3D Coordination is a referral and information service for children with Developmental, Attention or Behaviour needs links families/whanau with available/appropriate services
3D coordination is a collaborative effort between education and health. The role of this service is to improve access to services for children with possible Autism Spectrum Disorder, Attention Deficit/Hyperactivity Disorder, or Intellectual Disability, by streamlining the process to make efficient use of health professionals’ time, and by facilitating community supports. The criteria for accessing 3D Coordination is on the back of the enrolment form. Save, print, or copy just the Agency Enrolment Form as necessary. The list of criteria is for your information and does not need to be returned. A parent may enrol their child using the form in the parent brochure; this is for parents who have concerns which you may not share or who you would prefer to initiate the process. Only one enrolment form, either parent, agency or GP is needed to initiate the process. The concerned parent, GP, SENCO, Early Childhood Manager or other health or community agency enrols the child with 3D Coordination, then the family is contacted to discuss the process and to seek permission to engage with the educator in the first instance. Please give the parent the brochure to explain about 3D and ask their permission to email Questionnaires to the child/young person’s educator. Once 3D has the educator’s response, arrangements will be made to interview the parent. The gathered information is reviewed and if complex, is presented to the Multi-disciplinary team. As appropriate, the child or young person may be referred to Secondary Health Services, recommendations made be to engage with social or educational agencies or given information on issues of concern to them. Feedback will go to the parents, GP and the enroller. Parent Information Brochures are available as required. Please contact 3D Coordination for further information.
