5 Tips for Financial Success W
ELCOME to the latest edition of Fiscal Fitness! As we head into the New Year, now is a great time to get your financial house in order. Make your own Money Resolutions and start the year out right with a few basic tips, no matter what your age. Create Your Financial Plan Every successful business has a business plan, so why don’t you? There are many on-line tools to help you get started. Some of us need a little more help than a cookie-cutter questionnaire about our goals and objectives. Give up the new shoes and invest in a Certified Financial Planner® professional who will work with you side by side to create a plan with a purpose. Once you have the why, you can build the financial roadmap to get you there! Live Within Your Means Do you really know what you spend each month? It’s time to take an honest assessment of your expenses and keep track of frivolous spending. This can be challenging for many, considering we use debit, credit cards and cash for purchases. Ever hear the phrase “lifestyle inflation”? It means your cost of living rises as your income rises. I have worked with many wealthy individuals & families over the years and have noticed a common theme– as income levels rose, the cost of living remained remarkably the same. In other words, they lived below their means. By doing so, most were able to accumulate a substantial nest egg for retirement. Studies have shown that people who use plastic are unconsciously willing to spend more than someone paying with cash, a phenomenon often referred to as the “credit card premium”. If you had to go to the ATM machine to get cash, would you still buy those designer shoes? Better yet, put the item on hold for 24 hours. Chances are, you will not go back. Contribute to Your Employer’s Retirement Plan Did you know that less than 50% of individuals contribute to their employer retirement plans, leaving employer contributions on the table?1 If you are eligible for a 401(k), 403(b),
and most 457 plans, you can defer up to $18,000 in compensation in 2017. If you are age 50 and older, you can defer an additional $6,000 in 2017. If you cannot contribute the maximum, at least contribute the amount that your employer is matching. If you are married and not working, you may be able to contribute up to $5,500 with an additional $1,000 if age 50 or older (MAGI phase out limits apply). Plan for the Unexpected Now is a great time to review your insurance coverage, particularly if you have older policies. Have your needs changed? If you are suddenly single, do you still have the need for a large life insurance policy or would a policy with long-term care benefits be more appropriate for your financial future? Update Your Estate Plan Remember, your will or trust will not override what is named on the beneficiary designation form on a life insurance policy, annuity, or retirement accounts (IRA or 401(k) plan). For this reason, it is imperative that you update your beneficiary designations immediately to reflect your current wishes, particularly if you are recently divorced or widowed. Did you know the federal limits have been raised in NJ? The state estate tax exemption has been increased from $675,000 to $2 million for deaths as of January 1, 2017. Remember, the existing New Jersey inheritance tax remains in effect and is levied on siblings, nieces and nephews. Whether you believe it or not, the new administration will bring about changes to the investment landscape and how we plan for retirement. Now more than ever is the time to take an active role in managing your finances. “Now what” is not a strategy. Together we can make it happen...

