How he does it. “I only cut through skin, then push the muscle and tendons aside, without damaging them, and work through a very minimally invasive window to get to the hip,” Dr. Henderson explains. “There is very little bleeding and minimal pain.” Hip replacement typically puts a lot of stress on the knee joint as well, because the surgeon has to rotate the leg to the point where the foot is almost pointing backward. Not so with the Athello, which Dr. Henderson helped design in consultation with the manufacturer. It has a patented knee stabilizing system that protects the knee ligaments during external rotation. “I like to say that I don’t go to the hip; the Athello makes the hip come to me,” says Dr. Henderson.

An article in a magazine alerted Wendy Kaplan-Milstein to a better way to have her hip replaced. Maybe this article can do the same for you. Like her, you may undergo this minimally invasive procedure at Saint Barnabas Medical Center and go home the same day. Kaplan-Milstein, 61, is a Psychotherapist living in Chatham with her husband, Harold, 62, a Physician. She ﬁrst noticed hip problems while on vacation last October. “By November it was substantially worse, to the point where walking up the slightest incline was very painful,” she says. “And it got progressively more painful. I was limping, and I couldn’t straighten my leg. Some days it would make me cry.” The problem was diagnosed as osteoarthritis, the common degeneration of the soft tissues in the joint, which is associated with aging. Kaplan-Milstein lived with it for months, tried a cortisone injection— “the pain got worse rather than better,” she says—and was about to visit an orthopedist in May when she happened to see a local magazine article on minimally invasive hip replacement. It told of a lecture to be given by Timothy J. Henderson, M.D., an orthopedic surgeon at Gotham City Orthopedics. Surprisingly soon after her hip replacement procedure, Wendy KaplanMilstein was back to her demanding exercise routine. “People just don’t know about it, but they can be off crutches in a week and back to work in two weeks,” says Dr. Henderson. “I had a police ofﬁcer as a patient, and he was back to work in a month. This is a revolution in hip replacement surgery. The replacement your grandmother got, which made you afraid every time she tried to tie her shoes—that’s gone.”

This issue also reports on kidney transplants (page 41), free lung cancer screenings (page 40) and a woman who underwent successful surgeries for both brain and lung cancer—shortly after coming to work for the hospital as a receptionist (page 38). A-list celebrities apparently got the memo. Jennifer Lawrence, Kim Kardashian, Katie Holmes and many other stars have been spotted on the red carpet sporting top knots. And you should be wearing one too—if you have shoulderlength hair or longer—because it’s a universally flattering ’do. By adding volume at the crown, a top knot visually “lifts” the face, drawing the eye up and creating the illusion of more pronounced cheekbones. A top knot can be sleek and slicked back like Kim’s, relaxed and tousled like Katie’s or somewhere in between like Jennifer’s. They all start the same way—gather your hair into a high ponytail and secure with an elastic. Then twist the hair until it starts to coil around itself or divide the ponytail into two sections, twist them together and wrap the rope braid around the elastic. Pin the knot into place with a few bobby pins and you’re ready for a day at the office or a night on the town.

Five years ago, prolific New Jersey restaurateur Tim McLoone returned home to open a McLoone’s Boathouse in the area where he grew up. It’s appropriate, then, that the dishes on the extensive menu are upscale interpretations of classic comfort food: think mac ‘n’ cheese mixed with generous chunks of fresh lobster, succulent steak with crispy thin frites, and plump crab cakes with cloud-like whipped potatoes. But the sensational flavors of the food are almost outshined by the view. The restaurant’s dining room and outdoor seating area overlook the Orange Reservoir. Tip from the regulars? Cheryl D’Aries got a diagnosis wrong, but who can really blame her? She was only a brand new hospital receptionist, and the patient was herself. Recently hired, East Hanover resident D’Aries, 54, attended a new employees’ orientation at Saint Barnabas Medical Center last September and happened to sit next to new President and CEO Stephen P. Zieniewicz. The new director of thoracic surgery for Barnabas Health, Subroto Paul, M.D., was there too. She says she left the session “feeling really good about being part of Saint Barnabas.” But she wasn’t feeling so well—physically—a few days later. On Sept. 26, she began the day “just a little off.” Driving to work, she pulled over to call her husband to tell him something was wrong and discovered she couldn’t talk. “He asked if I was OK, and all I could get out was ‘No!’” she says. She drove herself to her office at the medical center and signaled to her co-workers to bring her a pen and paper. “I wrote S-T-R—,” she says. “I thought I was having

