AS YOU ENJOY THE LAST WEEKS OF SUMMER, KEEP IN MIND that the upcoming change of seasons brings a time for renewal. If you have let healthy habits “slide” these past few months—skipped workouts, indulged in too many desserts—fall is a perfect time to get back on track. Autumn, for example, is a wonderful time to exercise outdoors and enjoy the cooler temperatures. If knee pain is limiting your ability to live life to the fullest, perhaps it is time to visit the Joint Institute at Saint Barnabas Medical Center where total knee replacement surgery is performed, offering “custom” implants that exactly match the size and shape of a person’s knee. The benefits for the patient are less pain and a knee that feels much more natural. Read about the groundbreaking technology on page 40. Members of the community with Parkinson’s disease are exercising three times a week as part of a new program offered by Saint Barnabas Medical Center at the JCC MetroWest. These classes help participants build strength and improve balance, both of which are impacted by this debilitating neuromuscular disease. Turn to page 44 to learn more about this unique fitness and support program. Being overweight puts many limitations on people with diabetes, which is why we are pleased that the American Diabetes Association is now recommending weight-loss surgery as a treatment option for certain people with diabetes. Not only will bariatric surgery help diabetics lose weight, but it also will “cure” the disease in many cases. Read more about this life-changing procedure on page 36. Excess weight may also have the unfortunate side effect of impairing sleep, and it can lead to disorders like sleep apnea, which increases one’s risk of stroke and heart disease. But sleep apnea is a treatable medical condition, and the Saint Barnabas Center for Sleep Disorders can recommend technology or surgical procedures to reduce its impact. Turn to page 38 for more information about the complicated relationship between body weight and sleep. Berry season may be coming to an end, but there’s a plethora of other fresh produce just waiting to be picked at farms in and around Morris and Essex counties. Here’s a list of spots where “pickyour-own” is a popular and sometimes preferred harvest method. Take the entire family to Alstede Farms (1 Alstede Farms Ln., Chester, 908.879.7189; alstedefarms.com) for a full day of fruit and vegetable harvesting. Blackberries, peaches, nectarines and cantaloupe are in season through mid-September, while traditional autumn produce—apples, Asian pears, pumpkins—will be ripe for the picking from the start of fall through November. Admission is $5 during the week and $6.99 on weekends, and you pay for your fruits and veggies per pound. For your next home-cooked meal, bring back an assortment of fresh veggies from Bader Farm (290 Change Bridge Rd., Pine Brook, 973.227.0294; baderfarms .com). Pick-your-own crops include acorn and butternut squash, broccoli, cabbage, cucumbers, eggplant, sweet and hot peppers, tomatoes and zucchini. Looking for fall decorations? The farm has plenty

LOVE WHERE YOU LIVE Do you love where you live? Kinnelon resident Heather McManus wants to make sure that you do, and she has plenty of ideas on how you can transform an ordinary house into your dream home. If you haven’t visited Plum on Park recently, it’s time to return to the popular Montclair restaurant housed in a 1920s streetcar diner. For starters, owner Natalie Colledge has added a full dinner menu—try the pork chop Milanese, zucchini “spaghetti” or mussels and fries with sautéed chorizo—to go along with the popular breakfast, lunch and weekend brunch offerings. The cozy restaurant is still a haven for folks following a gluten-free diet, and now the dessert menu is safe for those with food sensitivities too. Once upon a time, traditional book clubs met in small groups to discuss the plots and twists of their latest reads. But the Green Brook Country Club’s version has taken the concept a step further, so much that New York Times best-selling author Erik Larson once called its gatherings and 400-plus membership a “book mob.” In fact, a meeting in mid-July that drew 350 attendees was considered a “small” event, says Marlene Cohen, who founded the club in 2005. So what’s the secret to their success? First, it’s open to anyone—country club membership is not a requirement. The second key is persistence to find and invite their guest authors. In addition to Larson, the club has attracted best-selling writers like Mitch Albom, Mary Higgins Clark, Amy Tan and actor Robert Wagner, among others, to the events. Celebrated writers Andrew Gross and Kristin Hannah have scheduled appearances in September. (Reservations for Hannah’s date are fully booked.) The book reviews draw the most visitors—they attract people from as far as New York City, Philadelphia and Connecticut, Cohen says—but the club also hosts luncheons and other social gatherings. bird-watching Attention ornithology enthusiasts: Our area is home to a wide variety of winged species and some great spots for ogling them. Learn more at njaudubon.org. Great Swamp National Wildlife Refuge (Basking Ridge): The spring and fall migrations bring the greatest number of species to this 7,600-acre site, including warblers and thrushes. It provides habitat for more than 240 species of birds, and several endangered birds breed here including American bitterns and northern harriers. Mills Reservation (Cedar Grove): This 150-acre oasis comes alive each spring with wave after wave of migrating warblers and each fall with the arrival of hawks. In fact, it’s one of the best spots in the state to observe the migration of warblers and hawks. Montclair Hawk Lookout (Montclair): From September through November you can visit this observation platform to view hawks in flight. October is the most interesting month to visit because it offers the greatest diversity of hawk species—from red-tailed to red-shouldered.

