Summer Program Enrollment Contract 2013 Parent/Guardian: _____________________________ Student: ______________________ Grade: _____ Western Mennonite School provides summer classes to meet the needs of students desiring work-based learning experiences (Internship), credit recovery courses (English and Math), and students desiring to “get ahead” on graduation requirements or free up their schedules by taking Health II during the summer. In consideration for the services provided by Western Mennonite School (WMS) on behalf of my child, I agree to pay: _____ $250 – Health II .5 credit (70 hour group class from 8a.m.-3p.m. July 15-26, M-F) _____ $250 – Internship .5 credit (90 hours in the local community with pre/post work required) _____ $250* – Individualized English .5 credit (10 hours of 1-1 instruction) _______________________ _____ $250* – Individualized Math .5 credit (10 hours of 1-1 instruction) _________________________ _____ $250 – Individualized SAT Preparation (no credit-10 hours of 1-1 instruction) _____ TOTAL ENCLOSED Because all summer programs are self-support, scholarships are not available and payment must be made in full at the time of registration. Enrollment is limited, and students will be accepted on a first-come, first-served basis until the program is full. There will be no full or partial refunds. I agree that my child will be bound by the regulations of the school including its standards of conduct and dress. These regulations and standards of conduct and dress may be communicated either in verbal or written form. By signing below, I agree to the above terms and conditions.

*Math and English are available ONLY for students who want to repeat a WMS class because they either failed the class or want to improve their grade. The $250 fee covers ten individualized, 1-1 sessions with the teacher. If a student finishes the class in less than 10 sessions, there will be no partial refund for unused sessions. If a student needs more than 10 contact hours of individualized instruction in order to complete the English or Math class, parents agree to pay an additional $30 per contact hour (this is unlikely to happen).
