Moreover its presentation of Beta-lactamase is not only confined to the resistance of Beta-lactam antibiotics.

The anaerobes isolate from healthy and infectious conditions were tested for the presence of the specific Beta-lactamase. Of all the anaerobes from the 24 healthy persons, 10.86% of Beta-lactamase producing strains were from the oral cavity and 11.9% from normal vaginal-canal. From a total 195 anaerobic strains from 98 patients, 23.07% were Beta-lactamase producing and of these, 20.5% were CI. Perfringens, 43.5% B. fragilis and 50% B. melaninogenicus.

None of the anaerobic cocci had the enzyme.

บุญลอ ศรีพยัตต์, นราทร ธรรมบุตร, ภิรมย์ กมลรัตนกุล, บุญเทียน พิทักษ์ดำ�รงค์กิจ, แม็คอัลเล็ค ดีเจ. การ ประเมินผลแอนแอโรบส์ ที่ขับเอ็นซัยม์ เบต้าแล็คตาเมส จากคนปกติ และผู้ป่วยโรคติดเชื้อ. จุฬาลงกรณ์เวชสาร 2528 ตุลาคม; 29(10) : 1105-1114 ได้ตรวจหา เอ็นซัยม์ เบต้าแล็คตาเมส จากแอนแอโรบส์สายพันธุ์ต่างๆจากคนปกติ 24 ราย ปรากฎว่า ในช่อง ปากของคนปกติ แอนแอรโรบส์ที่มีเบต้าแล็คตาเมส อยู่ร้อยละ 10.86 ในช่องคลอดมีอยู่ร้อยละ 11.9 ในผู้ป่วยโรคติด เชื้อต่างๆ จำ�นวน 98 ราย ปรากฎว่า ในแอนแอร์โรบส์ 195 สายพันธุ์ที่แยกได้นั้น มีสายพันธุ์ที่ขับ เบต้าแล็คตาเมส อยู่ถึงร้อยละ 23.07 แอนแอโรบิค ค็อคไค ไม่ขับเอ็นซัยม์ เบต้าแล็คตาเมสเลย ในแอนแอดรบิค บาคิลไล ชนิดแกรม บวกนั้น คล็อสตริเดียม เพอร์ฟรินเจนส์ สร้างเอ็นซัยม์นี้มากถึงร้อยละ 20.4 ส่วนในบาคิลไลชนิดแกรมลบนั้น แบ็คที รอยดีสด ฟราจิลิส และแบ็คทีรอยดีสด มีลานินโนเจ็นนิคัส สร้างเอ็นซัยม์นี้ได้ร้อยละ 43.5 และ 50 ตามลำ�ดับ การศึกษานี้ชี้ให้เห็นว่า แอนแอโรบส์ ในนอร์มัล ฟลอร่าของคนปกติ มีเบต้าแล็คตาเมสแฝงอยู่ และยิ่งในสภาวะ ที่เป็นโรคติดเชื้อ เอ็นซัยม์นี้จะยิ่งเพิมขึ้น อนึ่ง ได้ศึกษาความสัมพันธ์ของการดื้อเพ็นนิศิลลิน ของแอนแอโรบิค ปาโธ เจนส์ สายพันธุ์ที่ขับเอ็นซัยม์ดังกล่าว ปรากฎว่า แม้ว่าแอนแอโรบิค ปาโธเจนส์จะขับเบต้าแล็คตาเมสเอ็นซัยม์ได้ ก็ตาม แต่ก็ไม่ทำ�ให้แอนแอโรบส์นั้นดื้อต่อเพ็นนิศิลลินเต็มประสิทธิภาพ ร้อยละ 100 เสมอไป นอกจากนั้น คณะผู้ วิจัยยังวิจารณ์ปัญหาในสังคม เช่นการใช้ปฏิชีวนะเบต้าแล็คแตมพร่ำ�เพรื่อ จุลินทรีย์แอร์โรบส์แอละแอนแอโรบส์ ย่อมจะสร้างเอ็นซัยม์นี้มากขึ้นทุกที และจะก่อปัญหาอย่างยิ่งต่อการดื้อยากลุ่มเบต้าแล็คแตม ในอนาคต

Material and Methods A. The subject:The subjects consisted of 24 normal, healthy persons, 10 male Medical Students and 14 females from the Dept. of Obstetrics and Gynecology, and 74 patients (40 females, 34 males)

between 20 to 50 years of age. Thirty cases of PID,* 4 of septic-abortions, 20 of penile lesions (soft chancre-included), 2 of recurrent tonsillitis, 10 with leg-ulcers, 4 of AAC** and 4 of peritonsillar abscess participated in this study. The patients were from Chulalongkorn Hospital and Phahol-Yothin General Hospital, Bangkok Thailand.

B. The sampling technique : The process of taking the appropriate clinical specimens were obtained according to Narathorn et al. (2,3) C. The Laboratory procedures : C.1 The bacterial culture. The clinical specimens were immediately inoculated on different non-selective and selective-media for anaerobes described

by Narathorn et al and Holdeman et al.(2,3,8,9) C.2 Beta Lactamase assay. The isolated anaerobic strains were tested for the production of beta-lactamase by the “Well in specific media� as described by Narathorn et al. and by Chromogenic cephalosporin.*** (10,11,12) C.3 Antibiotic susceptibility test. All the anaerobic pathogens that haboured beta-lactamase enzymes-were tested for the penicillin-resistant pattern by the quantitative anitimicrobial susceptibility method.(13) The anaerobic isolation, the betalactamase assay and susceptibility test were done at the Anaerobe Unit, Dept. of Med. Microbiology, Chulalongkorn Hosp. Med. School, This project began in February, 1982.