Rated as a moderate trail for mountain bikers, the West Essex “Rail” Trail is a 5.5-mile trail near Little Falls. Enjoy obstacles created by fellow mountain bikers and explore secret side trails. Boasting a total of eight trails, Mahlon Dickerson Reservation in Jefferson Township is appropriate for riders of all ages and abilities. Looking for a more challenging ride? Visit Hugh Force Park in Dover and experience a fresh dirt trail. For a full-body workout, head to the Traction Line Recreation Trail in Morristown, which features 10 fitness stations along the twomile trail that runs parallel to the New Jersey Transit Line tracks. Another option is the Flanders Park bike path in Flanders, which has it all—paved paths, training-wheel paths and a play area for the kids. If you don’t like crowds, pedal over to Loantaka Brook Reservation in Morristown. Stay cool during hot summer days under the trees and enjoy the marked paths.

If you’re not the type of person who reads medical journals, you probably missed the groundbreaking June 2016 issue of Diabetes Care, the American Diabetes Association’s (ADA) most important publication. For the first time ever, the ADA, along with 44 other international medical organizations, is “urging that metabolic surgery be recommended or considered as a treatment option for certain categories of people with diabetes, including people who are mildly obese and fail to respond to conventional treatment.” In other words, weight-loss surgery is now considered a viable treatment option for some people with Michael Bilof, M.D. diabetes. “This is big,” says Michael Bilof, M.D., section chief for bariatric surgery at Saint Barnabas Medical Center. One of the groups endorsing this change is the Endocrine Society, which has long been opposed to using surgery, he notes. “Endocrinologists are the doctors who typically treat diabetics, and they just came out with a position statement reversing that,” he says. “Surgeons have been advocating for this for a number of years, and for the endocrinologists to endorse this approach is significant.” Bariatric surgeons, like Dr. Bilof, have been seeing the remarkable effects surgery has on diabetes for some time. “I was convinced a long time ago,” he says. “Within a week or so of surgery, the patient’s diabetes is gone.” He mentions one of his patients, who had battled diabetes for upwards of 15 years. “She had

gastric bypass surgery four years ago and was off insulin within days and off her pills in a week or two,” he says. “That is pretty compelling, especially when you see that day after day.” Other groups, however, wanted more scientific evidence, and now they have it, Dr. Bilof says. One of the more convincing studies, called STAMPEDE (an acronym for Surgical Therapy and Medications Potentially Eradicate Diabetes Efficiently) and carried out at the Cleveland Clinic, “clearly showed that surgery was far superior, with very acceptable complication rates,” he says. (See sidebar.) “All surgery has risks, that is true, but we have shown that we can do it quite safely, with very low complication rates and benefits that far outweigh the risks.” According to the new guidelines, metabolic surgery should be recommended to treat type 2 diabetes in patients with a body mass index (BMI) greater than or equal to 40, as well as in those with a BMI between 35 and 39.9 when hyperglycemia is inadequately controlled by lifestyle and medical therapy. It should also be considered for patients with type 2 diabetes who have a BMI between 30 and 34.9 if hyperglycemia is inadequately controlled. But surgery to treat diabetes is not for everyone, Dr. Bilof says. “If you meet the BMI criteria and you are diabetic, you are eligible, but that doesn’t mean all should have it. But it should be considered for all, and a significant percentage of those patients would benefit.” How does surgery seemingly “cure” diabetes—even before the patient has lost a significant amount of weight? “We don’t know the mechanism yet, so there is no real answer. But clearly something is happening metabolically that goes beyond straight weight loss. Somehow that translates to resolution of diabetes,

