EXPLANATION OF ENROLLMENT FORMS

ALL STUDENTS MUST HAVE THE FOLLOWING FORMS ON FILE PRIOR TO THE FIRST DAY OF SCHOOL:

• School Physical, dated after August 1, 2020 (Athletic Physical is sufficient for this requirement) • Florida Immunization Card • Copy of Birth Certificate • Medical Information and Authorization Form (see below)

This form is kept on file in the front office and contains emergency contacts, medical information, and consent for treatment. This form must be filled out completely and notarized. Must sign and return to the Registrar or upload to Magnus.

ATHLETICS FORMS (See Pages 12–24)

These forms must be completed and either returned to the Registrar or uploaded to Magnus, if the student will be participating in athletics. Please note, the physical form must be completed by a physician.
