Mammography advances through digital imaging

M

ammog raphy and breast cancer screening have undergone many changes in the past 40 years. Until recently, the greatest advance was the advent of digital mammography which resulted in almost double the breast cancer detection rates in certain groups. 3D mammography (known as tomosynthesis) has shown to be an even greater advance. Just like CT scans, which have revolutionized imaging of the brain, lungs and abdomen, tomosynthesis produces images in “slices.” This allows breast imaging radiologists to see through dense tissues that can make mammograms difficult to interpret. 3D mammography systems create hundreds of images compared with just a few images from conventional mammography so specialized viewing equipment and training are needed to be able to accurately view these images.

The published studies on 3D mammography have consistently proven

that it results in a significant reduction in “false positive” readings and has been shown to find more small, early stage, breast cancer than conventional digital mammography. Dense breast tissue can still lower the effectiveness of mammography, but the 3D images have improved our ability to “see through” the dense tissue.

Since October 2014, Carolina Breast Imaging Specialists has performed thousands of 3D mammography cases. We have been amazed at the improved image quality and immediately noticed a dramatic decrease in the need for additional images. Even more exciting has been the significant increase in detection of small breast cancers. In fact, our false positive rate has dropped by more than 40 percent. Even though we have needed less extra images and return visits, we have found more than double the number of breast cancers since implementing 3D mammography. Our patients have universally appreciated the more comfortable system and are no

longer required to hold their breath during the exposures. All screening exams at CBIS are performed with 3D and are interpreted by experienced breast imaging specialists.

emember the old adage: an ounce of prevention is worth a pound of cure?

When it comes to cancer screening we are headed in the direction of catching lung cancers earlier, giving smokers some hope. An estimated 45 million people in the United States smoke cigarettes, and cigarette smoking is the leading cause of preventable death in the United States, accounting for 159,260 deaths in 2014.

We know that those who have never smoked have the least risk of developing lung cancer so if you do not smoke, count your blessings. If you are still smoking talk to your doctor about ways to help you quit smoking.

For current smokers who fall into high-risk groups, it is now recommended that they undergo a special CT scan to look for signs of early lung cancer.

These CT scans use lower doses of radiation to look for cancer cells. A lung cancer screening trial studied people aged 55 years to 74 years who had smoked at least one pack of cigarettes

per day for 30 years or more. Heavy smokers who had quit smoking within the past 15 years were also studied. The trial used chest X-rays or low-dose CT scans (LDCT) to check for signs of lung cancer. The CT scans were better than chest X-rays at finding early lung cancer. Screening with LDCT also decreased the risk of dying from lung cancer in current and former heavy smokers. The risk of dying from lung cancer was decreased by 20 percent in those screened using CT scans.

What are the benefits of being screened for lung cancer? The main benefit of screening is that it helps doctors find cancer early. In these cases it should be easier to treat the cancer, and treatment may lower your chances of dying of lung cancer.

So what are the drawbacks to being screened for lung cancer? The drawbacks include receiving false positive results, which could lead to additional, more invasive tests. More tests may mean exposure to more radiation. All X-rays, including CT scans, expose you

to some radiation. Although the radiation dose from the first screening CT scan is low, you would need to have a scan yearly for three years, so your radiation exposure will slightly increase.

Of note, exposure to secondhand smoke was not identified as a risk factor that would warrant CT screening. For previous smokers, those who had quit within the past 15 years or those with a 20 pack/year history (a person who smoked one pack a day for 20 years, or who has smoked two packs a day for 10 years), with an additional risk factor (like diabetes, congestive heart failure or emphysema) could be screened.

If you, or a family member, are at risk for lung cancer, speak to your doctor about the benefits and risks of being screened. It would be helpful to have this or a similar article with you at the time of your discussion. Do not be timid about asking these important questions. It could save your life.

What is breast density?

Breast density describes the proportion of glandular tissue and fat within the breast. Breast density is not a measure of how the breasts feel, but rather how the breasts look on a mammogram. Glandular tissue includes lobules, ducts and connective tissue, and these appear as white structures on the mammogram, while fat appears black. High breast density indicates more glandular tissue than fat, while low breast density means there is more fat than glandular tissue.

Why is it important to know my breast density?

How do I find out my breast density?

Dense breast tissue lowers the sensitivity of mammography, or the ability to detect cancer. This is because cancer can appear as a white spot on the mammogram, similar to the appearance of dense breast tissue, making it difficult to distinguish from normal tissue. Secondly, according to studies in the New England Journal of Medicine and Journal of the National Cancer Institute, women with high breast density are more likely to get breast cancer than women with low breast density.