and that’s what is amazing. It does not just improve—it usually goes away.” There are some patients who do see a recurrence of diabetes, he says. “But when it does recur it is invariably less aggressive and more easily controlled, with just pills instead of insulin shots,” he says. Dr. Bilof hopes that, in light of the recent statement by the ADA and other groups, primary care doctors and other physicians will begin referring more of their diabetes patients to surgeons to discuss their options.

A STAMPEDE of good results The final report from the STAMPEDE trial shows that bariatric surgery’s beneficial effects on blood glucose control in mild and moderately obese patients with type 2 diabetes may persist for up to five years, with the advantage over diabetes medications-only approach widening over time. The five-year follow-up also reported that: n More than 88 percent of gastric bypass and sleeve gastrectomy patients maintained healthy blood glucose levels without the use of insulin. n 29 percent of gastric bypass patients and 23 percent of sleeve gastrectomy patients achieved and maintained normal blood glucose levels, compared to just 5 percent of those on medication alone. n Weight loss was significantly greater with gastric bypass and sleeve gastrectomy than with medications and was the primary driver for glucose control. Too much weight, too little sleep Being overweight is a risk factor for sleep apnea.

We Americans, by and large, are tired. Most of us don’t get the recommended seven to eight hours of sleep a night. But too many Americans don’t realize their poor sleep may be caused by a medical condition like sleep apnea, a common disorder that causes your breathing to stop or get very shallow for a few seconds or minutes and may occur 30 times or more an hour. Kristin Fless, M.D., a pulmonary and sleep medicine specialist with The Center for Sleep Disorders at Saint Barnabas Medical Center, says that about 10 percent of the population has been diagnosed with sleep apnea. “But that could be grossly underestimated, because having a body mass index (BMI) over 25 is a risk factor, and an estimated 65 percent of Americans have a BMI over 30,” she says. Indeed, body weight and sleep have a complicated relationship. Excess weight may impair sleep, and too little sleep in turn affects weight. “A few days of sleep deprivation can change the way the body responds to food,” Dr. Fless says. “We get a response that mimics how diabetics respond to

glucose. We also know that patients who are treated for sleep apnea have a decrease in abdominal fat, which is the metabolically active fat. This is probably related to insulin resistance, which Kristin Fless, M.D. can be seen with that sleep deprivation.” Sleep apnea and other sleep disorders may cause even more serious problems. “If not treated, people with sleep apnea have eight times the risk of mortality of those without the condition,” she says. “Sleep apnea increases the risk for stroke, heart disease and motor vehicle mortality. That’s the danger if people don’t recognize that their fatigue could be a sign of a treatable medical problem.” Treatable is the operative word. The most common treatment for sleep apnea is called continuous positive airway pressure (CPAP). A CPAP device pumps air under pressure through a face mask and into the airway of the lungs, keeping the windpipe open during sleep. Other treatments include using a customized mouthpiece to hold the lower jaw and tongue forward to keep the airway open. “Newer technologies include a pacemaker-like device that stimulates the upper airway muscles,” Dr. Fless says. In some rare instances, surgery may be needed to remove oversized tonsils or other soft tissues. A sleep study is the best way to diagnose a sleep disorder, and patients have the option of conducting a study in their home rather than in a sleep lab. “We now have very good technology for home sleep studies with results comparable to lab studies,” she says. “We show people how to use the equipment, and they take it home and bring it back to the lab the next day.”