“It was surreal...But I knew I was in good hands. I had the ‘A’ team and guardian angels above me and beside me.” —cheryl D’Aries a stroke.” Doctors in her office rushed to her aid and called 9-1-1. She doesn’t remember the next two days. It wasn’t a stroke. Tests soon revealed that her symptoms were caused instead by a malignant tumor in her brain that had traveled from a primary tumor in her lung, perhaps related to her history of smoking. Following treatment at Saint Barnabas, she is feeling much better. Because D’Aries’ brain tumor was solitary, she was a good candidate for surgery, says Otakar R. Hubschmann, M.D., chief of neurosurgery. It did worry him that the tumor was in an area of the brain responsible for speech and movement, but new techniques, some of which were developed at Saint Barnabas, improved her chances of avoiding long-term disability. Using state-of-the-art, real-time magnetic resonance imaging (MRI)-based frameless stereotaxic neuronavigation, Dr. Hubschmann and his team were able to precisely pinpoint the lesion’s location. “A computer creates a 3D-image of where the lesion is, and we then measure several distances along the curve of the skull to find the exact spot,” he explains. This allows him to make a small opening in the skull. He then makes a tiny, 1-centimeter (less than ½-inch) incision in the brain itself, minimizing damage to surrounding tissue. An ultrasonic aspirator is inserted into the brain to emulsify and then extract the tumor. After she awoke, D’Aries learned that she had just had brain surgery. “It was surreal,” she says. “But I knew I was in good hands. I had the ‘A’ team and guardian angels above me and beside me.” Her speech came back quickly, and over the next few weeks follow-up scans and pathology reports continued to show that the surgery had gone well. “I kept getting good news, and I felt great,” says D’Aries, who lives with her husband, Michael, 57, a contractor, and has four children ages 17 to 27. “My family yelled at me for doing laundry and cooking when I should have been taking it easy.” She still needed lung surgery. So she walked every day to boost her strength. Three weeks after her brain procedure, she had her lung lesion removed by Dr. Paul—who remembered her from the orientation a month earlier. “She was sitting right next to our new CEO, so I kept seeing her there,” he recalls. “What an odd constellation of events to bring her

to me as a patient!” Dr. Paul was able to remove the tumor using a minimally invasive approach that required just three small incisions. She left the hospital the next day. “She was one of the fastestrecovering patients I’ve ever had,” he says. Now six months removed from the surgeries, she says she feels great and, even though her cancer was Stage 4, her prognosis is excellent. “I still stutter a little when I am excited,” she reports, but otherwise she has a lot to be thankful for. She returned to work March 1, and in July she and about two dozen members of her family are taking a vacation in the Dominican Republic to celebrate her good fortune—which began, she believes, when she became an employee and attended that orientation. “One part of the presentation that day was about how you never know what people are going through, so you should treat everyone with compassion and kindness,” she says. “We have to wear our name badge over our heart to represent this idea, and I thought that was great.” The way things turned out, the medical center staff quickly proved that that sentiment was more than just words. “Everyone at Saint Barnabas showed great kindness to me and to my family and friends—I will be forever grateful,” says D’Aries. “Now I’ll be able to see my daughters walk down the aisle.” —david levine

To be eligible, participants must be current or former smokers between ages 55 and 74 with a history of 30 pack-years or more. (A pack-year is the number of packs smoked per day multiplied by the number of years of smoking. For example, 1 pack a day for 30 years equals 30 pack-years.) Also eligible are those ages 50 and older who have a 20 pack-year smoking history and also one of these: n exposure to radon, asbestos, silica, diesel fumes, chromium or nickel; n a personal history of prior lymphoma or smoking-related malignancies such as head and neck cancer, bladder cancer or esophageal cancer; n a family history of lung cancer, or n a diagnosis of chronic obstructive pulmonary disease (COPD) or pulmonary fibrosis.

