in partnership with WAbased group Alive and Kicking Goals, HITnet Innovations; Thoughtworks; Muru Marri Indigenous Health Unit UNSW and Young and Well Cooperative Research Centre. Samsung generously donated 150 tablets for the trial. Funding has since been obtained for a larger Randomised Controlled Trial through the National Health and Medical Research Council (Project Grant APP1060477).
KEY PERFORMANCE INDICATORS The ultimate success of CRESP will be reflected in lower suicide rates. However, the success of CRESP will also be measured by a number of process key performance indicators, which include:

EVENTS Launch of CRESP The NHMRC Centre of Research Excellence in Suicide Prevention (CRESP) was officially launched by the Hon Tanya Plibersek, Australian Minister for Health and the Hon Mark Butler, Australian Minister for Mental Health and Ageing at a ceremony on January 31st 2013. Minister Plibersek noted that a “whole of nation” effort is needed to prevent suicide in particular.

The Science of Suicide Prevention Workshop at TheMHS 2013 The Science of Suicide Prevention workshop, held at the TheMHS conference in Melbourne on August 20th 2013, brought together both senior and emerging researchers in the suicide prevention field, as well as those from other cognate disciplines such as genetics, psychiatry, biostatistics and epidemiology. New research on genetic and environmental risk factors that influence suicidality and completed suicide, and new interventions for suicide prevention were presented. Over 70 individuals registered for the Workshop from across Australia, as well as some from New Zealand and the US. Attendees included academics, researchers, clinicians, public servants from a range of federal and state departments, and employees from a variety of mental health organisations and health services.

Engagement with government and the community continued World Suicide Prevention Day 2013 Black Dog Institute and CRESP hosted the NSW launch of World Suicide Prevention Day (WSPD) on September 10th 2013, in partnership with Suicide Prevention Australia and on behalf of the National Coalition for Suicide Prevention. The Hon Kevin Humphries, NSW Minister for Mental Health opened the event. Other

speakers included Susan Murray, CEO of Suicide Prevention Australia, the Hon John Brogden, Chairman of Lifeline, and Professor Helen Christensen. The event included the launch of a world first clinical trial of a web-based self-help program, called Healthy Thinking, and the launch of the Lifeline Return on Investment Report. The Lifeline Return on Investment Report highlighted the growing preference among consumers to use the internet when seeking help during times of crisis, and demonstrated the value of suicide prevention services, showing a return on investment of $12 for every $1 spent. The National Coalition for Suicide Prevention’s (NCSP) campaign to “Vote 1 Suicide Prevention” to target suicide rates was also launched. It aims to halve suicide by 2023.
Prof Eric Caine, spoke about ‘Challenges for Suicide Prevention: Evaluating US Public Health Approaches’ on Monday Dec 2nd 2013 at the Black Dog Institute. Prof Caine is a leading international expert on suicide prevention, the John Romano Professor and Chair, of the Department of Psychiatry at the University of Rochester Medical Centre, and Director, Injury Control Research Center for Suicide Prevention. The audience included representatives from Suicide Prevention Australia, SANE Australia, R U OK? Foundation, Lifeline Foundation, NSW Health, Mindframe, On the Line, Carers NSW and beyondblue. Funding for Prof Caine’s visit from the US was obtained via a Brain Sciences UNSW seed funding grant.

WEBSITE The CRESP website www.cresp.edu.au was launched in April 2013. During 2013 there were 11,353 views of the CRESP website, made by 2,888 unique visitors across 4,086 visits. Website use spiked with the promotion of CRESP’s ‘The Science of Suicide Prevention’ Workshop held at the TheMHS 2013 Conference in August. The vast majority of visitors to the CRESP website were from Australia (80.4%), followed by the United States (3.4%), United Kingdom (3.2%). The remaining 7.5% of visitors originated from (in order) New Zealand, China, Hong Kong, Canada, The Netherlands, Germany, India and over 60 other countries. Traffic to the website was predominantly via web searches (55%), 21% came directly to the site (by typing in the address, using bookmarks or using email links), with another 21.8% via referral from partner websites. Since late July, around 150 visitors accessed the website via social media; of these 150 visitors, 73% arrived via Twitter, with 25% via Facebook (NB social media data is since late July only). Returning visitors accounted for 29% of visits to the website. The Home page was the most frequently visited page, receiving 19.5% (2,214) of all page views. Following this, the most popular page was a news item on the website regarding the release of an ABS report (‘ABS Report: Suicide still the highest cause of death among young people’) (5.4%, 611 page views), and the ‘Who we are’ page (4.7%, 537 page views).

MEDIA During 2013 CRESP, CRESP researchers, projects and activities were featured over 80 times across national, state and local media outlets, including print, radio, television and web. The 2013 Senior Researcher Best Research Paper Award was awarded to Professor Jane Pirkis from the University of Melbourne. Her paper, titled “The effectiveness of structural interventions at suicide hotspots: a meta-analysis,” was published in the International Journal of Epidemiology and demonstrated that restricting access to suicide “hotspots” using physical barriers such as fencing reduced suicide rates at those sites. The 2013 Early Career Researcher award was won by Dr Allison Milner, also from the University of Melbourne, for her paper entitled “Suicide by occupation: systematic review and meta-analysis”. This meta-analysis, published in the British Journal of Psychiatry, tested the theory that certain occupations have comparably higher rates of suicide. Whilst Dr Milner’s findings showed that individuals in low-skilled workforces had a higher suicide rate than those in higher skilled positions, the paper highlighted the urgent need to investigate the link between occupation and suicide risk.