It is well-know that aerobes and anaerobes or micro-aerophilic members dominate the indigeous, acquired and potential microflora of the skin, oral cavity, female genitor-urinary-tract and penile lesion (1,2,3) These organisms are not only abundant and highly diverse microbial flora of man but may occasionally cause primary or secondary infections. They may participate in a local or remote sepsis, often in mixed infections exhibiting pathogenic synergy and therefore pose problems in their detection, diagnosis and especially clinical management.(4) The fact that either the beta-lactamase enzyme is detected in a specific anaerobic pathogen or its production by anaerobic bacteria can interfere with the eradication of Group A streptococci in tonsillitis treated with penicillin brought us re-investigate our anaerobic strains sisolated from healthy and variously diseased subjects for the presence of beta-lactamase producing anaerobes (5,6,7) The purpose of the present article is to evaluate the roles of beta-lactamase producing anaerobes isolated from normal subjects, and those found in common in factious diseses, between the detectable beta-lactamase enzymes and the pattern of penicillin-resistance.

In the oral cavity, 5 strains of B. melaninogenicus haboured beta-lactamase andin the vaginal-canal the enzyme mostly found in B. fragilis and Bacteroides spp.

(Table 1). In the infectious conditions,B. melaninogenicus released highest amount (50%) of β- lactamase. (Table 2)

Out of the total 195 isolate anaerobic

pathogens from common infection diseases cases only 23.07% released β-lactamase enzymes. None of the isolate-anaerobic cocci haboured β- lactmase enzymes, while 20.4% of gram-positive Clostridium perfringens elaborated β- lactamase and 43.5% of gram-negative B. fragilis did (Table 2 )

The Laboratory data from Table 4 revealed the probable degree of β-lactamase activity and the significant role of penicillin resistance.

Discussion Bacteria can become resistant to drugs by the following mechanisms. Ordinary Mutations may occur in genes on the bacterial chromosome which once arisen in a bacterial population, may be transferred to others by transduction or conjugation on which mechanism

the species in question is capable of performing.(14) In aerobic pathogen, 60-90 per cent of resistance genes are carried on plasmids. Since many of these are self transferable and can mobilize other which are not, drug resistance can spread through a population of sensitive bacteria in eqidemic fashion.

However resistance to penicillin or other beta lactam antibiotics has been found

in virulent anaerobic bacteria to be plasmid determined. Whether the plasmids can undergo conjugal transfer is doubtful.

The specific plasmids can be carried from cell to cell and the majority of them now carry plasmids,(14,15) the acquisition of plasmids accounts the majority of the failures of beta lactam antibiotic therapy.(14) In Thailand, the bacterial beta-lactamase or formerly penicillinase enzyme was qualitatively detected since 1972.

(10) Since then many new promising methods have been recommended.(11) Beta–lactamase-enzyme-producing anaerobes in the normal healthy people demonstrate the probable role of latent beta-lactamase plasmids in the community. As Thai peple can buy antibiotics freely from any drug storn without a physician’s prescription, they often receive subtherapeutic doses of beta-lactam antibiotics.

Beta-lactamase-plasmids are latent in the anaerobes of the oral cavity and vaignal microbial flora (Table 3). However, beta-lactamase plasmids may be transferred from elsewhere especially from the aerobic flora.

In infectious conditions, beta-lactamase are undetectable in anaerobic-gram-positive and gram negative cocci. Resistance to penicillin may therefore be from other mechanisms as mentioned above. (Table 2). Many anaerobic gram-positive and gram-negative bacilli produce the specific enzymes. All the anaerobic pathogens are isolated from patient between 20 to 50 years of age without any underlying-diseases

or nosocomial infections (Table 3).

In our series, there were 43.47 per cent beta-lactamase positive B.fragilis and 50 per cent from B.melaninogenicus. The percentage of positive findings was lower compared to other studies(16) (Table 2) This may have been due to the naure of the plasmids and the local community.

As a matter of fact in a developing country like Thailand beta-lactamase-producing anaerobes ought to have been found more frequently than mentioned in Table 2-4

a. The probable 100 per cent beta-lactamase activity : The unidentified Bacteroides spp.,

B. gingivitis, and Fuobacterium spp. demonstrate the important to penicillin resistance, as follow: (Figure 1)

b. The over 50 per cent beta-lactamase activity : Althong beta- lactamase was seen in Cl. Perfringens, and B. Fragilis only penicillin was mostly destroyed. The activity of bacterial beta-lactamase enzyme in this situation was partial. (Table 5)

c. The 50 per cent beta-lactamase activity : Beta-lactamase activity producing exactly 50 per cent was not seen in our series. d. The less than 50 per cent beta-lactamase activity : This can seen in B.melaninogenicus that partially destroyed penicillin (Table 5)

and the latent beta-lactamase enzyme in the normalflora of the community. And will brings about a substantial change in the use of beta-lactam antibiotics in serious infections The “well in media” or the chromogenic cephalosporin method was equally sensitive, although the latter was more practical and less time consuming.

The “well in media” was complicated, time consuming and required special, experienced operators.

