ABOUT US OUR HISTORY

Mount Sinai Hospital was established in 1923 as a 33-bed maternity and convalescent facility. Today, we are a state-of-the-art, 472-bed teaching and research hospital but the reason we are here has not changed since the day we opened out doors — serving our patients and their families. More than 80 years later, Mount Sinai has become one of Canada’s pre-eminent patient care, research and academic health science centres. We are known internationally for our leading-edge discoveries, our outstanding doctors, nurses and support staff, and the compassionate care we provide to patients and their families. Mount Sinai is particularly well known for our programs in areas such as oncology, internal medicine, women’s and infants’ health, and chronic adult diseases including diabetes and arthritis. Our renowned scientists at the Samuel Lunenfeld Research Institute are working together with clinicians to increase their collective knowledge in these and other areas. By working collaboratively and in teams that span medical disciplines, we are leading the way. We are making important discoveries which we hope will lead to treatments and cures that will help patients at home and around the world. We know that science and medicine are moving forward at a rapid pace. We are continually changing and evolving to ensure that we can continue to teach and lead. As part of our growth, we are embarking on an exciting new expansion project called Renew Sinai. By updating our infrastructure and adding new space to develop our Women’s and Infants’ Health Program, we will continue to provide the best and safest possible care to our patients for generations to come.

CENTRES OF EXCELLENCE

Mount Sinai is one of the only health-care institutions in Canada to bring research and clinical practice together into five main areas of strength called Centres of Excellence. These Centres include: • Samuel Lunenfeld Research Institute • Frances Bloomberg Centre for Women’s and Infants’ Health • Christopher Sharp Centre for Surgery & Oncology • Daryl A. Katz Centre for Urgent and Critical Care • Centre for Inflammatory Bowel Disease • Centre for Musculoskeletal Disease Clinicians and scientists work collaboratively across these centres to improve patient care, advance new treatments and discoveries and bring therapies from the research bench to the patient bedside.

PATIENT AND FAMILY-CENTRED CARE

Patient and Family-Centred Care is not just a catchphrase or concept, but rather a tangible model of care, including initiatives, processes, principles and tools to do the job. It means we don’t just treat the patient, but engage our patients and their families in the care process. They demand it of us, and we demand it of ourselves. — Joseph Mapa, President and CEO, Mount Sinai Hospital At Mount Sinai Hospital, patients and their families are at the core of everything we do, and at the heart of our Patient and Family-Centred Care model is a genuine partnership between the health-care teams and our patients and their families. We are fully committed to this model of care, and seek to enhance it by developing the trust and respecting the voice, dignity and autonomy of our patients. We live the values of Patient and Family-Centred Care by adhering to the following principles: Dignity and Respect: As an inter-professional health-care team, we listen to and honour the perspectives, wishes and choices of our patients and their families. Patient and family knowledge, values, beliefs and culture are incorporated into the planning and delivery of high-quality, compassionate care. Information Sharing: As health-care providers, we openly communicate and share timely, complete and accurate information with our patients to help them understand their options, and encourage them to participate in decisions about their care. Participation: As a health-care team, we encourage and support patients and their families to participate in and make decisions about the care they receive. We recognize that having a voice in decisions about care maintains patient dignity and independence. Collaboration: As health-care providers, we work together with patients and families to design, implement and deliver the best care possible. This philosophy goes into everything we do, from program development and building design to professional education and research.

RENEW SINAI

Mount Sinai Hospital is growing for the future. Renew Sinai, our multi-year capital redevelopment project, is well under way and represents one of the biggest expansions in Mount Sinai’s 87-year history. When complete, Renew Sinai will provide for a bigger, brighter, safer, more technologically advanced and more pleasant environment for our patients, their families and our staff. It’s all about providing the best care in the best environment. The first part of Renew Sinai includes structural upgrades to the main building at 600 University Avenue and adding six new floors to its west tower to accommodate our Frances Bloomberg Centre for Women’s and Infants’ Health and other patient programs. We will be adding more private family spaces and incorporating state-of-art technology to care for the more than 1,000 infants a year in our Neonatal Intensive Care Unit (NICU). The improvements will also help provide the best medicine possible for the almost 6,700 babies our excellent health-care team delivers every year. We thank our patients for their patience during this time of noise and disruption. We appreciate your understanding as we improve Mount Sinai so we can continue to provide The Best Medicine. For more information and updates on Renew Sinai, visit our website at www.mountsinai.ca.