Lung cancer is one of the deadliest cancers—when it’s identified too late. But new surgical and oncological approaches have made early-stage lung cancer much more treatable and, in many cases, curable. To find these cancers early and get patients the care they need, the Lung Cancer Institute at Saint Barnabas Medical Center has joined the International Early Lung Cancer Action Program to offer a free, potentially lifesaving test to identify lung cancer in qualified patients. The program includes a low-dose computed tomography (CT) screening for individuals who are at high risk for developing lung cancer. More than 1,000 patients have been screened and about 20 cancers diagnosed since the program’s inception in 2013, reports Bridget Saggese, R.N., the patient navigator. “Most cancers were early-stage and curable,” she says. Patients identified as having cancer are treated by a multidisciplinary team, which consists of dedicated medical and radiation oncologists, pulmonologists, interventional radiologists and a thoracic surgeon. “We are dedicated to providing quality care and achieving the highest level of patient satisfaction,” Saggese says.

Leading the Northeast, Saint Barnabas Medical Center performed 301 of these procedures last year. Kidney disease is on a frightening rise. Among adults age 60 and older, it has more than doubled in the United States in the past 15 years. Experts predict that nearly 60 percent of Americans will develop kidney disease at some time—that’s higher than the lifetime risk of diabetes, heart attack or invasive cancer. Residents of New Jersey are fortunate to have one of the nation’s leading kidney transplant facilities—Saint Barnabas Medical Center. In 2015 surgeons at Saint Barnabas performed a record 301 kidney transplants, about 20 percent more than in a typical year, says Shamkant Mulgaonkar, M.D., chief of the Renal and Pancreas Transplant Division. The division has a dedicated team of eight transplant physicians, three transplant surgeons, a transplant urologist and a large team of transplant coordinators, social workers, a transplant pharmacist and dietitians. “That puts us in the top five transplant programs in the country in volume, and first in the Northeast,” he says, add-

ing that Saint Barnabas surgeons Stuart Geffner, M.D., and Harry Sun, M.D., are among the nation’s most experienced transplant surgeons. “We are helping a lot of people avoid or get off dialysis and achieve a higher quality of life.” There are several reasons why the incidence of kidney disease is rising. It’s often a byproduct of obesity and diabetes, and there’s an epidemic of each. These conditions can raise blood pressure and put overwhelming stress on the organs. In addition, the general population is getting older and living longer; aging itself is a risk factor for kidney disease. When coupled with the severe shortage of donor organs, these facts mean that most patients must wait an average of five years for a deceased-donor kidney. However, Saint Barnabas helps patients obtain organs from living donors—family members, friends and anonymous altruists—much, much sooner. “We started our Living Donor Kidney Transplant Institute to focus on promoting living donation through

a highly specialized approach,” Dr. Mulgaonkar says. “That has prompted a huge change. Last year, 135 of our 301 transplanted kidneys came from living donors.” His team conducts extensive research and participates in trials for new medications and surgical techniques. As a result of this and other research, transplant success rates have improved dramatically. Now almost 98 percent of living-donor kidneys survive the first year and nearly 90 percent survive five years, says the doctor. After last year’s success, for 2016 the focus is on growing the living-donor program even more. “We teach patients how to approach finding a living donor—you can’t just go ask someone to give you a kidney,” says Dr. Mulgaonkar. “We give them tools to have that conversation with family and friends.” He stresses that those who don’t have a compatible donor match may be eligible for exchange programs that link them with other candidates. “Even if you think you have no one who is compatible, we can help,” he says. —d.l.

