Program Waiver: PARTICIPATION AGREEMENT, SWIMMING AND SKATING EQUIPMENT RENTAL AGREEMENT, RELEASE OF LIABILITY, WAIVER OF CLAIMS AND ASSUMPTION OF RISK BY SIGNING THIS DOCUMENT YOU ARE ENTERING INTO A CONTRACT WHEREIN YOU MAY BE WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO Sue. TO The Edge Ice Arenas (“Arena”), Water’s Edge Aquatic Center (“Aquatic Center”), the Village of Bensenville, their owners, directors, agents, employees, and/or representatives of the above listed organizations. ASSUMPTION OF RISK: I am aware that ice skating and swimming involve certain inherent risks, dangers and hazards which can result in serious personal injury or death. I am also aware that ice skating arenas and the aquatic center contain potential dangers to the ice skating and swimming public. As such, I hereby freely agree to assume and accept any and all known and unknown risks of injury while participating in ice skating and aquatic center activities. I further recognize and acknowledge that the risks inherent in the sport of ice skating and swimming can be greatly reduced by taking lessons, abiding by the Skater Responsibility Code and the Swimming Responsibility Code (now known as your “Responsibility Code”) and by using common sense. RELEASE AND WAIVER OF CLAIMS AGREEMENT: In consideration of allowing me to participate in the Arena’s Ice Skating programs and the Aquatic Center’s swimming lessons; in consideration of the loan or rental to me of skating and swimming equipment, I hereby agree to the fullest, and extent permitted by law as follows: 1. TO ACCEPT FOR USE AS IS any equipment loaned or rented to me and to keep it in my possession at all times. 2. TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against the Arena and the Aquatic Center, any of the parties named above and/or the Rental Shop and 3. TO RELEASE the Arena, the Aquatic Center and any party named above and/or the Rental Shop from any and all liability for any loss, damage, injury or expense that I may suffer, or that my next of kin may suffer, arising from the ice skating and swimming programs at the Arena and the Aquatic Center and as a result of my participation in the Arena’s ice skating programs and the Aquatic Center’s swimming programs and the use of the equipment loaned or rented to me, due to any cause whatsoever, including negligence of breach of contract regarding the design, manufacture, selection, installation, maintenance or adjustment of the equipment and in the operation, supervision, design, or maintenance of the Arena and the Aquatic Center. ARBITRATION: In consideration of allowing me to participate in the Arena’s ice skating programs and the Aquatic Center’s lessons, and in the consideration of the loan or rental to me of ice skating and swimming equip‐ ment, I hereby agree to submit to binding arbitration any and all claims which I believe I may have against the Arena and the Aquatic Center’s, the above named parties and/or the Rental Shop arising from my participation in the Arena’s ice skating programs and the Aquatic Center’s swimming programs and use of the equipment loaned or rented to me. The arbitration shall be pursuant to the rules of the American Arbitration Association. The arbitrators shall apply the Federal Rules of Evidence to all proceedings. Arbitration shall be commenced within one (1) year from the date on which any alleged claim rst arose. Further, the arbitration shall be held in the town where the Arena and Aquatic Center is located, unless otherwise mutually agreed to by all parties. The submission to the American Arbitration Association shall be unlimited and the arbitration award may be enforced by any court of competent jurisdiction.