Many states, including North Carolina, have breast density notification laws requiring physicians to notify women if their mammogram shows dense breast tissue through the letter you receive in the mail, as well as the report your doctor receives. These laws intend to provide women the necessary information to decide on further action. The first state to pass breast density notification legislation was Connecticut in 2009, with 23 other states following suit, including North Carolina in January 2014.

Women with dense breast tissue should discuss their personal risk with their physician. This determines the need for additional screening tests or modified screening regimens. Factors increasing the risk of developing breast cancer include personal or family history of breast cancer, a known genetic mutation such as the BRCA 1 and 2 genes,

the use of oral contraceptives or hormone replacement therapy and lifestyle factors, such as obesity and excessive alcohol consumption. Women with a greater-than-20-percent risk of developing breast cancer based on these factors should undergo high-risk screening.

For any woman with dense breast tissue, the newest advance in breast imaging, 3D mammography or breast tomosynthesis, improves screening for breast

cancer. This is achieved by taking a series of images through both breasts, eliminating overlapping dense breast tissue. This allows for a clear and more accurate view of the breast, decreasing repeat visits and increasing the detection of small invasive cancers. Furthermore, the series of images for a 3D mammogram is obtained similar to a 2D mammogram, providing more information without changing the time in compression.

ancer. It’s a word that has long been associated with a death sentence. One of the world’s leading causes of death, it’s a disease that not only affects the diagnosed but also their families.

But today, cancer is not quite the omen of death it once was. And with several local initiatives raising both funds and awareness annually, the fight against cancer has become one the community is determined to win.

Carried out by masses of volunteers, the fundraisers directly benefit patients in the service area of the Marion L. Shepard Cancer Center.

Darleen’s Flamingo 5K is an annual fundraiser that donates all its proceeds to the center. The charity run immor-

talizes Darleen Smith, a Smithton woman, who lost her life in 2010 after a five-year battle with breast cancer.

Since, her children have spearheaded the initiative, bringing area runners out to participate, according to Nikki Klapp, Smith’s daughter.

“My brothers and I wanted to do something in (Smith’s) memory where the funds would also support patients of cancer,” Klapp said. “This initiative is important to me and my family because not only does it support cancer patients in Beaufort County, but it also helps bring the community together and that was a big thing my mom was passionate about.”

More than 200 participants, 40 volunteers and local business sponsors make the event possible each year. Last

year, $17,000 was raised from community participation, according to Klapp.

This year, the run will be held Oct. 17.

“Each year, my family is just humbled by the amount of support we have,” Klapp said. “We’re blown away at how giving the community is.”

The run isn’t the only program designed to advocate for local cancer patients. In accordance with October’s National Breast Cancer Awareness Month, Eastern Radiologists of Washington hosts its annual Paint the Town Pink event. The event raises money to benefit the center through the sale of T-shirts commemorating the event and encouraging area residents to wear pink, according to Tanya Dixon of Eastern Radiologists, who chairs the event. Local businesses also join

in as Pink Partners, permitting their employees to dress down and wear the T-shirts the day of the event and offering discounts to patrons who wear pink. Last year, it raised $10,500.

“We are raising money for those who have already been affected by cancer, but also this needs to bring awareness to women to get their breast exams done and take care of themselves, as we are taking care of the people who have already been affected,” Dixon said.

Other initiatives, such as Relay for Life, which benefits cancer research regionally, and the Shepard Cancer Foundation’s Let’s Tee It Up For a Cure golf tournament, Hope and Heroes Walk and the sale of purple bows, are held annually, as well.

The golf tournament and other components of the overall initiative raise tremendous amounts of money for patients in the area. This year, the three-tiered program garnered around $75,000 and continues to grow each year. Masses of community volunteers, much like other events held in the area, make the events possible, as do two local golf clubs — Washington Yacht & Country Club and Cypress Landing — whose members and residents head the initiative each year.

According to Pansy Champion, president of the foundation and an ovarian cancer survivor, the money yielded from the local events ensures a wide range of services, programs and therapies for patients seen at the center. This month, with a focus on

breast cancer awareness, the center is offering free breast cancer screenings in three main locations throughout Beaufort County.

The center will also host its annual Pink Power luncheon, an education program about how early detection, regular screenings and advanced treatment options can provide a good amount of power against breast cancer. The program includes a healthy meal, a Pink Power kit and guest speaker Mamie McCullough, a breast cancer survivor. The program will be held Oct. 13, from 11:30 a.m. to 1 p.m. at First United Methodist Church in Washington. The event is free but registration is required.

