PEDIATRIC SURGEON AT UTHSC/LE BONHEUR LAUNCHES ADOLESCENT BARIATRIC SURGERY PROGRAM Peggy Reisser, Communications Specialist It has taken more than a year and a half of work, but pediatric surgeon Ying Zhuge has established an adolescent bariatric surgery program in Memphis that is one of only a few in the country. An assistant professor of surgery and pediatrics at the University of Tennessee Health Science Center, Dr. Zhuge launched a bariatric surgery program for adolescents ages 16 to 21 at Le Bonheur Children’s Hospital on September 27, when she performed the program’s first laparoscopic sleeve gastrectomy on a 20-year-old patient. Once considered only for obese adults, bariatric surgery increasingly is seen as an option for the most severely obese adolescents. The weight-loss surgery program is one part of Le Bonheur’s Pediatric Obesity program, which includes a multidisciplinary Healthy Lifestyle Clinic designed to help children and teens manage their weight through a variety of options centered around behavioral and lifestyle changes. These include improving food choices, increasing exercise, and counseling for the child and family members. According to the Mayo Clinic, almost one-fifth of adolescents in the United States are obese, and approximately 7 percent have severe obesity. As these numbers climb in adolescents, so do accompanying health issues, including type 2 diabetes, hypertension, obstructive sleep apnea, cardiovascular disease, and mental health problems.
“The surgery is not the end, it’s a kick start,” Dr. Zhuge said. The surgery begins the weight loss that can then be amplified by healthier behaviors to last a lifetime.
As a result, the medical community is beginning to accept that for some young people, those with a BMI over 40 or weighing 300 pounds or more, surgery may be less problematic than those long-term health complications.
“It’s not that you have surgery and you’re fixed, it really is a lifestyle change,” she said. “It’s just a beginning, kind of like a springboard, and once they lose a lot of weight, they are more active and move around better and they can keep their healthy lifestyle.” The surgery on the first patient in the program took approximately an hour and a half. The patient went home two days later, and in the first two months after surgery, lost approximately 40 pounds. Born in China, Dr. Zhuge was 10 years old when her family immigrated to the United States. She received her bachelor’s degree from the University of Miami in Coral Gables, Florida, in 2002, and her MD from the Miller School of Medicine at the University of Miami in 2006. She came to Memphis after a general surgery residency at Jackson Memorial Hospital at the University of Miami. She had visited Memphis only once for her fellowship interview, but she remembers feeling so at home at Le Bonheur, that the match was welcome. Dr. Zhuge completed a pediatric surgery and pediatric surgical oncology fellowship at Le Bonheur and St. Jude Children’s Research Hospital in June 2015. 8

PEDIATRIC SURGEON CONTINUED
SAVE THE DATE!
She then joined the Division of Pediatric Surgery at UTHSC and Le Bonheur. She also became part of the team at the Healthy Lifestyle Clinic “I had done a fair amount of adult bariatric surgery,” she said. The movement to bring bariatric surgery to younger patients progressed, and she began to build the program in Memphis. She visited other successful programs, ordered equipment, identified potential patients, and negotiated insurance issues.

Even as Dr. Zhuge worked to improve the health of her patients, she faced her own health crisis. She was diagnosed with breast cancer in June 2017. Because her mother died from the disease at a young age, her father had urged her for years to have genetic screening. Busy first with school and then work, she put off the test. She got the screening after she finished training, and tested positive for the CHEK2 mutation. At age 35, her first MRI brought the diagnosis. “The stage was really early and I’m doing really well,” she said. She is engaged to Timothy Weatherall, MD, a vascular surgery Fellow at UTHSC. Dr. Zhuge said she is grateful for her partners at Le Bonheur who stepped in and helped when needed, as she went through chemotherapy and continued her surgical duties at Le Bonheur. Now, her focus is on building the adolescent bariatric surgery program. She said she aims to do 30 of the surgeries a year and will follow each patient for three years after surgery. Along with other members of the Healthy Lifestyle Clinic team, she plans to conduct research to contribute to the dearth of knowledge on long-term effects of the surgery on these younger patients.
