‘Alexa, Is My Heartbeat Healthy?’

O

ne in four U.S. households use smart speakers to check the weather, play music and query search engines. But a new technology may soon have folks asking, “Hey Google, how’s my heart?”

Researchers from the University of Washington, Seattle, have developed a skill for Amazon Alexa and Google Home that allows the devices to check heart rhythms.

Like a bat using echolocation to hunt for food, inaudible sound waves

radiate from the speaker and bounce off the surroundings before returning to the device to paint a detailed picture of the space — so detailed, in fact, that the technology can differentiate between a person breathing and her heart beating. This phenomenon is possible because your heart actually creates a tiny movement in your chest wall every time it beats.

“The question we were asking is, ‘Can we transform these smart speakers into ways in which we can do medical diagnosis?’” said re-

He’s an associate professor in the University of Washington’s School of Computer Science & Engineering.

“Specifically, in the study, we showed that we can use a smart speaker like an Alexa or Google Home to figure out if you have irregular heart rhythm.”

For the technology to work, users must be one to two feet away from the speaker, the researchers said.

They tested the accuracy of their work using 26 healthy participants and 24 hospitalized patients with heart conditions, such as atrial fibrillation and heart failure. Then, the scientists collected all participants’ heart rhythms using their sound technology as well as a standard heartbeat monitor.

“This is similar to how Alexa can always find my voice even if I’m playing a video or if there are multiple people talking in the room,” Gollakota explained in a statement.

“When I say, ‘Hey, Alexa,’ the microphones are working together to find me in the room and listen to what I say next. That’s basically what’s happening here, but with the heartbeat.”

Of the 12,300 or so heartbeats the researchers collected from the healthy individuals, the smart speaker’s reported time between waves was within 28 milliseconds of the standard heart monitor. For the nearly 5,600 heartbeats collected from people with cardiac conditions, the speaker’s time differed from the monitor by 30 milliseconds.

The findings were published online March 9 in the journal Communications Biology.

“I think the exciting part of the study is that we actually evaluated this with patients with cardiac conditions like heart transplants, irregular

heartbeats and a variety of different heart conditions,” Gollakota said.

“And it was really promising that even not just for regular participants, but even people who are actually in the hospital with cardiac conditions, it was able to deliver the results. So, that’s actually pretty promising.”

The ultimate goal of the technology is to allow people to monitor their heart from the comfort of their homes, the study authors said.

“One of the things with cardiac conditions like, for example, irregular heart rhythms is that they don’t appear often,” Gollakota explained.

“So, having a solution which is nonintrusive, contactless and it’s just sitting by your desk and monitoring your heartbeat all the time, can be pretty promising,” he said.

“The second advantage is that as we have seen with COVID, a lot of the [medical] practice has actually gone online to telemedicine,” Gollakota added. “So, in this telemedicine set-up, if you want to monitor someone’s heart conditions, or figure out if they have irregular heartbeats, a tool like this can be pretty useful to perform tests and understand how the heartbeat of the person is.”

This latest smart speaker adaptation is just one example of how existing technology is being reworked to try to improve the health of users.

According to a survey conducted last year by the Pew Research Center, about one in five Americans have smart watches, and the medical capabilities of these devices are rapidly expanding. Fitbit CEO James Park, for example, told Axios in February that his company’s product can now help detect a coronavirus infection and depression in the early stages.

es, you really are seeing double — more twins are being born now than ever before.

There are a number of reasons why, according to a new study.

Since the 1980s, twin births rose by a third worldwide — from 9 to 12 per 1,000 deliveries. About 1.6 million twins are born each year and one in every 42 babies is a twin.

A big reason for all those twins is an increase in medically assisted reproduction, including in vitro fertilization, ovarian stimulation and artificial insemination.

Another reason is that women in many countries are having babies at older ages. The chances of having twins increases with age, according to the study published March 12 in the journal Human Reproduction.

“The absolute number of twin deliveries has increased everywhere except in South America,” said study author Christiaan Monden, a professor of sociology and demography at the University of Oxford in England.

“In North America and Africa, the numbers have increased by more than 80%, and in Africa this increase is almost entirely caused by population growth.”

For the study, the researchers analyzed 2010-2015 data from 165 countries with 99% of the world’s population. They also looked at 19801985 data from 112 countries.

They found substantial increases in twinning in many European coun-

tries, in North America and in Asia.

In 74% of the 112 countries with data from both periods, the increase topped 10%. Asia saw a 32% increase and North America a 71% increase.

Only seven countries had a decrease of more than 10%.

“In both periods Africa had the highest twinning rates and there was no significant increase over time.

However, Europe, North America and the Oceanic countries are catching up rapidly. About 80% of all twin deliveries in the world now take place in Asia and Africa,” Monden said in a journal news release.

He noted that the twinning rate is so high because of the high number of twins born from two separate eggs. There has been little change in the rate of twins born from the same egg — which stands at about four per 1,000 deliveries worldwide.

The authors think rates of twinning may have peaked — particularly in Europe and North America — due to increasing emphasis in assisted reproduction trying to achieve singleton pregnancies.

It’s not clear if this is also the case in lower-income nations, and has important implications for the health of mothers and babies, as well as health care resources, according to the researchers.

Anthony D. Martinez, M.D. ECMC doctor discusses grant that will help his team to improve access to care and treatment of hepatitis C patients throughout Western New York Q: Erie County Medical Center recently landed the $1.5 million “Eliminating Hepatitis C by Improving Access to Care and Treatment” grant. Tell us about it.

A: We first had this grant going back five or six years when I first got to ECMC. At the time the grant was really focused more on combined hepatitis C and HIV co-infection.

This initial aim was to make some advances in treatments for people with both viruses. What happened was hep-C treatments really took off and evolved. The grant came to focus more on hep-C mono-infection, infection with hep-C alone. Hep-C remains a huge problem. New York state put out a new competition for the grant renewal. It’s really evolved over the last few years to focus more on hep-C as a standalone infection.

Q: What is the risk from hepatitis C? A: There are about 2.5 million people in the U.S. with hepatitis C, but 50% of them don’t know they have it, mainly because they haven’t been tested. And the reason for that is that hep-C is a mostly asymptomatic disease. So it’s replicating in your liver and creating inflammation. That inflammation over time may turn into scarring. By the time you do start manifesting any symptoms it’s late in the disease, when you’ve started to develop so much fibrosis or cirrhosis of the liver. Some people do get symptoms earlier, but they tend to be vague indicators like fatigue and joint pain.

Q: I always get the hepatitis strains confused. Which one is C again?

A: Hepatitis is pretty much an umbrella term for inflammation of the liver. It can be caused by a number of different things. There’s hepatitis A, hepatitis B and hepatitis C. Hepatitis A and B, we’re able to immunize you and protect you from those.

We sometimes see hep-B in patients who were born overseas — it’s still a huge epidemic worldwide. Hep-C is kind of a different thing. It’s a bloodborne virus. Some of the patients born between 1945 and 1965, the baby boomers, were exposed through blood transfusions back in the day.

Hep-C didn’t really exist until 1990.

