Practice Nurses’ Study Day Working Together To Promote Mental Well-Being.

Why? to develop skills for supporting clients’ mental well-being, support transition of mental health clients back to primary care, promote relapse prevention and improve communication and collaboration between primary care and mental health and addiction services.

Participants will be able to • Identify current Best Practice relating to Psychoactive IMI administration • Explain metabolic monitoring processes– in relation to mental health medications and “My Plan” Coordinated Care Plans • Access and utilise “My Plan” Coordinated Care Plans to find out about existing support goals, plans and people for clients. • Explain relapse prevention management – use of Wellness Plans, personal management strategies, introduction to risk assessment. There will also be networking and discussion opportunity Certificates will be provided for 6 hours of Professional Development
