This form is kept on file in the front office and contains emergency contacts, medical information, and consent for treatment. This form must be notarized, which can be completed in the front office. A new form must be completed each year. Must Sign and Return (REQUIRED).

AUTHORIZATION TO DISPENSE MEDICINE FORM (SEE PAGE 33)

This form is kept on file in the front office and gives the school permission to dispense prescription and nonprescription medication both on a regular basis, and as needed. A new form must be completed each year. Sing and Return (ONLY IF MEDICINE WILL BE KEPT IN OFFICE TO DISPENSE TO STUDENT).

LOCKERS & BACKPACKS Lockers are assigned to all students and are the property of the school. For security, lockers should remain locked at all times. Disciplinary action may be assigned to students who fail to keep their locker locked. A fine will be charged for all damaged and defaced lockers, including jamming the locking mechanism to prevent the locker from locking. There is to be no adhesive material (i.e. glue, tape, stickers, etc.) used on the outside of the lockers. Locker decorations are to be confined to the inside of the locker and affixed with the use of magnets. The school administration reserves the right to inspect lockers at any time. There are designated areas for athletic bags and equipment to be stored during the day. Backpacks are not permitted in the classroom.

TECHNOLOGY INFORMATION iPAD PROGRAM FOR ALL NEW STUDENTS

In order to aid in fulfilling the mission of CCHS, we have developed a school environment that includes iPads distributed to every student. The iPad is distributed before the school year. We have invested in management and filtering solutions that allow us to shape the digital experience of your student. At the conclusion of your four years at CCHS, the iPad is yours to keep. We distribute the iPads in a series of deployment days. Your student will need to attend one of these days to receive the device and gain valuable information on the basics of using it in our day to day classroom flow.

iPAD USE AND FINANCIAL AGREEMENT iPAD PROGRAM

Calvary Christian High School (CCHS) and (Parent/Guardian) enter into this agreement on behalf of (CCHS Student) on the date of the signatures below under the following terms: Students enrolled at Calvary Christian High School (CCHS) will be provided with an Apple iPad for the purpose of facilitating education. CCHS is leasing iPads from Apple on behalf of its students. As such, CCHS retains all ownership rights to the iPad until the student graduates or provides payment in full of the four year financial obligation upon withdrawal. CCHS may inspect the iPad and all stored information at any time with or without notice, and the student should not have an expectation of privacy as to anything stored on, sent by, or received through the iPad. The family will be assessed an annual technology fee of $250 per year $1,000 over four years, to help offset the expenses incurred by the iPad implementation, including lease of the device, software applications, manufacturer service agreement, and technical support. Upon graduation, the device will belong to the family. If a student enrolls at CCHS after the freshman year, then the family may choose to make the extra payment(s) to fulfill the four year financial obligation for the device, or the device must be returned to CCHS. If the family withdraws from CCHS before the senior year technology fee payment is made, then the family must either complete the full four year payment obligation, or return the device to the school.

STUDENT RIGHTS AND RESPONSIBILITIES

Many preference settings, such as the wallpaper or screen brightness, may be changed by the student. The purchase of any apps required by the school or course instructor will be managed by the CCHS I.T. staff. Students may not download or purchase apps on their school iPad without prior approval from the I.T. staff. Hacking or jailbreaking an iPad is not allowed. If a student’s iPad is found to be hacked/jailbroken, or if it is deemed that the iPad is being used inappropriately in any manner, the student will be subject to disciplinary action and possible financial penalties associated with harming the device.

DAMAGE OR LOSS

Students should report any damage to the iPad or loss of the iPad to the Technology Department, which will determine necessary action. All iPads are covered by a manufacturer service agreement in the case of defect or non-function due to manufacturer error or part failure in the course of normal operation. The service plan does not cover loss, negligence, or abuse resulting in damage to the device. If a negligent act results in a cracked screen, the family will be assessed a $50 fee for the replacement of that screen. The case chosen to protect the iPads has a very high protection rating. If the family chooses to use an alternate case, then the family will be responsible for all repair or replacement fees regardless of cause. If through negligence or abuse, the device is damaged to an unrepairable level or the device is lost, the family will bear the cost of replacement. A third-party voluntary insurance program will be made available to cover the costs incurred through damage or theft. Please see Safeware Insurance in this booklet.