Creative outlets help patients cope with cancer treatment. Diane Genco calls her second bout with breast cancer “earth shattering.” Genco, 64, of Westfield, suffered a recurrence in October 2014 after a mastectomy in 2010. She hadn’t needed chemotherapy the first time, but required four rounds of chemotherapy at Saint Barnabas Medical Center to fight the relapse. Her husband, Gene Schaefer, coincidentally, is an oncology researcher, “so I knew how damaging it can be,” Genco says. “I was a basket case.” She had dealt with her mastectomy by writing a blog about her care, but now she was in what is known as “chemo fog.” “I couldn’t write an email, let alone a blog,” she says. Prior to her treatment, she had learned about Saint Barnabas’ holistic nursing program, which includes using art therapy to help patients cope with the stresses of cancer treatment. “I wasn’t going to do it, but after the first round of chemo my brain was a sieve,” Genco says. “So I went to a holistic nurse, who was wonderful, and she suggested the art therapy program.” Art therapy is used in a variety of settings, including hospitals, nursing homes and schools, with a broad group of people, says Donna Dandrilli, M.A., ATR, the art therapist at Saint Barnabas. “Art accesses the right hemisphere of the brain, where different types of information are stored,” she says. “Research shows art can help with healing, and with perceptions that might be stuck—it can ‘un-stick’ them, so to speak, and transform negative emotions into healing emotions.” That research, Dandrilli says,

includes a 2013 United Kingdom study that found 92 percent of adults with cancer who used art therapy found it helpful. A 2006 study in the Journal of Pain and Symptom Management reported that art therapy reduced a broad spectrum of symptoms, including fatigue, pain and anxiety, she says. “Making art takes your mind off the fact that you feel sick,” she says. “It lets you create something when you feel very much out of control. With art you do have control— of the materials, of how you want to work with them, of making decisions that have a positive end result.” Genco was skeptical at first. “I come from a family of photographers and artists, but I am the one in human services. I don’t do art,” she says with a laugh. That’s a common misconception among those presented with art therapy. “It’s not about talent or past experience at all,” Dandrilli says. “Art therapy is about the process much more than the product. We are not there to teach you how to draw, though we will help. It’s really about eliciting your own resources and creativity. In the medical setting, art therapy helps patients use the mind and body in a positive way, and that mind-body connection is important in healing.” Indeed, there is more to art therapy than making art. “When I met Donna, I had no idea what to expect,” Genco says. “We just talked about my family, my anxiety, it was really amazing. I started seeing things clearer just talking to her. I realized it was not just about the cancer, it was about me being a mother, a grandmother, a woman,

the number of friends I have, and I decided to put a positive twist on it all. If I can’t write about it, I will try to use art to capture what I feel.” Sister Carol Jaruszewski, R.S.M., has been doing the same since she was diagnosed with ovarian cancer in 2008. Sister Carol, 71, of the Sisters of Mercy, lives in Little Falls. Her cancer has recurred several times since then, but as a lifelong artist and former art teacher, she already knew the importance of art in her life. “When I realized art therapy was an option at Saint Barnabas, I took advantage of it,” she says. “It has been a real source of strength and hope—it has become almost like prayer. There are prayers that are memorized or rote; then there are prayers that come from the heart, a spontaneous expression to your god or higher power. In expressing yourself, whether it is joy, praise and gratitude, despair or depression, you are in a connection with something beyond you. That is what I experience with art. I am able to tap into a deeper place that gives me greater clarity.” Dandrilli is available Tuesday, Wednesday and Thursday from 8 a.m.–4 p.m. to work with cancer patients in an out-patient setting. The art they produce is generally optimistic, she says. “I think there are times when they feel angry, which is normal, or sad, and those feelings definitely come out at times. But as we do art, we also talk and explore feelings, and it becomes more of an insight for them. Even with a life-threatening illness, people tend to hold on to hope.”