One notice that anaerobic B. melaninigenicus played a significant role in the oral cavity (Table 2), while B. fragilis and other Bacteroides spp. Were potent penicillin inhibition in infectious diseases (Table2,4) Finally, the anaerobes play an important role in the normai flora of human beings. However, they may incorporate B-lactamase plasmids as shown in this article.

e. The probably non benefit beta-lactamase activity : These strains habour specific enzymes which can-not destroy the beta-lactam antibiotics. The anaerobic pathogens were therefore sensitive to penicillin. (Table 5) Also from Table 4, Cl. difficile had no beta-lactamase activity yet, these anaerobes showed 100 per cent resistance to penicillin.

From our study. Beta-lactamase was the potent inhibitor of beta-lactam antibiotics in vitro. In vivo many factors mayenhance the resistance-pattern penicillin has a high protein-binding in serum, or sometimes is unable to attach and be absorbed into the bacterial cell-wall of anaerobes. However penicillin is still used in clinical practice due to its in expensiveness for most anaerobic and aerobic pathogens.

The percentage of penicillin resistance will increase in the future owing to the sub therapeutic usage by druggists,

1. มูลเหตุ : จุลชีพแอนแอโรบส์ (anaerobes)เป็นจุลชีพที่ไม่ชอบก๊าซอ๊อกซิเจน และก่อโรคปางตายหลายโรค เช่น โรค บาดทะยัก, โรคก๊าซแกงกรีนฯและภาวะเป็นหมัน ปัญหาที่สำ�คัญคือ การแยกวิเคราะห์จุลชีพเหล่านี้ยากและลำ�บากมาก จำ�เป็นที่ต้องมีตู้อบซึ่งเป็นอุปกรณ์ที่มีราคาสูงจำ�เป็นต้องสั่งซื้อจากต่างประเทศ จึงเห็นสมควรคิดประดิษฐ์ขึ้นใช้เอง ซึ่ง ผู้ประดิษฐ์ได้ทดสอบประสิทธิภาพอยู่นาน และพบว่าสามารถแยกจุลชีพที่เป็นเหตุหนึ่งที่ทำ�ให้มีบุตรยากในปัจจุบัน คือ anaerobic Bacteroides fragilis (BF) และสามารถแยกวิเคราะห์แอนแอโรบส์ที่ก่อโรคอื่นๆได้ด้วย

ผลของงานวิจัยเรื่อง “จุลชีพที่พบในช่องคลอด และปากมดลูกในสตรีปกติและติดเชื้อ” บ่งชี้ให้เห็นว่าในสตรีวัย เจริญพันธุ์ ถ้ามีเชื้ออักเสบชนิดที่ไม่ชอบก๊าวอ๊อกซิเจน anaerobic Bacteroides fragilis, (BF) บังเอิญเข้ามาอาศัยอยู่ใน บริเวณ ช่องคลอดและปากมดลูก นานเข้าจุลชีพนี้จะขับสารพิษออกมาปนเปื้อนบริเวณดังกล่าว และต่อมาพิษนี้จะถูกดูด ซึมเข้าไปในกระแสเลือด และจะไปทำ�ให้ไข่ที่สุกเต็มที่ที่รังไข; mature human ovum เกิดการเสื่อมสภาพ และฝ่อไปใน ที่สุด (deteriolate) ฉะนั้นตราบใดที่พบเชื้อนี้ โอกาสที่จะมี “ปฏิสนธิ”กับเชื้ออสุจิ ก็น้อยลงไปทุกที ด้วยเหตุนี้ จึงกลาย สภาพเป็นผู้ที่มีบุตรยาก

2. สรุป อาศัยเครื่อง Mini CO2 ‘Anaerobic Cabinet’ ที่ประดิษฐ์ขึ้นมานี้สามารถแยกวิเคราะห์ให้เห็นว่า บริเวณ ปากช่องคลอด (cervix) มีจุลชีพ BF หรือไม่ ? ถ้ากำ�จัดจุลชีพดังกล่าวได้ ไข่ (humanovum) ก็ไม่ฝ่อ ผสมเทียมก็มีโอ กาศสำ�เร็จมากขึ้น

ตั้งแต่ 10 มกราคม 2547 เป็นต้นมา* เครื่องมือ Anaerobic Cabinet นี้ช่วยให้มีบุตรได้ตามปรารถนา 3 ราย จากจำ�นวน 40 ราย (ร้อยละ 7.5) และบางราย เมื่อเลิกทำ�แล้ว (give up) ก็เกิดปรากฎการณ์ “Rebound” มีครรภ์ขึ้น มาอีก 4 ราย (ร้อยละ 10 ) คงเป็นผลเนื่องจากการ ปรับสภาพร่างกายให้สมบูรณ์นั้นเอง อนึ่ง เชื้อ BF ดูจะมีความสำ�คัญ ต่อการมีบุตรยาก เพราะมีผู้ป่วย 1 รายที่ได้บุตรสมใจแล้ว แต่ก็อยากมีทายาทอีก เมื่อมีครรภ์ได้ 3 เดือนก็มักจะแท้ง ต่อ เมื่อได้รักษาจนไม่พบเชื้อ BF ในปัสสาวะแน่นอนแล้วก็ได้ทายาทเพิ่มขึ้นอีกตามต้องการ ยาก

ฉะนั้น “ ตู้อบเพื่อแยกวิเคราะห์หาจุลชีพแอนแอโรบิค BF” จึงเป็นเครื่องมือที่สำ�คัญมากที่ช่วยผู้ป่วยที่มีบุตร

The tip of the speculum. The paired swabs were immediatedy placed in the prereduced transport media in the butyl rubber-stoppered-glass bottle containing CO2. c. Bacteriological processing. The specimens for bacteriological analysis were processed in the Department of Medical Microbiology, Bacteriology Laboratory (Chulalongkorn Hospital) within 2 hrs. of collection.