STANDARDS FOR iPAD USE & CARE STUDENT RESPONSIBILITIES

• Bring the fully-charged iPad and charging unit to school every day.

• Keep the iPad within sight at all times, or stored in the assigned locker with the door closed and locked. • Secure iPad in the school-provided case. • Other than parents or guardians, do not let anyone use the iPad. • Adhere to the CCHS acceptable use guidelines for technology use. • Report any problems, damage, or theft immediately to the Technology Department. • Create a Passcode on the iPad and keep the Passcode confidential. • Use the iPad in a manner that is consistent with the school’s policy and mission.

GENERAL CARE

• Do not do anything to the iPad that will permanently alter it in any way. • Do not remove any serial numbers or identification placed on the iPad.

• Keep the equipment clean. For example, do not eat or drink while using the iPad.

• Clean the screen with a soft, dry, anti-static cloth, or with a screen cleaner designed specifically for LCD type screens only. Do not use paper towels, which may scratch the screen.

SCHOOL EMAIL AND APPLE ID

The student will be issued a school email account. This account will be used for all digital communication with the student. The address is in a Google account managed by CCHS technology staff, and it will be accessible from any internet-connected device. This email address will be used to create the Apple ID for school issued iPads. The Apple ID that is initially used to set up the device must not be changed, as a change in ID will result in the loss of certain functionality.

SUMMER PROGRAMS SUMMER STRENGTH AND CONDITIONING

Calvary provides an opportunity for students to participate in a supervised strength and conditioning program during the summer months in the school’s state-of-the-art training facility. The schedule below outlines the schedule for the summer. Students must have a current FHSAA Athletic Physical on file to participate in workouts. An FHSAA Physical Evaluation form will be on the following page for your use.

ATHLETIC FORMS In order to participate in Calvary sports, including tryouts, each student must submit an FHSAA physical (EL2). Consent and Release from Liability, Heat and Concussion and Consent and Release for Cardiac Arrest and Concussion (EL3). These forms are included on the next several pages. Calvary has partnered with AFC Urgent Care, Clearwater who will provide an athletic physical for only $15. They are located at 1500 N. McMullen Booth Rd. in the Publix Plaza about one mile north of Calvary. They are open from 8:00AM–8:00PM Monday–Friday and 8:00AM–5:00PM on Saturday and Sunday. No appointment needed. You must bring the forms (EL2 & EL3) with you to the doctor. Students who are entering Calvary after completing one semester or more in high school must also complete an Affidavit with Compliance with the Policies on Athletic Recruiting (GA4). *This form must be notarized.

A collared shirt with CCHS logo purchased form the Warrior Wear House. CCHS team or organization collared shirt may be worn if the color is white, navy, red, or gray. During the second semester seniors may wear shirts with an imprint of the college name and/or logo of the university/college where they have been accepted.

SWEATSHIRTS OR JACKETS

Only CCHS sweatshirts or jackets may be worn during the school day. If the shirt under the sweatshirt or jacket is visible it must comply with above shirt standard of dress.

Docker-style, made of cotton twill in navy, gray, black or khaki - (WHITE IS NOT AN APPROVED COLOR) purchased anywhere. No skinny, bellbottom, low rider, hip-hugger, or cargo pants (outside pockets) are permitted. A belt must be worn if pants do not fit securely around the waist.

SCHOOL UNIFORM ORDERING PROCESS DEADLINE TO GUARANTEE ORDER DELIVERY BEFORE THE FIRST DAY OF SCHOOL: AUGUST 1, 2018

AxCelMark is the provider for school day apparel for Calvary Christian High School. To order your students shirts, we have linked our website via www.cchs.us, the primary school website.

1. ORDERING PROCESS

• Open the website by accessing www.cchs.us on the navigation bar at the top: select “Student Life.”