To get blood to the rest of your body, your heart pumps it into the aorta, the main artery, through a valve that opens and closes, maintaining blood flow in the right direction. As you age, calcium deposits may cause that valve to become stiff and inflexibleâ&#x20AC;&#x201D;to the point where eventually it doesnâ&#x20AC;&#x2122;t open fully. This is called aortic stenosis, and it can cause a decrease in blood flow from the heart. Left untreated, it can lead to shortness of breath, loss of consciousness and ultimately death. Until the past few years, the only way to repair an impaired aortic valve was with open-heart surgery. Those procedures were very successful for otherwise healthy individuals, but since aortic stenosis tends to afflict older patients who also have other health concerns, the operation was more than many people could handle. For them, there was no reliable treatment. Since 2012, however,

patients at high risk for surgery have been able to take advantage of a minimally invasive procedure called transcatheter aortic valve replacement surgery, or TAVR. And in April, Saint Barnabas Medical Center opened a new operating room specifically designed to accommodate this cutting-edge procedure and offer it to patients. TAVR works in a fashion similar to the way interventional cardiologists insert a stent into a blocked artery. It involves threading a catheter through an artery in the groin—or occasionally through an artery in the chest—to the heart. Instead of removing the old, damaged valve, which is the approach used in surgical aortic valve replacement, TAVR advances a collapsed replacement valve inside the damaged, narrowed valve. The new valve is then expanded to push the old valve to the sides and regulate blood flow. Cardiac surgeon Mark Russo, M.D., director of the Aortic Center at Saint Barnabas, says the benefits are significant. “With open-heart surgery, you need to use a heart-lung machine, the operation takes two hours and the patient is in the hospital five to seven days with a recovery period of six to eight weeks,” he says. “With TAVR, we don’t stop the heart, we don’t cut out the old valve, the procedure takes about 30 minutes and patients commonly go home the next day or the day after. Most patients feel better when they wake up and are back to normal activities within a week.” More than that, TAVR reduces the risk of death within 30 days from aortic stenosis by 75 percent in these high-risk patients, who are not healthy to begin with. “This therapy prolongs survival and increases functional status, and it makes older people feel better,” Dr. Russo says. The new operating room where TAVR takes place is called a hybrid OR, because the procedure requires the skill sets—and the tools—of both a cardiac surgeon and an interventional cardiologist. The hybrid OR is ready for both methodologies. “The procedure is a collaboration between the interventional cardiologist and the cardiac surgeon,” says Bruce Haik, M.D, an interventional cardiologist who is the cardiac catheterization lab director for Barnabas Health. The team also includes cardiac imaging specialists in echocardiography to monitor the procedure as it happens in real time and in three dimensions. TAVR has been so successful in helping high-risk patients, it is now being studied as an alternative for low-risk patients as well—and the early results are promising there too. “These are overwhelmingly good outcomes,” Dr. Russo says. “Surgery is still a great option for patients. But eventually, TAVR will be the standard therapy for the overwhelming majority of patients.” —d.l.

the future of TAVR

Usually doctors don’t like to make predictions, but when it comes to the procedure called TAVR—transcatheter aortic valve replacement (see main article)—Saint Barnabas Medical Center clinicians are boldly bullish. “I think within five years it will displace openheart surgery as the gold standard of care for treating aortic stenosis,” says Mark Russo, M.D., director of Saint Barnabas’ Aortic Center. Interventional cardiologist Bruce Haik, M.D, agrees. “Until recently, the question was, ‘Who should have TAVR?’” says Dr. Haik, who is director of the Barnabas Health cardiac catheterization labs. “Now it’s, ‘Why would you send someone for open surgical aortic valve replacement?’” Dr. Haik and his colleagues at the Barnabas Health Heart Center would know. They recently celebrated their program’s 500th TAVR procedure. (This was done at Newark Beth Israel Medical Center, also a Barnabas Health hospital.) A party was held to mark this milestone, and several patients came—including an 83-year-old man who’d had the procedure just a few days earlier.

About aortic stenosis

Aortic stenosis—a narrowing of the aortic valve opening—occurs in about 2 percent of people over age 65. It is found more often in men than in women. Most people with aortic stenosis do not develop symptoms until the condition is advanced. The diagnosis may be made when a clinician hears a heart murmur and performs tests. Four New

ORs

How does a hospital show its commitment to the future? With words, for sure—a clearly articulated statement of mission is important. But action counts too. And these days, construction busily under way on the Saint Barnabas Medical Center campus is proof positive that the institution is growing to meet tomorrow’s needs and is supporting that effort with major resources. Four new operating rooms are being added that are designed to fit well not just with today’s latest technology, but also with the innovations of years to come. Surgeons, of course, want to perform in a state-of-the-art environment that allows them to use their skills to their fullest. That’s what the new facilities will do. “With these new ORs, patients in the region don’t need to go anywhere else in

