Annual mammography screening begins at 40

very October we hear about Breast Cancer Awareness.

Pink ribbons everywhere, news stories, fundraisers and free gifts when you get your everso-pleasant mammogram. For those of us who have dedicated ourselves to finding breast cancer, our jobs don’t end on Nov. 1. In addition to reviewing mammogram after mammogram, now each containing hundreds of images since 3D mammography has become standard, we also spend a great deal of time performing breast biopsies and counseling patients, including the always challenging task of delivering the dreaded news that a patient has breast cancer.

The good news is that breast cancers found at screening (before symptoms and lumps develop) have the best prognosis and are the most curable.

Unfortunately, despite decades of scientific evidence that mammographic screening saves lives, we also spend quite a bit of time defending our specialty from naysayers. Every few

They analyze decades-old studies performed on obsolete technology, before modern training and oversight rules were developed. Last October this panel concluded that younger women may choose to wait until they reach 50 to begin routine mammography.

They also acknowledged that the maximum benefit of screening is achieved when it begins at age 40. However, they concluded that screening in that age group was not worth the “costs and harms.” They cited anxiety, false positive results and imperfect mammograms as well as economic factors. They even propose that some cancers may never become life threatening, so perhaps they should not be found or treated. Some of these concerns are reasonable, but if they had used modern standards, they may not have reached that conclusion. Many of the issues they mention are addressed by today’s 3D mammography, which is rapidly replacing traditional digital mammography (the panel studied data on images that weren’t even digital since many were performed in the

Cancer Center celebrates 10 years of personal, quality care

or more than a decade, the Marion L. Shepard Cancer Center has been changing lives in Beaufort County and beyond.

The center began as a small outreach clinic, branching out from East Carolina University’s School of Medicine, and a vision of Dr. Zack Waters. He reached out to Dr. Mary Raab, who is a top-notch oncologist, to lead the cancer clinic.

“He said, ‘Once you’re my patient, you’re my patient and you’re not going anywhere,’” Pam Shadle, public relations manager at Vidant Beaufort Hospital, said of Waters. “It was really just an across-the-table gentlemen’s agreement.”

First located in the hospital, Raab and nurse Geneva Everett saw patients for years, before being joined by medical director Dr. Jennie Crews in 2003 and moving into the state-ofthe-art facility in 2006.

divided into two sections: medical oncology and radiation. Its 25 staff members help with treatments for a lot of cancers, ranging from breast and prostate, to lung and blood (leukemia), according to Kim Ledoux, practice manager.

The Cancer Center offers almost all types of cancer treatment, with the exception of in-patient chemotherapy, in which a patient would stay overnight or longer, she said. It

includes a 12-chair chemotherapy infusion bay, a procedural room for minor procedures such as bone marrow biopsies and a six-room clinical facility, Ledoux said.

The Cancer Center has its own inhouse pharmacy, as well as on-site patient registration. These services make it easier for patients, as they don’t have to go between the center and the main hospital for treatment, Ledoux said.

Ledoux said she is also proud of the center’s work in clinical trials — work that she refers to as “cutting edge.”

“Our clinical trials staff are absolutely outstanding in the work that they do,” she said. “We have been able to provide services that I think we are very fortunate to be able to do.”

Cancer Center is known for serving the whole patient, and not only that, patients are recognized by who they are, not by what’s wrong, according to Ledoux.

Other holistic treatment options include yoga classes, massage therapy and a meditation garden for patients to get outside.

“We don’t want you to feel sick,” Ledoux said. “We need to promote your health.”

As for the future, Ledoux said she hopes to see the center reach out to even more people. The center is already partnering with health care providers in Aurora and Belhaven, and most recently has expanded to Hyde County.

The Vidant system has also debuted its MyChart program, in which patients can access their medical records online, and staff is continuing to adjust to the new system.

Ledoux said she would like to see nutritional services grow as a component of cancer treatment, as well.

“It’s everybody. Everyone is engaged,” she said of the staff. “I would like every person in Beaufort County, Martin County, Hyde County … to know how awesome this Cancer Center is, and that we have such a gem here in eastern North Carolina.”

3D technology brings improved imaging, biopsy From Eastern Radiologists

I

t’s no secret that breast cancer affects 1 in 8 women, or that it’s the second most common form of cancer in American women.

It’s also no secret that mammography is the most successful screening

tool for breast cancer — the only one that has consistently proven to reduce cancer deaths. But conventional digital mammography does have its limitations. That’s no secret either, particularly to the 10 percent of women who are called back after a mammogram.

Conventional digital mammography

produces two-dimensional images, yet the breast is a three-dimensional structure. It only makes sense that there’s room for error there. And that error comes in the form of overlapping breast tissue, especially in women with dense breasts.

Additional imaging often clears these suspicious areas, but false positives can understandably make women pretty anxious (and prove costly).

But studies show that it can cut the other way as well: Conventional digital mammography may miss up to 20 percent of cancers. Some cancers can hide deep within the tissue, completely obscured on a 2D mammogram.

Here’s where 3D technology comes in. 3D digital mammographyTM, or tomosynthesis, produces multiple consecutive “slices” of each breast, minimizing obstructions and improving visualization. The resulting images are more detailed and accurate, revealing structures that may otherwise remain obscured. By providing a clearer and more detailed image of the breast tissue in the first place, 3D mammographyTM is able to reduce the number of callbacks.

Tomosynthesis also has been shown to improve cancer detection,

especially with the more aggressive and potentially fatal invasive cancers.