We didn’t have adequate testing for it until around 1992.

Q: When you say it didn’t exist before 1990, do you mean it wasn’t identified until then?

A: Right, there was a hepatitis A, hepatitis B and non-A, non-B hepatitis. Eventually they just went with C because it was next in the alphabet.

Q: How do you know who to screen?

A: In the old days we used to screen based on risk factors like a history of injection drug use, blood transfusions or non-commercial tattoos. Or, if you’ve ever been on dialysis or accidentally stuck with a needle while on the job. Now, we didn’t do risk-based screening very well in the ‘90s and early ‘00s. The CDC changed the guidelines to not just continue risk-based screenings, but screen everyone born between 1945 and 1965 regardless of risk factors. That helped, but it still didn’t get us to where we needed to be in identification of these patients. So within the past year, the CDC and Preventive Task Force started recommending screening for any adult ages 18 through 79 regardless of risk factor in addition to screening high-risk individuals outside that age group. They recommend screening all pregnant women.

The risk of transmission from mother to child is about 6%. The reason so much attention was focused on the baby boomers is because for a long time they were 75% of all patients with hep-C. What we’ve seen is a shift toward patients outside of that cohort to people younger than 40. The curve has changed and is attributed to the injection drug use crisis we have in the country right now. So hep-C is a downstream complication from that.

There’re about 50,000 new cases each year, and most of that is attributable to injection drug use.

Q: How treatable is hepatitis C? A: It’s completely curable. That’s the amazing thing. The therapies have evolved to the point where we give you oral regimens from eight to 12 weeks. The cure rate is about 98%, regardless of comorbidities.

The regimen is really well-tolerated, with minimal side effects.

This is really exciting for us because we spent so many years with older, injection-based therapies that could only cure about 30-35% of the time.

Then the treatments advanced to a 50-75% success rate. Now we’re closing in on 100%.

Q: As far as the injection crisis goes, what’s your program able to do? Or is that outside of its scope?

A: No, actually, ECMC has a

The way we do this is that our hep-C patients who also have addiction disorders can be managed within the walls of our clinic. We recognize that in our younger patients it’s important to deal with both of these disease states. If I just cure the hep-C and don’t deal with addiction process, there’s a chance those patients could become re-infected. So a successful program means managing both of these disease states simultaneously.

I’m trained in addiction medicine as well. We have an outreach program with the addiction centers in our area.

We work collaboratively to screen patients and link them to care. As a result, we have a very high show-rate in our clinic. We do about 6,000 visits a year. Unfortunately our clinic is as busy as ever. The goal is to eliminate hep-C globally by 2030. The U.S. isn’t on target for that, unfortunately. At our current pace we wouldn’t be able to hit it until 2037 at the earliest. So it’s good news that we’re reaching a lot of folks. But, bad news because it’s a signal that we still have a huge problem with the substance use crisis.

Q: How successful are the addiction interventions typically?

A: It’s an interesting question when people ask about success in this context. I always respond back, “What do you mean by success?” It’s a chronic disease state. You don’t cure it. It doesn’t go away. You manage it much like you would diabetes or hypertension. These require lifelong management. So our cure rate of the hep-C is very high. We do also have a high retention rate for patients who have received the hep-C treatment we still continue to manage them for the addiction. So it really depends on what you consider success to be. Our goals are to make sure they can be productive members of society, take care of their kids, get through school and stay out of the legal system. It’s a really comprehensive thing that doesn’t have an easy metric you can point to.

Vitamin D: Good for Health, Fights COVID-19 V itamin D is an essential nutrient, and recent research has suggested it may also help guard against severe COVID-19.

But how much is enough, and how hard is it to get the right amount of vitamin D? “We know that a large percentage of the population has suboptimal levels of vitamin D. In fact, as many as half of the U.S. population may be deficient in vitamin D,” said Kristin Gustashaw, clinical dietitian at Rush University Medical Center in Chicago. “This can possibly lead to symptoms including fatigue, tiredness, hair loss, delayed wound healing, decreased immune health, muscle pain and more, with no other known causes.

“Part of the difficulty of maintaining vitamin D levels is because there are not a large variety of foods that contain much vitamin D,” Gustashaw added in a medical center

news release.

The vitamin is accessible to people through some foods, supplements and even sunshine.

Food sources include egg yolks, milk, cheese, beef or calf liver and certain fish, such as salmon, mackerel, tuna and sardines. Other foods are fortified with vitamin D, including certain cereals, breads, soy milk and orange juice.

Gustashaw also recommends that people should get out in the sun for at least 15 to 30 minutes a day, but says they should be sure to get a constant source of the nutrient from their diet and supplementation.

Adults should get a minimum of 600 IU of vitamin D each day and 800 IU if over age 70. Children should get 600 IU each day. And infants up to the age of 12 months should get 400 IU/day. Gustashaw says you can determine your vitamin D levels through a blood test.

If you do have low levels of vitamin D, it’s always best to talk to your health care provider or dietitian about the best way to boost your intake, Rush University experts said.

Certain medications can affect vitamin D absorption. These include steroids, the cholesterol-lowering drug cholestyramide, and the seizure medications phenobarbital and phenytoin.

While vitamin D toxicity is rare, there’s no proof that taking more than the upper limit of the recommended dosage is beneficial, the experts said.

In some cases, excess amounts of vitamin D can lead to renal failure, calcification of soft tissues throughout the body, including in coronary vessels and heart valves, cardiac arrhythmias and even death.

assed in March, the $1.9 trillion package afforded more relief to people living at poverty levels who are insured through the ACA — currently, those under 400% of federal poverty guidelines qualify for prorated premium subsidies. The lower your income, the greater the subsidy. These subsidies are paid by the federal government directly to the insurer selected by the member.

(Federal poverty levels, for examples, are $12,880 for an individual and $26,500 for a household of four.) For 2021, members with incomes more than 400% of poverty guidelines will to have to pay more than 8.5% of their income for insurance. The number of people seeking health insurance on the exchanges has surged due to the pandemic and related loss of employer-sponsored insurance.

As payers transition away from feefor-service and towards capitation or bundled payments, (see below) the issue of unnecessary services or overutilization becomes moot.

That is how NYS prosecutors described the prescription pad of Nassau County general practitioner George Blatti, 75. He is facing a 59-count indictment including five counts of second-degree murder and 11 counts of first-degree reckless endangerment. The physician showed “depraved indifference to human life” as he illegally prescribed “massive amounts” of opioids to patients despite increasing warnings and pleas from patients, family and friends. Blatti, “often working out of his car like a common drug dealer,” prescribed opioids to people who were not officially his patients, (no medical record or having done an exam.) He was back in court last month for sentencing. He could receive 25 years to life.

House health subcommittee chairwoman Anna Eshoo wants to make Medicare’s temporary expansion of telehealth services due to the pandemic permanent, across the entire country. Right now, only certain areas of the country will remain eligible (typically rural) once the pandemic ends. (Commercial pay-

A bundled payment to a certain provider covers all services delivered by all associated providers for 30 days related to a specific surgical procedure. For example, an orthopedic group receives say $50,000 from Medicare to replace a member’s hip.

