The Hanover & District Hospital (HDH) works with its partners to provide a full range of primary acute care hospital services and selected secondary services to meet the needs of the population of Hanover and surrounding rural townships
The Board of Governors, staff, physicians and community partners work together to accomplish seamless care that
provides core rural health services close to home and formulizes a clear pathway for referrals to additional services HDH partners with peer acute hospitals, community agencies, long term care homes, mental health and addictions and social service providers
HDH provides the people we service access to the care they need through a 24/7 Emergency Department, Acute Care Unit (inclusive of medical/surgical beds and multipurpose ICU), Physiotherapy Program, Surgical Services Department, Family Centred Birthing Unit, Hemodialysis Unit and Palliative Care Services.
Access is provided within the organization to Community Mental Health and Addiction Services, Chaplaincy Services, Grey-Bruce Diabetes, Home and Community Support Services, Home and Community Care SWLHIN and the Hanover Family Health Team
HDH has been "Accredited with Exemplary Standing" in three consecutive surveys (2013, 2017, 2021) This is the highest level awarded by Accreditation Canada and reflects that our Board, staff, volunteers and physicians strive to surpass the fundamental requirements of the accreditation program
What Should I Bring to the Hospital?
When preparing for a stay at Hanover & District Hospital (HDH) be sure to bring the following items:
Your Ontario Health Card
A piece of Government issued photo ID
All of your medications (in original bottles) along with a list of your current medications including non-prescription medications
If you use mobility, vision or hearing aids, bring those devices with you.
If you have additional health coverage, bring your insurance information with you
Any personal belongings or toiletries (soap, shampoo, toothbrush and toothpaste) you might need during your stay including pajamas, rubber bottom slippers, robes etc
Please don’t bring any valuables with you for your hospital stay HDH is NOT responsible for any lost or stolen items This includes, but is not limited to electronics, jewellry, cash and credit cards
Do I Need to Pay for My Hospital Care?
If you are a Canadian resident, OHIP will cover most, if not all, of your hospital services Some services are “de-listed” by OHIP. Your insurance may still cover some of these services
OHIP does not cover private or semi-private rooms Check with your insurer before you arrive if possible If your insurance covers these accommodations, let us know right away Every effort is made to provide you with the appropriate accommodation, private, semi or ward room. If your choice is unavailable, you will be moved when a suitable option becomes available
Please note that it is the patient/family's responsibility to transport or arrange transportation to and from the hospital

Telephones & Televisions
Telephones are available in each patient room with no charge for local calls Hearing impaired phones are available upon request Personal televisions are also available upon request. A daily rate will apply. Televisions are owned and operated by the Hospital Auxiliary
Nutritional Services
HDH patient meals are provided by Sysco and follow heart healthy and Canadian Food Guide Portion guidelines "Retherm" technology is utilized where food is assembled on a cold tray The trays are then put in the retherm oven Half of the tray is heated while the remaining half is kept cold
Your Privacy is Important to Us!
HDH will protect your personal health information by keeping it safe and secure When you are admitted, you will be given a number you can give to your family for inquiries. When they call, they will be asked for the number. No information or condition update will be given without the number
The use of recording devices is not permitted without consent. We reserve the right to review media records at HDH to protect privacy and confidentiality
There may be times when you overhear a conversation between a roommate and the health care provider We ask that you respect the confidentiality of others

Our Spiritual Care Providers are professional, multi-faith and compassionate members of your healthcare team Services for patients/family and caregivers include one on one visits, prayer meditation, rituals or reflection, help to connect with local community of faith, facilitation or arrangement of prayers and other religious rituals, help families process grief and loss A Spiritual Care Provider can be reached by asking a nurse at any time 24/7.

You may request an interpreter for yourself or a family member If you need an interpreter, please tell your Health Care Provider.

