FIVE TRENDS POINTING TO TELEHEALTH AS A SUPPLEMENT TO IN-PERSON HEALTH CARE Mental Health Appointments Remain Primarily Virtual Mental health is the only specialty where there are more virtual bookings now than during the early months of the pandemic. In May 2020, 74% of mental health bookings were virtual. That number rose to 85% in May 2021 and 87% in May 2022. Patients surveyed noted convenience, the comforts of home and a perception of increased intimacy as reasons for virtual visits with mental health providers.

Since the onset of the COVID-19 pandemic, where and how adults in the United States seek and receive health care has evolved. Telehealth went from a novelty to a necessity, and now Americans expect more choices and connection in their health care experiences whether inperson or online. We bring you some valuable insights and data to help you make decisions regarding the use of telehealth for your health care needs. Data points to a preference (with the exception of mental health care) for in-person care and the use of telehealth as one part of the overall continuity of care. This and more was discovered as part of The Healthcare Experience: 2022, a comprehensive report that includes data from patient and provider surveys and an analysis of appointment booking trends from May 2020 to May 2022. Conducted by Zocdoc, a digital health care marketplace where people can find and book in-person or virtual health care appointments across more than 200 specialties and 12,000 insurance plans, the report also uncovered these trends: Telehealth Has Not Replaced In-Person Care Prior to the COVID-19 pandemic, telehealth represented approximately 1% of care, according to the Kaiser Family Foundation. However, amid stay-at-home orders and concerns about contracting COVID-19, many Americans turned to telehealth to safely get the care they needed with 33% of appointments booked conducted via telehealth in May 2020. By May 2022, that number declined to 17%, and excluding mental health, just 9% of appointments were attended virtually. This signals a return to in-person care as Americans adapt to a new normal; in fact, 77% of patient survey respondents indicated they will utilize a combination of telehealth and in-person care in the future.
Telehealth Still Has Some Problems To Work Out Highlighting some of the issues providers have experienced with online appointments, 37% of providers said it was ‘more difficult’ or ‘much more difficult’ to build a relationship with patients via telehealth versus in-person, and 58% indicated it was harder to examine patients virtually. Additionally, 58% of providers indicated connectivity issues or trouble with technology negatively impacted their delivery of virtual care.
Patients Are Still Catching Up On Care Since the onset of the pandemic, many Americans delayed or canceled health care appointments. In fact, 63% of patients said they put off preventive care during the early stages of the pandemic and 24% said they have not yet caught up on scheduling those appointments. However, 39% of patients have either caught up or made progress on catching up on preventative care. Offline Factors Impact Online Care With 81% of Americans using some form of video calling and conferencing during the pandemic, according to data from Pew Research Center, people have gotten used to distractions and interruptions during these interactions. However, the circumstances of a virtual visit can make it harder to deliver or receive care with providers reporting seeing a patient’s pet (36%), seeing a patient’s family member or roommate (31%) or seeing patients outside their homes (42%) during telehealth appointments. Your immune system may get stronger and begin to fight infections that may have been hidden in your body. Tell your healthcare provider if you have any new symptoms after you start taking BIKTARVY. If you develop new or worse kidney problems, they may tell you to stop taking BIKTARVY. Tell your healthcare provider right away if you get these symptoms: skin or the white part of your eyes turns yellow, dark “tea-colored” urine, light-colored stools, loss of appetite for several days or longer, nausea, or stomach-area pain. The most common side effects of BIKTARVY in clinical studies were diarrhea (6%), nausea (6%), and headache (5%).
Worsening of hepatitis B (HBV) infection. Your healthcare provider will test you for HBV. If you have both HIV-1 and HBV, your HBV may suddenly get worse if you stop taking BIKTARVY. Do not stop taking BIKTARVY without first talking to your healthcare provider, as they will need to check your health regularly for several months, and may give you HBV medicine.
ABOUT BIKTARVY BIKTARVY is a complete, 1-pill, once-a-day prescription medicine used to treat HIV-1 in adults and children who weigh at least 55 pounds. It can either be used in people who have never taken HIV-1 medicines before, or people who are replacing their current HIV-1 medicines and whose healthcare provider determines they meet certain requirements. BIKTARVY does not cure HIV-1 or AIDS. HIV-1 is the virus that causes AIDS. Do NOT take BIKTARVY if you also take a medicine that contains: dofetilide rifampin any other medicines to treat HIV-1

HOW TO STAY SAFE WHEN EXERCISING OUTSIDE THIS SUMMER the day goes on. Yet, you can always opt for an indoor workout on a hot summer day. Head to your local gym or try an at-home workout and enjoy the sunshine some other day.