• Under the Student Life tab: select “Parent/Student Resources.” • Under “Parent/Student Resources” double click the School Day Apparel option. You will be directed to the Warrior Wear House. We invite you to read the information on the “home page.” It will provide latest policy and important information relating to the ordering process. • Once opening the Warrior Wear House, scroll down the left-side navigation bars and double click your item category preference, i.e. polo, woven, etc. • The items displayed on the Warrior Wear House are the only approved items in compliance with the current uniform policy. The item prices include embroidery of the garment with the Calvary shield logo, Calvary Athletic Sword, or Large “C” logo for hats only. AxCelMark has already programmed its systems to adjust embroidery thread color with the garment color you select. • Make your selection by double clicking the item. The item will be displayed with more detailed information to help with finalizing your decision. Please click “VIEW SPEC SHEET” (in orange at the upper right of the screen) to determine item size, based on your student’s measurements. • Select the color and size then enter the quantity. To enter the item, click “ADD TO SHOPPING CART” (in orange) at the upper right of the page. • Once you have selected an item, scroll down and click “UPDATE CART.” Enter your next item and follow the same process. • Once your entries are complete, you will be taken to the verification screen, “VIEW SHOPPING CART” that lists all the items selected. • Next, complete your order by clicking “SUBMIT SHOPPING CART.” Complete the personal information entries for order processing and shipping. • The final step is to enter your payment information. Your payment will be processed by PayPal which is one of the most secure methods of processing payments for websites. You do not have to have a PayPal account to order. • You will receive a confirmation of payment from PayPal via email. Please save receipt for your records. *This completes the ordering process. 33

• When you receive your credit card statement, the charge will reflect AxCelMark Corporation as the source of transaction. We are the approved service provider for preparing Calvary’s school day apparel. • Operations and shipping: The Warrior Wear House website is operational year round: consequently, we are continuously processing orders. Your order will be shipped to the address listed in the “ship to” portion of your email order confirmation. Shipment tracking and status will be provided when your order ships. Outbound shipping is free for Florida orders only. We will provide a quote for out-of-state orders upon request. • Customer Service Support: Questions regarding apparel options and sizing should be addressed to our Customer Service Team. Please do not hesitate to call. Our phone contact information along with our email address is available via the “Contact Us” tab on the website. • Return Policy: Because each order is prepared exclusively for your student, there are no refunds or exchanges, so verify before ordering.

DROP-OFF/PICK-UP STUDENT DRIVERS DROP- OFF/PICKUP

Students in the 9th and 11th grades are to be dropped off/picked up at the south entrance of the CE building. Students in 10th and 12th grades are to use the west doors of the high school building for drop-off/pick-up. All doors are locked and students must have their student ID to enter the building. Students are not to use the front entrance doors to the school lobby. A map that shows the car line directions is included in this booklet.

STUDENT DRIVERS

Students driving a car to school must register the car with the school office and purchase a parking tag. A registration form is included below. Students are to display the tag on the rear view mirror at all times. Students will be assigned a parking spot. Students are required to drive in a safe manner and follow safe driving practices while on CCHS property. Violations are subject to school discipline.

LUNCH GENERAL INFORMATION

• Lunch Entrees are prepared on campus and served several days each week. Lunches are occasionally catered from outside vendors such as Chic-fil-a, Queens Pizza, and Buffalo Wild Wings.

• On Monday and Wednesday, lunch is catered from outside vendors such as, Chick-Fil-A, Queen’s Pizza, Buffalo Wild Wings, etc.

• A daily “self-serve” salad buffet includes a variety of fresh lettuces, vegetables, fruits, cheeses, meats and other salad bar items.

• Daily freshly prepared wraps, soups, pasta and fruit salads, peanut butter and jelly sandwiches, yogurt, chips, and other snacks, We will also have a variety of homemade desserts available each day.

• Iced water is offered in the café at no charge; additional drinks and snacks are available for purchase from our vending machines.

• Student lunches may not be delivered by an outside vendor.

• Student may bring packed lunches from home. CCHS does not provide refrigeration facilities for student use.

• Microwaves are provided by the school for students to heat lunches.