Knifeless surgery Targeted radiation cuts out prostate tumors and leaves no scars. W h e n a d o c to r b e c o m e s a patient, you can be reasonably sure he or she will know where to turn for reliable information on the best treatment options available. So when Stephen F. Bergen, D.D.S., was diagnosed with prostate cancer, he did his due diligence. And the path he took led him to an amazing radiology machine called the CyberKnife. As its name implies, it acts like a surgical knife, but without any Alison Grann, M.D. actual cutting, scarring or blood loss. For certain cancers, like Dr. Bergen’s, it provides excellent results with minimal complications or side effects. Dr. Bergen, 71, is a maxillofacial prosthodontist, a dentist specifically trained to restore the teeth and other structures of the mouth. He works primarily with cancer patients, so he knows first-hand what cancer and its treatments can do. His cancer was discovered last fall, when he found himself urinating more frequently during the night. His PSA scores, markers for prostate cancer, were still in the normal range; nevertheless, his urologist performed a digital rectal exam and felt a nodule on his prostate. Follow-up scans done by ultrasound and magnetic resonance imaging and a biopsy confirmed the cancer. “My doctor recommended surgery as one option but left it open for me to explore different options,” says Dr. Bergen, who lives in West Orange with his wife, Juliet, who is also a prosthodontist. “I explored 12 or 13 different options for prostate cancer, and the one I felt—both in the medical literature and from what I heard from people I talked to—was the best option for me was CyberKnife.” A big reason, he says, was that his recovery time would be minimal, a plus for a doctor still working full time. “CyberKnife can be as effective as surgery,” he continues. “Even though you keep your prostate, it essentially kills the cancer cells, but there is no cutting involved, so no down time going back to work.”

Prostate cancer: Know the symptoms For many men, early-stage prostate cancer has no symptoms. He received treatments in May 2016 under the direction of Alison Grann, M.D., chair of the Department of Radiation Oncology at Saint Barnabas. The medical center installed the newest model of the CyberKnife this past January, making it one of the first hospitals in New Jersey to get the latest iteration. CyberKnife delivers very high doses of radiation over fewer treatments—up to four times the energy per dose from traditional radiation—she says. “For certain cancers, higher doses of radiation on each treatment area are more effective. Some types of cancer, like lymphoma, do better with low doses, but for certain cancers, such as prostate cancer, higher doses per treatment are more effective.” Early stage lung cancer, small brain tumors, melanoma and some pancreas and liver cancers also do well with CyberKnife treatment, Dr. Grann says. Dr. Bergen received five doses, preceded by the placement

of target markers on the prostate. Side effects from the treatment may include inflammation in the prostate and urethra, which can lead to temporary urinary difficulties. There is also a very small risk of bowel changes, and all prostate treatments can interfere with potency. Dr. Bergen experienced a little pain urinating at first, but nothing that ibuprofen and Flomax couldn’t take care of. The goal of his treatment is to see a decreased PSA level. But PSA numbers aren’t the only marker of cancer, Dr. Bergen says. “My PSA was never very high (3.7), so that would be my warning to men: Don’t just rely on your PSA.” And if presented with a decision like his, do your homework, Dr. Grann recommends. “The key to decision making is to understand your options,” she says. “Patients have choices and need to be well informed of those choices and their side effects. It is not one size fits all.”

Strengthening our minds and bodies An exercise and support program helps patients with Parkinson’s. Stuart Cooper, of Livingston, and Michele Sutton of West Orange, are both 74 and suffer from Parkinson’s disease. And they share one more thing in common—the two are regulars at the Parkinson’s Disease Support Program, a collaboration between Saint Barnabas Medical Center and the JCC MetroWest. The program offers exercise and support groups for patients with Parkinson’s and other neuromuscular/movement disorders and their family caregivers. Exercise classes are held three days a week. On Monday, it’s tai chi chih, a series of slow, controlled movements that focus on the development of an internal energy that the Chinese call chi. Wednesday’s class helps attendees build strength and improve balance, both of which suffer from the debilitating effects of neuromuscular disease. On Thursday, participants get into the pool for water exercises. “I don’t think we will vanquish the disease, but the classes improve strength and balance, and invariably I learn something,” Cooper says. The support groups meet one Friday every month. “We discuss new research and treatments, the challenges and frustrations the group members face, the importance of physical activity and nutrition, and how to manage the disease,” Cooper says. “We share what works and what doesn’t work. The caregiver support group is primarily spouses, and we talk about both the good and bad, as well as the frustrations they face as caregivers.” Both Cooper’s wife, Karen, and Sutton’s husband, David, attend that group. “Couples go through this disease together,” Sutton says. Occasionally, the groups invite guest speakers. “Michael J. Fox once said that Parkinson’s is the gift that keeps on taking,” Sutton says. “The support groups are a lifeline for me and my husband.” Part of that lifeline is made up of the connections made with others battling the disease, which is a primary goal of the program. “These programs have brought this community of individuals living with this disease closer,” Stephen P. Perfect for snacking, This fuzzy little green bean is a healthy Handful—and fun to eat too. If you’ve ever eaten at a Japanese restaurant, you’ve likely sampled edamame (pronounced eh-dah-MA-may). These young green soybeans are typically served in their fuzzy pod, steamed and sprinkled with coarse salt, and are eaten by dragging the pod between the teeth, releasing the beans. The nutty, buttery flavor and somewhat crunchy texture make edamame a popular snack or appetizer, but the beans are also low in calories, high in protein and fiber, and incredibly easy to prepare at home.