MOUNT SINAI HOSPITAL AUXILIARY When Rose Torno founded the Mount Sinai Hospital Auxiliary in 1954, she began a strong tradition of community support that continues to grow each year. With more than 1,300 members consisting of 400 active volunteers, the Auxiliary is the largest of its kind in North America. With a mandate to support the Hospital through fundraising initiatives and events, including an annual gala and Perspectives magazine, the Mount Sinai Hospital Auxiliary has been committed to supporting the Hospital for over 50 years. Other initiatives supported through the Auxiliary include: Baby Photography: Parents can have their baby’s photo taken before they leave the Hospital. This program is run by the Auxiliary through Just Arrived Baby Photographers — trained to photograph newborns with care. Give a Gift Twice Program: The Mount Sinai Hospital Auxiliary has an affiliate arrangement with Somerset Flowers and Gifts called Give a Gift Twice. When someone purchases floral arrangements or gift baskets through www.somersetgifts.com by phone or online, Somerset donates 10 per cent of sales back to the Auxiliary. Floral arrangements and gift baskets are priced lower than retail outlets, and all purchases can be delivered throughout the Greater Toronto Area. ShopSinai.ca: Is an online branded boutique with items for men, women, baby, spa and more. For added convenience, the Auxiliary will beautifully wrap and deliver your gifts within the Hospital free of charge. Local delivery is available for a nominal fee. Proceeds support the Auxiliary’s $4.5-million pledge to women’s health benefitting Mount Sinai Hospital’s major pillars — patient care, research, technology and education. The site also features links to online retailers selling baby baskets, flowers, chocolates, personalized gifts and more. Affiliate retailers donate a percentage of the sale back to Mount Sinai Hospital. For more information please visit www.shopsinai.ca or call 416-586-8290.

MOUNT SINAI HOSPITAL FOUNDATION The Mount Sinai Hospital Foundation raises and stewards funds to support Mount Sinai Hospital. Each year, thousands of donors are inspired to contribute to Mount Sinai, making it possible for us to upgrade clinical areas, purchase state-ofthe-art equipment, support our world-renowned researchers across the Hospital and at the Samuel Lunenfeld Research Institute of Mount Sinai Hospital, and train the next generation of health-care workers. The generosity of our donors helps provide the Hospital with the flexibility it requires to meet the diverse and complex needs of our patients. Mount Sinai’s first steps on the path to medical excellence were made possible by a culture of philanthropy. We owe our very existence to passionate volunteers and donors. And just as in 1923, when four visionary community members saw the potential and raised $12,000, much of it in nickels and dimes, we need you. Without philanthropy, Mount Sinai would be an ordinary hospital. With you, it is extraordinary.

There are many ways you can give: • Make a Donation Your gifts are applied where funds are most urgently needed, to purchase new equipment and technology, and enable Mount Sinai to do all it can for our staff, patients and families. • Support “Right from the Start” Pledge your support for our revitalized and expanded Frances Bloomberg Centre for Women’s and Infants’ Health, now under construction on top of our Murray Street wing. • Support “Research Creates the Best Medicine” Support the Samuel Lunenfeld Research Institute of Mount Sinai Hospital, one of the world’s leading biomedical research institutes, where scientific discoveries are used to create better diagnostic tools and treatments for our patients. • Order Our Tribute Cards Beautiful donation cards can be ordered to express your thanks to your caregivers at Mount Sinai, to mark special occasions, such as holidays and anniversaries, and to honour a loved one. The minimum contribution is $10 and all contributions are income tax receipted. For your convenience, we accept Visa, MasterCard and American Express. • Join the Sinai Stars By joining the Sinai Stars monthly giving program, your donation helps fuel The Best Medicine all year long. • Join Leadership Sinai Leadership Sinai is a growing community of dynamic, young philanthropists and volunteer leaders working to advance the mission of Mount Sinai Hospital. Leadership Sinai stands as one of the Hospital’s most energetic and visionary volunteer fundraising groups.

