Changes in Requests for Home Care Support Services (Non ACC) Health Network Communication for Covid-19 response This notification is being distributed to alert you to a number of changes to Home Care Support Services (HCSS) requests as part of the current COVID-19 healthcare response. These changes, detailed below, will help us plan and respond to the delivery of our HCSS during this time. They have been made in order to: 1. reduce exposure and spread of Covid-19 2. respond to a reduction in our available HCSS provider caregivers 3. respond to a potential increase in community requests for HCSS 4. reduce pressure on Acute services.
Key change GPs and community-based clinicians can now request HCSS direct though CCC (Community Care Co-ordination). No admission required

We need to reduce the risk of COVID-19 exposure to our workforce so we can continue to provide home-based care to those that need it most across the Bay of Plenty. To help minimise this exposure risk we will be reducing non-essential home-based domestic care to low risk clients. These decisions will be based on the person’s individual needs. We encourage family members who are isolated with other family members to help support them during this time. We want to reassure clients that we will be checking on them via phone throughout this time to remain updated on their situation and aware of any changes should they occur.
A diagram detailing the new process for requests is shown on the following page. Should you have any questions please do not hesitate to contact me.