Power Up One cup of unshelled edamame rings in at just 190 calories and yields a hefty eight grams of fiber. It’s also a complete protein (meaning it contains all nine essential amino acids), boasting 17 grams a cup—about as much as three eggs. Edamame is an excellent source of folate, manganese and vitamin K too, as well as a good source of copper, phosphorous, magnesium, thiamin and iron. And, according to the American Institute for Cancer Research, it contains a variety of phytochemicals that may be beneficial in preventing and fighting cancer. So munch away!

Buy/ Store/ Serve Edamame can be purchased fresh, frozen and dry-roasted with various seasonings (sea salt, wasabi) for snacking right out of

the bag. The fresh variety—available during late summer at most Asian grocery stores—lasts only a day or two in the fridge. Once cooked, the pods can be stored in the refrigerator for up to a few days. Frozen edamame, on the other hand, will keep in the freezer for about a year. To prepare, boil the fresh or frozen pods for about four minutes, drain and immerse in an ice bath to stop the cooking. (You don’t want them to get mushy!) Sprinkle with coarse salt and, if you’d like, other spices such as garlic or cayenne. Or remove the beans from the pods (gently squeeze with your fingers to release them) and incorporate into your favorite dips, salads and stir-fries.

Some folks prefer to steam edamame, which is the way it is prepared in many Asian restaurants—purists argue this method retains more of the nutrients. To steam, place a steamer basket into a large saucepan and fill the pan with about an inch of water, making certain that the water does not touch the basket. Bring the water to a boil and place the frozen edamame in the basket. Cover with a lid and steam until tender, about two minutes. Another option: Edamame can be shelled and roasted (350°F for 30 to 40 minutes) with extra-virgin olive oil, salt and pepper.

Edamame comes from the Japanese words eda (branch or stem) and mame (beans), translating to “beans on a branch” or “stem beans” because the beans were often sold while still attached to the stem. Although edamame has been enjoyed in East Asian countries for more than seven centuries, it didn’t become popular in the United States until the 1980s when a TV miniseries, Shogun, brought about a boom in Japanese food and culture in California, according to the 2009 book History of Edamame, Green Vegetable Soybeans, and Vegetable-Type Soybeans. And the word edamame didn’t appear in the MerriamWebster dictionary until 2008. —Liz Donovan

DIRECTIONS n Start by marinating the shrimp: Put the shrimp into a large glass or ceramic bowl and add the juice and finely grated zest of 1 lemon, garlic, crushed red pepper, oregano and a good seasoning of salt and black pepper. Mix well, cover and let marinate in the fridge for a couple of hours. While the shrimp are marinating, cut the watermelon into chunks or slices and remove the skin. Arrange on a large platter, cover and chill in the fridge. Preheat a grill or a ridged grill pan on the stove over medium to high heat. Cook the shrimp for 1 to 2 minutes on each side until cooked through and nicely charred. Arrange the shrimp over the chilled watermelon and scatter with the feta and scallions. Drizzle the olive oil over the whole dish, top with a flourish of Greek basil or mint and serve with extra lemon to squeeze over.