Facing new
challenges A uniquely skilled surgeon treats the complex health challenges of a past gastric bypass patient.
When Anne Haas had gastric bypass surgery years ago, she did her research and found the right bariatric surgeon for her needs. But in recent months, when ensuing medical problems required complex procedures, she faced a challenge: finding the right doctor to bring her back to health. Anne ultimately turned to accomplished bariatric surgeon Frank J. Borao, M.D., FACS, FASMB, director of the Center for Minimally Invasive Esophageal and Gastric Surgery at Monmouth Medical Center. Today, she feels very fortunate for her decision. “I thank my lucky stars that I found him,” says the Monroe resident. “I had to have had someone looking over me.”
New issues arise Anne had dieted throughout her life with no lasting success. In 2003, she underwent standard gastric bypass surgery. Though she maintained a 75-pound weight loss for nearly a decade, her situation grew more complex. Afflicted by unrelated kidney problems since adolescence, in 2010 she received a kidney transplant. And the transplant required Anne to take the steroid prednisone over the longterm, which weakens the body’s tissues. In the past few years, she also developed severe acid reflux—a sign something had gone wrong with her older bypass. “I gained 20 pounds in a year and a half. Up until then, my weight had been pretty stable,” says Anne, 67. “I just didn’t feel good; I knew something was wrong.” Anne’s gastroenterologist diagnosed her with a gastrogastric fistula—a connection between the small stomach pouch created as part of her bypass and the rest of her stomach. So Anne began looking for a bariatric surgeon to fix her
gastric bypass, and she found Dr. Borao “by happenstance.” “I came upon his name, scheduled an appointment and fell in love with him,” she recalls. “Anne needed surgery,” says Dr. Borao. “In addition to the fistula, Frank J. Borao, M.D. she had a hiatal hernia with a significant amount of stomach in her chest. Although it is a formidable surgery, it can be done through a few small incisions.” That message was a wake-up call to Anne. “Until I went to Dr. Borao, I didn’t think it was a big deal,” she says.
Intricate procedure Dr. Borao performed an eight-hour operation, which included removal of a portion of Anne’s stomach and small intestine and repair of her hiatal hernia, with a takedown of the fistula. “It wasn’t an easy surgery but everything went well,” says Anne, who was out of bed the next day, and returned home one day later. “The staples wouldn’t hold because of the condition of my tissues from the prednisone use, so Dr. Borao had to hand-sew all of the connections.” Three weeks after her surgery, Anne returned to her job in real estate. She tired quickly at first, but after a short time, she felt “a lot better.” Plus, Anne’s reflux and heartburn are “totally gone,” she reports. “Dr. Borao is amazing,” says Anne. “I would recommend him to anyone. Given a second chance
When faced with health challenges, two patients turn to the hospital’s colorectal surgery program Colon cancer runs rampant in Michael Feindt’s genes—affecting multiple people on the same side of his family, including his grandfather, father and uncle. Michael’s grandmother and older sister succumbed to uterine and ovarian cancer at young ages. Twice, in his 40s, the Jackson resident was diagnosed with colon cancer and underwent partial colon resections. After being cancer free for 15 years, Michael underwent a routine, annual colonoscopy at age 60 performed by gastroenterologist Joel Kurtz, M.D. A polyp, detected on the screening, was tested and pathology confirmed it was cancer.
It’s in the genes Genetic testing confirmed Michael tested positive for the mutation in the MSH2 gene, which is associated with some cancers, especially with hereditary nonpolyposis colorectal cancer (HNPCC), and Lynch syndrome, an inherited condition that gives a person a higher risk of cancers of the digestive tract and other organs. And like Michael, people who have Lynch syndrome have a significantly increased risk of developing other cancers, including colorectal cancer. Additionally, the average age for colorectal cancer to be diagnosed with someone with Lynch syndrome is 45 compared to the average age of 72. Michael turned to Roy M. Dressner, D.O., FACS, FACCRS, a practicing boardcertified colorectal surgeon at Monmouth Medical Center for colon cancer surgery. He appreciated Dr. Dressner’s diligence

in doing everything possible to perform a successful colon resection and avoid attaching a colostomy bag—a prosthetic medical device that collects human waste. Prior to surgery, Dr. Dressner brought Michael’s case to the hospital’s tumor board. “At these meetings, all of the specialists work together to review the cases and come up with a consensus of how we can provide the best possible treatment approach,” notes Dr. Dressner.
Surgery yields excellent results Dr. Dressner performed a colon resection surgery via a laparoscopic or minimally invasive approach. He removed the cancerous part of Michael’s colon, located near one of the previous resections and meticulously incorporated the previous surgical sites. Dr. Dressner notes that the surgery was further complicated due to scar tissue and a previous hernia from the
two previous colon resections performed through “open surgery” rather than a minimally invasive approach. Michael’s wish came to fruition: Dr. Dressner performed a colon resection surgery without the use of a colostomy bag. Since Michael is high risk, he remains on close surveillance and has follow-ups every 3–6 months for three years. “I’m feeling great now and am confident that Dr. Dressner and the team at Monmouth Medical Center are providing me with the best care possible,” Michael says. “While I can’t control my family history, Monmouth is helping me fight colon cancer and stay on course.”