Summertime weather motivates us to spend more time outside, but before lacing up those workout shoes and heading out the door, it’s important to take precautions when exercising in the heat. Training in the heat adds an additional level of stress to the body because it must work harder to maintain a balanced core temperature. Being exposed to extreme heat for long periods of time may cause our natural cooling system to fail. Our body sweats to cool down, but excessively in the heat, and losing that additional fluid results in loss of performance and can lead to more serious health consequences and threatening heat-related illnesses. Fortunately, there are several simple safety tips you can follow while exercising this summer outdoors. Check The Weather Be aware of the expected weather conditions for the duration of your outdoor activity. According to the National Weather Service, a heat index at or above 90 degrees Fahrenheit poses a significant health risk when engaging in physical activity outdoors. However, this temperature can vary as each person has a different heat tolerance and fitness level. Where you live can also play a factor in what is too hot.
Dress Appropriately Wearing appropriate clothing to help cool the body and wick away sweat will help you to stay cool and dry. Your outdoor gear should include loose and lightweight clothing that will facilitate heat loss and shield you from the sun’s UV rays. Avoid wearing hats and instead opt for visors. As the body loses heat through the head, a hat may hold in the heat while a visor will protect your face from the sun and allow your head to lose heat efficiently. Regardless of what you wear, you can take additional steps to stay protected from the sun’s UV rays just by where you choose to work out. Find a shaded route to avoid direct sun and help keep you cool. Parks with trails usually are shaded by trees, making them the perfect path for a hot day. Hydrate It’s important to stay adequately hydrated to make up for fluid lost through sweating. The amount of fluid each person needs will vary based on how much they sweat, their size and level of fitness, but as a good baseline – for every 20 minutes of exercise try to sip at least four ounces of fluid or more. Following this standard will allow you to remain in constant state of hydration and aid your body’s cool down process. Since most people need several hours to replace the fluids lost, it's best to replenish your body and avoid dehydration as soon as possible. Remember, our bodies lose more than just water when sweating, so you may want to consider a sports drink like Herbalife’s H3O® Fitness Drink to promote hydration with electrolytes to keep a good sodium/potassium balance.
It’s also extremely important to be aware of humidity. Humidity is particularly dangerous as it raises the overall heat index and the moisture in the air, preventing the body from using its typical cooling mechanism of sweating and evaporation to cool down. In addition, due to blood flow changes as your body sends blood closer to the surface of the skin to cool down, muscles are deprived, and this can cause cramping and other circulatory distress for the heart. If it’s humid and hot outside, don’t take a chance and instead choose a cool time of day to avoid midday sun. Early morning exercise is optimal since humidity rises as 10 WIREMAG.COM #7 2022

Listen To Your Body It’s important to know the signs of heat exhaustion as you’re exercising. There are many signs to watch for including, but not limited to, nausea, confusion, fatigue, excessive sweating, irritability, low blood pressure, increased heart rate, visual disturbance, and muscle cramps. It’s important to act immediately if you notice any of these symptoms. You should quickly stop all physical activity and find a cool place to rest. Get to a shady spot or an air-conditioned building nearby and take small sips of water. If these symptoms don’t subside after sufficient rest and fluids, medical attention may be necessary.

There’s no need to cancel your exercise plans completely when it’s hot out. By taking the right precautions, many people can safely continue exercising outdoors. Nothing feels better than hitting the fresh summer air, but remember that the risks far outweigh the rewards when it comes to training in extreme heat. So, make sure to take care of your body and avoid pushing yourself too hard during outdoor exercise this summer.

Rafa Carvajal: Where are you originally from and where do you live? Alek Maximus: I was born and raised in Fort Lauderdale, Florida. My family is originally from Poland.
RC: What do you do for a living? AM: I do part-time personal training, fitness modeling and DoorDash for a living. RC: How do you stay in such great shape? AM: I always train, do my cardio regularly and eat well-balanced nutritious meals.

