Welcome to Holland Bloorview Kids Rehabilitation Hospital Holland Bloorview Kids Rehabilitation Hospital is Canada’s largest children’s rehabilitation teaching hospital, fully affiliated with the University of Toronto. As a world-class teaching hospital, Holland Bloorview trains a wide range of future health-care specialists in the field of childhood disability. During the course of your treatment or stay at Holland Bloorview, you can expect to meet healthcare trainees from a variety of specialties. As a client at Holland Bloorview, you play an important role in advancing their education. Your care is delivered in a team approach. Holland Bloorview Kids Rehabilitation Hospital is Canada’s largest children’s rehabilitation hospital. We pioneer treatments, technologies, therapies and real-world programs that give children with disabilities the tools to participate fully in life. Holland Bloorview serves approximately 7,000 children each year, with about 600 inpatient admissions and 58,000 outpatient visits. Holland Bloorview is a world-class teaching hospital fully affiliated with the University of Toronto. We train future healthcare specialists in the field of childhood disability. The Bloorview Research Institute is located onsite, allowing us to integrate cutting-edge research and teaching with frontline care to improve children’s quality of life.

WORKING TOGETHER TO MAKE DECISIONS Working together to make decisions At Holland Bloorview, the client, family and health-care team work together to make informed decisions about treatment choices. Treatment can include receiving medicines, check-ups, physiotherapy, life skills training and coaching. The goal is to help your child take part in making decisions about their health care, if they are able to make these decisions.

Who gives consent? Before starting a plan of treatment or treatment, members of the healthcare team must have consent. Saying “yes” to treatment is called consent. Saying “no” to treatment is called refusal. A decision about treatment is only made after a discussion between the client, family and health-care team, and all questions have been answered. Ontario law (called the Health Care Consent Act) helps people and families to make treatment decisions. A person is capable to decide about his/her own treatment if he/she is: • able to understand the information needed to make the decision • able to understand the harms, benefits and outcomes of having or not having the treatment

Driving Holland Bloorview is located in north Toronto at 150 Kilgour Road. The only public access to Kilgour Road is off of Bayview Avenue, north of Eglinton Avenue East and south of Lawrence Avenue.

1) From Davisville Station on the Yonge line, take the 11 Bayview bus Northbound and get off at Glenvale Road. Walk North to Kilgour Road and then East to 150 Kilgour Road 2) From Bayview Station on the Sheppard line, take the 11 Bayview bus Southbound and get off at Glenvale Road. Walk North to Kilgour Road and then East to 150 Kilgour Road 3) From Eglinton Station on the Yonge line, take any of the following buses: 54 Lawrence East, 51 Leslie, 34 Eglinton East, 100 Flemington Park and get off at Rumsey Road (3rd stop past Bayview Ave.) Walk North (600 metres) on Rumsey Road until you reach Kilgour Road. Â Holland Bloorview is straight ahead.

PARKING INFORMATION AND RATES To accommodate families and clients at Holland Bloorview Kids Rehabilitation Hospital, we provide accessible parking close to the main entrance and a covered drop-off area.

Parking rates*: • • • • •

Per half hour or less - $1.75 Daily (24 hour) weekday maximum - $9.00 Evening maximum (6 p.m.-6 a.m.) - $6.00 Weekend daily maximum (Sat 6 a.m. - Mon 6 a.m.) - $6.00 Monthly pass (available at reception) - $60.00

Holland Bloorview offers 20 minute free parking for drop offs. Monthly passes are also available at a cost of $60 per month. A ticket will be issued upon entering the Holland Bloorview parking lot. Payment is made using the pay machine (cash, debit and credit cards accepted) located inside the front entrance of the building. Payment can also be made when entering the parking gates with a credit card, the same card needs to be used on the way out. More pay and display parking is located outside of Holland Bloorview’s main parking lot, by Toronto Rehab and along the north side of Kilgour Road. Families who require in and out privileges should park in the pay and display area outside the main lot. *Parking rates are subject to change without notice.