Parents and kids are invited to bring their own stick, puck, and helmet to the ice and have some good old fashioned hockey fun. Adults are required to wear helmets and children are required to wear a helmet with a face mask. Additional protective equipment such as shin pads and elbow pads are recommended. Learn To Skate The Edge Ice Arena offers Learn To Skate classes for all ages, beginning from 3 years old to adults, and of all abilities. Whether you are an aspiring future Olympian or just want to skate for the fun and exercise, there is a class for you. Our experienced, professional gure skating staff are eager to share their knowledge and love of the sport. Reasonably priced classes are offered during the week and on Saturdays to accommodate busy schedules. For more information regarding group skating lessons or private instruction, please contact the Figure Skating Director. Basic Class Information Detailed class descriptions are available at the front desk. All Learn To Skate classes are held at The Edge Ice Arena John Street. Tots - Beginning skaters between the ages of 3-5 years old. Beginners- Beginner skaters between the ages of 6-10 years old. Pre-Alpha 1 - Skaters ages 6 and older with some skating experience. Instructor approval needed. Alpha - Forward skating skills, stroking and crossovers. Pre-requisite; pre-alpha or instructor approval. Beta - Backward skating skills, back crossovers, t-stops, and turns. Pre-requisite; Alpha or instructor approval. Gamma - Advanced review of above and variety of turns. Pre-requisite; Beta or instructor approval. Delta - Advanced review of above, turns, edges, and lunges. Pre-requisite; Gamma or instructor approval. Freestyle Levels - Variety of footwork, jumps, spins, varying in difficulty by skill level. Pre-requisite; Delta for FS 1, FS 1 for FS 2, FS 2 for FS 3, etc. or Figure Skating Director approval. Teen/Adult - Open to all adults. No skill level required. Instruction is geared toward each individual’s ability. Students are encouraged in a non-intimidating environment. Synchronized Skating - Synchronized Skating, a fast growing discipline, consists of 8-20 skaters moving as one. Skaters experience teamwork, skills building, and long lasting friendships. Learn To Skate Basic Information Skate Rental - Figure and hockey skates are available to rent for $2.00 per lesson for Learn To Skate classes and $3.00 for every Public Skate. Dress for warmth but no need to bulk. Light jackets or sweatshirts are usually ne. Pants or sweatpants are preferred, no snow pants or snowsuits. GLOVES ARE A MUST. Helmets are optional. Skaters progressing to freestyle levels are required to wear gure skating attire; no jeans permitted. Classes meet once a week, but you may enroll in more than one class. Most classes run 40 minutes. Tot classes are 30 minutes of instruction and 10 minutes of fun time. Parents are only permitted to join them on the ice during this time (skate rental fees apply if needed). Classes are held at the John St. Ice Arena, 545 John St. Bensenville, IL 60106. Make-up Policy Make-ups will be permitted ONLY if class space allows. Only 1 make-up per session. Skater must obtain a “makeup” slip prior to showing up for class. No class make-ups on the rst 2 weeks of a session or last week of a session. NO MAKE-UPS WILL BE PERMITTED DURING THE WINTER SESSION. Refund Policy Refund requests and/or credits will be granted only until the end of the second week of classes, less the fee of classes attended. Registration fee is non-refundable. Full refunds and credits will be given only in cases of medical necessity, with a doctor’s note. Registration Fee There is a yearly registration and processing fee of $15.00. Registration fee includes membership to the Ice Skating Institute, the largest recreational gure skating organization in the country (non-refundable). Registration fee will be waived if skater can provide proof of current ISI membership. Copy of card must accompany class registration. Registration Deadline Class registration deadline is 1 week before the start of class. The Edge Ice Arena is committed to giving our skaters the highest quality of instruction and taking pride in preparation for class. Registration will only be taken if there is class availability.

The 201 5 American Red Cross Learn-to-Swim Program The American Red Cross Learn-to-Swim program is designed to give students a positive learning experience in the water. This program teaches aquatic safety skills in a logical progression through six levels of courses. The objective of this program is to teach children to swim and be safe in, on, and around the water.

Level 1 - Introduction to Water Skills (Approx.6 years - 14 years) The objective of Level One is to help students feel comfortable in the water and to enjoy the water safely while developing effe ctive swimming habits and true locomotion skills. Students will learn elementary water skills which they can build on as they progress through the various levels. Level 2 - Fundamental Aquatic Skills (Approx. 6 years - 1 4 years) Students who wish to enter this course must have a Level 1 certi cate or must be able to demonstrate all the completion requirements in Level 1 . The objective of Level 2 is to give students success with fundamental skills. Students learn to float without support and recover to a vertical position. Fun And Skills… … the right place to start… About the Program Pre-Hockey/Learn to Skate classes are structured so your child will have the opportunity to learn the basics of hockey from experienced professional instructors in an atmosphere that is relaxed, noncompetitive and fun to learn. No skating or hockey experience is needed. Classes with Pre-Hockey/Learn to Skate programs concentrate on skating basics and con�idence building. Our hockey program prides itself on the quality of our coaching staff. Our program makes it a priority to train our coaches to ensure your child receives the best coaching instruction available. Upon completion of our Pre-Hockey program, it is our goal to have prepared your child to be ready to join a team. Your child may then enjoy the excitement of playing on a hockey team with children of his or her own age. Evaluation will take place on �inal class to determine if your child should return to another Prehockey class or move up to a house league team. Equipment Because our classes are instructional in nature, we do not require complete hockey equipment. We do require your child wear a HECC approved hockey helmet with face-mask, hockey elbow pads, shin pads, hockey gloves, jersey and has a hockey stick. Rental skates are available to rent for one dollar but purchasing a correctly-sized, quality skate is of great bene�it to the skater. The Water’s Edge Aquatic Facility The Water’s Edge is an indoor aquatic facility attached to the Edge Ice Arena at 545 John Street. The Water’s Edge offers diving boards in a 13 foot diving well and an 8-lane, 25 yard lap pool ranging from 5 feet to 6 feet deep. To access this facility, patrons are asked to use the main entrance of the building. The pool and locker rooms can be accessed directly through the ice arena restrooms on the west side of the building. Cash is the acceptable form of payment for daily use. Aquatic sta cannot accept bills larger than $20.00. Small bills requested. Patrons must provide their own swimming attire, towels, and locks for securing property. Lockers are provided for use; free of charge. The Village of Bensenville is not responsible for lost, stolen, or damaged property.

Monthly pool schedule is subject to change without notice AquaFitness Courses The Water’s Edge Aquatic Center o ers AquaFitness programs to participants of all ages. These courses offer no-impact cardio work outs designed to help you lose those extra pounds without the stress on your joints. Classes are typically held in our dive well while you are suspended in the water. Flotation devices are provided. Classes are FREE with your membership, or simply pay the daily entrance fee to join. Patrons interested in receiving the discounted Resident, Senior, or College Student rates MUST present valid identi cation.

