Our Service Stands Out • VON Hastings, Northumberland, Prince Edward delivers care, 24 hours a day, 7 days a week, all year long. • VON delivers a variety of nursing care, specialized services, home support services, and Community Support Services. • VON offers personally designed care for the needs of our clients, caregivers and families. • As a charitable not-for-profit health care agency, donations are used to provide free or subsidized services to persons who cannot afford to pay. • For expert care and friendly, professional services, you can depend on VON’s carefully chosen health care team of Registered Nurses, Registered Practical Nurses, Health Care Aides, Personal Support Workers and Nurse Practitioners. • Experienced and specially trained staff provide a variety of in-home quality Nursing, PSW, and Community Support Services to people of all ages, backgrounds and needs. • Continuing education is a VON priority. VON staff attend specialized training and advanced courses in community health care to ensure they remain up to date with current practices and regulations. • For services to help you remain at home, actively and independently, VON offers Meals on Wheels, and a full range of Home Care and Community Support Services. • VON Nurses link people with helpful community resources. • Every effort is made to speak with clients in their preferred language. • General Visiting Nursing All VON nursing services are provided by RNs or RPNs, holding current, valid Certificates of Competence issued by the College of Nurses of Ontario. The objective of VON Nursing Services is to assist people to remain at home or to return to their normal daily activities as soon as possible. VON nurses help individuals to make informed decisions about their health, and enable clients to attain and maintain their highest level of independence and quality of life. VON Nurses provide support to caregivers and families in their caregiving roles. VON is committed to working with the Access Centres and other community partners to provide all residents of Hastings, Northumberland, Prince Edward access to the best quality nursing services.

Professional Nursing Services
VON provides 24 hour a day service. A professional nurse and manager are on call 24 hours a day to provide urgent services to clients outside of regular working hours.
• Palliative Care Program VON understands just how devastating the diagnosis of a potentially non-curable illness can be, not only to the person with the illness but also to his/her family and caregivers. At VON, we have nurses with specialized training in Oncology (care of clients living with cancer) and Palliative Care (nursing with a focus on promoting optimum quality of life at all stages of a non-curable illness). We are committed to providing care and compassion and the most up to date knowledge and skills available. We understand that in these circumstances only the best will do. Our Palliative Care Team’s goal is to provide optimum knowledge to all nurses at VON in matters of pain control, symptom management, and on-going support of the client, family and/or caregivers. This is achieved through active participation with Cancer Care Ontario, the Palliative Pain and Symptom Management Programs through the Ministry of Health, the Quinte Regional Integrated Palliative Care Team and focus groups in the community. VON Palliative Care nurses also collaborate with the client and caregivers to provide client-focused care. Any individual faced with a life threatening illness can be confident that the care they receive from VON will include ongoing concern for their physical comfort and emotional well-being as well as ongoing compassionate support to the client, family and/or caregivers, thereby greatly enhancing quality of life for those involved. Services may be paid by the provincial health plan, insurance companies, or on a private pay basis. VON HASTINGS NORTHUMBERLAND PRINCE EDWARD

Comfort
• Intravenous Therapy Program VON’s Intravenous Therapy Program delivers skilled nursing care and teaching to individuals in need of intravenous medications and/or solutions to manage and to maintain their health. These services are provided in the comfort of your own home. This enables you to remain in your home and actively participate in your community. Care is provided by Registered Nurses proficient in I.V. therapy, pump management, ambulatory infusion and central lines. Fee for services may be covered by the provincial health plan, private insurance or on a private pay basis.
• Wound Specialists People with ostomies, fistulas, ulcers, and incontinence have very special needs. VON Wound Specialists are nurses who are specially trained to deal with the problems associated with these conditions. They keep abreast of state-of-the-art equipment and treatment programs. The aim of VON Wound Specialists (WS) is to promote a person’s dignity, independence, and participation in family and community life. Services are available in home, in long term care facilities, and clinic settings. Services are directed to the person involved as well as their family and caregivers.
• Ostomy Care A stoma is a surgically created opening in the abdomen that is made when a bowel or bladder diversion is needed. The Wound Specialists (WS) offer pre-operative counselling to help the person prepare for the change in body function and body image after surgery. Post-operatively, the WS provides instruction on the techniques and appliances necessary for care of an ostomy. The WS also provides emotional support to the individual and their family.
• Dialysis Program VON’s specially trained Dialysis Nurses offer teaching, assistance with, and performance of dialysis in your home. These services are available 24 hours a day, 7 days a week, all year long. VON nurses provide a professional link and ongoing communication between the client, physician, and the Home Peritoneal Dialysis Unit.