New Jersey or New York for the highest-quality surgical care,” says Ronald Chamberlain, M.D., chairman and surgeon-in-chief of the medical center’s Department of Surgery. The new operating rooms represent a $14 million investment by Saint Barnabas Medical Center, with a budget for new technology of an additional $3 million, says Dr. Chamberlain. “Medical technology advances day by day, like new apps for smartphones,” he says. “These ORs are planned to anticipate new technology for the next decade in terms of their size and shape and the ability to accommodate new imaging systems. They are also designed to meet the future, as more and more procedures and specialties move from open surgery to more minimally invasive options.”

In all, the new ORs add about 30 percent more space, he says. This will allow surgeons to perform up to 40 more procedures a day on top of the 90 or so they were able to schedule before the expansion. And the improvements will enable the medical center to renovate existing rooms. Perhaps the showpiece of these new rooms is the hybrid OR. It allows surgeons to perform minimally invasive cardiac, vascular and neurological operations, with the option of converting quickly to open surgery should that need arise. “This OR in itself represents a $6 million investment,” Dr. Chamberlain says. The principal procedure performed here, at least for now, is transcatheter aortic valve replacement, or TAVR. (See page 42.) TAVR involves two surgical specialists, a cardiac surgeon and

The hybrid OR affords them and their assistants more room and has a specially designed operating table that converts quickly from an interventional to an open surgical platform. It also houses the high-tech imaging equipment needed for these types of operations. Another new OR is perfect for brain procedures. “It allows neurosurgery to move into the modern era as it makes the transition to minimally invasive and robotic brain surgery,” he says. This room is built to house an MRI imaging system, which allows neurosurgeons to perform what is known as stereotactic radiosurgery—a technique that despite its name actually uses radiation, not a knife, helping the doctor pinpoint the precise location of any tumor that needs to be removed. This means he or she needs only make small openings in the skull, thus avoiding cra-

niotomy—the removal of a portion of the skull—and its attendant risks of infection and longer healing time. The other two ORs are also better adapted to the future of laparoscopic and robotic surgery. Thad R. Denehy, M.D., a gynecologic oncologist at Saint Barnabas, says the new rooms, with more space and two surgical robot systems (including the very latest model, the daVinci Xi), are “very exciting. They give us more opportunities to operate, of course. These two rooms are much larger than our standard operating rooms and specifically designed for the robots, with all the amenities and materials and instruments in permanent rooms rather than being switched from room to room or in a converted room. The design helps maximize the work flow needed to use the robot, and can accommodate increased numbers of staff as well as the new large amounts of equipment required for some of the more state-of-theart and more complex surgeries.” The ORs also make video conferencing available, with video equipment built into the laparoscopic towers to transmit images of the surgery. Dr. Denehy also appreciates the improved ergonomics and support structures, such as booms, that make using the latest equipment easier and help prevent surgeon fatigue. Dr. Denehy says he performs more than half of his surgeries using the robot now. That number should only increase. “A study released in March that assesses the benefits of robotics compared 3,000 cases across four gynecologic surgical

approaches. It found that patients having robotic benign hysterectomies had better outcomes and fewer complications compared with those undergoing conventional operations. This was true even though they were often more complicated surgeries. They still had better outcomes and less pain. That is why robotic surgery is overtaking other approaches.” These ORs allow orthopedic and other specialist surgeons to perform cutting-edge robotic procedures too, Dr. Chamberlain says. “People come to where the technology is, and surgeons want access to that technology.” To be sure, one reason for these new ORs is to provide for more surgical volume, especially same-day and outpatient surgery, which is growing rapidly. The OR expansion is the first of three phases of growth at the medical center, Dr. Chamberlain says, and a new hospital building under construction will house some inpatient and outpatient services, for which these rooms will be used. Saint Barnabas Medical Center’s surgical roster includes 364 staff surgeons, he says. “The neurosurgery staff has grown by 700 percent and the thoracic surgery staff by 100 percent,” he says. About 25 new OR nurses, tech and support staffers have also been added recently. In all, these improvements put Saint Barnabas at a level equal to the best medical centers in the country. “They show that Saint Barnabas is investing in the future, and in better care and outcomes for patients,” says Dr. Denehy. —d.l.