Research at Mount Sinai Hospital… The Samuel Lunenfeld Research Institute

Mount Sinai Hospital is committed to excellent health research. Located within the Hospital is the world-class Samuel Lunenfeld Research Institute where award-winning scientists work toward a better understanding of cancer, gastrointestinal disease, osteoporosis, mental health conditions, healthy pregnancies, arthritis and diabetes.

Did you know…?

• Research leads to new treatments and improved patient care. • Many of our researchers are also physicians and surgeons. • Cancer is one of the most researched diseases at the Samuel Lunenfeld Research Institute. • Breast cancer is the most frequently diagnosed cancer among Canadian women. Our scientists are looking at how Vitamin D, exercise and diet, and genes affect breast cancer. • In Canada, more than two million people have diabetes. Our researchers have found that certain genes may lead to improved treatments for Type 2 diabetes. • 20 per cent of Canadians will experience mental illness in their lifetime. Our scientists have found genes linked to schizophrenia and bipolar disorder. • Mount Sinai Hospital delivers more than 6,700 babies every year, making us one of the top three labour and delivery sites in Canada. Our researchers work together to find ways to predict early labour, and improve the health of pregnant women and infants. “We hope to one day be able to offer a real cure for the many people suffering from conditions that are now considered incurable.” — Dr. Andras Nagy, Senior Researcher, Samuel Lunenfeld Research Institute For more information on the Samuel Lunenfeld Research Institute, please visit www.lunenfeld.ca.

CARE MAIL

At Mount Sinai Hospital, we recognize that patient-centred care starts with support from family and friends. Mount Sinai offers Care Mail services — this program allows loved ones to stay in touch during a hospital stay. Care Mail: Patients at Mount Sinai Hospital are now a click away from their loved ones with the launch of the Hospital’s new online patient greeting service, Care Mail. Family and friends need only visit the MountMount Sinai Hospital website, www.mountsinai.ca and click on the E-mail a Patient tab on the left-hand side of the screen, to send their best wishes and greetings to patients in Hospital. The basic service is free: the individual fills out an online form with a greeting and the patient’s name and room number (if known). Hospital volunteers will print and deliver the email message in a sealed envelope within 24 hours of receiving it. Email received on weekends and holidays will be delivered the next business day. Unfortunately, we cannot deliver email messages after a patient has been discharged from the Hospital and gone home.

CELLULAR PHONES

Cellular phones can be used in most areas of the Hospital. To ensure that patients receive the best care — and the most rest — cell phones are restricted in patient-care areas or those areas marked with “no cellphone zone” posters.

CHAPLAINCY

Mount Sinai Hospital respects the right of all people to have their individual beliefs and religious affiliations and is committed to service that is characterized by care, compassion and concern for the whole person. This includes care for physical, emotional, social and spiritual needs. When you are facing difficult decisions, or when you are feeling alone or in pain, your spirituality can be a source of guidance and hope. The chaplains at Mount Sinai Hospital are trained to offer multi-faith spiritual care and support. We are represented by a rabbi, an ecumenical chaplain, a Roman Catholic chaplain and a chaplain specializing in ICU and the Women’s and Infants’ Health Program. If you would like us to contact someone from your own faith group, we will try our best. Belonging to a religion is not a requirement for chaplaincy support. Our chaplains are available for spiritual care and support 24 hours a day, seven days a week to patients, visitors and staff. Prayer, meditation and worship services are posted on the Chaplaincy board outside the Spiritual Oasis, 4th floor. For more information, please call us at 416-586-4800 ext. 8567.

The Hospital Synagogue is located near the Emergency Department on the (3rd) main floor. It is open all day and can be opened by Security if needed at night.