• Pre-dialysis assessment of client, family and home environment to determine suitability for home dialysis; • Follow-up teaching at home with new clients starting on dialysis; • Perform dialysis exchanges and exit site care using a variety of equipment when clients and/or family are unable to do so;

• Foot Care Program

• Carry out dialysis exchanges, administration of intraperitoneal medications when a client’s condition necessitates; • Teach and monitor medication administration in the bags and/or by mouth; • Assist client in ordering dialysis supplies on a monthly basis; • Provide respite care for a family to take time away; • Provide continuing education and health teaching to other health care agencies, clients, client families, and the community. • Veterans’ Independence Program Assessment Visits. Working with the Federal Department of Veterans Affairs, the VON Nurse helps veterans with long-term health problems and determines the kind of assistance they need.
VON Hastings, Northumberland, Prince Edward offers foot care in clinic settings. Healthy lifestyle, optimum mobility and independence are important for people of all ages. The average person walks approximately 325,000 kms in a lifetime and puts 700 tonnes of weight on each foot every day. Occupational activities, athletics, improper foot wear and individual differences in gait also impact foot health. Special consideration also needs to be given to individuals who have certain medical conditions, such as: diabetes, arthritis or neuromuscular disorders. Some medications also affect the structure and health of the foot. Our high standards of care are based on evidence and research. Information is kept current and monitored by the national Best Practice Team. The nurses who provide care in this program have education in basic, advanced and diabetic foot care.
Services include:
• A general overview of your health and a thorough assessment of your feet • Trimming of nails • Treatment of ingrown and thickened nails • Callus and corn care • Dry skin care • Foot care education • Comfort measures • Referral to other health care professionals where necessary

Home & Personal Support Program Home Support Services enhance an individual’s quality of life by providing them with the personal assistance and support needed to make them comfortable in their own homes.

Specially Trained Staff Our dedicated staff of Personal Support Workers provide assistance with light household duties and caregiver respite.

(Seniors Maintaining Active Roles Together)® Benefits of Exercise
Participation in physical activity programs like SMART® can help: • Improve physical health, mobility, independence and energy • Increase and maintain strength, coordination, balance, and flexibility; helping to prevent falls • Manage weight and improve lung and heart function • Prevent or manage: high blood pressure, cholesterol, respiratory conditions, stroke, stress, cancer, arthritis, osteoporosis, and many other sudden or chronic conditions • Provide opportunities to meet new people and socialize in group settings

SMART
Participants can enroll themselves or be referred by healthcare providers, family or friends. Participants are encouraged to work at their own pace, wear comfortable clothing and supportive shoes.
Fall Prevention & Exercise Program
A free 12 week, education and exercise program aimed at seniors. The program will help you stay strong, on your feet, healthy and injury free. Registration is required and there is a pre- and post assessment. Learn to minimize the risks of falls by making easy changes.
Group Exercise
The SMART Group Program provides exercise classes to seniors who wish to improve their strength, balance and flexibility, regardless of their current ability, free of charge. Classes consist of low impact aerobics followed by strength, balance and flexibility training. Chairs are available and exercise bands are available for purchase. There is no mat work. Registration is required. Classes are held in the community; in church halls, condominiums and apartment buildings. The sessions are typically twice a week, each lasting an hour, and adapted for all participant levels. VON HASTINGS NORTHUMBERLAND PRINCE EDWARD

Community Support Services The Southeast Local Health Integration Network partially funds community support, volunteer based programs. These services are provided to persons with physical and/or age related cognitive disabilities and their caregivers. VON supports individuals to remain healthy, active and independent, so they can remain in their own homes for as long as possible. Some user fees apply. Subsidies available. • Meals on Wheels: Nutritious, hot and frozen meals are delivered to the client’s home by caring volunteers. This service is available for a small fee for residents within the Quinte West community. Hot meals, including dessert, are prepared by a local caterer. Our frozen menu includes entrees, breakfast foods, soups and dessert items. Special diet menu items are also available through our frozen meal supplier. • Volunteer Visiting: Trained volunteers provide companionship and a link to the community. Regular weekly visits break the isolation of living alone. • Congregate Dining: Noontime dinners are offered in Trenton once a month. Social interaction and educational opportunities are provided. • Volunteer Transportation: A transportation service is available, for a small fee, to provide clients with rides to medical appointments, errands and social outings. This service helps individuals remain independent, and provides regular contact with friendly volunteers. • Seniors’ Fitness Programs: VON has several fitness programs for older adults encouraging increased physical activity as part of daily living. They include monitored fitness activities and wellness education to enhance mobility, decrease risk of falls and injury, delay the progression of certain diseases, and assist with stroke recovery. Exercise programs are facilitated by qualified fitness leaders and are available at home or in a group setting. • Telephone Reassurance: Volunteers make a friendly call in the morning providing family and caregiver reassurance. There is no charge for this service.