Patient Safety/Risk Management
We are committed to a culture of patient safety where patients, families and Health Care Professionals feel supported and empowered to keep the Hanover & District Hospital safe for all. The Patient Safety/Risk Management Committee's major goal is to improve safety and develop systems to support staff and patients
Fall Prevention: HDH screens all patients for fall risk on admission, 48-hours after admission and then weekly It is important to keep you safe! If you're deemed to be at risk of falling, a falling star symbol will be placed outside your door to alert the team You will be given a yellow armband to wear that communicates to the care team that you are at high fall risk While in the hospital, please make sure you have non-slip socks We may also lower your bed to the lowest position, use night lights, call bells, alarm mats for beds and chairs, beds that alarm, and various mobility aids.
Patient Safety Boards: At each bedside, there is a white board that provides key information for you and your Health Care Provider Please do not write on these boards If you have any questions about the information on the board, please ask your Health Care Provider
Pressure Ulcer Prevention: Upon your admission you will be screened for risk of developing a pressure ulcer A complete skin assessment will be completed and documented on your chart You will be assessed for the development of pressure ulcers every week.
Blood Clot Prevention: Every admitted patient is screened for risk of developing a blood clot upon admission. If you ’ re deemed to be at risk of developing a blood clot, your care team will ensure that this is addressed with medication and/or placing a mechanical pump on your legs
Two Patient Identifiers: Before we give you a medication or a procedure, we will ask you to identify yourself twice everytime For example, your name and date of birth This is important to keep you safe
Infection Prevention & Control

Visiting Hours
Visiting restrictions may vary due to the ongoing COVID-19 pandemic Please check our website for the current visiting policy in place
When no restrictions are in place normal visiting hours on all units are from 8:00 a.m. to 6:30 p m and from 7:30 p m to 10:00 p m
Patients admitted to hospital may have two (2) visitors at the bedside at a time Exceptions may be made on patient need. In our Emergency Department, Surgical Services and Outpatient Clinics each person may be accompanied by one (1) support person
One (1) support person may escort a patient for scheduled appointments, tests, out-patient procedures or treatments
Visitors may be asked to exit the room for nursing staff or physicians to provide patient care.
Exceptional circumstances will be managed on a case-by-case basis for pediatric patients, birthing families, patients receiving palliative care, or patients in the Intensive Care Unit.
Parking

Masking and Hand Hygiene
Please wear a mask, watch your distance and wash your hands while visiting the hospital. These are the best defenses for preventing and controlling the spread of infection to the patient While you are visiting the Hospital, we encourage you to use the hand sanitizers conveniently located throughout the hospital Please wash your hands before entering a patient room and when leaving.
Infection Control Guidelines for Visitors
Visitors must read and follow the infection control guidelines posted at all entrances If an isolation sign is posted on a door, all visitors must report directly to the nursing station where they will be provided with instructions.
To protects against the spread of infection, visitors are asked NOT to;
Use the washrooms in the patient's rooms - there are public washrooms located throughout the hospital
Share the patient's food or use any of their utensils, glasses or cups
If a visitor is not feeling well or has tested positive for COVID-19 or has been exposed in the past 10 days, DO NOT visit the hospital.

Trillium Gift of Life Network (TGLN) - Organ & Tissue Donation
HDH collaborates with the TGLN working to save lives through organ and tissue donation and identification of potential donors HDH reports every patient death to TGLN so that specially trained staff can identify potential donors and work with families to discuss options If a death occurs, your family will be contacted via phone by TGLN
As a community member, you can make a difference by registering to be a donor today at BeADonor.ca Evidence shows when families are presented with proof of registration they consent to donation, but in the absence of registration, consent drops dramatically. Hospital Amenities
Public Washrooms are located in the main lobby (accessible), in the Emergency Department waiting room (accessible), on the second floor across from the stairwell (accessible) and behind the elevators across from the quiet room.
Vending Services: The vending machine is located on the Main Floor of the hospital and is available 24/7 It provides a variety of food and hot/cold beverages
Gift Shop: The Gift Shop is located on the main floor and is operated by the hospital's Auxiliary volunteers Hours of Operation are Monday - Friday from 9:30 a m to 2:30 p.m. Hours are subject to change based on availability of volunteers. The proceeds from the shop are donated to the hospital
The Gift Shop has a variety of personal care products, books, decor, cards, baby gifts, purses, jewellery and small gift items offering no tax on purchases Cash/Debit are accepted Come and check it out - You will not be disappointed If you are interested in becoming an auxiliary volunteer see the "Giving Back" section.
Pet Therapy Program: St John Ambulance Therapy Dog volunteers and their certified dogs have become a familiar sight in many areas of the hospital.
Hospital Fit Trail: The Fit Trail is a 1 5 km track located on the Southwest corner of the Hospital property available to anyone wishing to participate in healthy activity The trail and 10 fitness stations are fully accessible The fitness stations are designed for novice and advanced participants and include required equipment and instructions The Fit Trail was a staff initiative built and maintained by volunteers and donors.
Change Machine: There is a change machine located at both entrances of the hospital.