If you have any problems related to your discharge, please speak to your nurse immediately. Please make sure that you have all of your belongings, as well as your discharge care instructions and any needed prescriptions before you leave the Hospital. Please arrange to have your accounts settled before you leave. The Cashier’s Office is located on the main floor and is open from Monday to Friday, 8:30 a.m. to 3 p.m. You can also pay in the Admitting Office after hours, on weekends and on statutory holidays. HEALTH-CARE TEAM

Mount Sinai Hospital is affiliated with the University of Toronto. As a teaching hospital, Mount Sinai is committed to the education and training of future doctors and other health-care professionals. Your caregivers are dynamic people who have completed rigorous academic programs. While in the Hospital, your medical team may include: • Attending physician: the staff doctor responsible for your complete medical care; • Resident: a doctor continuing his/her education to specialize in one field of medicine; and • Medical student: a student doctor in his/her final two years of medical school. We also participate in the education of students from many health professions and academic institutions such as the University of Toronto, Ryerson University, George Brown College and the Michener Institute for Applied Health Sciences. They are here to learn and do not make decisions or provide treatment. Your health-care team may also include primary nurses, care managers, clinical nurse specialists, nurse practitioners, service assistants and unit clerks. During your stay, you may receive care from a variety of other professionals including dietitians, physiotherapists, occupational therapists, social workers, pharmacists, respiratory therapists, technicians and technologists from various services.

Teaching Attending System: Information for Patients in the Department of Medicine

There are four medical teams in the Department of General Medicine. Each medical team includes an attending physician, several residents and senior medical students. The team works under the supervision of an attending physician. Attending physicians work on a rotation system so it is possible that your care may not be supervised by the same physician throughout your stay. However, be assured that each attending physician is fully informed of your health-care needs. If your personal physician is not assigned as your attending physician, he or she is encouraged to visit you regularly in the Hospital and to keep in close touch with the attending physician. You may also wish to call your personal physician to tell him or her about your hospital stay. When you are discharged, your personal physician will receive a complete report of your hospital stay and will resume responsibility for your care. If you have questions about these arrangements, please speak to your primary nurse, personal physician or attending physician.

PREVENTING INFECTIONS

Mount Sinai Hospital staff work hard to prevent infections and provide the safest possible environment. Certain health conditions and treatments, such as surgery or chemotherapy, put patients at risk for developing an infection. The best way to protect yourself against infection, inside and outside of the Hospital, is by washing your hands regularly and thoroughly. Hospital staff, patients, and visitors should clean their hands every time they enter a patient room. The easiest way to do this is to use the waterless hand rub that is mounted outside each room. One pump provides enough hand rub to spread over all the surfaces of your hands to remove germs. We routinely check for antibiotic-resistant germs to keep them from being passed from person to person. These germs are not harmful to healthy people, but they can be a problem for those who are seriously ill. Some patients might not be sick from these germs, but could be carrying these germs on their skin, nose or in their bowels. Knowing if you are carrying these germs will help your doctors to choose the right antibiotics for treating infections. We regularly ask patients to have a nose and rectal swab done to test for these germs. If you are carrying an antibiotic-resistant germ, Hospital staff may use special precautions when they take care of you. This might include a private room and additional steps, such as using gowns, gloves and masks for your care. Please let us know when you are admitted if you have ever been in contact with an antibiotic-resistant germ or if you have been in contact with people with infections before coming to the Hospital. If a family member or friend is ill themselves, particularly if they have a “cold”, vomiting, or diarrhea, they should not visit you in the hospital. If you are not sure about whether someone should visit, please check with your nurse. If you or your visitors have questions or comments about infections or the prevention of infections, please call the Infection Control Office at any time at 416-586-3118. You are welcome to visit our website at: www.microbiology.mtsinai.on.ca.

PRIVACY

Effective health-care delivery depends on an atmosphere of trust between patients and caregivers. Rest assured Mount Sinai Hospital respects your rights to access your personal health information, to have it kept private and confidential and to insist on its accuracy. If you have questions about why we collect your personal health information and how it is used and collected or disclosed, look for a copy of Mount Sinai Hospital’s Information Practices, available in many patient care areas. If you would like to obtain a copy of your record or have access to it, please contact our Health Records Department at ext. 2649. If you have any further questions or if you would like to access or correct your personal health information, or make a complaint please contact our Privacy Office at ext. 2101 or privacy@mtsinai.on.ca.