The need for prostate cancer screenings is different for everyone

I

t seems that every month there is more controversy in the media on prostate cancer screening. Believe me, medical professionals are just as confused as is the public. Depending on which medical advice you listen to, screening is either a good idea and recommended, or it is not reliable and should not be performed. What should you do? There is unfortunately no easy answer, but the more you know about prostate cancer and the risks and benefits of screening, the better you will be able to make your own decision. I have this discussion with patients several times a week, and I will give you the same advice that they receive. I thought it might be most useful to do this in a question-and-answer format, much as it would be at an office visit.

Should I be screened for prostate cancer?

This is the most common question and the most difficult to answer. Why? Because not even the different medical societies can agree. Obviously those at increased risk should strongly consider being screened. This would include those with a close family member with prostate cancer, such as a father or brother. These individuals carry a genetic risk of developing prostate cancer earlier and a more aggressive type. African-Americans also have a higher risk of prostate cancer. These individuals should consider having at least a baseline PSA at age 45. As urologists, we recommend all patients have a baseline PSA starting at age 50.

However, the U.S. Preventative Task Force has previously recommended to not perform routine screening in

the general population. They state that to find one “significant” cancer, 1,000 men must be screened. Note that I said “significant.” This means a cancer that would potentially be lethal if untreated. Screening would find more cancers, but some of these would be slow-growing and in men who, because of their advanced age or poor health, may well die from other causes before their prostate cancer would ever grow. The American Urologic Association has since revised their guidelines on screening. Although they still recommend screening, the frequency and timing of PSAs has changed.

What does screening mean?

We recommend a PSA blood test as well as a rectal examination. Many

patients want to know if they can just skip the rectal exam. Unfortunately, not all cancers raise the PSA level, and the only way they can be diagnosed is by examining the prostate. I will find a few cancers every year in this way.

The good news is that this only has to be done once a year. Believe me, I moan and groan just as much as the next guy when it is my turn. It’s just one of those things we have to do.

What is a PSA? Simply put, PSA is a protein in the bloodstream. Normal prostates make it in small amounts; prostate cancer makes it in large amounts. The normal range is from zero to four. Any number above four is abnormal but does not necessarily mean you have cancer. PSA is a wonderful test in that we can pick up a cancer very early, often months to years before we would find it at a rectal exam. However, that sensitivity comes at a price. Other things can affect PSA. Recent sexual activity, prostate problems such as difficulty voiding and particularly infection may affect the level.

cancer was detected by early screening did have a higher survival rate. I feel that some of the controversy about PSA has to do with how your doctor interprets the test. What that means is that you have to interpret the PSA level as it applies to each individual. A PSA of four in a 41-year-old man means something totally different than it does in a 77-year-old man. Unfortunately, you cannot apply the zero-to-four scale in the same way for each patient.

This is where you should discuss your PSA with your physician. Not everyone who has a PSA more than four needs or should have a biopsy. I have heard that almost all men, if they live long enough, will get prostate cancer.

Your lifetime risk of developing prostate cancer is about one in 11. Does that mean that all those men will need treatment? No. Again, each individual must be individually considered. Your age, any additional medical problems, family longevity and aggressiveness of your cancer all weigh in on the decision to treat your cancer. Many patients who have prostate cancer will never require treatment, and their cancer will never be a threat to them. That is a discussion your urologist and you would need to have.

Opponents to screening point out that some individuals will have biopsies even though they don’t need them because they don’t have cancer. Unfortunately, that will always be true because nothing in life is 100 percent. I try to be as sure as I can that when I do a biopsy, it is as absolutely necessary as possible. However, the happiest patients are those with a negative biopsy.

If I have cancer, what can be done?

The good news is that today there is even more that can be done than ever. From watchful waiting to surgery to ultrasound to radiation to robotics, the choices are myriad. Once again, every patient is different and what might be best for your neighbor may not be best for you. This requires a long discussion with you, your urologist, possibly oncologist or radiation oncologist. Suppose the entire population of Beaufort County died of the same disease in just one year. As we try to comprehend the magnitude of that we should realize that this is actually happening in other ways. Each year in the United States nearly 50,000 people die of a preventable illness. Yet we still find people who won’t take the simple steps to prevent getting this disease.

Colorectal cancer is a silent killer. The sad reality is that most cases of colon cancer are preventable. The good news is that screening exams to detect colon cancer are effective and are readily available.

In my practice there is hardly a week that goes by without my identifying a new colon cancer in a patient. What is frustrating is that upon questioning, many of these patients had the opportunity to undergo colon cancer screening in the past but elected not to. In up to one-third of the patients, I find pre-cancerous polyps that are easily removed at the time of their procedure. These patients have saved themselves from a potential cancer.