Christopher A. Settles was losing his three-year battle with ulcerative colitis—a chronic disease of the large intestine that causes severe stomach pain and diarrhea. Medications were no longer helping, so the 21-year-old needed to have his colon removed. The good news: the Neptune man is now symptom-free and living a normal life, after a series of successful colorectal surgeries last year at Monmouth Medical Center (MMC). “I feel like I have a second chance. It’s a whole new perspective,” says Christopher, who recently returned to college and his job. He was not alone in this struggle: About 10 percent of patients with ulcerative colitis will need surgery to eliminate the disease.
Emergency care Diagnosed three years ago during his first semester of college, Christopher had been able to control his condition most days with medications. Then one night, he suddenly felt worse than ever. “I couldn’t eat, I couldn’t sleep, I couldn’t lie down, the pain hurt so much,” he recalls. Christopher’s father took him to the Emergency Department at MMC, where he was admitted and put on intravenous (IV) antibiotics. But when his symptoms didn’t subside, doctors determined he needed most of his colon (large intestine) removed. That operation would be the first in a series of three planned surgeries that would lead Christopher back to a regular, healthy life. After the initial surgery by MMC surgeon Ernest Ginalis, M.D., Christopher’s colitis pain was gone. But he would need to live with an ileostomy bag—an external bag that collects waste through an opening in the abdomen—for several months while his body healed. Three months later, Michael Arvanitis, M.D., FASCRS, section chief of colon and rectal surgery and acting chair of surgery at MMC, performed the second procedure—an intricate operation that included creating a “J-pouch” inside Christopher’s body, from part of his small intestine. The J-pouch would replace the ileostomy bag once the surgical areas had healed. For the interim, Dr. Arvanitis made a temporary detour in Christopher’s intestines, enabling waste to exit his body “upstream” from the J-pouch. That required creating a new
stoma—the opening from the intestine to the ileostomy bag— through a different site on Christopher’s abdomen. “My partner Dr. Roy Dressner and I are the only surgeons in the area who regularly perform these procedures,” notes Dr. Arvanitis, who is double board-certified in both colon/rectal surgery and general surgery, and is a clinical associate professor of surgery at Monmouth’s teaching affiliate, Drexel University School of Medicine.

Expanding care for the
youngest emergency patients Kids can say the darnedest things. So the thumbs up from children treated at the Thomas & Ann Unterberg Pediatric Emergency Department are extra special. Raj Kaur has two active children— which has meant many trips to the Emergency Department. And each time, the Holmdel mom has taken her kids to the Unterberg Children’s Hospital at Monmouth Medical Center, where they receive specialized care in the Thomas & Ann Unterberg Pediatric Emergency Department (ED). While other hospitals are closer to Kaur’s home, she’s willing to drive an extra 20 minutes or so to Monmouth Medical Center whenever possible. “They’re experts when it comes to kids,” says Kaur. “They’re super compassionate and they know how to deal with children. They treat you like family.” Plus, the pediatric ED recently began providing care around the clock—making it even more accessible to families from across the region.
Kid-friendly staff For Kaur’s son, his first visit came at age 2 after he hit his head on a toilet. “When the ambulance came, I requested they take him to Monmouth Medical Center,” recalls Kaur. “That’s where I went when I was a kid. And I feel more secure with an Emergency Department that specializes in children.” In 2014, Kaur’s kids both ended up in the ED at the same time. Her daughter Kaylyn Sidhu, now 11, fell and hurt her wrist at ten-