Smoky poblano peppers are the perfect counterpart for sweet cherry preserves. The creamy brie brings them all together. n ½ tsp. olive oil n 1 whole poblano pepper, sliced n 1 clove garlic, minced n 1/8 tsp. salt n 2 tsp. butter n 4 slices hearty white bread n 4 slices brie cheese (rinds removed) n 2 generous Tbs. cherry

Heat olive oil in a skillet over medium heat. Add poblano peppers. Cook peppers for 3 minutes. Add garlic and salt. Cook an additional minute. Remove from heat. Spread ½ teaspoon of butter on one side of each piece of bread. Place two slices of bread buttered side down on a hot grill pan. Top with two slices of brie and a tablespoon of preserves on each sandwich, followed by pepper slices. Cover with remaining bread, buttered side up.

crab cake sandwich with artichokes and crème fraîche The problem with crab cakes is that you never get enough. So here’s a version that offers dependable crab-cake satisfaction whenever the craving strikes. n n n n n n n n n n

8 oz. lump crab meat ¼ cup mayonnaise 1 egg, lightly beaten 1 tsp. lemon juice ¼ tsp. salt 1 Tb. parsley ¾ cup panko Japanese bread crumbs 1 Tb. butter 4 small sandwich rolls 4 Tbs. chopped marinated artichoke hearts

In a bowl, combine crab meat and mayonnaise. Stir in egg, lemon juice and salt. Add parsley and panko and stir until combined. Do not overmix. Refrigerate mixture for 1 hour. In a large skillet, melt 1 tablespoon butter over medium heat. Shape crab mixture into 4 patties and cook for 5–8 minutes per side, or until golden brown. Place one crab cake on the bottom of each sandwich roll. Top with 1 tablespoon chopped artichokes and 1 tablespoon crème fraîche. Sprinkle ½ teaspoon of fresh tarragon over each crab cake. Top with roll top.

One cup of jackfruit provides 37 percent of your daily value of vitamin C and 25 percent of B6. It’s also a good source of potassium (21 percent), magnesium (12 percent) and fiber (10 percent). Its seeds, which are edible and can be roasted or boiled like chestnuts, are high in protein and low in fat. Added bonus: A cup of the sweet-tasting fruit contains just 150 calories, making it an ideal snack for waist watchers.

Jackfruit is available in many Asian markets, usually in a can. (For sweet dishes, look for the ripe variety preserved in syrup; for savory ones, opt for the young green variety preserved in brine.) It’s also sold dried or frozen, and you may be able to score a fresh fruit. To prepare fresh jackfruit, cover your work surface with newspapers to catch the natural latex that will ooze out of the fruit when you cut it. (It’s extremely sticky!) Next, coat your hands and knife blade with cooking oil to prevent the hard-to-remove sap from sticking. Cut the fruit in half and remove the flesh, which will be clumped together in bulbs. Use a paring knife to extract the remaining pulp strands from the bulbs and to slice open the bulbs and take out the large seeds. Try not to be dissuaded by the musky, onionlike smell— as you get deeper into the fruit, a sweet tropical aroma will take its place. In Indian and Thai cuisine, jackfruit has long been used in curries or soups. The savory kind is favored by vegetarians as a meat substitute because of its fibrous texture. Border Grill chef Susan Feniger, for example, famously served BBQ “pulled pork” jackfruit on boa buns. The sweet variety, which has a tropical fruity flavor, can be eaten fresh, juiced, turned into jam or whipped up into a dessert such as ice cream.

a beauty on the beach

Luxury has a Gallic accent at the Cheval Blanc St.-Barth Isle de France. By Rita Guarna

From top, should you tire of relaxing on the beach, a tranquil pool awaits at the Cheval Blanc St.-Barth Isle de France. Enjoy cocktails for two with a breathtaking view from this private ocean-facing perch.