The bundled payment covers basically everything from soup to nuts: including the surgeon, the surgery center, nursing, the anesthesiologist, lab and X-rays, supplies, equipment, the artificial hip and post- operative

therapy. It would be up to the orthopedic group to pay the associated providers. The Rand Corp. studied 2,400 bundled procedures including hip and knee replacement, bariatric weight surgery and spinal infusions.

Medicare saved an average $4,200, or 10%, per case versus what Medicare would have paid everyone individually fee for service. Even better, patients saved an average $500 out of pocket. For some reason, commercial carriers are not totally on board yet with bundled payments.

World vaccination

While we are focused on vaccinating 330 million Americans, there are 8 billion people worldwide that must be vaccinated as well if we are to effectively end the pandemic.

Vaccine manufacturers have teamed up with other companies to distribute vaccines world-wide over the next two years. Pfizer and BioNTech have partnered with 10 other companies to produce two billion doses this year. Product development starts in St. Louis, goes to Andover, Mass., for additional work and then on to Kalamazoo, Mich., for final touches.

In Europe, after it starts in St. Louis, the vaccine goes to Belgium then to Germany before final distribution.

Britain-based Astra Zeneca is scheduled to produce three billion dosed via the Netherlands and Belgium.

They will supply Mexico and South America. Novavax will produce two billion and will supply Japan and the South Pacific. Moderna will produce two billion vaccines focused on Asia and Africa.

Patients are able to personalize the lighting in the room and select video entertainment while inside the MRI tube. There are already about 2,000 of these. Kids can select animated shorts featuring characters like Mickey Mouse, Aladdin and the Avengers.

CVS invests in communities.

The pharmaceutical chain is addressing racial inequality and the social determinants of health, like poverty and food insecurity, that negatively impact the health of the communities it serves. CVS is investing more than $12 million to build 60 affordable apartment units in south Phoenix, where 80% of the residents are Black or Latino. A housing study revealed 50% of the residents there spend more than 30% of their income on housing which ultimately leaves little income for other living expenses, let alone healthcare. Last year, CVS spent $114 million on 2,800 units in 30 cities in 20 states. CVS also provides, at no cost, mobile preventive health screening via RVs to underserved neighbors.

In helping others, they help themselves.

Last month, I heard from a woman whose husband abruptly left her and their three teenage children after 28 years of marriage. She was in shock and consumed with pain.

She shared with me how comforting it would be to hear from her minister and church friends, but they didn’t know her situation. It had been years since she had been to church and those relationships had fallen by the wayside.

Similarly, a neighbor of mine confided that he stopped going to temple after he lost his “bride” to breast cancer. Mad at the injustice of it all, he couldn’t bring himself to attend services without her. This led to years of isolation, during which he was overcome with loneliness and guilt. I wasn’t surprised when he talked of numbing his pain with alcohol.

My message to both was simple: Renew your spiritual connections.

It’s easy to imagine how difficult it might be to contact a minister, priest, rabbi or other spiritual adviser and ask for help if you’ve been absent from services for many years or when loneliness has you in its grip.

But any religious organization worthy of its mission will respond with open arms, even in COVID-19 times.

A metaphorical warm embrace is waiting for those who reach out by phone, email or participation in livestream or limited in-person worship services.

Healthy Spiritual Life Can Uplift Those Who Live Alone

A

pril showers bring May flowers and so much more.

In addition to warmer weather, new growth and budding possibilities, April is also a time for worship and family gatherings that embrace reflection, contemplation and celebration.

Easter, Passover and Ramadan all traditionally take place in the springtime when hope springs eternal.

The coinciding of these religious holidays reminds me of the healing power of faith.

Almost without exception, the women and men who have found their way after a divorce or the death of a spouse have been bolstered by a healthy spiritual life.

Many of the participants in my “Alone and Content” workshops take advantage of every spiritual connection they can find. They regularly attend worship services, find

comfort in prayer and hymns, and faithfully observe religious holidays.

Others march to a different drummer and nourish their spiritual life in private moments: listening to music, communing with nature, reading, writing or sitting in contemplative silence.

In all its varied expression, my observations tell me that those who find sustenance in spirituality fare better when challenged by life’s transitions.

They take time to spiritually connect with themselves and others and, in doing so, find the inner strength to navigate unchartered waters and to help others along the way.

Even when their own sense of self-worth is fragile or failing, I’ve seen devout divorcees and widows reach out to others with encouragement and emotional support. Spiritually grounded, their empathy runs deep and their desire to uplift others

Likewise, if other paths beckon — if nature or poetry or meditation feed your soul — go forth and embrace those opportunities.

My spiritual battery gets recharged when I take time out from my busy schedule to be still and meditate. Quiet time in an inspirational setting allows me to contemplate my most deeply held beliefs, let go of fear, and set my intentions. I emerge more grounded, grateful, and ready to embrace the day with an open heart.

The power of a spiritual life is mighty. When you pursue your spiritual goals and teachings, life can be more manageable and your ability to cope with loss, loneliness and everyday struggles is strengthened.

However you pursue a spiritual connection, I encourage you to make that connection or re-connection now.

Today. A belief in something greater than yourself and the company of people who share your faith can lead to a richer, more meaningful, and peaceful existence.

You do not have to go through life alone. And what a blessing that can be.

Experts weigh in on employees’ mental health

What Employees Can Do

When it comes to managing the stress and anxiety from work transitions, there are steps employees can take to stay in control of their mental health.

arly in March, Texas announced it would reopen all businesses to full capacity and lift mask mandates. Shortly after, South Carolina Gov. Henry McMaster called state employees back to the office. Here in Western New York, COVID-19 cases continue to decline as more people get vaccinated. If this trend continues in the coming months, local businesses will revisit plans to bring employees back.

While some companies will remain remote, others will require employees to work a hybrid of remote and office work or return to the office completely.

Managing the anxiety and stress of retransitioning to a public setting (or any post-pandemic transition) can be a challenge.

Individual situations vary, but people throughout Western New York must now face these challenges head on.

Sara Schambach, a Springville native, has been in the office since the pandemic started and says employees have their own offices and can socially distance. Child care, however, has been an ongoing concern. On the other hand, Darcy Kennedy-Ellis of Buffalo started a new, in-person job just weeks ago.

“It’s been very weird to readjust to humans who aren’t my husband or my infant!” she said in a Facebook post.

The New New Normal

Moss’s Advice

Mental Health Counseling private practice in Buffalo, said transitioning back to office life can drudge up new stressors and cause anxiety. People have adjusted to their new normal, and most now have the tools necessary to be successful. In this next phase of COVID-19, they will once again have to navigate challenges like childcare and being around other people.

Moss navigated the retransition to office life herself after months of working from home. With that retransition came new procedures and safety precautions like deep cleaning surfaces, installing hand sanitizer dispensers and UVC plug-in purifiers, and putting up plexiglass as a barrier to keep patients safe. But even new office protocols have been a point of stress for many people in their jobs.

“Seeing the new changes is a lot.

