Community Care Coordination Update Kia Ora, The Bay of Plenty Community Care Coordination (CCC) demonstration site is in progress since March this year. The service triages and manages all district nurse referrals and referrals for the Body in Motion in Home Falls prevention programme. A significant proportion of the work undertaken by CCC is to send out funded routine wound care to Primary Care. As a result, CCC has managed to redirect 10% of district nurse wound care referrals to Primary Care. This is a very positive change as we try and get people to receive care at the right setting by the right provider at the right time. Please remember to inform CCC immediately if you are unable to accept the referral. CCC is establishing cohesive relationships with Primary Care partners and as the service evolves this will continue to be strengthened. As we go forward, CCC requests that Primary Care providers please be considerate when sending requests for district nursing services after 3pm for visits on the next day. Requests arriving after 3pm for next day visits places significant impact for district nursing services as they have to find appointments to slot people in. This is especially challenging for those people requiring home visits on the next day. We realise that that patients arrive late, hence it is unavoidable that referrals have to be sent late in some cases.