The standard method isolation and identification of the aerobic organisms was used in this study. Media for isolating aerobes and facultative anaerobes were Trypticase soy agar with 5% sheep blood and 0.005% cysteine for the a primary isomary isolation, 1% Lactose brom-thymol blu agar and MacConkey’s agar for Enterobacteriaceae or non fermentative Gram negative rods,(10) APT agar (BBL)(8) For Lactobacilus sp., specific media for Corynebacterium vaginale, (9,11) blood chocolate media for Neisseria group.

All Staphylococcal infection were subjected to plasma Coagulase test. The Gram-negative bacilli were traced – to biochemical reactions.

Media for isolation anaerobic bacteria were PRAS* cooked meat glucose broth, fresh 5% sheep’s blood Trypticase soy agar, and specific media for Bacteroides sp., Clostridial sp.(12,13) Anaerobic plates for strictly anaerobes were duplicated anaerobes were duplicated studied within Anaerobic chamber, Anae-robic incubator, CO2 Anaerobic cab-

inet for strictly anaerobes and Anaerobic jar with Gas generating kit (OXOID) for obligate anaerobes.

Identification of anaerobes were made on the basis of cellular morphology (Gram’s stain) and colonial characters.(14) Final identification was based upon the manual of the Virginia Polytechnic Institute of Anaerobes.

From June, 1980 to March, 1981, 258 women in Gynecologic Clinic of Chulalongkorn Hospital were sampled for culture of both aerobic and anaerobic organisms. All subjects were classed as follow : 1. Normal healthy women 96 cases.

1.1 Total paired specimens for culture 198 samples.

2. Infected genital tract infection…112 cases.

2.1 Total paired specimens for culture 224 samples.

3. Unsatisfied subjects (gynecologic diseases, i.e. myoma uteri, procidentia or incomplete cuestionares, etc.) 50 cases 4. Total well and infleced women 258 cases.

This project was studied mainly for bacterial – isolation and identification. The fungi, the parasites or other causative agents were beyond the scope.

Result Of the total 258 subjects, 50 cases were unqualified while 96 normal healthy (37.2 per cent) women were chosen for study and 112 cases (43.4 per cent) were the infected gynecologic cases.

Out of 208 specimens from cervical areas, the total aerobic and anaerobic isolation were 317 strains (Table 3) : 134 strains (42.3 per cent) from healthy and 183 (57.7 per cent) from the infected women. The specimens yielded polybacteria growth. The total isolated aerobes were 165 strains (57.7 per cent) from the infected women. The specimens yielded polybacteria growth. The total isolated aerobes were 165 strains (52.0 per cent) and anaerobes were 152 strains strains (48 per cent).

In the healthy subjects the commonest facultative bacterium was the Lactobacillus sp. (24.7 per cent). There was no Staphylococcus aureus growth in the cervical area. The next common bacilli was E Coil, other pathogenic and non pathogenic bacteria are shown in table 1. The anaerobes of the healthy subjects were Bacteroides fragilis (15.1 per cent) and B. melaninogenicus (18.9 per cent) in 53 subjects. The commonest anaerobes were Peptostreptococci in cases with clinical infection.

Of 112 gynecologic-patients exam-ined, Staphylococus aureus was highest identified (17.65 per cent) each in vaginitis from the cervical area. Other significant findings were Lactobacillus sp.

(33.3 per cent) from vaginal abscess, E. Coli (40 per cent) from cervical area, Pro* PID = Pelvic inflammatory diseases.

teus sp. (20 per cent) from cervical area, Pseudomonas sp. (8.33 per cent) from PID, C. vaginale (25 per cent) from cer-vicitis (Table 1), Neisseriae sp. (25 per cent) from cervicities area (Table 1) Peptostreptococci (80 per cent) from cervical area of vaginal abscess (Table 2), C. perfringens (20.5 per cent) from cervical area of PID (Table 2) B.fragilis (15.2 per cent) from vaginal area (Table 2), and B. melaninogenicus (6.5 per cent) from cervicitis (Table 2), Two strains of Fusobacterium sp. were seen.

The incidence of isolated organisms were tabulate and compares both items in Table 3. Mixed isolation of aerobes and anaerobes were high in infected women (41.7 per cent) while isolated aerobes alone were high in normal healthy subjects. Only anaerobes were recovered at high at high percentage in cervicitis. However 14 ( 40 per cent) infected women and 14 (40 per cent) normal individuals were sterile (Table 3).

The amount of infected patients were 40 cases (35.7 per cent) of cervicitis, 30 cases (26.8 per cent) of PID, 30 cases (26.8 per cent) of vaginitis, 8 cases (7.1 per cent) of vaginal abscess (3.6 per cent) Of septic abortion (Table 3). The highest aerobic isolation was in vaginitis (42.7 per cent) and 3631 per cent anaerobic indentification in cervicitis (Table 3).

According to the age distribution, 29 (30.2 per cent) health subjects were highest between 26-30 years old, while 11 (27.5 per cent) of 26-30 years and 11 (27.5 per cent) of 31-35 years were

highest in number from cervicitis. Out of 30 PID, 10 (33.3 per cent) were between 26 – 30 years old, with 10 (33.3 per cent) of 26 – 30 years old and 10 (33.3 per cent) were highest number of vaginitis (Table 4).