It’s a reminder that something’s happened that has changed our normal,” Moss said.

Laura Ott, a counselor in Cheektowaga, points to several different fears that her patients experience.

Some worry that their managers won’t be supportive of changes impacting their children. Others worry about job security and missed opportunities due to a lack of flexibility.

Still others struggle with acclimating to their company in person or fear that they may contract COVID-19 while there.

ighmark Blue Cross Blue Shield of Western New York Blue Fund has opened its 2021 grant cycle, and is now accepting applications online through April 16 for grants to organizations and initiatives that align with at least one of Blue Fund’s key health areas that support Western New York’s greatest needs.

This includes behavioral health, cardiovascular health, health care workforce development and healthy children.

Since the nonrofit health plan launched Blue Fund in July 2018, it has awarded more than $6.9 million to organizations and initiatives that work collaboratively to address key

health areas and demonstrate enhanced measurable outcomes in the Western New York community.

Submitting a brief letter of intent (LOI) using Blue Fund’s online application system is the required first step in the grant-making process. Selected applicants will then be invited to submit a full application, with funding decisions announced in the fall of 2021. Organizations interested in applying are encouraged to review program guidelines and focus areas as they may have changed from previous years.

During these transitions, Moss suggested practicing self-care outside of work, connecting to a support system at work (in a safe manner), and having open, honest conversations.

Keeping a stress ball or silly putty on hand and practicing deep breathing can also help.

Self-care can take the shape of a favorite activity or involve taking a moment for oneself, Moss said.

Whatever self-care looks like to someone, it should go behind the simple act of doing something and allow a person to really connect to the moment.

Above all, communicating about what’s needed and necessary can make a difference. Moss emphasized that it’s vital to tell people you need space and make sure you’re comfortable. Though this can be difficult for those with anxiety, people should respect your request when it’s coming from a place of authenticity.

“I have to remind people that there is no rulebook for this. A lot of employers and people are learning as they go. They need you to say something so they can adjust,” Moss said. “We’re all carrying things right now, and it’s important that we all come together and understand one another.”

Ott’s Advice

According to Ott, maintaining healthy boundaries with family, friends, and colleagues is a critical component of creating a healthy environment and having meaningful relationships. People should also check in with themselves often and

learn what will help them feel better connected to the people and things they enjoy.

Lastly, Ott said that people should acknowledge and embrace the fact that opposite feelings coexist.

“We can feel eager and anxious simultaneously. We can want a break from constantly being with members of our household and also feel sad that this period of togetherness is ending.

What Employers Can Do

Transitions impact people emotionally because they are often linked to stress, Ott says. People transitioning back to work need support from their employer as well as a welcoming, supportive space to share challenges and identify solutions.

“What we know about stress and trauma is that access to support is essential for promoting effective coping and resiliency,” she said.

“Things such as flexible or staggered office hours, availability of human resources, having management open to feedback, and free emotional support services are a few things that will help employees better transition.”

Moss similarly advised that employers should promote employee assistance programs and remind people about available resources.

Employers can also provide materials on stress management, speak to staff about mental health, or bring in a counselor to present on the subject.

Above all, both Moss and Ott encourage employers to talk to their employees.

“The best thing an employer can do is ask staff how they are doing and offer support if the answer isn’t ‘fine,’” Ott said.

Last Thoughts

Though we are getting better at talking about mental health, a stigma still exists.

“If you’re a person who’s struggling with the idea of going to a counselor, it’s important to know that everyone needs someone to talk to, especially now. It’s okay to get help,” Moss said.

Community Health Center of Buffalo Now Carries COVID-19 Vaccines

he Community Health Center of Buffalo, Inc., now has COVID-19 vaccines available to the public.

CHCB’s participation in the federal vaccine supply program will allow the health center to expand the breadth of its mission to get vaccines into communities of color that disproportionately do not have access. These doses have been designated to complete the vaccination of its employees and their immediate family members.

Daniel’s Law Seeks To Help Mental Health Crises The legislation is ‘a benefit to a person in crisis’

n Feb. 4, New York Assemblyman Harry B. Bronson (D-138) and Sen. Samra Brouk (D-55) introduced legislation to establish state and regional mental health councils to bring mental health care professionals to the scene of mental health emergencies and train 911 dispatchers to summon the needed personnel.

The proposed law is named “Daniel’s Law” after Daniel Prude, a man who died in March 2020 of asphyxiation days after an altercation with Rochester Police.

“It’s essential in order to effectively learn how to help someone who’s having a psychiatric emergency,” said Anna Shurmantz, licensed clinical social worker and owner of Shurmantz Counseling, PC, in Cheektowaga.

“I had to go to graduate school to learn and be able to practice those skills. It’s not law enforcement personnel’s area of expertise. They’re there to serve and protect the com-

munity. They’re not mental health experts and it’s a disservice to them and to the community to ask them to take on this role that requires significant education and experience to do.”

She said that some law enforcement officers with whom she has worked in her practice relate that they do not want to go on calls involving mental health crises. She anticipates that like other, similar programs in other parts of the nation that New York involving mental health professionals in these crises would also be successful.

Brittany H. Bennett, licensed mental health counselor and owner of Bridge Over Troubled Water, PLLC, in Tonawanda, also sees benefit in trained mental health professionals assisting with mental health crises as they “are able to identify particular characteristics that could reflect mental health symptoms or conditions, yet also recognizing the potential for someone being under the influence of substances — drugs, including alcohol — which can mimic mental health symptoms and or exacerbate existing mental health symptoms.”

Telling the difference can aid all the professionals involved to address the situation in a way that will offer the best chance for a positive outcome, both for the individual and for public safety.

Amy Cross-Viola, licensed clinical social worker in private practice and adjunct professor at University at Buffalo, would like to see a behavioral health unit working as a professional equal but separate from law enforcement personnel — including equal pay.

“The whole point is that law enforcement will not necessarily be called,” she said. “We’d like to see a separate behavioral health unit so it removes the police entirely. The truth is that mental health providers

are trained to handle mental health crises. Police are not. It’s no fault of theirs. They’re there to handle crime.”

She added that the arrival of police officers tends to escalate the emotions of someone in a mental health crisis, which may lead to injury if the situation gets out of control.

“There’s a further divide between the police and the community and people are further traumatized,” Cross-Viola said.

Instead of responding to 911 calls with the police as the primary response, “We need to catch up with the times,” Cross-Viola said.

Daniel’s Law includes a provision that will train 911 dispatchers in how to know whether mental health services or law enforcement is needed.

“I’m not saying the police would have no place in a mental health crisis,” she said. “If behavioral health responds and decides there’s a need for police response, they can be called in.”

She said that in a similar program in Eugene, Oregon, police were called by mental health professionals to assist with a mental health crisis 0.6% of the time.

Cross-Viola anticipates rolling out a statewide version that would provide service to each county, as rural areas would not need the same number of mental healthcare providers.