IF THE HOTEL ST.-BARTH ISLE DE FRANCE WERE A WOMAN, you could say she married well. French luxury brand LVMH (Louis Vuitton Moët Hennessy) could have its choice of lovely “mademoiselles” when searching for a suitable match in North America, but the boutique hotel on Anse des Flamands, probably the prettiest beach on the island, won out and after a year-long engagement (read: renovation), it joined the exalted Cheval Blanc family. (Other family members include Courchevel in France and Randheli in the Maldives.) Perhaps the best part of the union is that it appears that little has changed at the beloved hotel, now called the Cheval Blanc St.-Barth Isle de France. The staff—the hotel’s heart and soul—remains the same: affable, accommodating to a fault and decidedly French. The only difference is that now they don charming seersucker uniforms. Similar too are the guest quarters—40 suites, bungalows and villas, which feel less like hotel rooms than the bedrooms of a Provençal family. The whitewashed country furnishings have been upgraded with the brand’s signature taupe plus accents of the palest salmon-pink, which you’ll find on everything from beach towels to pillows to glassware. Thankfully, the popular daily fashion shows remain too. They feature resort wear from the hotel’s closet-sized boutique stuffed with everything from Pucci bikinis and stylish caftans to straw hats and jewelry. Don’t be surprised if the model, Roxane, looks familiar: When she’s not strolling the sandy catwalk, she’s taking your dinner order in a charmingly halting English. Oh, yes, the food. As a French territory, St. Barts not surprisingly has a cuisine that reminds you of dining in the south of France. Chef Yann Vinsot oversees a pair of excellent eateries on this property: La Case de L’Isle, featuring sophisticated French-Caribbean plates (plus a 150-odd selection of wines and champagnes) and the more casual La Cabane de L’Isle, site of the fashion shows. Only eight miles across, St. Barts (short for St. Barthélemy and sometimes spelled St. Barth) is a hilly (thanks to a number of volcanic peaks) speck popping out of the northeast Caribbean in the French West Indies. Discovered by Christopher Columbus in 1493 (who named it for his brother Bartolomeo), the island was settled by the French and owned for a while by Sweden before returning to French control. It became chichi after the Rockefellers and Rothschilds fell for its charm in the mid-’50s.

With no direct flights, getting to St. Barts isn’t easy. Some folks fly to St. Maarten and take a short flight or ferry across. A more civilized crossing (read: easier) is flying to San Juan, then boarding a tiny puddle jumper. One of Tradewind Aviation’s multiple daily flights will do nicely. Its Pilatus PC-12s are comfortable and sturdy—important features as the eight-seater threads between two jagged peaks before touching down on a teensy runway. (Flights from St. Thomas and Antigua are also available.) While the atmosphere is relaxed, folks do dress to impress while shopping or dining in the capital city, Gustavia, with its yacht-lined harbor. (The island hosts one of the world’s most thrilling yacht races, Les Voiles de St. Barth.) Luxury brands abound along with unique boutiques, often outposts of exclusive Parisian designers. Prefer to test your sea legs with a more gentle cruise? Rent a catamaran with crew for a day-long or half-day tour. Our captain, Miguel of St. Barth Sailor, anchored in the bay near Colombier, where we swam to the beach (the only one of 16 beaches not easily accessible), after which we sipped champagne and nibbled on a gourmet lunch prepared by Cheval Blanc (lest we miss the restaurants’ gastronomic delights too much). Back on terra firma, we proceeded to Bonito, where fashionistas flock to enjoy a delicious Latin American menu alongside unrivaled views of the harbor from an open-air pavilion. If your “cruise” doesn’t offer enough of a respite, the spa back at Cheval Blanc will chase away any lingering stress. It offers signature Guerlain treatments. (It’s the only Guerlain spa in the Caribbean.) Try the Solar Escape, a body massage combined with a facial, or let a beauty coach choose an indulgent experience for you. Do you think you could enjoy an island with no casinos, no all-inclusives, no cruise ships in port? An island with no poverty, no crime, no beach vendors? With unfailingly polite people, awe-inspiring vistas, white sand (or shell or stone) beaches and top-notch cuisine? I do.
