The Treatment Innovation and Dissemination Research Group (TIDR) is a collective of faculty, post-doctoral fellows, graduate students, staff and community partners housed within the University of Michigan School of Social Work. Our research group focuses on community-based intervention research that utilizes technology to deliver evidence-based interventions within various underserved and underrepresented populations. TIDR investigators and teams develop and test mental health interventions within different communities of need. Investigators also focus on functional outcomes, such as employment. Collectively, TIDR works as a group to share resources, provide mentorship and promote intervention research. TIDR Investigators obtain funding to support their work through federally funded grants, private grants and internal grants. During this reporting period, over 30 projects were funded.

Exposure and response prevention versus stress management training for adults and adolescents with obsessive compulsive disorder: A randomized clinical trial

Cognitive Behavioral Therapy featuring exposure and response prevention (ERP), is the most extensively investigated psychosocial treatment for obsessive-compulsive disorder (OCD) in youth and adults, but it is not commonly available to OCD patients Alternatively, SMT is commonly offered but has no scientific support in the treatment of OCD

Despite ERP’s demonstrated efficacy, differential effects at different ages are not well known This randomized trial assigned adults and adolescents to ERP or stress management training (SMT) allowing for a direct comparison of efficacy in different age groups. It also accurately compared the efficacy of ERP versus SMT using the same number of therapy sessions and equivalent therapist attention. Finally, this study provides the first RCT comparing the effects of ERP versus SMT for adolescents with OCD.

Entertain Me Well (EMW)

was developed in 2018 by TIDR Members Addie

Weaver, Anao Zhang and Joseph Himle EMW is an online platform to deliver online Cognitive Behavioral Therapy in an entertaining, cost efficient and less stigmatizing way EMW began with one project in 2019 and has grown to house 10+ projects and trainings.

Professor Weaver collaborated with the Lenawee County WIC clinic to tailor and pilot test EMW for perinatal WIC clients with depression, resulting in the MBFBT program. Nineteen perinatal WIC clients enrolled in the pilot study, completing an average of six of eight MBFBT sessions Participants experienced decreases in depression and anxiety, and increases in parenting sense of competence

Mindyourtotalhealth(myth)

Professor Zhang partnered with Michigan Medicine’s Adolescent and Young Adult (AYA) Oncology program to tailor EMW for AYA cancer survivors with depression, creating the MYTH program. Zhang conducted a pilot feasibility trial of MYTH compared to Beating the Blues (BtB). Findings show MYTH is feasible, as 80% of participants randomized to MTYH completed at least six of eight sessions Participants randomized to MYTH reported significantly lower post-treatment depression than the BtB group.

Professors Weaver and Himle partnered with Northville Public Schools Superintendent RJ Webber to create the Mental Health Can’t Wait Summer Internship Program. EMW is...

QUICKY&EASILYTAILORABLE ENGAGING

EMW is the first technologyassisted CBT designed to allow for quick, easy, low-cost tailoring of selected content while retaining core, evidence-supported components of CBT.

EMW uses a three tiered approach of teaching CBT to users: a character-driven storyline, videobased educational content, and tailorable text and image-based panels. Billi’s story ends each episode with a cliffhanger that keeps viewers coming back!

Bornheimer’s Suicide Prevention, Treatment, and Research (STaR) lab aims to prevent suicide death among adults with serious mental illness, and particularly schizophrenia spectrum disorders (SSDs).

The STaR lab focuses on addressing the critical public health problem of suicide by:

Cognitive Behavioral Suicide Prevention for psychosis (CBSPp): a 10-week individual cognitive-behavioral suicide preventionfocused treatment with tailoring for psychosis symptoms and focus on information processing biases, appraisals and schema.

Ellis was named a James S. Jackson Emerging Scholar Award, a Rogel Cancer Center Health Equity Scholar and received a Career Development Award (K01) from National Cancer Institute! This federally funded grant allows Ellis to further strengthen her skills in intervention research and cancer genetics The grant supports her research on developing and testing a tailored, eHealth intervention for African American families that encourages uptake of recommended cascade genetic testing by improving communication about genetic testing results and targeting drivers of and barriers to behavior change.

In fall of 2023 Professor Smith was named Associate Dean for Research. He is committed to supporting faculty research that advances equity, social justice and social change. Smith’s portfolio of active, federallyfunded research focused on improving employment outcomes for autistic young adults, adults with severe and persistent mental illness, and returning citizens Smith’s appointment is effective in January 2024.