“There is a lot of reactivity that’s going on,” said Shawn Marie Cichowski, a provider with AbleTo, a virtual behavioral health provider headquartered in New York City and a certified life coach and owner of Western New York Life Coaching in Williamsville. “Having someone available to have that perspective in a high intensity fight-or-flight situation where there are mental health concerns is only beneficial.”

‘Falling Behind Doesn’t Mean Dropping Out’

T

hanks to the power of social media, runners in the Run the Year Challenge can connect with participants across the nation for support and encouragement.

Members of the Facebook group share their runs and celebrate various milestones, whether it’s hitting mile 100 or mile 1,000. They also ask for tips and advice.

Last week, one particular runner posted about how turbulent his journey had been so far and asked how to avoid sabotaging himself on those particularly challenging days. His post resonated with me for myriad reasons, the most obvious being how relatable it felt.

I ran consistently for the first part of the year, but bad days pile up. Life is catching up to me as it so often does. It all started by missing one day of mileage. The next thing I knew, I started missing a lot of days.

Like my fellow runner on Facebook, I find myself disgruntled at this, ready to give up on the whole thing.

The strange part? When this random stranger posted his sentiments, I found myself screaming out to him that a string of bad days wasn’t a reason to stop fighting for the good days. “Forgive yourself and keep going,” I mentally shouted to this man.

Eventually, I sat down and penned a response to my new friend, a complete stranger who asked, “How do I avoid sabotaging myself on my bad days?” Here’s what I wrote: “Remind yourself that falling behind doesn’t mean dropping out.

Stay the course and embrace your best days. Forgive your worst. You’ll get there as long as you’re gentle with yourself in the process.”

It’s funny, right? As people, we’re often so much more compassionate toward others than ourselves. We’re more apt to forgive someone else for dropping the ball than we would be if we dropped the ball. Why is that?

Better yet, what would we accomplish if we just let ourselves go at our own paces?

As soon as I wrote my response, I knew I needed to heed my own advice.

Right now, I’m closing in on 400 of 2,021 miles for the year — not bad, all things considered. My run streak is, well, currently at one day.

I walked four miles yesterday but couldn’t bring myself to pick up the pace. Today, I embraced a good day and crushed six miles with fresh legs. To me, this serves as proof that falling behind doesn’t mean dropping out.

Like so many things in life, our journeys rise and fall with the tide.

You may be crushing your fitness goals or taking a breather on the sideline. Either way, you’re on the same path as everyone else. How you get to the finish line is up to you. Just don’t forget to be gentle with yourself in the process.

As Medical School Applications Spike, Is Dr. Fauci To Thank?

Jacobs School of Medicine has seen an increase of roughly 40% from this year over last — is it caused by the Fauci Effect?

A

recent national spike in applications has been dubbed “The Fauci Effect” based on the notion that the increase is driven by a widespread admiration for the work of Dr. Anthony Fauci, director of the U.S. National Institute of Allergy and Infectious Diseases and the chief medical advisor to the president.

In few places has that rise been so dramatic as right here, in Buffalo.

According to physician Dori Marshall, associate professor of psychiatry and director and associate dean of admissions for the Jacobs School of Medicine at the University at Buffalo, while the national rate of applications for medical school are up roughly 18%, the Jacobs School has seen an increase of roughly 40% from this year over last.

Marshall noted that while Fauci has certainly become a household name, that only happened in March or April of 2020 and one cannot simply decide to apply to medical school that summer without having prepared in many ways. She instead attributes the increase to other factors, including students who had potentially been thinking of taking a gap year before starting medical school, but had those plans derailed and decided to go directly to medical school instead.

Marshall also pointed out that because all interviews have been conducted virtually, applying to far-

flung schools has been less cost-prohibitive for many students.

“They didn’t have to incur travel expenses,” she said. “Because all of our interviews have been virtual, for people from say California or Maine, it’s easier to get here.”

And cost is certainly a factor for many students. According to Marshall, the number of students requesting financial aid this year increased by 111%.

The number of students and amount of financial aid aren’t the only things that have changed this year, either. According to Marshall, the number of first-generation applicants has increased by 59% and the number of individuals who identify as Black, Hispanic or Native American has increased by 3%.

While Marshall is hesitant to chalk up any of the changes to Fauci himself, there has been an underlying

theme she has noticed when it comes to what’s driving applicants. “[In interviews] I haven’t heard anybody talk about Anthony Fauci, so I don’t think he’s having an impact. But, the pandemic certainly is,” she said.

When interviewed, many applicants have discussed how the pandemic itself has laid bare major disparities in health care throughout the country and how the pandemic itself is having a disproportionately negative effect on already underserved communities.

“Many students are committed to going into medicine to address some of those disparities and make the delivery of healthcare more fair and equitable,” she said.

For Joya Ahmad, 28, and Alexandrea Adams, both of whom have been accepted to the Jacobs School, that reality is a major factor in what is driving them into the medical field.

As a Detroit native, Adams said she has seen those disparities play out first-hand, and hopes one day to be among those trying to end that trend.

“Definitely being from Detroit has encouraged me to be a physician, just seeing the lack of health and healthcare within that setting and it’s the reason why I want to serve in a marginalized, underserved community,” she said.

In her undergraduate studies, Adams double-majored in biology as well as public policy, a choice that she said has become ever more pertinent as the pandemic goes on.

“I think the pandemic has further showed me how important it is for medical professionals to have a public health background,” she said.

Ahmad has worked with underserved populations in the past, but said that the response to COVID revealed even more stark disparities that exist within public health and health care.

“That really stoked the fire for me,” she said.

So much so that she began working with a personal protective equipment delivery nonprofit organization called Med Supply Drive, where she has been active for roughly a year.

“That work really drove home the idea of two countries that exist within one,” she said.

She said that when she does pick a school — neither applicant has made a final choice yet — opportunities for community involvement will play a role in that choice.

Ahmad also said that racial tensions and issues such as police brutality have further demonstrated the existence of two realities within one border, and further galvanized her to go into work where she feels she can make a difference in marginalized communities.

The soon-to-be medical school student said that there is a daunting amount of work to be done. She worries that in some ways, things will get worse before they get better.

“Vaccine rollout and how re-opening goes is really going to lay bare more societal and structural issues,” she said.

While that outlook may seem less than sunny, it is that uncomfortable and painful reality that she hopes will drive her and other to make real change as members of the medical community.

“I’m sad that this is where we’re at,” she said. “But it feels better to know that I can have a hand in doing something about it.”

Popcorn Is Popping Good for You!

y love affair with popcorn began after my first child.

Desperate to shed pregnancy pounds, I reached for the puffy stuff daily. Oh, the satisfaction! It filled me up, I lost weight, it became my BSF: Best Snack Forever.

The main perk of popcorn is that it’s a whole grain snack. Unlike refined grain snacks that have been stripped of some nutrients, popcorn, with its entire seed intact, boasts a slew of health benefits.

As with other whole grains, consuming popcorn may reduce

inflammation, lower cholesterol and decrease the risk of Type 2 diabetes, heart disease, obesity and some forms of cancer.

A three-cup serving of popcorn provides a whopping 3.5 grams of fiber, which is about 14% of the recommended daily intake of fiber for women and about 12% for men.