Overnight accommodations at Holland Bloorview Holland Bloorview is pleased to offer overnight guests a stay in our accessible, comfortable and private Accommodation Suites on the fifth floor of our building. The Accommodation Suites provide Holland Bloorview families a secure and home-like environment while their child is an inpatient, or when accessing outpatient services. Each room has an accessible washroom, telephone, flat screen television with DVD player, small fridge and ceiling lift (family must provide own sling) in the bedroom. Accommodations can provide a commode chair, shower chair/bench or I.V. pole on request. *Respiratory equipment is not provided or serviced in accommodations. Guests enjoy a common cooking and eating area. Coin operated laundry facilities are also available within the accommodations area. Holland Bloorview is a non-smoking environment. The use of alcohol or illegal drugs is strictly forbidden.

Who can stay? Our priority is to serve Holland Bloorview families who are actively receiving services. Note: inpatient clients cannot stay in Accommodations. Rooms may be made available to other guests. Please check with the Accommodations Coordinator for availability and rates. Holland Bloorview reserves the right to restrict reservations.

Family and client rates We have studio rooms and suites available. Studios There are seven studio style rooms with two single beds. $45 per night. Suites There are three one-bedroom suites with two single beds and a double-size sofa bed in the living area. $65 per night Note: Rates are subject to change without notice. *Families needing financial support should approach their social worker for information about funding.

Sleeping chairs Inpatient rooms have arm chairs that pull out into sleeper chairs that a parent can use to sleep overnight at their child’s bedside. Only one parent is allowed to stay in the child’s room.

What to bring if your child is being admitted as an inpatient If you are coming to stay at Holland Bloorview in one of the inpatient units (rehabilitation or complex care), you should bring the following: • Your child’s clothes, including pajamas, bathrobe, sweaters, t-shirts, pants, slippers and shoes • Toys or family photos to remind your child of home (please no stuffed or plush toys) • Personal supplies such as a tooth brush, tooth paste, shampoo, soap, brush, lip balm, hand and body lotion *Please remember to put your name on everything. **Please note we are a scent free environment, we ask that you avoid any scented products.

For Your Safety

SAFETY AND SECURITY Fire In the event of a fire, a staff person will tell you what to do. Fire exits are clearly marked throughout Holland Bloorview. At Holland Bloorview, we take the safety of our clients, staff and visitors very seriously. Staff are trained regularly to follow fire procedures and the alarms are tested frequently.

Emergency number In the event of an emergency while you are at Holland Bloorview, call Holland Bloorview’s emergency number at ext. 5555 from any internal phone and state the nature of the emergency and your location for help to be sent.

Photo I.D. All Holland Bloorview employees are required to wear photo identification badges that include their name and title. You are entitled to know who is providing services to you at all times. If you are ever not sure who is working with you, ask to see his or her identification badge.

Smoking policy Holland Bloorview is a non-smoking environment. To accommodate visitors who wish to smoke, benches are located along Kilgour Road. Please be sure to use the cigarette receptacles provided. Smoking by minors (clients under 19 years old) is strictly prohibited. Anyone supplying cigarettes to a minor is in violation of the Smoke-Free Ontario Act. Permission to smoke by a parent or guardian does not override this policy.

When visiting our facility, please keep in mind • Fragrance free policy - Strong scented products such as perfume, hairspray, cologne and aftershave can cause or trigger health issues. In consideration for others, we request that you and your visitors do not wear scents when coming to Holland Bloorview. • Cell phone policy - Holland Bloorview insists upon limited use of cell phones and other personal electronic devices in our building. Cell phones, smartphones and walkie-talkies are not allowed in client rooms on 3 East and in the Bloorview school due to the potential interference they may cause with medical equipment. • Balloons - Please do not bring latex balloons or other latex products into Holland Bloorview due to the serious health problem for clients and individuals with allergies. • Nuts - Although nuts (including peanuts) can be purchased through vending machines in the cafeteria, we strongly urge you not to bring any nut-products to the inpatient area on the third floor or to any community programs such as swimming, spiral garden, etc.

Please wash or sanitize your hands when you enter the hospital, and when entering and exiting client rooms. Hand sanitizing stations are placed throughout Holland Bloorview, including at the front entrance. Hand washing and sanitizing are two of the best ways you and your healthcare team can prevent the spread of many infections. Everyone has an active role to play in ensuring good hand hygiene.

There are two ways to clean your hands: 1. Hand rub provided by the hospital

After cleaning, make sure your hands are dry before you touch anything.