Kindred: Family Centered Approaches to Promoting Cascade Screening for Hereditary Cancer Syndromes among African Americans

Research has shown that African Americans are less likely than oth engage in genetic testing for hereditary cancer syndromes, which can uncover pathogenic variants that increase cancer risk African Americans also have lower rates of cancer cascade testing, a process for providing genetic counseling and testing to relatives of African Americans who are known carriers of specific cancer-related pathogenic variants.

Kindred is a five-year study with the overall objective of increasing communication about genetic testing among African American families and the uptake of recommended cascade testing. We aim to learn about new and improved ways to support African American individuals and families before, during and after they participate in cancer genetic testing The stages of the study include:

Phase 1: Identify facilitators and barriers to African American family communication about genetic testing and cascade testing through interviews and focus groups discussions

Phase 2: Develop and beta-test a tailored, eHealth intervention for African American families that encourages uptake of recommended cascade genetic testing by improving communication about genetic testing.

Phase 3: Conduct a pilot test of the intervention with African Americans families to evaluate the eHealth intervention’s feasibility and acceptability, participant engagement, and its potential impact on cascade testing.

Michigan Men's Diabetes Project abouttheproject

Previous literature has indicated that Black men are two times as likely to develop Type 2 diabetes compared to their non-Hispanic white counterparts and are also more likely to have associated complications. Furthermore, Black men have lower access to quality health care, and masculinity norms have been shown to hinder them from seeking the limited care that is available. In this study, we aim to investigate the effect of peer-led diabetes self-management education and long-term ongoing support on glycemic management.

Phase 1: Modification of existing diabetes education content to be more appropriate for the population.

Phase 2: We will conduct a randomized controlled trial to test the intervention. Participants randomized to the intervention arm will receive diabetes self-management education, structured diabetes self-management support, and a more flexible ongoing support period. Participants randomized to the control arm will receive diabetes selfmanagement education. Diabetes self-management education will be taught by certified diabetes care and education specialists, while the diabetes self-management support and ongoing support period will be facilitated by fellow Black men with diabetes who will be trained in group facilitation, patient-provider communication strategies and empowerment techniques.

Phase 3: Post-intervention interviews and dissemination of findings to the academic community. The primary goal of our study is to determine whether long-term peer-led support groups in conjunction with diabetes self-management education are a promising solution to improve self-management behaviors and decrease A1C levels.

Exploring PTSD Symptoms, Barriers and Facilitators to Mindfulnessbased Stress Reduction for Justice-Involved Black/African American Female Adolescents and Parents/Caregivers

High rates of trauma and post-traumatic stress disorder (PTSD) associated with delinquency (and recidivism) persist for justice-involved individuals. This enduring problem denotes a significant gap in our treatment approach. PTSD, the primary outcome of the proposed study about healing informed mindfulness-based stress reduction (MBSR), combines stressinflammation/cortisol reactivity, recidivism, personal agency, racial identity and parent-child relationships (secondary outcomes), which contribute to the overall health equity and well-being of justice-involved Black/African American female adolescents (JI BAFAs). The study will investigate the barriers and facilitators to the MBSR intervention with JI BAFAs and their parent-child relationships MBSR has been successfully implemented as a treatment for PTSD and is a promising new approach for healing to reduce health disparities among both JI BAFAs and their parentchild relationships. We will use a healing-informed MBSR approach based on an intersectional trauma-responsive framework: Intersectionality theory, Healing Centered Engagement, and Theoretical Domains Framework.

Web-Based Cognitive Behavioral Therapy for Depression Among Homebound Older Adults: Empower@Home

Empower@Home is an internet-based cognitive behavioral therapy program for older adults with depression Unlike many online mental health programs, it focuses on the unique concerns of this age group. The program was developed collaboratively with community partners, older adults and other stakeholders, resulting in an easy-to-navigate platform. It's unique for incorporating Empower Coaches, who are staff members without specialized mental health training, to support the adoption and engagement of the online program. The development of this program received financial support from the Michigan Health Endowment Fund.

Promoting Awareness and Engagement with Suicide Prevention Resources and Treatment in Community Mental Health

The Suicide Prevention, Treatment, and Research (STaR) Lab, led by Bornheimer, has completed Aim 1 of of an NIMH-funded pilot effectiveness clinical trial focused on modifying and evaluating the acceptability and preliminary effectiveness of Cognitive Behavioral Suicide Prevention for psychosis in partnership with Washtenaw County Community Mental Health (WCCMH).

