User Pathway Each user has followed similar pathways, depending on their needs: 

 

 

 

The user is given an initial assessment as to their needs. Using the assessment across projects allows for us to assess their wider needs and not solely address the immediate issue. The key worker for the project will meet with the user and create a plan of action, detailing the interventions we will use. Contact details will be passed, with the agreement of the users, to the external project staff who will contact the user to conduct a project specific action plan. If the provision of services requires the involvement of external agencies, the user is consulted throughout the entire process. During their engagement with the project, the lead project worker will maintain casework notes for the user. Evaluations will be conducted on a biannual basis throughout the duration of the intervention, as well as after specific activities, such as workshops or events. These evaluations will allow for families to express their feelings with regards to their current situation in a culturally sensitive way. Regular case conferences will take place every month to discuss the progress of the user through the intervention programmes. At the end of the intervention, a final evaluation and assessment is conducted to determine if any additional work is needed.