• Home Help Maintenance: This program is a brokerage system in which VON helps clients find independent contractors to assist with yard work, routine housework tasks, snow shoveling, and grocery shopping. Fees are agreed upon between the worker and the client. • Assisted Living Program: The VON Assisted Living Program provides services to eligible seniors living within designated geographic areas in Belleville, Trenton, Cobourg and Campbellford. Services include personal support assistance such as dressing, personal hygiene, assisting and monitoring medication use and other routine activities of living; homemaking services including shopping, housecleaning and meal preparation; security checks or reassurance services including visits to ensure client health or safety; and care coordination. Funded by the LHIN, services are flexible to the needs of the client and available 24 hours a day, 7 days a week on both a scheduled and unscheduled basis. Eligible seniors include those at high risk of requiring institutional care without this flexible and client-centred style of enhanced support. The VON Assisted Living Program allows these individuals to remain in their own homes with the support they need to maintain their health and safety. Access to this service requires a referral from the Community Care Access Centre.
VON Adult Day Services VON Adult Day Programs offer client-centred, compassionate care for individuals in the community living with cognitive and physical challenges. Caregivers experience worry-free time away while their loved one attends either full or half-day programs that focus on supporting strengths and capabilities of the individuals who attend. Caring and specially trained staff support individuals to remain active, engaged and secure while participating in the activities designed to meet their social and recreational needs. Services are available in the following communities: Quinte West, Picton, Brighton, Cobourg, Bancroft, Belleville, Port Hope and Campbellford. SMILE Seniors Managing Independent Living Easily
General Information 1. What is the SMILE Program? The SMILE program makes it possible for frail seniors, who are at risk of loss of independence, to remain in their own homes. The program provides access to services for the seniors (and their caregivers). Dignity is a matter of choice, and the SMILE program offers seniors options in managing their own care. Seniors can choose what services they need, when they need them and who will provide those services. 2. Why was the program developed? • To assist seniors in receiving services that meet their needs in a way that it is best suited to their circumstances • •
To reduce the risk of premature dependence and institutionalization To improve the quality of life of seniors
3. Who is eligible? To be eligible the senior must meet the following criteria. • Require assistance with multiple household tasks and activities due to either a functional or cognitive impairment • Be over 75 years of age or living with a disease of aging • Live alone or with a caregiver unable to assist • Be at risk of admission to hospital or a Long Term Care Home. 4. Is there a fee for service? Services are arranged based on the needs of the senior and individualized care plans and budgets are developed to meet their needs. There is no set fee for any service authorized by the SMILE Program. Service Providers are chosen by the client and are not employees of VON. 5. Will the assessment include disclosure of my financial status? No. The assessment is related to your health and well being and ability to manage your activities of daily living.

In-Home Respite for Caregivers In-Home Respite is a program designed primarily to provide caregiver relief for families caring for loved ones with cognitive or physical challenges. Consideration is also given to the frail elderly.
Features of the program are: • An experienced team member will visit the home, and together with the caregiver develop a suitable program of support for the caregiver and the care receiver; • Services include: ongoing caregiver support, counseling and information/education; • A trained respite worker will be matched with the client, and where possible, that pairing is maintained during their time in the program to provide continuity. The focus is on encouraging and supporting client preferences and abilities while offering caregivers free time and respite from caregiving; • Respite hours are based on needs and staff availability. The service is available 7 days a week.

Community Corporation VON Hastings, Northumberland, Prince Edward Community Corporation is the VON equivalent to a volunteer Board of Directors; responsible for raising funds to support local programs and services. They also participate in a variety of activities promoting community based health care, and advocate on behalf of seniors and caregivers in the region.
Did You Know??? Provincial Governments are responsible for healthcare in Canada; and to address the ever-increasing need, fundraised dollars are also required to help cover costs. The money raised by our VON Community Corporation comes from a variety of sources and is used to: provide client subsidies for those unable to pay, purchase specialized medical equipment, and to provide ongoing training and education to our professional staff and volunteers. Annually we need to raise in excess of $150,000 from the community. Please help us reach our goal by making a donation or organizing a special event.