nis camp, while her son Dylan Sidhu, now 7, appeared to have a broken nose after getting hit with a hockey stick at sports camp. “When I picked them up that afternoon, I was concerned, so I took them straight to Monmouth,” she says. “The doctors in the Emergency Department were amazing. They were going back and forth between the two of them.” As it turned out, Kaylyn was the one with the broken bone—she had a fractured wrist. Lawrence Stankovits, M.D., a pediatric orthopedist at Monmouth Medical Center, quickly treated her with a cast, and the family continued to follow up with him at his office. “They’re amazing, from the technicians and nurses to the physicians. They know how to talk to the kids to keep them calm,” says the working mom, an executive at RWJBarnabas Health since 2013. “They were always coming in to check on us, offering to get us food. You don’t feel like a
number. My friends who have gone there all say the same thing.”
Outstanding care Last September, both kids returned again to Monmouth: Kaylyn came when she broke her ankle while cheerleading. Two weeks later, Dylan went after suffering a concussion during a football practice. As always, says Kaur, the service was “superior. Attend a religious service Frequent church or synagogue attendees have lower blood pressure than others, says a study published in the International Journal of Psychiatry in Medicine. Why? It could be that “people who attend services are exposed to sermons that provide guidance for healthy decision-making and coping with stress,” says Harold G. Koenig, M.D., of Duke University, one of the study authors. Group singing may also help cut stress, he says.

Fix that flaw A tiny red spot on the face? It could be broken capillaries easily repaired with a laser treatment by a dermatologist. Many cosmetic procedures are less expensive and invasive than you’d think. If a facial or skin flaw bugs you, check it out and decide if it’s worth corrective treatment. To Cleanse or Not to Cleanse?
Two dietitians weigh in on the pros and cons of a juice fast.
These days, it’s not surprising to hear of Hollywood’s leading ladies doing dietary cleanses. To lose weight for a role, Beyoncé once completed a “master cleanse,” limiting herself to herbal laxative tea, salt water and a concoction of lemon juice, maple syrup and cayenne pepper. Salma Hayek co-founded a company called Cooler Cleanse, which offers an assortment of organic fruit and vegetable juices and nut milks. And Gwyneth Paltrow endorses several detox programs and cleanses on her goop.com site. So what is a cleanse? In short, it’s essentially a semi-fast, most often involving a juice-only menu. It typically lasts anywhere from 48 hours to 10 days or more, and enthusiasts insist it rids the body of toxins while causing the skin to glow and the pounds to melt away. But before you head to the grocery store to stock up on juicy supplies, proceed with caution. Far from a surefire health booster, a juice cleanse or detox can actually be harmful. Laura Allen, R.D., a clinical dietitian at Monmouth Medical Center notes some of the drawbacks of juicing. “Given the natural sugar content of fruits, juice cleanses can exceed recommended sugar intake and can provide excess calories,” she says. Those on a cleanse, Allen adds, often do not get essential fatty acids nor the required amount of daily protein. “With juice cleanses, you’re drinking a diuretic that usually includes laxatives,” adds Jacqueline Ehlert-Mercer, Ed.D., M.Ed., R.D., who has lectured at several New Jersey colleges and universities. “You’ll lose a lot of water and electrolytes while depriving your colon of good microbes and bacteria. That’s not a positive thing to do.” Because the weight you lose in a cleanse comes from fluids, “the pounds will likely

come right back when you’re off the fast and eating normally again. You can get trapped in a yo-yo diet,” she cautions. For a long-term weight loss or management plan, go the old-fashioned route—eat regularly but in moderation, limit salt and sugar, drink plenty of water and exercise daily, Ehlert-Mercer recommends. Those with medical conditions and histories, such as diabetes, should be cautious about starting a cleanse, Allen says.
Ehlert-Mercer, who has authored several studies on cleanses, also believes juice fasts are a good way to rid allergens from the body. Both dietitians recommend that everyone, whether starting a cleanse or not, seek advice from a physician to determine the best weight-loss or detox plans.
A time to juice People often begin a cleanse to kick off health-related New Year’s resolutions or to flush impurities from their bodies, says Allen. “Typically after the holidays or after an extended period of unhealthy eating, individuals will start a juice cleanse to detox their body or kick-start the habit of eating healthier or to promote weight loss.” The benefits come from the daily increase in vitamins found in fruits and vegetables—the main ingredients used for the juices. Taking in more “greens” will also flush sugars from the body and potentially lead to a path of better eating habits once the cleanse has ended. “The ideal cleanse,” according to EhlertMercer, “is one that increases intake of organic fruits and veggies and offers plenty of fiber and vitamins.” The founder of Ramapo College’s Havemeyer Edible Garden, Ehlert-Mercer notes that one of the keys to an effective detox plan is fiber, which helps remove waste and toxins from the body.