These days, with my finicky digestion and high cholesterol, all that fiber makes me deliriously happy, not to mention, remarkably full. A recent study, in fact, found that 15 calories of popcorn was just as satisfying as 150 calories of potato chips.

Beyond its fiber, popcorn is a good source of polyphenols, which are powerful antioxidants that have been linked to better circulation, improved digestion and a reduced risk of certain cancers and age-related diseases.

Looking to lose some weight?

Popcorn is a dieter’s best friend. It’s

super low in calories (a three-cup serving of air-popped popcorn has fewer than 100); it provides prolonged satiety because its complex carbohydrates take longer to digest; and it has scant fat and salt.

Of course, popcorn can quickly become an unhealthy snack if it’s doused with oil or butter, salt or sugar. A big tub of movie theater popcorn, for example, can inflict over 1000 calories and several days’ worth of salt and fat. So, mindful preparation is key. Air-popped popcorn is your healthiest bet because it provides some control over the seasonings added afterwards. Microwave popcorn can be a healthy choice, too, especially since most brands have removed trans fats and the chemical linked to “popcorn lung” — the wheezing disease that resulted from fumes inhaled when a bag was opened.

he air feels and smells crisp and clean. The space feels safe, calming and pure. Glowing pink Himalayan salt lamps line the walls. Pebbles of soft salt cover the floor.

When you enter the quiet and dimly lit room, it feels like walking on a sandy beach at sunset and entering a cave beneath the rocks.

There are all kinds of health trends these days. From special diets and exercise plans to holistic spa treatments.

It seems that physical and mental wellness are at the top of our lists, especially during a pandemic.

One of the latest growing trends has been around for centuries but has gained more recent attention for its healing effects. Salt cave therapy not only allows you to relax in a beautiful, serene environment, it also improves lung function and heals troubled skin.

AURA Salt Cave and Wellness is Western New York’s first halotherapy center. Kelly DeBerg opened in 2017 and has made it her goal to focus on providing holistic therapies and products to help people return to a natural state of wellness.

“Our salt cave has about 22,000 pounds of salt brought in from Poland and is made to look like an actual salt cave,” DeBerg said.

The halogenerators used in these salt caves grind pharmaceutical grade salt into aerosol and distribute it throughout the room. Inhaling these tiny particles removes toxins from the lungs, reduces inflammation in the respiratory system and widens airways.

“It’s good for respiratory health, especially right now,” DeBerg said.

“Keeping your lungs and your respiratory system healthy and clean is a huge preventative.”

Pam Toole, owner of Southtown Salt Cave, is a registered nurse. She always leaned toward more natural options for health and wellness. After trying a salt cave for the first time with her husband, she knew that she wanted to offer this alternative to those struggling to get some relief.

Integrative Health Sees Uptick During Pandemic

octors practicing integrative and holistic health are seeing increased interest from new and returning patients because of the pandemic.

Marlene Rogers, family nurse practitioner and co-owner of Advanced Integrative Care in Williamsville, said that the practice is growing and many patients are more interested in imRogers mune strategies and wellness than before.

“We have a nutritional IV clinic that we’ve seen definitely an increase with the high doses of vitamin C, magnesium, B vitamins, immune products, and energy products,” Rogers said At the beginning of the pandemic, many people delayed care because of the virus and for some, financial

constraints have caused them to continue to delay. Lab work is covered by insurance, as is traditional medication; however, some services are not covered.

At Northeast Integrative Medicine in Williamsville, acupuncturist Ariel Wachowiak, who holds a Master of Science in acupuncture and Oriental medicine, said that she has the same number of patients; however, they have shifted their interest since before the pandemic.

“They have strengthened their commitment to treatment in order to boost their immune systems,” Wachowiak said. “I believe part of the issue in Western New York is that there is a lack of exposure to knowledge about how integrative medicine can help improve one’s health.”

Creating a healthy lifestyle with patients is the main goal of Kimberly Fenter, doctor of naturopathy and certified holistic nutrition health coach at Audubon Women’s Medical Associates, an integrative practice.

Lately, she’s had her work cut out for her with some patients.

“Unfortunately, during the pandemic, a lot of the consciousness of healthy lifestyles has taken a backseat to being comfortable in what they eat, wear, and adapting to the ‘new norms’ Fenter they’ve been dealing with,” she said.

Especially while quarantining, it is easy to let healthful habits slide and comfortable—but unhealthful ones—creep in.

“I’ve not seen an increase in patients in the past year, but currently, I’m seeing more of an uprise in my client base looking for help for holistic way for mental health and inclusive of that their whole health mind, body, spirit,” Fenter said.

She said that before COVID-19, people had little consciousness of how the quality of their food affects their health. Now that they feel a little overweight and sluggish, they are looking to Fenter for help in controlling their weight through food and activity.

The stressors of working and educating from home have ramped up patients’ stress levels. Without their normal healthful ways of coping, like getting together with friends or going on outings, it has been more difficult to maintain lower stress levels.

“I get back to the basics of how you are creating a healthy environment with what you fuel your body and mind with,” Fenter said.

Instead of ruminating on past mistakes, she encourages clients to make positive health changes.

High stress levels also affect sleep.

“I’m seeing a lot of interest in sleep hygiene, which affects how people feel health-wise in general,” Fenter said.

She added that many seek help for digestive issues, as poor diet, little sleep and stress can contribute to poor digestion.

ary Travers Murphy, a former television consumers affairs journalist in Buffalo and a former town of Orchard Park supervisor, became executive director of the Family Justice Center of Erie County in January 2010. he was inspired to take the job after a close friend and another women were brutally murdered as a result of domestic violence within six months of each other in

Orchard Park.

Her friend, Aasiya Hassan, was beheaded by her husband in 2009 and it shocked the community and raised many questions about domestic violence and the services offered to victims of abuse.

Murphy said those deaths are the reason she “jumps out of bed every day to work at the Family Justice Center to help shine a light on this horror show in our society.”

Q: How serious of an issue is domestic violence in Erie County and Western New York?

A: “It is a public health crisis.

The definition of the abuse we are talking about is control. The control the victims are under involves talented control freaks working out of the same handbook and one of their most effective tools is to convince the victim they are causing the problem and are responsible for it. That is not true but these victims are brainwashed to believe it. One in three girls in the 1012 and 24-26 year old age group are victims of a controlling, abusive and often times violent relationship. One in four women and one in seven boys and men are victims. This is all about control and long before these relationships turn physical, there is the breakdown of psychological, emotional, verbal, financial, sexual and social media abuse that can go on for years before it becomes physical and sometimes deadly. The abusers will do and say anything to maintain that power and control, including threats of killing the victims or the children if they leave or say anything to anyone.”

Q: How does the Family Justice Center help these victims and when did it begin?