From Table 1.2 and 3, the most common isolated microorganisms from the infected women’s cervical area were; 1. E. coli and Peptostreptococci sp. (cervicitis)

(PID) 3. E. coli and Peptostreptococci sp. (septic abortion) 4. Lactobacillus and Peptostre-ptococci sp. (vaginal abcess) 5. S. epidermidis and Peptostre-ptococci sp. (vaginitis) The statistic significance were also seen In Table 3A.

Discussion If the cervical area of the female genital tract is the central location, one may classify the gynecologic infections into 2 groups; a. the infections around the cervical area the septic abortion, the pelvic inflammatory diseases and the cervicitis, etc. b. the infections in the vaginal area are the vaginal abscess, the vaginitis, etc.

Originally, we had intended to obtain the data concerning the bacterial isolation between the vaginal and cervical areas of normal healthy and differential gynecologic conditions but lately found that the bacterial isolation of the two locations were almost the same amount and species.* Thus the dual samples were just for double check up.

However, several samples either from the normal or infected individuals revealed substantial per cent of “Growth”.

Many catagories of this comparative can be drawn; 1. normal healthy subjects and the gynecologic patients regarding to the amount of isolates bacteria (Table 1,2).

The aerobic Lactobacilli were higher in normal health while the amount were lower in the infected women, C.vaginale were higher than normal in the septic abortion, vaginal abscess. Only with the exception of E. coli were isolated higher in healthy than other infections.

Tococci sp., Bacteroides sp., C. perfringens Were higher in the infections around the cervical area 2. number of normal healthy subjects and infected women habouring the organisms (Table 3). The isolated aerobes with anaerobes were seen highest in the normal healthy but the infected gynecologic case that haboured the combination or the single aerobic isolation were lower individually. The infection of pelvic inflammatory disease and the cervicitis cases were haboured by the higher amount of anaerobes.

Table above 2 catagories can be explained according to the fact that in puberic period large amount of the specific carbohydrate (glycogen) present in the vagina. Lactobacilli attract the particularly carbohydrate and contribute for the maintainance of acidity (PH).

There appears to be the disturbance mechanism in preventing the disturbance mechanism in preventing the establish-ment of harmful microorganisms.

In the infected women, the Lactobacilli incidence were lower in the vaginal vault (Table 1,2).

3. The normal healthy and infected gynecologic conditions on the amount of isolate aerobes and anaerobes.

In normal healthy the amount of aerobes in the vaginal canal were higher than the infected gynecologic cases (Table 3).

Two possibilities concerned this problem, the alterative degree of

Oxygen-tension was the primary effected growth of aerobes and anaerobes.

Secondly, the cervical mucous played the important role by possessing lysozyme, the antibacterial activity, which reduced the incidence of microorganisms in the cervical area. In the pre-puerperal infections the mentioned normal defense mechanisms were disturbed, the percentage of the survival isolated microorganisms were probably increased in both areas.

Moreover, in our series, the total amount of aerobes number ed the anaerobes with the unknown explanation.

4. The normal healthy and infected gynecologic diseases on the amount of “No growth.”

Several sample either from the normal or infected individuals revealed substantial number of “No growth” because of technical errors and media preparation. Anaerobic-no growth were out number aerobes. (Table 1,2) Unfortunately very few anaerobic Fusobacterium sp. and also unidentified organisms were recovered. These isolated organisms were worth-nothing to mention about.

One can notice that there are 2 groups of isolated aerobes and anaerobes.

The first group of isolated organisms was part of the microbial flora in the genitor urinary (GU). The other minor group was accidental bacteria, such as S. aureus or S. epidermidis However the common infec-tions by these organisms have the potential of producing the serious se-

quel of shock, disseminated intravascular coa-gulation, hemolysis, renal failure and death. (16,17,18) However this preliminary bacterial isolation offers the interesting points as follow.

1. In order to obtain the correct causative or ganisms, one should take the proper speciments from the area that relates to the clinical diagnosis and also knows how to collect the excellent samples for culture.

2. The disturbance of genital tract causes, firstly the high number of microbial flora, and these microorganisms are potentially pathogens. Secondly the accidental induced microorganisms are harmful.

3. The difference between microorganisms in genital tract in health and infected women seemed to be significance (Table 3 A 1-3) 4. The data about the most common bacterial isolation of gynecologic infections may be used as a presumptive indication for the initial treatment of choice until the specific bacteria are identified.

Early antibiotic treatment based on fixed antibiograms can prevent the serious complications. (15,16) Summary 258 women-subjects were chosen for study, but 50 subjects did not meet the study criteria and dropped out, etc.

The total of 317 bacterial strains were identified with 134 strains (42.3 per cent) from healthy and 183 strains (57.7 per cent) from infected women

One hundred and twelve common infected gynecologic patients of cervicitis (35.7 per cent), PID (26.8 per cent), vaginal abscess (7.1 per cent) and vaginitis (26.8 per cent) were found to have 52 percent positive bacterial culture of cervical and swab-samples.