Registration Guidelines Registrations can be completed at the Edge Ice Arena at 735 E. Jefferson Street, over the phone at 630-766-8888, or online. The Edge Ice Arena will be accepting registrations during standard hours of operation; please call to verify hours. Residents of Bensenville will be required to show proof of residency at time of registration. Non-residents are welcome to participate in all aquatic programs. Registration Deadlines Class registration deadline is 1 week prior to the start of class. The Water’s Edge Aquatic Center is committed to giving our swimmers the highest quality of instruction and taking pride in preparation for class. “Walk-Ins” jeopardize this structure and a $10.00 increase may be applied to registrations completed day of. The Wahoo Swim Team is a recreational team that competes against local teams throughout the year. The Wahoos compete in the Central Suburban Swim Conference during the summer months. For ages 6-18 years, the Wahoo Swim Team is designed to develop individual swimming skills, while promoting healthy lifestyles and sportsmanship. The Green Teen Zone is a drop-in center for Bensenville youth, funded by the Village of Bensenville. Our program serves fourth grade through eighth grade children, enrolled in Bensenville schools. The Green Teen Zone is a fun, safe place for children to come EVERY day after school and during the summer. The best part about the Green Teen Zone is that it is absolutely FREE!!! The Green Teen Zone o ers a great deal of activities and opportunities for students. Our program also provides free daily meals!

Do you know about our food program? The Green Teen Zone has partnered with the Northern Illinois Food Bank to provide daily snack and dinner to children every day!! The Green Teen Zone will also provide meals to any Bensenville resident 18 years old or younger! Even if you are not of an age where you can participate in our program, you can still come in every day for free meals!! ASSUMPTION OF RISK: I am aware that ice skating and swimming involve certain inherent risks, dangers and hazards which can result in serious personal injury or death. I am also aware that ice skating arenas and the aquatic center contain potential dangers to the ice skating and swimming public. As such, I hereby freely agree to assume and accept any and all known and unknown risks of injury while participating in ice skating and aquatic center activities. I further recognize and acknowledge that the risks inherent in the sport of ice skating and swimming can be greatly reduced by taking lessons, abiding by the Skater Responsibility Code and the Swimming Responsibility Code (now known as your “Responsibility Code”) and by using common sense. RELEASE AND WAIVER OF CLAIMS AGREEMENT: In consideration of allowing me to participate in the Arena’s Ice Skating programs and the Aquatic Center’s swimming lessons; in consideration of the loan or rental to me of skating and swimming equipment, I hereby agree to the fullest, and extent permitted by law as follows: 1. TO ACCEPT FOR USE AS IS any equipment loaned or rented to me and to keep it in my possession at all times. 2. TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against the Arena and the Aquatic Center, any of the parties named above and/or the Rental Shop and 3. TO RELEASE the Arena, the Aquatic Center and any party named above and/or the Rental Shop from any and all liability for any loss, damage, injury or expense that I may suffer, or that my next of kin may suffer, arising from the ice skating and swimming programs at the Arena and the Aquatic Center and as a result of my participation in the Arena’s ice skating programs and the Aquatic Center’s swimming programs and the use of the equipment loaned or rented to me, due to any cause whatsoever, including negligence of breach of contract regarding the design, manufacture, selection, installation, maintenance or adjustment of the equipment and in the operation, supervision, design, or maintenance of the Arena and the Aquatic Center. ARBITRATION: In consideration of allowing me to participate in the Arena’s ice skating programs and the Aquatic Center’s lessons, and in the consideration of the loan or rental to me of ice skating and swimming equip‐ ment, I hereby agree to submit to binding arbitration any and all claims which I believe I may have against the Arena and the Aquatic Center’s, the above named parties and/or the Rental Shop arising from my participation in the Arena’s ice skating programs and the Aquatic Center’s swimming programs and use of the equipment loaned or rented to me. The arbitration shall be pursuant to the rules of the American Arbitration Association. The arbitrators shall apply the Federal Rules of Evidence to all proceedings. Arbitration shall be commenced within one (1) year from the date on which any alleged claim rst arose. Further, the arbitration shall be held in the town where the Arena and Aquatic Center is located, unless otherwise mutually agreed to by all parties. The submission to the American Arbitration Association shall be unlimited and the arbitration award may be enforced by any court of competent jurisdiction. BINDING EFFECT OF AGREEMENT: In the event of my death or incapacity, this Agreement shall be effective and binding upon heirs, next of kin, executors, administrators, assigns and representatives. ENTIRE AGREEMENT: In entering into this Agreement, I am not relying upon any oral or written representation other than what is set forth in this Agreement I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE ARENA AND THE AQUATIC CENTER, ITS OWNERS, MANAGERS, EMPLOYEES AND/OR THE RENTAL SHOP.