Move over, quinoa—freekeh is quietly staging a takeover.
Tired of quinoa and want to shake up your meal plan? Then give freekeh a go. This ancient whole grain (pronounced free-kah) has long been a staple of Middle Eastern and Northern African diets but only recently began gaining popularity in the United States. Freekeh comes from durum wheat that’s harvested when it’s young and green and then fire-roasted. After the charred outer shell is rubbed off, the grain in the middle, which retains a subtle smoky, nutty flavor, can be boiled and used in dishes in place of rice, farro, barley, millet and other grains.
POWER UP Compared with superfood power-player quinoa, freekeh comes out on top with less fat (1 gram in ½ cup cooked compared to 2 grams in the same amount of quinoa), and more protein (6 grams vs. 4 grams) and fiber (4 grams vs. 3 grams). At only 130 calories per serving, it’s a better choice than rice or pasta for those hoping to lose weight. It’s also rich in magnesium, which helps with energy levels and digestion; iron, which oxygenates red blood cells; lutein, which promotes eye health; calcium, which benefits the bones; and vitamin B6, which boosts the immune system. Plus, freekeh ranks low on
the glycemic index, making it a smart choice for those who have diabetes or are at risk of the disease.
Buy/Store/Serve The grain is sold already removed from its shell and looks similar to barley or wheatberry. To prepare it, simply cook it as you would any other grain. Bring 2½ cups of water per 1 cup of dried freekeh to a boil. Lower the heat and allow it to simmer until the water is absorbed, and then fluff and allow to cool. You can also soak freekeh in water overnight and heat it up the next day. It’s available both cracked and whole—the cracked variety cooks in about half the time as the whole, similar to rolled oats vs. steel-cut oats. Once cooked, use the grain in salads, in soups, as a pilaf, or on its own as a side dish, served with meat or fish.
DID YOU KNOW? Freekeh is also called farik, which comes from the Arabic word for “to rub,” referring to how it is prepared. According to some sources, the grain was discovered two millennia ago after a Middle Eastern wheat field was set on fire during an ambush. Ingredients FOR THE CHICKPEAS n 1 cup beet juice n 1 Tbs. balsamic vinegar n 1 14-oz. can of chickpeas,
drained, rinsed and dried n Salt and pepper
FOR THE SOUP ½ small leek, finely chopped 1 garlic clove, chopped 1 celery rib, coarsely chopped 1 large zucchini (approximately 9 oz.), coarsely chopped n ½ Tbs. olive oil n 2¼ cups vegetable or chicken stock n 3 sun-dried tomatoes n 3½ oz. fresh white crabmeat n Few sprigs of parsley or tarragon, finely chopped n Squeeze of lemon juice n 12 oz. watercress n 1½ cups unsweetened soy milk