RC: What are the three exercise routines that have helped you achieve your fitness goals? AM: One of my favorite exercise routines for chest is starting out with an incline dumbbell chest press almost like shoulder pressing, but focusing on the upper chest, then incline dumbbell flies, upper cut cable flies, neutral grip machine press, flat bench, machine dips, and, lastly, push-ups. For my leg routine, I like to start with machine hamstring curls, then quad extensions, into a wide stance leg press, lying down hamstring curls, E-Z curl barbell lunges, Romanian legged dumbbell deadlifts, and, lastly, barbell squats. For my arms routine, I start with a machine rope overhead extension, then into an overhead seated E-Z curl barbell triceps extension, rope pull downs and unilateral cable pushdown. I follow with biceps alternating dumbbell hammer curls, then onearm hammer rope curl, E-Z curl barbell wide grip curls, preacher curls, transitional reverse dumbbell curls (supinating then pronating on the way down), forearms E-Z curl barbell behind the back, and neutral grip dumbbell wrist curls hanging on the bar. All these routines are slow tempo based, 3-6 working sets for top weight done with 10-16 reps, 1 hr. to 1 hr. 30 mins. workout, 30 seconds to a minute rest between sets, and the last exercises I do are a few drop sets for an ultimate finisher pump. #7 2022 WIREMAG.COM 13
RC: What makes you happy in life? A M : Wo r k i n g o u t , s h o o t i n g w i t h photographers, talking to friends, and traveling. RC: What do you enjoy the most about modeling for photography? AM: It makes me happy when I get to express who I am in the moment when posing for a photo. I get to show you my authenticity and hard work to hopefully inspire someone to live their life to the fullest.
RC: What is the most important fitness or nutrition tip you can share with our readers? AM: I'd say consuming enough greens, fruits, adequate protein, and carbs. Hydration intake throughout each meal is vital for achieving your nutritional goals, with supplement intake omegas, zinc, boron and vitamin D. For fitness you want to rest enough by getting 7-9 hours of sleep a day. Also, rest from training one to two days for optimal recovery and muscle growth. Do your cardio regularly, warm up before you train, and after the workout stretch your muscles to allow them to grow properly.

RC: What are your favorite things to do in your free time? AM: I like to go out to the beach, hang out with friends, go play pool, bowling, and watch TV. RC: What is the biggest dream you would like to achieve? AM: To make a living out of modeling, bodybuilding, and social media. To become the youngest best natural fitness model/bodybuilder in the world someday, inspiring many and helping others transform their lives. Running a successful brand, owning my own gym, and doing what I love together with my life partner would be my wildest dream. RC: Is there anything else you would like to share with Wire Magazine readers? AM: You are in control, you are safe, and you are guided in life. CARE RESOURCE OPENS NEW MIDTOWN MIAMI HEALTH CENTER Rafa Carvajal: What are the hours of operation of the new Midtown Miami Health Center? Jonathan Welsh: The hours of operation at Care Resource’s new health center located at 3510 Biscayne Boulevard are Monday, Tuesday, Thursday, and Friday from 8:00 a.m. to 5:15 p.m. Wednesday hours are extended from 8:00 a.m. to 7:30 p.m. Patients can also make an appointment for visits on Saturday. The health center is closed on Sunday.

Care Resource opened a new Midtown Miami Health Center at 3510 Biscayne Blvd which reimagines the delivery of community health. Care Resource is a South Florida-based nonprofit and Federally Qualified Health Center (FQHC). According to Care Resource’s CEO Rick Siclari, “Our new health center significantly expands access to health services and transforms the delivery of vital health care and social services for Miami’s rapidly growing populations. We designed our health center to be inviting and welcoming for all our patients. This warm and relaxing atmosphere removes the stress and lets our patients know they’re in good hands.” The center’s open spaces, large murals and artwork, and cozy colors are all part of their goal to provide improved physical and emotional wellness. Care Resource estimates its new Midtown health center will provide medical and pediatric care to over 7,000 individuals with over 21,000 annual visits, plus see a large increase in the new dental care services. Additional services include behavioral health, HIV prevention and linkage, social support services, and access to healthy food and nutritional counseling through the health center’s Food for Life food pantry and nutritional center. As an accredited Patient- Centered Medical Home (PCMH), all the health center’s services are delivered in one location. “When people have access to affordable and high-quality primary care, they won’t delay seeking assistance for health concerns, thus avoiding an emergency, and enjoying better health outcomes,” explained Rick. We sat down with Care Resource Associate Director of Development and Communications Jonathan Welsh to learn more about the new health center and the care you can receive as a patient.