Discharge
Your health care team wants to ensure that once your acute medical needs have been addressed, you can safely be discharged back to your home Please vocalize any concerns that you have regarding your discharge care with your care providers so that it can be addressed and included in your plan of care. Questions regarding your healthcare will be answered before discharge including prescriptions and follow-up appointments, if necessary
Transportation
You will be told your time of discharge Please arrange for transportation home If you are unable to get transportation, the Hospital will help arrange transportation. You will have to pay for the transportation provided If you have any questions about going home, ask a health care provider
After Discharge
Within 48 hours you will receive a telephone call from our nursing staff to ensure your transition home is a success Also, if you have a Hanover family physician you will be contacted for a follow-up appointment with your physician
Non-Compliance with Discharge
HDH must ensure appropriate use of Hospital beds When your health care provider feels that you are ready to go home or be transferred to another facility, you will be required to leave If you refuse to leave, you will be charged for your accommodation

You may receive a patient satisfaction survey prior to leaving the hospital The survey is available electronically through the hospital website as well Please take a few minutes to complete the survey and provide your feedback. Your opinion is important to us as we continually try to improve services for our patients and families

Day-of-discharge help to aging or frail adults or adults with disabilities to ensure their safe and comfortable return to independent living at home
Provide transportation from the hospital, perform errands such as prescription and grocery pick up, as well as provide a complimentary meal.
Help find resources available in the community to support ongoing safety and independence
Hanover & District Hospital has an active medical/surgical program that utilizes a multi-disciplinary approach to care The Acute Care Unit offers 22 active inpatient beds that care for a wide variety of patients with surgical and medical health issues of all ages The medical and nursing staff who care for these patients utilize a patient centered approach in providing care
Intensive Care Unit (ICU)
Hanover & District Hospital has a two-bed ICU that contains specialized equipment and monitoring devices for seriously ill patients. Within the ICU, patients can be monitored using various methods including invasive monitoring methods Patients can be ventilated for up to 24-hours in our unit Patients who are cared for in the ICU will receive specialized care from nursing and medical staff
Rehabilitation Services
Rehabilitation Services operates Monday to Friday providing services to inpatients Services are provided by registered physiotherapists. If you are in the hospital and are in need of Rehabilitation Services, your physician will order these services and the physiotherapist will visit you in your room to organize the therapy
In-Patient Pharmacy
The pharmacy is responsible for overseeing safe and effective drug distribution in the hospital for inpatients and provides a source of drug information for health care professionals, patients and families Medication counselling, discharge counselling and medication summaries are available on request The Pharmacist is part of the multidisciplinary team and routinely works with the team to review patient drug therapy ensuring the best possible treatment
Family Centred Care Suites
Hanover & District Hospital has two Family Centered Care suites that have been designed to meet the complex needs of patients and their families The rooms are designed to have a “home like” atmosphere for patients and their families, with a fully equipped family room attached to a functional patient room. The suites themselves are utilized by a wide variety of patients and their families from birth to palliative care
The Hanover & District Hospital through a partnership with Diabetes Grey Bruce offers support to those with diabetes Diabetes Grey Bruce is a regional team of Registered Nurses, Registered Dietitians and a Nurse Practitioner who offers support, education and treatment options for those living with diabetes and for those at risk These services are provided free of charge and do not require a physician’s referral

The Diagnostic Imaging Department provides a variety of x-ray and ultrasound exams to service inpatients, outpatients and emergency patients They perform approximately 12,000 examinations per year Our professional team consists of Registered Medical Radiology Technologists, Registered Diagnostic Medical Sonographers and Consultant Radiologists The department is completely digital utilizing Picture Archive and Communication System (PACS) technology All imaging exams must be ordered by a Physician or Approved Health Care Professional

The Hanover & District Hospital Hemodialysis Unit is a satellite of London Health Sciences Center The unit offers hemodialysis for the clinically stable full care and selfcare dialysis patient
All patients are assessed in London before using the Hanover & District Hospital Unit. Transient or vacationing patients are accepted after being approved by the Kidney Care Centre in London The Dialysis Unit is staffed by specially trained Registered Nurses and Dialysis Unit Assistants.

