TOGETHER, SAVING LIVES & BUILDING HOPE
Every day tens of experienced, qualified, certified, medical personnel leave Syria, especially among workers in the health sector who suffered from detention, shelling, and kidnapping. This led to a lack of medical specialists and a decrease in the number of active medical personnel. Syria has been dealing with the decrease of highly educated professionals due to a high migration rate for years, after March 2011 migration rates grew higher, causing a semi-collapse in many service sectors, especially health and education. We will not look the other way and hope the situation improves itself, that’s why we started to respond quickly through the establishment of a high quality training center in northern Syria. We brought experts from Syria and from Arab and foreign countries to conduct training and qualification programs for medical and non-medical staff that work inside Syria, in order to fill the gaps that threaten the whole Syrian society. We have provided special courses that cover the main requirements in wartime and other courses for maternity and child care, psychosocial support, and gender based violence. Furthermore, we established a nursing school and we aim to open others in other parts of Syria in the near future. Finally, we have launched projects for training in various administrative fields to provide others, such as Syrian civil society organizations, and local councils with the lessons that we have learned thus contributing to the reconstruction process of the health system in our country, Syria. We will keep moving forward, qualification and training is an ongoing process that does not end, we need innovative solutions to challenge the “Disastrous� reality, and we will not stop until we make a difference.

Introduction UOSSM, in partnership with Physicians Across Continents (PAC), conducted a survey on the hospitals in Syria, the results showed a clear shortage in medical staff. A few examples are: 90 medical students were forced to quit medical school due to the crisis. There are only 24 anesthesiologists among all the hospitals that were surveyed throughout Syria, and there are only 50 specialists throughout Syria (not including surgeons.) There were many midwives that were not adequately trained, probably due to the fact that they do not work in hospitals, and in hostile areas in Aleppo, there is only one Gynecological/Obstetrician per 45,000 people. Almost one third of technicians are not properly educated, there is a need to train these staff and to properly qualify them for their jobs. Comparing similar surveys throughout the world with Syria we found that in 2008 there were five doctors per 10,000 citizens, where the average throughout the world is 13 doctors per 10,000. Today in Syria, there are only 2 doctors per 10,000. There is an average of 28 qualified nursing staff per 10,000 throughout the world, while in Syria in 2009 there were 14 per 10,000. Today in Syria, there are less than 5 nurses per 10,000, this includes non-certified nurses as well, this is 17% of the accepted average worldwide. There are equal amounts of qualified and non-qualified nurses, which elevates the need for training and education for this staff.

“If I Only Knew… I Could Have Saved My Friend” It was a typical day in the Hama suburbs, when all of a sudden, the area came under fire and an explosive barrel blew up near a young 20-year-old woman. “Aisha” didn’t have any apparent wounds or shrapnel lodged in her body. People ran to the area of the explosion to give her warm wishes because she miraculously was not harmed by the explosive barrel. Her friend came to her thanking God for the miracle of her safety, when all of a sudden “Aisha” said “I don’t feel too good” to her friend. She said she was hearing a loud ring in her ears. Her friend smiled and told her that it was from the sound of the explosion. After a few moments, “Aisha” started bleeding from her ears, and experiencing shortness of breath. She then started turning blue in the face. “Aisha” was transferred to a hospital, but lost her life on the way to the emergency room, leaving her friend with so many unanswered questions about why she died despite the fact that there was no apparent injury to her body. “Aisha’s” friend decided to attend a medical training course where all of her questions were finally answered. She learned what explosive shock trauma was, its signs, and how to respond and deal with such an injury. After her training she said with a heavy heart, “If I only knew, I could have saved my friend.”

General Overview During 2015 (between January 2015 ,1 and December 2015 ,31) The Qualification and Training Department reported 248 different activities throughout Syria, with maximum use of human and technical resources in both Bab Al Hawa Training Center (Syria), Reyanli Training Center (Turkey), and Adan Training Center (Lebanon). Activities were categorized into the following groups:

“Hurry Before it is Too Late” One day in a city in Syria, as people were going about their everyday routine, there was an airstrike campaign coming from a warplane overhead. The strikes hit civilian homes causing fires which led to fireman coming to try and put out the fire. Search and rescue operations were under way in hope of finding survivors. Among the dead were three children with their mother, their untimely deaths were caused by the airstrikes and fire afterwards. The brother of the children was also found, barely alive, fighting for his life. He was rushed to a nearby hospital. “Ibraheem” one of the instructors in the medical training and education center in Bab al Hawa, admitted the young boy. He began to examine the scared young boy who was crying, he was taking 24 breaths a minute, and the boy hearing extremely loud noises, and black soot was coming out of his mouth. “Ibraheem” layed the child in the correct position and gave him oxygen, he went on to give the boy an IV to rehydrate him from all of the fluids he lost. (Burns cause a loss in fluids and proteins in the body.) “Ibraheem” went on to examine the boy’s ears afraid the boy was experiencing explosive shock trauma because the boy was hearing extremely loud noises. “Ibraheem” found that 25% of the boy’s body was burned badly, he had burns on his face, and his ears were burned externally and internally in the airways. This type of burn requires the insertion of a tube to insure the airway remains open. “Ibraheem” requested an ENT specialist to perform an endotracheal intubation on the child. The expert examined the child and refused to perform the procedure claiming that since the child is conscious there is no need for the procedure. “Ibraheem” replied to the expert saying, “we learned from the French Professor Raphael Pitti and his training team, that when someone is burned in the face and inner ears and shows symptoms like the little boy, that a endotracheal intubation is crucial, before the tube becomes blocked, after a few arguments the expert finally agreed to follow “Ibraheem’s” recommendation. While performing the procedure, the doctor noticed that the tube started to get tighter. He was able to successfully perform the procedure, and he admitted that the consequences would have been devastating had he not listened to “Ibraheem.” The airway would have become so constricted that they would have had to perform a tracheal biopsy.

4. On-Field Training Aim to improve response to war-related traumatic injuries and chemical attacks. On-Field mobile training team is ready to provide training in emergency situations such as the frequent use of chemical warfare and the increased need to raise awareness among medical staff. In addition to the dramatically increased intensity of fighting and the urgent need to evaluate the preparedness to manage war-related traumatic injuries in the following provinces:

6. One-Day Activities Qualification And Training Department respond to some emergency situation by launching awareness campaigns within Bab Al-Hawa Training Center, and also encourage guests from outside Syria who visit Bab Al-Hawa hospital with unique specialties to provide basic training or education within their specialties, to improve the efficiency of medical staff.