In fact, recent studies show 3D mammographyTM detects 41 percent more invasive breast cancers than traditional 2D mammography. It’s most useful for women with dense breast tissue or a greater amount of glandular tissue in their breasts (as dense tissue patterns can limit the sensitivity of standard 2D mammography), but all women can benefit from 3D mammographyTM. “More information is better for cancer detection, and that’s what tomosynthesis provides,” said Dr. Ericka Griffin, director of breast imaging for Eastern Radiologists.

“It allows us to literally peel back the layers and view each one individually.

There’s overall less room for error, which is a significant advantage.”

3D technology’s impact on breast health doesn’t end there. Physicians are also using 3D breast biopsy to explore the most subtle lesions.

is simplifying hard-to-detect and difficult-to-access cases, facilitating greater accuracy and faster procedures — all at a lower radiology dose.

“We’re thrilled to have the Hologic 3DTM breast biopsy system,” Griffin said. “It’s the most advanced option available, and the first of its kind in eastern North Carolina. We pair this system with 3D mammography, which together yield a 21-percent improvement in biopsy predictive accuracy. It’s fantastic to be able to provide our patients with that kind of assurance.”

n average, every two minutes in the United States, a woman is diagnosed with breast cancer and about one woman will die from breast cancer every 13 minutes. It is estimated that in 2016 there will be 246,660 new cases of breast cancer diagnosed in the U.S. ‌ approximately 1 in every 8 women will be diagnosed with breast cancer. Worldwide, breast cancer is the most common cancer among women.

Breast cancer is the uncontrolled growth of cells in the breast. Breast cancer most commonly occurs in women, but does occur in men as well. Both men and women should be aware of the symptoms of breast cancer. Common symptoms include: lump in the breast, pain in the armpits or breast (unrelated to one’s menstrual cycle), redness of the skin of the breast, rash around the nipples, swelling, change in breast size or shape, nipple

discharge and/or change in nipple appearance.

Every woman wants to know how to reduce her risk of breast cancer. Some factors such as being a woman, age and genetics cannot be changed. However, factors such as being overweight, lack of exercise, smoking and eating unhealthily are factors that one has the power to change. When one chooses healthier options and improve their lifestyle, it helps lower the risk of breast cancer.

In addition to a healthy lifestyle, breast exams are very important.

Self breast exams and mammograms help detect a cancerous lump early, which increases the chances of survival.

Mammog rams are to be conducted by a physician practice, but self breast exams can be done in the comfort of the home. When conducting a self breast exam, lift the arm of the side being examined above the head. Then using the pads of the fingers,

move around the entire breast in a circular pattern; moving from the outside inward. Switch breasts and repeat the steps. If one notices any thickening, lumps or hardened knots, one should contact their medical provider. Self breast exams help people learn what is normal for their body/breasts, therefore if something was to change they would recognize it. If assistance with how to conduct self breast exams is needed, please visit or contact the Beaufort County Health Department. BCHD has models that show the motions of self-examining, as well as what a lump may feel like. Self exams are beneficial, but they alone are not enough. Mammograms should be conducted at least once a year. Mammograms help detect cancerous cells before they can be felt. Early detection is the best protection!

Beaufort County Public Health Department offers a Breast and

Cervical Cancer Program. This program provides cancer screening for low income women, ages 5064, who do not have Medicare B or Medicaid. As part of the program, women will also receive a pap smear, breast exam, basic laboratory studies and will be referred for a mammogram. What exactly is cancer?

C

ancer can affect anyone. Sometimes it strikes with no warning, while other times people may have a genetic predisposition. Various medical organizations say there are between 100 and 200 different types of cancer. Everyone has heard of cancer, but some are still unsure of what cancer is.

Defining cancer The organization Cancer Research UK defines cancer as abnormal cell growth.

Cancer cells are cells that divide in an uncontrolled way. New human cells normally grow and divide to form new cells as the body requires them. As healthy cells grow old or become damaged, they die off and new cells take their place. However, when cancer develops, this process goes haywire.

Damaged cells become even more abnormal and can survive when they would normally die. These cells keep multiplying and eventually can form lumps or masses of tissue called tumors. This is the case in most cancers, with the exception of leukemia, wherein cancer prohibits normal blood function due to abnormal cell division in the bloodstream.

Not all lumps in the body are tumors. Lumps that remain in place and do not spread to other areas of the body can be harmless or benign. According to the American Cancer Society, cancerous tumors are malignant, which means they can spread into, or invade, nearby tissues.

Cancer stages actually are determined based by how far cancerous cells have spread beyond their point of origin.

Cancer stages Cancer is staged according to particular criteria based on each individual type of cancer. Generally speaking, lower stages of cancer, such as stage 1 or 2, refer to cancers that have not spread very far. Higher stages of cancer, such as 3, mean cancer has branched out more. Stage 4 refers to cancer that has spread considerably.

Common forms of cancer Cancer can occur just about anywhere in the body.

Cancers of the breast, lung, colon and prostate affect males and females in high numbers.

Classifying cancer involves understanding where the cancer originated.

Cancer Treatment Centers

of America offers these classifications: ➤➤ Carcinomas begin in the skin or tissues that line the internal organs. ➤➤ Sarcomas develop in the bone, cartilage, fat, muscle or other connective tissues. ➤➤ Leukemia begins in the blood and bone marrow. ➤➤ Lymphomas start in the immune system. ➤➤ Central nervous system cancers develop in the brain and spinal cord.

Cancer is treated in various ways and depends on the cancer’s stage, type and effects

on the body. A person’s age as well as his or her current health status also may play a role in treatment decisions made by both the patient and his or her medical team.

Surgery may be conducted to remove a tumor, while chemotherapy employs chemicals to kill cancerous cells. Radiation therapy, which uses X-rays to direct radiation toward cancerous cells, is another potential cancer treatment. The side effects of each treatment vary, and there are ways to mitigate these effects.