Think about the times that you would clean your hands at home, such as: • Before and after eating • After using the washroom

• After sneezing or coughing

Make sure you wash your hands at these times and even more often when you are at Holland Bloorview. Your health-care team plays a big role in preventing the spread of infection. They are committed to giving you clean, safe care. There are important moments when your health-care team should wash their hands, such as: • • • •

After touching objects in a client’s room and going to another room Before and after treating clients Before and after special procedures, such as giving a needle After contact with body fluids, such as blood or urine

Flu vaccination or mask policy for care providers During the fall and winter flu season, you may notice some of your care providers wearing a mask. Staff, students and volunteers will either be vaccinated against the flu or wear masks in most areas of our facilities to reduce the spread of the illness and help keep our clients safe. There are many reasons why care providers choose to either get vaccinated or wear a mask. Please respect the decision and privacy of each person who provides care for your child.

All visitors to the inpatient units (third floor) are required to wear an inpatient visitor badge. Each family receives four of these badges upon admission, and will be asked to return them upon discharge. All other inpatient visitors – including children who are not clients – are required to sign in at the front reception and receive a temporary visitor label. Visitor labels are dated and can be thrown out at the end of the visit. There is no need to sign out at reception.

For Your Safety

VISITOR POLICY

Third floor staff will approach any person without a proper visitor label or family identification and direct them to reception. A maximum of four visitors can visit at one time. Visitors under the age of 12 must be accompanied and supervised by an adult.

Visiting Hours Parents are permitted to visit anytime.

For other relatives, siblings and friends, visiting hours are: • • • •

Monday to Thursday – 4 p.m. to 8 p.m. Friday (and any weekday before a statutory holiday) – 4 p.m. to 10 p.m. Saturday – 10 a.m. to 10 p.m. Sunday – 10 a.m. to 8 p.m.

Visit only if you are well Holland Bloorview encourages visitors to self-screen before entering the facility and during visits. Please reschedule your visit if you have one or more of the following symptoms not associated with a pre-existing condition: • • • • • • •

Cough Fever Shortness of breath Severe headache Unexplained muscle aches Unexplained extreme fatigue Vomiting and/or diarrhea

Social workers Social workers are professionally trained to support children, teens, young adults and families to cope with the impact of illness, disability, complex health needs and trauma. They care for the social, emotional and service needs of children, teens, young adults and their families, while respecting the need for client and family confidentiality.

How can a social worker help me? Social workers provide parents/caregivers with emotional support at various key points in their child’s development, for example the time around diagnosis, during school years or at points of hospitalization. Social workers are also available in helping adjust to adolescence and transitioning to young adulthood.

How do I contact a social worker at Holland Bloorview? Most programs at Holland Bloorview have a social worker available to help. Holland Bloorview offers: • The Garden Grill Cafeteria (located on the main floor, cash only) • Tim Horton’s Kiosk (located on the main floor, cash only) • Vending machines (located on the main floor and the third floor) The Garden Grill provides cafeteria service, which includes hot entrees, pizza, soups, sandwiches and daily made-to-order international cuisine, all prepared on-site. Microwaves are available in the cafeteria so that clients and families can heat their own meals. The Tim Hortons Kiosk sells freshly brewed coffee, hot beverages and freshly prepared in house sandwiches, snacks, take-away meals and bottled beverages.

Hours of operation: The Garden Grill Monday to Thursday 11 a.m. to 2 p.m. Fridays from 8 a.m. to 2 p.m.

Grocery Foundation Resource Centre We invite clients, families, staff, related professionals, students and volunteers to visit our resource centre to learn about Holland Bloorview’s programs, services, equipment and disability-focused information. The resource centre offers information on a range of topics related to growing up with a disability and different supports for children and youth as they develop and grow. Workshops and parent group activities offer families opportunities to meet other families. By networking with other parents and resource staff families can learn about different resources available to them. Health Sciences Library Serving clinicians, researchers, students, staff, clients and families, Holland Bloorview’s Health Sciences Library is a full teaching hospital library of the University of Toronto and part of the Health Science Information Consortium of Toronto. The library collection is focused on the subject areas of programs offered at Holland Bloorview, including child development, parenting, nutrition, cerebral palsy, spina bifida, neuromuscular disorders, brain injury, cleft palate and genetic disorders.