Of ninety six well individuals, 134 trains (42.3 per cent) of aerobes and anaerobes were recovered from cervical area. The comparison of isolated bacteria revealed the significance relation-ship (Table 1-5). The project expressed, the interference of cervical resident bacterial flora to become potentially pathogens,

Owing to the fact that fully equipped-anaerobic chamber is a very delicate-expensive scientific instrument and due to shortage in the budge try shortage, the auther has developed a “Home-made” anaerobic chamber using materials available in Thailand. ราวๆศตวรรษมาแล้ว ท่านบรมศาสตราจารย์หลุย ปาสเตอร์ พบจุลินทรีย์ชนิดหนึ่งที่ไม่พึ่งอ๊อกซิเจนเพื่อ การดำ�รงชีวิต จุลชีพเหล่านี้มีอยู่ทั่วไปในสถานที่ที่เป็นภา วะไร้อ๊อกวิเจน (anaerobic condition) เช่น ก้นทะเล ลึก บริเวณที่มีสิ่งเน่าเปื่อยทับถมกัน ซากสัตว์ที่ตายใน ดิน โดยเฉพาะอย่างยิ่งในทางเดินอาหารของมนุษย์และ สัตว์ซึ่งมีสภาวะ reduced(1) ในห้องปฎิบัติการที่แบกวิเคราะห์หาจุลชีพก่อโรค โดยทั่วไปมักจะขาดการสนใจจุลินทรีย์แอนแอโรบส์ ทั้งนี้ เพราะขาดวิธีการและเครื่องมือในการแยกเอาแอนแอ โรบส์ที่ก่อโรคติดเชื้อรุนแรงมาศึกษาได้ (2) การสร้าง แอนแอโรบิค แชมเบอร์ (Anaerobic chamber) ก็คือ การส้างขอบเขตเนื้อที่จำ�กัดที่ไร้ก๊าซอ๊อกซิเจนในห้อง ปฎิบัติการและให้มีก๊าซต่างๆที่จุลินทรีย์แอนแอโรบส์ ชอบและต้องการเพื่อแยกแอนแอโรบส์เผ่าพันธุ์ต่างๆ

เพื่อการสึกษาวิเคราะห์เป็นประโยชน์ต่อสังคมและ ต่อผู้ป่วยที่ต้องทนทุกข์ทรมานโดยจุลชีพก่อโรคเหล่านี้ สิ่งสำ�คัญอันหนึ่งในบรรดาวิธีการที่สำ�คัญในการนำ� แอนแอโรบส์มาเพาะเลี้ยง ก็คือการสร้างสภาวะไร้อ๊อกซิ เจนในที่ๆจำ�กัด (anaerobic closed space) ก็คือ การประดิษฐ์แอนแอโรบิค แชมเบอร์ และทดสอบ ประสิทธิภาพแชมเบอร์ (chamber) ที่ประดิษฐ์ขึ้นใช้ใน หน่วยแอนแอโรบส์ คณะแพทยาศาสตร์(3) เหตุที่ต้องประดิษฐ์แอนโรบิค แชมเบอร์ขึ้นเพราะ 1. สั่งซื้อจากต่างประประเทศราคาแพงมาก (รูปที่ 1) 2. ประดิษฐ์แอนแอโรบิค แชมเบอร์ โดยใช้วัสดุที่ หาได้ง่าย ราคาถูก และมีในประเทศไทย

วัสดุและวิธีการ การประดิษฐ์แอนแอโรบิค แชมเบอร์ขนาดเล็ก ซึ่ง ใช้ส่วนประกอบที่มีอยู่ในประเทศไทยทั้งสิ้น แบ่งส่วน ประกอบออกเป็น 5 ส่วน ดังนี้ ก. ส่วนเนื้อที่ไร้อ๊อกซิเจน (anaerobic compart-

ment) เป็นเนื้อที่มิดชิด (closed-space) ทรงสี่เหลี่ยม ผืนผ้า (รูปที่ 2) ใช้พลาสติคใสหนา 0.2 มม. ผนึกรอยต่อ โดยความร้อนอัดกด** ขนาดตู้กว้ง 35 ซม. ยาว 99 ซม. สูง 93 ซม.

ค. ส่วนภายในตู้ (interior compartment) ภายในแชมเบอร์มี NaHCO3 และไพโรแกลลอน (Pyrogallol) ผสมใส่เพื่อดักทำ�ลายดูดก๊าซออกซิเจนที่ หลงเหลือในตู้ออกบ้าง นอกจากนั้นเป็นที่วาง media และดรรชนีชี้บ่ง methylene blue indicator (รูปที่ 3) ที่ปรุงขึ้นเอง เพื่อบอกสภาวะไร้ก๊าซออกซิเจนภายในตู้ การ streak plates สามารถทำ�ได้ง่ายภายในแชมเบอร์นี้

ง. ส่วนบริการก๊าซ (gases compartment) ก๊าซที่ใช้คือก๊าซคาร์บอนไดอ๊อกไซด์และก๊าซไนโตรเจน ซึ่งการปล่อย ก๊าซนั้นใช้ส่วนประกอบที่ประดิษฐ์ขึ้นเอง เช่น บอลลูนยาง ขวดแก้วดูการเดินทางของก๊าซ (gas-detective flask) copper flint apparatus เพื่อกำ�จัดก๊าซอื่นๆที่ปนมากับก๊าซ N2 และ CO2 และเครื่องกำ�จัดความชื้นก่อนผ่านเข้าสู่แชม เบอร์ (dryer) (รูปที่ 4)