So what is the problem?

The simple answer is that despite widespread availability of screening tools, only about a third of eligible patients actually have a screening test performed. The rest refuse, ignore their physician’s advice or choose to take their chances.

Below are the most common colorectal cancer screening tests as advocated by various national organizations on colon cancer prevention and detection. These guidelines apply to both men and women as both genders are equally susceptible to the consequences of this disease.

1. The simplest screening test is known as the fecal occult blood test.

This is actually a quick test to determine if blood is present in your stool.

It is easy to do, but unfortunately doesn’t detect non-bleeding polyps and has limited value.

2. A second available test is a flexible sigmoidoscopy exam. This is where a thin flexible lighted tube is advanced into the rectum and distal colon. This is usually done in an office setting and

often without the need for a sedative.

Its main drawback is that it typically views less than a third of the colon and rectum. More proximally located polyps might be missed 3. The “gold standard” in colon cancer prevention is to perform a colonoscopy. This is an endoscopic procedure designed to look inside the entire colon with a long flexible lighted tube. It is a very safe and relatively comfortable procedure (aided by sedation). With colonoscopy, most identified polyps can be safely removed during the procedure. This can significantly reduce the incidence of colon cancer. This is the preferred screening test for detection and removal of colon polyps.

Colonoscopy should generally be performed initially at age 50, and then every 10 years if no polyps were seen. If there is a family history of colon cancer or polyps, an earlier colonoscopy might be advised.

African-Americans are advised to start colon cancer screening around age 45.

Obviously, screening exams are designed to find disease at an early

stage before symptoms arise. Although polyps and the early stages of colorectal cancer are usually present with no symptoms, there are warning signs for which to watch. ➤➤ A change in bowel habits ➤➤ Blood in the stool (even “fresh” blood) ➤➤ Narrowed stool ➤➤ Bloating and fullness or a feeling that the bowel cannot empty completely ➤➤ Frequent gas pains ➤➤ Unexplained weight loss without dieting ➤➤ Continued fatigue ➤➤ Unexplained anemia These are all potential warning signs for colon and rectal cancer.

If present, you should consider having a formal evaluation of your colon, even if you are younger than the usual screening age.

Don’t let yourself or loved ones become a cancer statistic. When you think about colon cancer, know that it is a preventable disease. And prevention is only effective if you are willing to undergo the proper screening tests.

Talk to your physician. And if you are in the recommended age group, take a few minutes to schedule your screening exam. Colonoscopy screenings are covered by most insurance policies and can often be made without a referral.

Foods that help fight cancer While there are no magic foods that can guarantee men and women won’t one day develop cancer, there are many foods that research has shown are capable of reducing a person’s risk of developing this potentially deadly disease.

W

hile no single food or ingredient can prevent people from getting cancer, research has shown a link between excess body fat and various cancers.

A poor diet that’s high in fat can contribute to excess body fat, which the American Institute for Cancer Research has linked to higher risks for developing cancers of the esophagus, pancreas, colon and rectum and gallbladder, among others.

The AICR notes that research on foods that fight cancer is ongoing, but the following are some of the foods that can play a role in cancer prevention.

Broccoli also is a great source of the B vitamin folate as well as potassium. The AICR notes that studies have shown folate helps to maintain healthy DNA and keep cancer-promoting genes inactive. But men and women should speak with their physicians regarding how much folate to include in their diets, as animal studies have suggested that high amounts of folate may promote development of certain types of cancer, including colon cancer.

Cranberries Like apples, cranberries are high in dietary fiber and vitamin C. Diets high in foods containing vitamin C have been linked to a reduced risk for cancer of the esophagus. In addition, studies have shown that vitamin C protects cells’ DNA by trapping free radicals, which can damage the body, and inhibiting the formation of substances called carcinogens, which are capable of causing cancer in living tissue.

Legumes Legumes include kidney and black beans, yellow split peas and red lentils. Dry beans and peas are great sources of fiber and good sources of protein, and each also makes excellent sources of folate. In addition to their ability to contribute to the protection of colon cells, legumes also contain various phytochemicals, which researchers feel may decrease chronic inflammation, a risk factor for many cancers. In addition, these phytochemicals may increase the self-destruction of cancer cells.

While there are no magic foods that can guarantee men and women won’t one day develop cancer, there are many foods that research has shown are capable of reducing a person’s risk of developing this potentially deadly disease. More information on cancer-fighting foods is available at www.aicr.org.
