Vision and hearing tests will be performed for students in specific grades in the fall for students who have not yet completed their annual vision and hearing tests.

Students wishing to participate in athletics should complete and submit physical forms by the first day of school, August 16, 2023 or first practice. Completed physical forms can be faxed (713-773-9753), emailed to nurse@veritasca.org, or submitted in the School Office.

Athletic Physical Forms must be in by the first day of school!
