Your Future of Hope Survivorship Program was developed by the Cancer Treatment and Prevention Center of UT Health Northeast. It is based on the guidelines and recommendations of the Institute of Medicine, the American College of Surgeons Commission on Cancer Program Standards of 2012 with the benefit of Equicare CS™ Case Management Software.
Your Future of Facing a diagnosis of cancer is one of life’s most difficult challenges for both patients and family members. What will this journey be like? How do you prepare? Will you travel this path alone, or will there be people to walk by your side – people who will hold your hand, care for you, encourage you, and celebrate your progress? Will they understand what you’re going through?

One thing is certain – cancer brings with it a host of emotions, questions, and concerns. Every patient of the Cancer Treatment and Prevention Center at UT Health Northeast will be surrounded by a caring and committed team of highly trained specialists, support staff, and volunteers to provide them with the best care possible. Steps
You have completed your initial treatment for cancer, so now what? Where do you go for answers to your questions now? How do you make sure you are getting the right medical care as a cancer survivor? And how are you supposed to feel - will you always worry about your health? The Survivorship Program at UT Health Northeast’s Cancer Treatment and Prevention Center was designed to provide cancer survivors with a comprehensive care summary and a structured follow-up plan that reflects their treatment and addresses the many post-treatment needs to improve cancer survivors’ health and quality of life. Patients of UT Health’s Cancer Center as well as individuals who have completed their treatment elsewhere are qualified to participate in the Your Future of Hope Survivorship Program. If you received your treatment elsewhere, we will need copies of all of the medical records pertaining to your cancer care and treatment.
Record of Care Once you complete your cancer treatment, you will be given a record of all of the care you have received as well as the important characteristics about your disease including:
Questionnaire You will be asked to complete questionnaires about your health and personal needs so that we can provide you with personalized and ongoing care to meet your specific needs.
Patient Portal As part of the services we provide, you will be able to access your records through our secure, online website called Equicare CS™. You and your care providers can use Equicare CS™ to manage your care. This site will enable you to access appointments related to your care, complete questionnaires assigned to you, and review the educational materials that have been created for you.
Educational Materials Your cancer team will identify the educational information they feel you would most benefit from based on your diagnosis, treatment, and other factors related to you. This information will be updated as needed to provide you with the best possible learning resources.
Follow-Up Plan Following the completion of your treatment, your care team will customize a 5- to 10-year plan created specifically for you that will outline the lab work, exams, diagnostic testing, and health screenings we recommend. As the timeframe approaches for each of these, we will schedule specific appointment dates and times for them.
• The diagnostic tests performed and the results.
Referrals
• Your tumor characteristics – site, state, grade, hormonal status, marker information.
Based on your individual needs, we may refer you for additional support services that may include:
• Therapies you received including surgery, chemotherapy, radiotherapy, transplant, and hormonal, gene, or other therapies. Also included will be the agents used, treatment regimen, and total dosages.
• Nutritional counseling related to healthy eating choices and weight management.
• Clinical trials (if any) and their identifying number and name. • Contact information on institutions and key providers for your treatment, as well as a coordinator for your continuing care.
• Support group participation or counseling services. • Physical therapy, occupational therapy, and exercise programs to help increase your stamina, functionality, and quality of life. • Financial planning.

Cope
Support
Group
Our survivorship support group is based on the American Cancer Society’s I Can Cope program for adults facing cancer. Conducted by certified leaders, the sessions will include information on cancer diagnosis and treatment, treatment side effects, pain management, cancer-related fatigue, and nutrition and physical activity. Communicating concerns and feelings, self-esteem and intimacy, financial concerns, community resources, and staying well in mind, body, and spirit are also part of the program.