Hanover & District Hospital’s Emergency Department is open 24 hours a day, 7 days a week with a physician on-site The triage system ensures that all patients will be seen by a nurse within 15 minutes of arrival. Our fully qualified emergency physicians and nurses proudly provide emergency services to over 17,000 patients per year
When Should I Go to the Emergency Department?
The Emergency Department's focus is to provide emergency medical care Don’t hesitate to visit the Emergency Department or call 911 if you are experiencing;
Chest pain;
Difficulty breathing, unexpected heavy breathing or shortness of breath;
Severe bleeding or head trauma;
Sudden onset of weakness, dizziness;
Loss of consciousness;
Severe abdominal pain;
Injuries to eyes, hands, ears;
Sudden change in speech or vision;
Broken bones; Major sprains;
Children experiencing a persistent fever (>38.5°C/100.4°F), persistent vomiting/diarrhea or will not eat or drink (particularly young children)
For minor health problems (e g coughs, colds, sore throats, rashes, prescription renewals) call your family doctor or nurse practitioner first. If you do not have a family doctor;

The Health Records Department is responsible for maintaining the confidentiality, accuracy, integrity, and quality of client/patient information for Hanover & District Hospital

Responds to inquiries and processing of requests for client/patient information
Coding, abstracting and submission of clinical data on inpatients, day surgery, outpatients and emergency patients.
Monitoring and improvement of data quality
Monitoring and compliance with chart content, completion and documentation requirements.
Transcription service and scanning of health records
Chart assembly, filing, retrieval, tracking and delivery
Assists in protecting the legal interests of the patient, facility and staff

A B O R A T O R Y
The Laboratory provides a broad range of testing, both within the department and by referral to other Laboratory facilities We are accredited by the Institute for Quality Management in Healthcare (IQMH). The Laboratory is located on the first floor of the Hospital right beside Diagnostic Imaging and is available to support inpatient, outpatient clinics and the Emergency Department on a 24/7 basis
An HDH requisition is required in order to receive outpatient services Outpatient services are available Monday - Friday from 9:00am - 12:00pm by booked appointment.

The physician or nurse practitioner (NP) will conduct their clinic, both with consults and procedures, allowing the community to be serviced closer to home rather than travelling to a larger center for this care Each clinic is equipped with the required equipment specific to each specialty

At Hanover & District Hospital we have two state-of-the-art surgical suites furnished with innovative technology Our staff are highly skilled Registered Nurses and Registered Practical Nurses that are certified in Perioperative nursing along with Registered Nurse First Assistants on staff, enabling us to provide advanced care to our surgical patients
The Surgical Services Department supports the Emergency Department and Family Centred Birthing Unit providing patient with efficient, superior surgical care

Patient & Family Advisory Committee (PFAC) – the steering committee for the Patient & Family Advisory Program at Hanover & District Hospital Members of this committee can also participate in various hospital committees including, Health Equity, Ethics Professional Practice, and Patient Safety Unit Based Committees and/or Focus Groups
Patient & Family Advisor may be asked to participate on various hospital projects as they occur to enhance patient and family centred care throughout the hospital

All funds raised through our Foundation directly benefit patient programs, equipment purchases, staff education, and renovations, with the main goal of enhancing patient care at the Hanover and District Hospital. Past projects such as the portable X-Ray Machine, an Ultrasound Machine and our Operating Room renovation are just a few examples of what the Foundation was able to fund thanks to the generosity of the community. Through appeals, our memorial garden, and several events such as our biennial gala and Brew Side Bingo there are various ways the community can participate in our fundraising goals and achievements