Goat’s Milk Labneh with Carrot, Beet and Pomegranate Salad

Serves 4 to 6

INGREDIENTS n 18 oz. full-fat goat’s milk yogurt n Salt and freshly ground black pepper n Bunch of small carrots n Bunch of mixed small beets n 3 Tbs. olive oil n 3 garlic cloves, unpeeled n 1 sprig thyme, leaves picked n 2 Tbs. coarsely chopped flat-leaf parsley n 1 Tb. coarsely chopped mint n Seeds from 1 pomegranate n ½ cup sliced pistachios n 1 Tb. pomegranate molasses n 1 to 2 Tbs. pistachio or extra virgin olive oil n ½ tsp. ground sumac

DIRECTIONS n Season the yogurt with salt and black pepper. Line a finemesh nylon sieve with a cheesecloth and scoop the seasoned yogurt into the sieve. Suspend it over a bowl, cover with plastic wrap and leave in the fridge for 24 hours to allow the water to drain from the yogurt. The result is a soft cheese called labneh. n Preheat the oven to 375°F. Scrub the carrots and beets and remove the stalks and leaves. Cut any large carrots in half and add to a roasting pan, drizzle with the oil and season with salt and black pepper. Tuck the garlic cloves in with the carrots, scatter with the thyme and roast on the middle rack of the oven for about 30 minutes, depending on size, until tender. n Cook the unpeeled beets in a saucepan of boiling salted water until tender, drain and leave until cool enough to handle. Peel and cut into bite-sized wedges. n Put the carrots and beets into a bowl, add the chopped herbs, squeeze in the roasted garlic from its skin and season with salt and black pepper. Mix the veggies gently using your hands and arrange on a serving platter. Dot the labneh over the top and scatter with the pomegranate seeds and pistachios. Drizzle over the pomegranate molasses and pistachio or olive oil and season with a sprinkling of the sumac. Serve immediately.

Caramelized Mandarin Salad INGREDIENTS n 8–10 mandarins or clementines— choose firm ones, which are easier to peel n ¾ cup superfine sugar n Juice of 2 large oranges n 1 to 2 tsp. rosewater or orange flower water n Seeds from ½ pomegranate n ½ cup shelled pistachios, coarsely chopped n 2 to 3 sprigs mint, coarsely chopped

DIRECTIONS n For a more elegant look to the finished salad, rather than using your hands to peel the mandarins, cut a thin slice off the top and bottom of each and sit them upright on the cutting board. Using a sharp, serrated knife, and working from top to bottom, cut the peel away in strips using a swaying action. Try to cut all of the pith away, leaving the fruit in a near barrel shape. Cut each mandarin in half through the middle and arrange in a serving dish.

n Meanwhile, heat the orange juice until just below boiling in a separate saucepan or in a bowl in the microwave. Slide the caramel pan off the heat and carefully add the hot juice—the caramel will splutter furiously, so keep your hands covered by a cloth and your face away from the pan. Return to the heat to melt any hardened caramel, bring to a boil and simmer for 1 minute. Remove from the heat, add the rosewater or orange flower water and let cool for a few minutes.

n To make the caramel, put the sugar in a heavy-bottomed saucepan, add 2 Tbs. of cold water, and set over low to medium heat to dissolve the sugar without stirring. Increase the heat to medium, bring the syrup to a boil and cook until it becomes amber-colored, swirling the pan to ensure that it cooks evenly.

n Pour the caramel over the mandarins and stir gently to coat the fruit. Leave for 1 hour or so for the caramel to flavor the little oranges and vice versa. n Scatter with the pomegranate seeds, pistachios and mint, and serve.

This Caribbean hideaway is the perfect place to make like a stage star—or a sea turtle. By Rita Guarna

Before St. Vincent and the Grenadines—100 MILES west of Barbados—served as the setting for the swashbuckler film series Pirates of the Caribbean, this island chain with the cumbersome name was known primarily to yachties and seclusion-seeking music and movie stars. But the luxurious, all-inclusive Buccament Bay Resort no doubt will appeal to other Caribbean connoisseurs on the prowl for some under-the-radar fun. And the good times come in several notable flavors. The Harlequin Performing Arts Academy, for example, gives guests the rare opportunity to be coached (yes, it’s included in the price) in voice, acting and dance by world-class professionals. Pros live on site for three-month stints. Recently, Phil Cavill, an actor, vocal coach and West End star—who was asked to audition for and was cast as Jean Valjean in the West End production of Les Misérables, the world’s most successful musical—and Netta Evans, an actress, drama coach and founder of The Windmill Theatre School in London, were helping guests channel their inner divas.