The library is staffed by professional library personnel to provide reference services, literature searches, interlibrary loan and circulation, etc. There are four computer workstations, located near the entrance, that offer access to the library catalogue and Internet. Photocopying and printing are available for 10 cents per page.

Legal assistance program In partnership, Pro Bono Law Ontario (PBLO) and Holland Bloorview offer a free legal assistance program to low and moderate-income Holland Bloorview clients and families. The program’s onsite lawyer can assist clients and families with legal advice about non-medical issues that may impact a child’s health or a family’s capacity to care for their child. The types of legal issues can include housing, education, immigration and employment, to name just a few. To learn more about this program, talk to your social worker, physician or clinician or visit a family support specialist in the Grocery Foundation Resource Centre, located on the first floor.

Telephone information Warmline The Holland Bloorview Kids Rehabilitation Hospital Telephone Information Warmline assists you with information about our programs and services as well as a range of topics and resources related to childhood disability. The Warmline is made possible in partnership with Cisco Canada. This telephone service offers live assistance from one of our family support specialists between the hours of 10 a.m. to 12 p.m., Monday to Friday. If you call the Warmline outside the hours of operation, please leave the spelling of your name and your area code, telephone number and any questions you may have, and a family support specialist will return your call by the next business day.

Get involved One of Holland Bloorview’s core values is Client and Family Centred Care (CFCC). It means that we are committed to a family centred approach in all we do. At the core of CFCC is the Family Leadership Program. The Family Leadership Program creates partnerships with families so that their unique wants, needs and preferences are included in all areas of program planning, delivery and evaluation at Holland Bloorview. The Family Leadership Program was formalized in 2011 and now consists of 90+ volunteers that sit on 35+ standing committees and external groups, and 15+ working groups (as of 2013). Family leaders also provide one on one peer support from Monday to Friday and teach at new staff orientation, educate students and speak at conferences globally. Depending on how much time you have to volunteer, your experience or if you want to participate from home, we will find the best fit for you as a family leader. Training will be provided for all positions.

Ways to get involved Be a family advisor We look to family advisors to share their ideas and experiences to inform hospital programs, research, events and written materials. Family advisors participate on hospital committees, on project teams or share feedback individually. Family advisors can take part in most opportunities in person or by phone. Be a family mentor Family mentors provide valuable peer support to other Holland Bloorview family members by providing information and resources. Mentors may volunteer in the family resource centre, answer the Holland Bloorview telephone Warmline or support families in different clinic areas. Be a teacher Family teachers create a learning opportunity for health care providers by sharing their family’s health care experience. Family teachers participate in small group discussions designed to train health care providers or present at new staff orientation, grand rounds and conferences. Be a member of the family advisory committee (FAC) The family advisory committee is a dedicated group of parents that meets monthly with senior hospital leaders to discuss ways to advance the practice of client and family centred care in hospital programs and processes. Be a member of the Bloorview Research Institute family engagement committee (BRI FEC) The Bloorview Research Institute Family Engagement Committee works with senior leaders from the Bloorview Research Institute to define how we engage families in research, including review of all proposals and grants, how we recruit for research initiatives and how we’ll communicate the findings. Review of grants and proposals is a great way to participate from home.

Join the children’s advisory council (CAC) We know the best care happens when families and staff work together. Holland Bloorview’s Children’s Advisory Council is a place where kids can share their ideas on how to make our services better. Clients and their siblings aged three to 13 are welcome. Join the youth advisory council (YAC) The Youth Advisory Council (YAC) has been an important part of Holland Bloorview since 1996. The council’s members are youth aged 14 and older who have received services at Holland Bloorview. With meetings every month from September through June, YAC provides a sounding board for staff, clients, families and students. YAC provides input into the development and evaluation of Holland Bloorview’s programs and services. It also offers youth and young adults the opportunity to develop advocacy skills and be involved in helping to create possibilities for younger generations of Holland Bloorview clients. BLOOM – Holland Bloorview’s magazine for families BLOOM is Holland Bloorview’s how-to magazine on parenting children with disabilities. BLOOM addresses the unique joys and challenges of parenting kids with disabilities by combining firsthand family insights and advice from leading professional experts. We also mail to families who have used our services in the previous eight months.