ขนาดเล็กคือ นอกจากประหยัดเงินที่ต้องซื้อจากต่าง ประเทศแล้ว แชมเบอรืขนาดเล็กนี้ยังช่วยแยกวิเคราะห์ แอนแอโรบส์ที่เจริญเติบโตยาก (strictly anaerobes) แอนแอโรบส์ประเภทปานกลาง (obligate anaerobes) ทำ�ได้โดยง่าย นอกจากนั้นสามารถทดสอบแอนแอโรบส์ กับปฎิชีวนะที่สำ�คัญ ได้ง่าย (paper disc anaerobic susceptibility test) และสามารถทำ�การแยกวิเคราะห์ ได้คราวละหลายๆแซมเปิลส์ (clinical samples) เพราะ มีเนื้อที่มากกว่าโหลแอนแอโรบิค (anaerobic jarGasPak)(4) ข. ข้อบกพร่อง อุณหภูมในตู้พลาสติกยังควบคุม ให้คงที่ไม่ได้ แต่บังเอิญประเทศไทยอุณหภูมิทั่วๆไปมี “คามอุ่น” ที่แอนแอโรบส์นี้ชอบอยู่แล้ว เนื่องจากแชม เบอร์นี้มีขนาดเล็ก การทำ� media จึงทำ�ได้ไม่สะดวก นัก คงทำ�ได้แต่การ streak plates เท่านั้น ข้อเสียอีก ประการหนึ่งคือถุงมือยางทำ�ให้เกิดความร้อนอบ เหงื่อ อกง่ายซึ่งการ manual manipulation ทำ�ได้ครั้งละ 1-2 ชั่วโมงเท่านั้น และต้องใช้ Talcum powder ทามือแขนก่อนปฎิบัติกิจกรรมเสมอ

ภายนอกตู้ยังมี water manometer ที่ผลิตขึ้นเอง เพื่อดูความดันของก๊าซภายในแชมเบอร์ และมี vacuum เพื่อดูดอากาศออกจากแชมเบอร์ วิธีการ ก. การไล่อากาศออกจากแชมเบอร์ ต้องมีความรู้ พื้นฐานที่ดี เช่น ต้องทราบว่าอากาศมีก๊าซออกซิเจนร้อย ละ 79 ไนโตรเจนร้อยละ 20 ที่เหลือคือก๊าซเฉื่อย และ ต้องรู้น้ำ�หนักเปรียบเทียบของก๊าซชนิดต่างๆ ข. การปล่อยก๊าซต่างๆเข้าไปในแชมเบอร์ ถ้าแชม เบอร์ไม่มีรูรั่ว (leakage) ดรรชนี methylene blue จะ เปลี่ยนจากสีฟ้าเป็นไร้สี แสดงถึงสภาวะไร้ก๊าซอ๊อกซิเจน ภายในแชมเบอร์ วิจารณ์ การใช้แอนแอโรบิค แชมเบอร์ ในระยะแรก ขลุกขลักและได้ผลไม่ดีพอ แต่เมื่อได้ซักซ้อมบ่อยๆเข้า บุคลากรที่คุมแชมเบอร์ รุ้งานดี จึงนับได้ว่า แชมเบอร์ ขนาดเล็กนี้มีประสิทธิภาพและมีข้อดีและข้อเสียดังต่อไป นี้ ก. ข้อดี ประโยชน์ของแอนแอโรบิค แชมเบอร์

A home-made “mini-cabinet” has been developed to produce saturated-atmosphere. To the home-made cabinet was affixed a “home-made water manometer”, a specific portable hygrometer and a humidifier. It was suitable for the enhancement of fibroblast cell culture in the laboratory diagnosis of antibiotic associated colitis and other serologic tests. The “home-made moist cabinet” was a non-expensive apparatus. The substantial advantages and disadvantages are also discussed in this artile.

บรรยากาศที่ ห่ อ หุ้ ม โลกย่ อ มประกอบด้ ว ยก๊ า ซ หลายชนิด นอกจากนั้นยังมีความชื้น (humidity) จาก การระเหยของน้ำ�ปะปนอยู่ในบรรยากาศ ความชื้นนี้มา จากไอน้ำ� (water vapour) ซึ่งนับว่าเป็นก๊าซชนิดเดียว ในบรรยากาศที่มีมากหรือมีน้อย ขึ้นอยุ่กับอุณหภูมิของ บรรยากาศนั้น บรรยากาศที่มีอุณหภูมิต่ำ� ย่อมมีไอน้ำ� น้อย แต่ในบรรยากาศที่มีอุณหภูมิสูงย่อมมีไอน้ำ�มาก ความชื้นสมบูรณ์หรือความชื้นอิ่มตัว (absolute humidity, saturated, water vapour) จึงหมายถึงจำ�นวน ไอน้ำ�มากที่สุดที่พึงมีได้ในบรรยากาศที่มีปริมาตรจำ�นวน หนึ่ง (given space). ในปัจจุบันมีกระบวนการ (Processes) หลายวิธีใน การหาจำ�นวนความชื้น (water vapour) ในบรรยากาศ วิธีที่สำ�คัญในการหาค่า water vapour ในอากาศมี 2 วิธี คือ ก. การหาค่าความชื้นสัมพันธ์ (relative humidity) หมายถึงหาจำ�นวนของความชื้นจริงที่มีอยู่ใน บรรยากาศตามปริมาตรและอุณหภูมิที่กำ�หนด นำ � มาเที ย บกั บ จำ � นวนของความชื้ น มากที่ สุ ด ใน

- ส่วนบริการก๊าซ (gas supply) : มีก๊าซคาร์บอน ไดอ๊อกไซด์ที่ทำ�ให้แห้งโดยผ่านไปยัง warming unit ที่ ประกอบด้วยหลอดไฟฟ้าขนาด 60 วัตต์ ต่อจากนั้น จึง ให้ ก๊ า วนี้ ไ หลไปตามท่ อ อย่ า งแข็ ง เข้ า สู่ ตุ้ เ ก็ บ ความชื้ น

-เครื่องมือวัดความชื้น (hygrometer, pyschrometer) : เครื่องวัดความชื้นภายในตู้เป็นแบบ OTA-type-portable hygrometer