A “Twenty years ago the Erie County executive called together nearly 100 people, from survivors of domestic violence, police and the many agencies that deal with these issues and said we have a problem — the number of calls for abuse and the number of homicides because of it are increasing. After walking through a victim’s steps seeking help for themselves and their children to get out of an abusive relationship, it was easy to see there were too many steps which led them to ask, ‘Why not put all of these services under one roof with an advocate to hold a victim’s hands from that first phone call and

through the process, including obtaining a restraining order from the courts.’ That is how we opened our downtown office and the satellites and it has been a 100% game changer for the victims. We have representatives from every agency they need right here ready to assist them and it is all done in strict confidentiality.”

Q: How has the pandemic affected the Family Justice Center?

“Except for a few weeks in the summer we have all been working remote. In a way, COVID has been the biggest blessing in disguise because calls from victims have increased by 74% and it continues to climb but, more important, we found we could operate efficiently.

We saw an increase of 1,000 new and returning clients in 202 for a total of 4,362 clients compared to 3,094 in 2019. A majority of the client calls were considered a high lethality risk, which is unprecedented in our agency’s history. The numbers are increasing because of the convenience of someone calling our hotline and having our advocate begin the process with them. All of our services are free and when the courts went into quarantine, they went to teleconferencing for orders of protection. It was like a miracle because it spares our clients hours and hours of time in court. One of the most effective tools for these control freaks is the ability to isolate so when they quarantine mandate came down it was a dream come true for them and a nightmare for victims.”

Q: What should victims of abuse do in those situations?

A: “Call our hotline at 716558-SAFE and talk with one of our advocates who will immediately let them know they are not alone, the cycle of abuse repeats and mostly, this is not their fault. We begin with an assessment of questions and we use these responses to obtain an order of protection from court for them.

During the pandemic, while victims are working at home, it has been easy for them to chat with us online or by phone. It is all confidential and their safety and the safety of their children is very important.”

Evergreen Health Receives Adult Outpatient Mental Health Clinic License Another milestone as organization looks to expand mental and behavioral health services

vergreen Health recently announced that it has received its adult outpatient mental health clinic license from New York State’s Office of Mental Health.

“We’ve always known that mental health is a key component to overall health and wellness. Now more than ever, it’s important for us to strengthen our efforts in this critical area in our patient’s health journey,” said Khristopher Decker, vice president of behavioral health at Evergreen Health. “We are enthusiastic about this exciting step to grow our capacity in the behavioral health area within the many vital services offered at Evergreen.”

Evergreen’s new adult outpatient mental health (OMH) clinic license will enhance its Center for Behavioral Health, which is focused on services pertaining to an individual’s mental state, emotions, and the patterns of behaviors they adapt to cope with adversity.

The OMH license will broaden the organization’s mental health services and create a sustainable model of care for its existing mental health program. The license also includes a new insurance reimbursement model, which will allow Evergreen to eventually expand, enhance and sustain group therapy, family services, medical assessments, and health edu-

cation groups over the long term.

Since 2019, members of Evergreen’s Center for Behavioral Health have worked towards this credential from the New York State’s Office of Mental Health, the state’s governing body that permits agencies to provide licensed mental health services in New York state. The Office of Mental Health also provides guidance on intake, policy and assessment, which positively impacts the quality of the services that agencies provide to patients.

Dogs Serving People with Autism

‘Man’s best friend’ is much more than just a family pet

A

ssistance dogs can perform more tasks than guiding.

Their role has expanded considerably to provide help for a variety of needs, including helping people with autism.

Veronica Federiconi, CEO of Autism Services, Inc., in Amherst, said that service dogs trained to help children with autism may be used for helping them transition from one place to another.

“The dog is familiar to the child and makes it a little easier,” she said.

Since 60% of children with autism have other, unrelated neurological conditions, service dogs can be trained to assist with other tasks such as sensing seizures so the child can lie down in a safe place and minimize the risk of injury or altering

diabetic emergencies.

“Primarily, we use dogs for detecting if the child is going to have a seizure,” Federiconi said. “Others are for navigating and keeping them safe. Safety is a huge reason to have a dog.”

While pet dogs can offer emotional comfort to people of any ability, service dogs that help a child with autism can do that and so much more. It is not a matter of finding a nice puppy or even a well-mannered adult dog.

“You really need to go to someone that can provide an animal that is highly trained for this,” said Kathy Eiss, president of the Autism Society of Western New York in Buffalo.

A therapy dog and a service dog have different levels of access to pub-

lic spaces. Therapy dogs are trained and can be certified to aid in providing comfort and emotional support.

Service dogs, including those trained to assist children with autism, are protected under the Americans with Disabilities Act to go with their child and handler to any publicly accessible place. Service dogs can also provide emotional support but are also trained and certified in specific tasks to help mitigate the effects of a disability.

Katy Ostroff, licensed clinical social worker and manager of client services for NEADS World Class Service Dogs in Princeton, Massachusetts, said that her organization trains yellow and black Labradors as autism service dogs for children aged 8-16. Their training is at the same level as any other service dog, only with skills that can help someone with autism. The organization ordinarily places 50 dogs annually and has placed more than 1,900 since its founding in 1976.

“The dogs are trained with the parent to help the child mitigate the symptoms of autism,” Ostroff said.

“We find for many of our children who apply, there’s a concurrent diagnosis of anxiety, depression, obsessive compulsive disorder, or Tourette’s. The work that the dog does with the parent is all about helping

As some families may still feel the strain from work layoffs and furloughs, others work for businesses that have boomed during the pandemic. Depending upon their type of donor funding, autism organizations may be struggling or doing fine financially.

“Everything has been really affected by COVID,” said Kathy Eiss, president of the Autism Society Western New York. “Donations have been affected.”

The organization typically benefits from a golf tournament each summer, hosted by the American Concrete Institute for many years.

Although the event did not happen in 2020, the organization donated $1,500 in lieu of the typical $5,000

the child regulate emotions.”

The dog’s “tools” may include providing gentle pressure by lying across the child’s lap, which some autistic children find soothing and can help lessen meltdowns. By minimizing the number and extent of meltdowns, families can feel freer to go places and spend time in public with their children. The dogs can perform tasks like alerting parents of non-verbal children when they need help.

“We had many instances where kids were petrified of the dentist, the doctor or a restaurant that is too loud,” Ostroff said. “The dog gives them a focal point, reliability and consistency.”

In addition, service dogs perform helpful tasks that are not trained, such as providing a social bridge for children who typically feel left out and stigmatized because of their differences. Typical children spot the service dog and gravitate towards the child with autism. These interactions can help foster friendships — something that can challenge children with autism.

“We’ve found the dog is a great tool for socialization,” Ostroff said.

“It’s easy to memorize the dog’s name, weight, birthdate and easy commands.”

Many children with autism struggle with learning how to make eye contact and understand facial expressions. Service dogs offer a nonjudgmental individual to practice on.

Ostroff said that the dogs are trained to give children eye contact and that transfers to relating better to people.

NEADS raises purpose-bred dogs and uses puppy raisers as foster homes to provide socialization and basic obedience training, followed by training on their campus. Before the pandemic, the organization also worked with a prison-based program that involves inmates training dogs.

These dogs go home with foster families on weekends for additional socializing.

Those potentially receiving a dog are carefully screened, first by Skype and then in-person at the NEADS campus before beginning the process of matching a dog to the family. The dogs must show aptitude for the tasks and for working with children.