Respite care Holland Bloorview offers a range of overnight and day respite services. Respite care is a family support service that provides temporary relief from the physical and emotional demands involved in caring for a child with a disability. Holland Bloorview offers overnight respite services, as well as day respite services. Dental Services and Cleft Lip and Palate Program In our state-of-the-art facilities, our specialists provide a full range of dental services in two key areas: pediatric dentistry and the cleft lip and palate program. Pediatric dentistry specializes in providing general dental services to clients with complex medical needs, to children who are undergoing rehabilitation. We see both inpatients and outpatients whose treatment is best managed in a multidisciplinary environment by a team who understands the needs of children with disability and their families. Cleft lip and palate dental services are offered to children who have facial differences due to congenital or hereditary factors, trauma, tumors or acquired or congenital dental dysfunction. Our focus is a family-centred approach to care that includes assessment, treatment, surveillance and follow-up. Services are offered to siblings of clients. A referral is required from either a physician or dentist. Orthotics and prosthetics The prosthetic and orthotic team serves clients of all ages, from infants to adults. Families are presented with devices and treatment options for children as young as three months of age. Paying close attention to the changing needs of our clients, our multidisciplinary team works closely with clients to make sure their device is designed to meet their individual needs and lifestyles. Music and the Arts Holland Bloorview’s Centre for the Arts offers a wide range of year-round art and music programs, workshops and special events for children and youth of all abilities. All children are welcome, however priority is given to those with special needs unless otherwise stated.

School Preschoolers in the inpatient units attend a bright, accessible nursery called the therapeutic playroom, while school-aged students go to the Bloorview School Authority or are bussed to Toronto schools with one-to-one nursing support.

Snoezelen Snoezelen rooms are multi-sensory environments that use music, lighting effects, gentle vibrations, tactile sensations and aromatherapy to encourage relaxation, social skills and heighten sensory awareness. Holland Bloorview has an inpatient Snoezelen room, as well as a community Snoezelen room and Snoezelen pool that are open to both Holland Bloorview and non-Holland Bloorview families, service agencies and schools for children and adults with disabilities. Ronald McDonald Playroom The Ronald McDonald playroom is a supervised fun zone for outpatient clients and their siblings to play and relax. Children can keep busy and have fun while caregivers have a chance to consult with doctors and therapists privately, research information in the Grocery Foundation Resource Centre, attend information sessions and workshops, or take a quick coffee break. This drop-in service is free to all clients and their siblings age two and a half years and up visiting Holland Bloorview for a clinic, doctor’s appointment or consultation. Parents are given a pager to tell them if they are needed in the playroom. Hours of Operation: Monday – Friday: 9 a.m. - 12 p.m. and 1 p.m. - 4:30 p.m. Booking is limited to a maximum of two hours during busy times. Community Programs and Services: Aquatics Recreational or instructional aquatic programs are offered seven days a week to the general community serving infants to seniors. Our aquatics programs are fully integrated and open to everyone, however registration priority is given to swimmers who have a disability and their siblings. Our pool has unique features to promote accessibility: our water temperature is 33ºC (92ºF), our change rooms (including family style) are fully accessible and equipped with ceiling lifts and we have an excellent variety of water chairs and a ramp entry into the pool. During swimming lessons, volunteer support is available to swimmers who require one-on-one support in order to participate safely. The Bloorview Research Institute is dedicated to improving the lives of children with disabilities and their families through research. Research conducted in the institute focuses on enhancing care, developing assistive technologies, and evaluating all the barriers restricting inclusion and participation of children with disabilities. During your care at Holland Bloorview, you may be asked to participate in a research study. Participating in a research study is completely voluntary and it is up to you and your family to decide if participating is right for you. If you choose not to participate, it will not affect your clinical care at Holland Bloorview. The study may not help you personally, but the results could help other children with similar health issues in the future.

Volunteer Resources recruits, screens and trains qualified and caring volunteers from the community to support staff in providing the very best programs for clients at Holland Bloorview. There are approximately 1,000 individuals a year, from across the greater Toronto area and speaking over 60 languages, who volunteer at Holland Bloorview. You’ll see volunteers throughout the building from 8:30 a.m. until 8:30 p.m., seven days a week.
