Nutrition Services for Patients with type 2 Diabetes/ Prediabetes and CVD Risk Factors
Western Bay, Primary Health Organisation Dietetic service are on a mission to tackle lifestyle conditions such as type 2 diabetes and cardiovascular disease risk factors in the Western Bay. Currently around 5% of the NZ population have type 2 diabetes, around 18.6% of the NZ population have prediabetes and 1 in 3 deaths in NZ are attributable to CVD.

However,
Lifestyle change in people with prediabetes reduces their chance of developing type 2 diabetes by ~ 50-60% over 3 years and 27% over 15 years.
And

Studies show that lifestyle intervention that is holistically focused including sleep, stress, nutrition, activity and behaviour change can be effective in management of lifestyle based chronic disease such as type 2 diabetes and CVD.
Our self-management groups run by the dietetics services are lifestyle intervention, education groups to empower patients to make positive and meaningful changes to their health. This is in keeping with.
The Ministry of Health’s Quality Standards for Diabetes Care 2020 that state that all people with diabetes:
• Will receive high quality, culturally relevant, structured self -management education, tailored to their individual needs.
• Will receive personalised expert advice on lifestyle choices such as good nutrition and regular physical activity.
Our Mission:
Our mission is to offer every person in the Western Bay of Plenty diagnosed with type 2 diabetes or prediabetes the opportunity to attend a self-management group. These groups are developed and led by Dieticians and Nutritionists to help educate WBOP patients on their health and what to do about it. We empower patients to better understand and manage their condition and risk factors through lifestyle changes.
We can take referrals from GP’s, Nurses, HIPS, Health Coaches or self-referrals. These free groups are ongoing throughout the year and based in many locations across the Western Bay or online to capture as many of our community as possible. We are calling on all the health professionals of the Western Bay to help us to offer all relevant patients in our community this opportunity.