• CCHS operates as a closed campus; therefore, students will not be allowed to go off campus for lunch except for designated days for Juniors and Seniors

AFTER SCHOOL SUPERVISION

• After School Supervision is provided for students each day in a designated classroom. Students on campus 15 minutes after the dismissal bell must report to the room designated for supervision if he/she is not in an activity supervised by a coach, teacher, club sponsor, or other staff member.

• Students may remain in After-School Supervision from 3:10–5:30 PM for no additional cost. A student remaining from 5:30–5:45 PM will incur a $15 per day charge. A student remaining after 5:45 PM will incur a $50 charge per 30 minute increments.

• The athletic facilities, including the weight room, are for use by teams under the supervision of a coach. The athletic facilities, including the weight room, are not an “open” area where students may go after school dismisses unless they are a part of a team using the facility for a school-related purpose.

• On days when there is an athletic event on campus, students remaining on campus to attend the event must be supervised in the room designated for After-School Supervision until time for the event to start. Students may not congregate in the hallways or outside an athletic facility on campus waiting for the event to begin.

• A student remaining after school and not in a supervised location may be assigned to attend Saturday School ($25 fee) as a disciplinary measure for being in an unsupervised area.

BUS TRANSPORTATION PROGRAM Calvary currently offers bus transportation service from the Westchase area. The transportation is for the morning hours only. In the afternoon, students may remain in after-school supervision (no later than 5:30 PM) until parent transportation arrives.

Bus departs at 7:00 AM from the Publix parking lot located at 12139 W. Linebaugh Ave, Tampa, FL 33626.

ST. PETERSBURG AREA

Bus departs at 7:00 AM from the Suncoast Cathedral located at 2300 W. 62ND Ave, N. St. Pete, across from St. Pete Christian School

FEE

Family must register student for bus transportation service. The bus transportation may not be used on an as needed basis unless registration for the year has been made. There is an annual fee of $750 to participate in the bus transportation service. The fee may be paid as a lump sum or included in the monthly payment with tuition and related fees. ADMISSIONS STUDENT LEADERSHIP TEAM Admissions Student Leaders are a valued and integral part of the culture and ministry of Calvary Christian High School. These students firmly commit to support and promote the mission of the school and assume the responsibility of being a role model both on and off campus. Admissions Student Leaders serve as hosts for school events, student shadows, and school tours for prospective families as well as assist teachers and administration in various leadership tasks all while maintaining a minimum 3.0 GPA. While Admissions Student Leaders enjoy serving as a team and forming relationships with our staff, their participation in this program also affords them the opportunity to earn community service hours.

REQUIREMENTS FOR CONSIDERATION for the Admissions Student Leadership include:

REQUIREMENTS FOR CONTINUATION as an Admissions Student Leader include:

• Affirmation as a model student by CCHS staff at periodic intervals during the school year.

• Maintaining strong academic standing with a 3.0 GPA.

COMMUNIT Y SERVICE One of the goals we have for our students at Calvary Christian High School is to develop the Christian character trait of servanthood. Calvary encourages students to serve their community, their local church, and local agencies committed to serving those less fortunate. Therefore, a minimum of 100 hours of Community Service is one of the requirements for graduation.

THERE ARE SEVERAL FACETS ABOUT COMMUNITY SERVICE TO CONSIDER

• Work for the Glory of God (1 Corinthians 10:31).

• Scholarships may be tied to Community Service (Bright Futures, for example).

• Forms should be filled out completely by the STUDENT (not parent).

• Community Service forms are to be turned in the same semester as completed.

• Community Service is a great way to learn about possible careers - if you are interested in medicine, for example, volunteer at a hospital, nursing home, etc.

• Students may not earn community service by working for a family member or individual; it must be completed through an institution.

• Rising 9th graders may begin logging Community Service on June 1.

• Senior Privilege - Seniors must meet requirements stated in current year handbook to earn Senior Privilege.

• Documentation: In the Community Service description area we require a complete description of the service performed. We will no longer accept one word descriptions. For example, a form with “VBS or Basketball Camp” will not suffice. You must detail your role describing how what you did was of service to the community.

• CCHS will send out opportunities for community service as we learn of them. You are not limited to these opportunities.