RC: What type of services are going to be provided at your new Midtown Miami Health Center? JW: Services include general adult primary medical care, women’s and family services, pediatric care, HIV specialty care and same day life-saving medications (Test & Treat/ Rapid Treatment Access Program), transgender health services, immunizations, diagnostic laboratories, dental care, behavioral health, and substance abuse care. The health center’s outreach team seeks out those who might otherwise be lost to care and provides no-cost, rapid HIV testing and wellness screenings, as well as education and referrals for HIV prevention services. Their linkage and retention team links patients diagnosed with HIV to primary medical care, medication, and support services. Their case managers assess patient eligibility for
government programs, provide housing assistance, food pantry vouchers and other services to help support wellness. Their Food For Life Network provides groceries, meals, and nutritional education to eliminate malnutrition and starvation in those living with chronic illnesses in Miami-Dade.

JW: It is the largest non-profit, Federally Qualified H e a l t h C e n t e r s e r v i n g S o u t h F l o r i d a’ s d i v e r s e underserved and uninsured communities, and Care Resource’s history here goes back almost 40 years when it emerged out of the ashes of the HIV/AIDS epidemic, bringing optimism and hope to the distraught. It has since evolved into a multi-faceted resource with programs to support wellness, primary care, behavior, mental health, pediatric and dental care.

The challenge presented to the leadership of the organization was to expand needed space for services Care Resource offers to the local community – and add resilience along with required parking – while maintaining its prominent central location on Biscayne Boulevard. The new structure preserves the organization’s mid-century headquarters – which has been a symbol of hope and wellness for so many – by building around and above the existing building, expanding its footprint, height and presence while creatively complying with the City’s new zoning ordinances. RC: How is the design of the health center different from that of traditional medical facilities? JW: Care Resource is unique in many ways and is an enormous benefit to all of Miami and indeed South Florida. Built during the pandemic, the new facility is distinguished from traditional medical centers and includes such amenities as: • Advanced mechanical equipment providing fresh, filtrated air throughout through the building. • Isolation rooms to buffer, protect and ensure that the building’s various spaces are kept sanitary in case of future pandemics. • Dental facilities with state-of-the-art panoramic X-ray equipment not usually found in community health centers. • On-site labs for immediate testing and quick results as needed for all the various medical specialties. • Full, in-house pharmacy where all prescriptions can quickly and easily be filled onsite. • Doubled the size of Food For Life food bank enabling it to provide greater service to clients. 18 WIREMAG.COM #7 2022
•Multiple social services conference rooms to accommodate community programs within secure, state-of-the art spaces. • On-site dedicated parking for easy access to the facility. RC: Who is your target population? JW: No matter one’s health needs, citizenship, religious beliefs, gender, age, or if they are insured or uninsured, Care Resource cares for all, regardless of one’s ability to pay. Care Resource is a multicultural community-based organization that cares for more than 27,000 individuals in Miami-Dade and Broward Counties. The health center’s patients reflect the unique diversity of the local area including people of color, women, children, adolescents, adults, veterans, and others, as it maintains its longstanding commitment to services for the LGBTQ and HIV communities. The vast majority of the health center’s patients are below the Federal Poverty Level (FPL) or have incomes at or below 200% of the FPL. Approximately half of Care Resource’s patients are uninsured. Care Resource cares for the young who still have much to see, for those in transition and for those who seek a helping hand. Care Resource cares for those who feel forgotten, for those who come from afar, and for those who lack the resources to live a healthier life. RC: Is there anything else you would like to share with Wire Magazine readers about your new Health Center or Care Resource? JW: Visiting your doctor can often feel cold, impersonal, and stressful. Care Resource’s new and innovative approach toward health care is reflected in the design of our Midtown Miami Health Center. The health center was designed to be inviting and welcoming for all our patients. This warm and relaxing atmosphere removes the stress and lets our patients know they’re in good hands.