To support the dissemination of research findings into the community, the STaR Lab received a MICHR Promoting Academics and Community Engagement Dissemination Grant. As part of the grant, Bornheimer’s team partnered with WCCMH to host a number of events during National Suicide Prevention Month to raise awareness, foster dialogue and promote community engagement in suicide prevention efforts. Community members also contributed to an art project that will be displayed at various WCCMH locations and the School of Social Work to inspire hope and continue the conversation about the community’s role in suicide prevention Aim 2 of the pilot effectiveness clinical trial is currently underway and is expected to complete recruitment in the next year.

This project aims to increase accessibility and decrease stigma for mental health care among adolescents and teens. Five students from Northville Public Schools participated in the Mental Health Can’t Wait Summer Internship Program in 2023. They worked with the U-M team to tailor Billi’s story on the Entertain Me Well Platform to be acceptable and applicable to high school students. The long term goal is to redesign the EMW platform to be optimized for mobile use and iteratively tailor educational content, images, text and videos for high school students to utilize if they are experiencing symptoms of anxiety and/or depression.

A Hybrid Effectiveness-Implementation RCT of Virtual Interview Training for Autistic Transition-Age Youth

Job interviews are a barrier to employment for autistic youth, who want training to improve their skills yet experience anxiety and low motivation to interview. Many autistic youth receive pre-employment transition services, but there is a major lack of evidence-based practices to improve job interview skills and employment outcomes for these youth To address this gap, Smith previously partnered (R34 MH111531) with autism stakeholders to adapt an efficacious internet-delivered job interview simulator to support autistic youth (Virtual Interview Training tool for Autistic Transition Age Youth; VIT). Building on his R34, Smith is now conducting a multi-site randomized controlled trial using a Hybrid Type 1 effectiveness-implementation approach of VIT by comparing autistic youth engaged in both pre-employment transition services and VIT to autistic youth receiving pre-employment transition services and an active control. In collaboration with co-investigator, Mary Baker-Ericzén, this R01 will evaluate if the VIT Tool for Autistic TransitionAge youth enhances employment, as well as if the benefits are a mechanism of improved mental health. It will also identify facilitators and barriers to implementation.

Partnering to Advance Psycho-Oncology Services for Underrepresented Adolescents and Young Adults (AYAs) with Cancer

Adolescent and young adult (AYA) cancer survivors are an age-defined population (15-39 years old), with studies reporting nearly 90,000 newly diagnosed AYA patients annually in the United States and 633,000 survivors of AYA cancer in 2020. Despite an overall improved five-year survival rate among AYA cancer survivors in the past two decades, these improvements have not transferred equitably to underrepresented specifically Black, Indigenous and people of color (BIPOC) and sexual and gender minorities

The project team will utilize a transdisciplinary approach to enhance a national research infrastructure through the AYA CAncer RESearch (AYA CARES) program at the U-M The team will engage in a series of stakeholdercentered activities for the purposes of conceptualizing, developing and disseminating patient-centered outcomes research/comparative clinical effectiveness research (PCOR/CER) studies that capitalize on the collaborative efforts, experiences and brainpower of (embodied) researchers, clinicians, AYA cancer survivors, caregivers, program administrators and other stakeholders.

Family Safety Net: Developing a family-focused suicide prevention approach to encourage safe firearm storage in remote Alaskan homes

Suicide is a major and rising public health problem in rural America. In Alaska, suicide rates are consistently triple the national rate. There, 60% of the suicide fatalities are due to firearms In rural Alaska Native (AN) communities, virtually all homes have multiple guns for subsistence hunting, which increases suicide risk significantly. Our study developed a ‘family-focused’ screening to identify households with firearms and self-assessed suicide risk (i.e. households where someone is experiencing mental health struggles) that could benefit from an intervention designed to reducing quick access to firearms in people’s homes, thus lessening household environmental risk for suicide. Overall, the FSN intervention was acceptable to and appreciated by participants who entered the study Over the three waves, our recruitment strategies were refined and resulted in a three-item, family-focused screening that was endorsed about 30% of the time. There are signals of impact related to participants’ perceived firearm risk and their motivations to take safety precautions (our mechanisms of change). Importantly, many participants took action to secure their household firearms and medications to reduce quick access. These promising preliminary results suggest a feasible innovation to screening protocols that can enhance safety and prevent suicide deaths by engaging adult family members in creating safer environments within their homes. Further research is needed to test the efficacy of this novel intervention.