-เครื่องนี้ทำ�ด้วย chrome benzel มีหน้าปัดสีขาว และเข็มชี้สีแดง แก้ววางอยู่บนตู้เก็บความชื้น (ภาพที่ 4) นอกจากนี้ ส่วนประกอบอื่นๆอีก เช่น เทอร์มอมิเตอร์ที่ อ่านเป็นเซลเซียสและฟาร์เรไฮด์

3. การใช้เครื่องมือเก็บความชื้นขนาดเล็ก (operation of CO2 – home made moist cabinet”) - เติมน้ำ�แข็งให้เต็มบีกเกอร์ขนาด 250 มล. ท้ะง 2 ใบ ต่อมาจัดวาง microtiter plates ที่มีเซลล์ไฟ โบรบลาส - ค่อยๆปล่อยก๊าซ CO2 ให้ผ่านเครื่องบอกการไหล ของก๊าซไปสู่ตู้เก็บความชื้น (2 lb./sq. inch) - ระวังอย่าให้ความดันในตู้สูงมาก โดยสังเกตดูจาก water manometer ที่ติดอยู่ข้างตู้ ปกติให้มีความดัน เท่ากับส่วนสูงใน U-tube ประมาณ 3 inch ประมาณ 3 ชั่วโมง เครื่องมือวัดความชื้นจะขึ้นเป้นร้อยลละ 100 คือ มีความชื้นอิ่มตัว ทิ้งตู้เก็บความชื้นไว้ตลอดคืน 4. วิจารณ์ ตู้ เ ก็ บ ความชื้ น ขนาดเล็ ก ที่ ป ระดิ ษ ฐ์ ขึ้ น เองนี้ เริ่ ม

ใช้ตั้งแต่ต้นปี พ.ศ. 2528 โดยใช้ร่วมในการทำ� Tissue culture assay for Cl. Difficile cytotoxin* นอกจาก นั้น ยังใช้ประโยชน์อย่างมากในการ “seeding” เนื้อเยื่อ foreskin fibroblasts ที่ได้จากหนังหุ้มปลาย (prepue) เด็กที่เป็น phymosis (4,5) นอกจากนั้นตู้เก็บความชื้นนี้ มีประโยชน์ในการเสริมการทดสอบ AD Nase B** titration (modified after Widdowson and Nelson)(6) อนึ่งก๊าซ CO2 ที่ใช้ในตู้เก็บความชื้นยังช่วยเร่งปฎิ กิริยา cytotoxic ต่อ fibroblast ได้ดีและเร็ว นอกจาก นั้นก๊าซนี้มีราคาถูกและปลอดภัย เพราะไม่ใช่ก๊าซติดไฟ การที่ให้ก๊าซผ่าน warming unit เพื่อป้องกันไม่ให้ก๊าซ ที่ไหลผ่านท่อยางก่อความเย็นจับกันเป็นก้อนน้ำ�แข็ง*** ตู้เก็บความชื้นที่ประดิษฐ์ขึ้นเองนี้ มีข้อเสียหลาย ประการเป้นต้นว่า ประการแรก ไม่สามารถควบคุม อุณหภูมิในตู้ให้คงที่ได้ ที่เป็นเช่นนี้เพราะ ฝาตู้เป้นกระ จกถึง 3 ด้าน และไม่ได้บุฉนวนป้องกันการแผ่รังสีความ ร้อน ยิ่งไปกว่านั้นไม่มี automatic thermostat ช่วยใน การควบคุมอุณหภูมิ อย่างไรก็ดี บังเอิญอุณหภูมิในห้อง ปฎิบัติการมีประมาณ 35-37 องศาเซลเซียสอยู่แล้ว ข้อ เสียประการที่สอง คือ ตู้นี้ต้องติด Safe valve ที่ดีเพื่อ ปิดก๊าซ CO2 ไม่ให้เข้าไปในตู้ เมื่อพบว่าความดันในตู้สูง เกิน 3 นิ้ว

ข้อเสียอีกข้อหน่งคือ ตัวทำ�ความชื้นเป็นเพียง ปิด เกอร์ที่ใส่น้ำ�แข็งความชื้นในตู้จะค่อยๆลดลงทีละน้อยจาก ร้อยละ 100 ในตอนกลางคืน จนกระทั่งถึงเพียงร้อยละ 90 ในตอนเช้ามืด (อ่านจากเรื่อง hygrometer) Hygrometer แบบที่นำ�มาใช้นี้ อ่านค่าความชื้น สัมพันธ์ได้แม่นยำ�และรวดเร็ว (ripid, accurate relative humidity determination) ตั้งแต่ร้อยละ 10 ถึงร้อยละ 100. ที่เป็นเช่นนี้ เพราะเครื่องวัดความชื้นนี้เป็นแบบใช้

diaphragm type element ที่มีความไวสูง (maximum sensitivity) และไม่ต้องมีค่าแก้ใดๆ (no correction chart) อย่างไรก็ดี เครื่องมือเก็บความชื้นที่ประดิษฐ์ขึ้นเอง มีประโยชน์มากกว่าข้อเสียที่กล่าวมาแล้ว เหมาะอย่างยิ่ง ในการใช้สำ�หรับประเทศไทยซึ่งเป็นประเทศกำ�ลังพัฒนา การซื้อหาตู้เก็บความชื้นซึ่งมีราคาแพงจากต่างประเทศ เป็นการเสียดุลย์การค้าไปโดยไม่จำ�เป็น