Directions To make the chickpeas, place the beet juice and balsamic vinegar in a small saucepan and bring to a boil. Simmer for 20 to 25 minutes, until the liquid has reduced to about ¼ cup and has a thick syrupy consistency. Preheat the oven to 400°F. Place the chickpeas in a small baking pan and pour over half of the syrup, mixing well. Season and roast for 10 minutes. Remove from the oven, pour over the remaining syrup and roast for another 10 to 15 minutes, keeping an eye to make sure the syrup does not burn. When ready, set aside. Sauté the leek, garlic, celery and zucchini in the olive oil and 1 tablespoon of water for about 5 to 8 minutes. Add a splash more water if necessary. Pour in the stock, then the sun-dried tomatoes. Bring to a boil and simmer for 15 minutes. Mix the crabmeat with the chopped parsley or tarragon, and then season with salt, pepper and a squeeze of lemon juice. Refrigerate until needed. Removed the sun-dried tomatoes then add the watercress and soy milk. Simmer for a couple of minutes to soften. Blend until smooth and season to taste. Serve, topped with a spoonful of crabmeat and a scattering of chickpeas.

carrot, coconut and ginger with cilantro sambal
Yield: 4 servings Calories: 227 per serving
Ingredients n 1¼ cups fresh coconut, grated n ½ tsp. cumin seeds n ½ tsp. coriander seeds n 2 shallots, diced n 1 garlic clove, coarsely chopped n 5 Tbs. ginger, peeled and grated n ½ tsp. chile flakes n ½ Tbs. coconut oil n 1½ lbs. carrots n 3 cups vegetable or chicken stock
Directions If using a whole coconut, prepare it by piercing the three eyes with a skewer, and drain out the liquid. Then, wrap it in a towel, place on a hard surface, such as the floor (or take it outside), and give it a firm blow with a hammer to crack it open. Use a spoon to gently lift the flesh away from the harder exterior and remove the brown skin using a potato peeler if desired.

Add the carrots, fresh coconut and stock. Bring to a boil and simmer for 30 to 35 minutes until the carrots are soft.

Toast the cumin and coriander seeds in a small frying pan for about 2 minutes, then grind in a mortar and pestle. Sauté the shallots, garlic, ginger, chile flakes and ground spices in the coconut oil and 1 tablespoon of water for about 5 minutes, stirring occasionally. Add a splash of water if the mixture sticks to the pan.
To make the sambal, place the cilantro, grated coconut, ginger and chile into a mini food processor and pulse to a coarse, dry paste. Stir in the lime juice and season to taste. When the soup is ready, blend until smooth, season and serve topped with the sambal.

Yield: 4 servings
Ingredients n 1 ½/3 cup black quinoa n Pinch of salt n 1½ cups unsweetened soy milk n 9 oz. smoked haddock n 1 bay leaf n 1 onion, peeled and halved n 6 peppercorns n 1 medium leek, finely sliced n 1 garlic clove, chopped n ½ Tbs. olive oil n 1 fresh corn on the cob n 3 cups vegetable stock n 5½ oz. spinach, finely shredded n 3 Tbs. chives, finely chopped

Directions Place the quinoa in a strainer and rinse under cold water. Transfer to a small saucepan, cover with 2½3 cup water, add a pinch of salt, bring to a boil and cook with the lid on for 10 minutes. Remove from the heat and leave undisturbed for 5 minutes so that the grains absorb any remaining water. Return to the strainer and run under cold water again to cool completely. Set aside until needed. Place the soy milk in a medium saucepan with the smoked haddock, bay leaf, onion halves and peppercorns. Slowly bring to a boil and simmer for 3 minutes. Remove from the heat and let stand, covered, for 5 minutes. Strain and set the cooking broth aside. Clean the saucepan and return to the heat. Sauté the leek and garlic in the oil and 1 tablespoon of water over low heat for about 5 minutes, until soft and translucent. Stand the corn upright and slide the blade of a sharp knife along the length of the cob, removing the kernels as you do so. Add these to the saucepan, along with the reserved cooking broth and the vegetable stock. Add the cob for added flavor. Bring to a boil, immediately reduce to a simmer, and cook for 7 to 10 minutes. When ready, remove the cob and used a slotted spoon to skim any foam that has formed on the surface. Add the cooked quinoa, along with the spinach and fish. Bring to just below boiling point, season to taste, garnish with some chopped chives and serve immediately.
