VACATION CARE TERMS AND CONDITIONS
Consent & Indemnity
1. By making this booking, I give consent for my child/children to:
(a) attend the vacation care program (Program) operated by West HQ Limited (SGAC);
(b) participate in all activities forming part of the SGAC Program; and
(c) travel between SGAC and ONE55.

2. I acknowledge that during my child’s/children’s participation in the activities my child/children will be exposed to the obvious risks associated with those activities.
Gymnastics, tumbling, and trampoline are inherently risky sports which regardless of how careful participants and instructors are, the risks cannot be eliminated.
SGAC has conducted risks assessments in respect of the activities within the Program which are available upon request.
3. I indemnify SGAC, its officers and agents and keep them indemnified against all loss, claims, demands, actions and liabilities of any kind (other than due to wilful negligence) arising from my child’s/children’s participation in the activities and the Program.
4. I understand that it is my responsibility to fully enquire of and disclose to SGAC staff and/or any other authorised person, any circumstances which could affect my child’s/children’s safety or any activities that I do not wish my child/children to participate in.
5. I authorise SGAC staff to organise medical or hospital treatment (including transportation by ambulance) as they deem necessary. I understand that I will be responsible for any costs incurred.
6. I acknowledge that additional fees will apply in the event that I fail to collect my child/children by 4.00pm. The late penalty fee will be charged at the rate of $30 for each 30 minutes.
7. Conduct
I understand that my child/children and myself are bound by the Conditions of Entry for SGAC and ONE55 and Code of Conduct (Policies)

I acknowledge that initial registration payments may be reimbursed if SGAC determines that the needs of child/children are outside the scope of the program.
I acknowledge that if my child/children breach any of the Policies I will be contacted and my child may be removed from the Program without refund.
I understand that, should my child/children cause disruption or behave in a way that detrimentally affects the enjoyment of other participants in the Program, he/she/they may be asked to leave for the day and/or the entire Program and I will be contacted to collect they/them.
8. Attire
(a) Aquatic participants must wear recognised swimwear; street wear or other clothing is not permitted in the pool. All other relevant policies must be adhered to, including but not limited to, Aqua Nappy Policy, Hygiene Policy, Pool Safety Policy and Public Swimming Policy.
(b) Gymnastic and gym participants must wear suitable gym attire and appropriate footwear at all times.
9. Marketing
I give permission for my child/children to be photographed/recorded by SGAC staff while participating in the Program and consent for the photos/recordings to be used for marketing, training, end-of-day email communication and publicity purposes.
10. Fees and Refund
(a) Cancellation and refund of Program fees will only be considered for participants who have suffered a major illness/injury, eg contagious infections, broken bones or hospitalisation. A doctor's certificate must be provided in support of the refund request. The request must be made within seven (7) days of the commencement of the Program.
(b) Refunds will not be granted for any reason other than those set out in 10.(a).
(c) Cancellation will only be accepted for the entire Program. Fees associated with any missed days of the Program will be forfeited.
11. Privacy
SGAC recognises the importance of protecting the privacy and rights of individuals concerning their personal information.