Those who do not “make the cut” are available for other service jobs for which they might show aptitude or for adoption as family pets.

Other funding comes from organizations like United Way, Combined Federal Campaign, and private donors. The organization also hosts educational events — now on Zoom — although these are for just a nominal fee per attendee.

While gifts from donors require little overhead to acquire compared with fundraisers, events are a way to raise awareness about autism and an organization’s mission and needs.

“Fundraisers are also about people getting together,” Eiss said.

Pandemic Creates Learning Challenges for Children with Autism Parents have several options to assist with a child’s education

he COVID-19 pandemic has brought a plethora of changes to everyday life, including to children newly educating at home.

For children with autism spectrum disorder, learning in the classroom was already challenging. Filtering lessons through Zoom or other remote means adds an additional layer of difficulty.

Schooling at home may seem a big relief and a welcomed retreat; however, Kathy Eiss, president of Autism Society Western New York in Buffalo, said that “it’s very difficult for the families. A lot of the children area used to the structure and routine and that is being totally destroyed.”

The “competition” for the same donated dollars has made it tough for organizations to raise funds, especially during a time when some families are experiencing financial setbacks of their own.

Autism Services, Inc., in Amherst typically hosts an art auction each June. That was canceled both in 2020 and for 2021, despite its being a 20-year, successful event that includes visual and performing arts.

“This year, I directly made calls to the sponsors and said, ‘We can’t hold the event but we still need sponsorship to buy the materials for everyone and keep the program going,’” said Veronica Federiconi, chief executive officer

A recent study of 3,500 families published in the Journal of Autism and Developmental Disorders reports that 64% of caregivers said that the changes brought by the pandemic “severely or moderately impacted” their child’s autism symptoms, behaviors, mental wellbeing or related challenges. Further, 75% of parents reported that they felt extreme or moderate stress because of the disruption to their lives.

“Many parents are just trying to get by,” said physician Michelle Hartley-McAndrew, medical director of The Children’s Guild Foundation Autism Spectrum Disorder Center at Oishei Children’s Hospital. “Some are being really creative and homeschooling their children but that is the

That encouragement pushed her to keep calling. Some did not give as much as they typically would, but Federiconi said that many were very generous so that the organization received close to what the fundraiser typically makes. Instead of the in-person gallery, it was a virtual gallery on the website where the organization sold art.

Autism Services’ other big fundraiser is a 5k in June, which was also canceled for both 2020 and 2021.

The money raised supports services in the Southtowns area.

“Without two years of the run, it hurts,” Federiconi said.

The organization also hosts a

“The social skills component of interacting in person with other people is non-existent,” Hartley-McAndrew said. “Many of the kids go to school all year long because they will lose some of their skills if they are not consistently practicing them. The effect of basically losing a full year is devastating for these kids who need it most. In clinic, the level of regression and behaviors we are seeing is unprecedented.”

Autism Services, Inc., in Amherst has schools for children with autism from age 5 through 21. The school received donations of Chromebooks so that the children lacking them could learn from home.

“It has been extremely challenging with remote learning,” said Veronica Federiconi, chief executive officer.

Some children are capable of remote learning, but many cannot.

For the latter, the school has offered in-person learning opportunities on Monday, Tuesday, Thursday and Friday with the usual protocols of masking wearing and social distancing. Wednesday is designated for deep cleaning the facilities.

“Many have to have a parent sitting next to them at home,” Federiconi said. “The parent isn’t there in school every day to learn and understand the way we’re communicating with the children and the strategies we’re using to get them involved.

“Parents have been very frustrated and some are very unhappy, calling to ask when their kid will go back in person. When we had them in school and then had to send them back several months back. When we were identified as our area as being in the orange zone, our people could not go to program, children or adults, until that was lifted.

“For autistic people, those kinds of things are difficult to understand.

They can’t put it together. Seeing people through Zoom isn’t something some are used to.”

To help make up for the fundraising shortfalls, Autism Services is working with a restaurant to host a new fundraising event and the organization may begin new fundraising efforts in 2022.

A few unexpected donations have helped keep Autism Services going, such as $15,000 from the board of JP Foundation because they could not host their normal 5k event.

“It was out of the kindness of their heart,” Federiconi said.

“They’re committed because they see there’re a lot of autistic people in their community who could not get services. Fundraising is a top discussion at board meetings. Not just for us, but all the nonprofits.”

Tips to Help Children with Autism Cope with Studying at Home Physician Michelle Hartley-McAndrew, medical director of The Children’s Guild Foundation Autism Spectrum Disorder Center at Oishei Children’s Hospital, offered a few ways that parents and educators could help children with autism cope with educating at home.

Make a schedule or consistent routine, preferably with a visual support showing the different activities. At home: wake up, school, breaks, lunch and snacks, screen time or whatever is part of their day. Individuals on the spectrum are very visual and a schedule will help them to know what to expect.

Keep a calm environment.

Minimize distractions and clutter while the child is trying to focus on therapies or online learning.

Offer child choices to the extent possible. “Would you like to do math or reading first?”

Consider using the child’s interests in teaching new skills: counting dinosaurs or wheels on trains.

Incorporate physical activity. Researchers have found that children on the spectrum who exercised three times per week showed significant reductions in repetitive and aggressive behaviors. The behavioral improvements may last for at least two hours after exercising and improve sleep.

Try relaxation techniques: music, breathing, yoga, reading or whatever is relaxing to you and your child.

Maintain good communication with your child’s school or service provider so you are on the same page and have realistic expectations.

Involve your child in activities of daily life like cooking, laundry or chores.

Don’t be too hard on yourself or your child. These are unprecedented and extremely stressful times; sometimes you need to remind yourself of that, lower expectations and recognize most people are doing what they can just to get by. Brighter days are ahead.

It is okay to admit that you are having a hard time and seek help. There are online parent support groups that provide connections to and share information and resources with other caregivers in similar situations. Telehealth and Early Intervention: The Advantages and Disadvantages It comforts and empowers parents and caregivers

arly intervention is a nationwide program that aids parents, like myself, of children with developmental delays or disabilities.

This crucial resource helps infants and toddlers (from birth to age 3) reach physical, cognitive, communicative, social-emotional or adaptive milestones.

In times of worry and uncertainty, it comforts and empowers parents and caregivers.

Through E.I., service coordinators link parents with local therapists who come directly to the home and work one-on-one with children like my daughter.

Since the pandemic peaked last March, many of these children have not been able to see their therapists in person. They have, like other school-aged children, been restricted to virtual visits only.

Telehealth’s greatest advantage is the ability to safely deliver necessary services. But, what are the disadvantages compared to in-person interactions? How do these COVID-19 restrictions affect the quality of therapy? How do they affect the bonds between children and their therapists? When children struggle with tasks that most of us take for granted, these therapists make them fun and rewarding. They not only become teachers to my daughter and I, but friends to us both.

Danielle Tallo, a registered and licensed occupational therapist, weighs in on the benefits and drawbacks she found while transitioning from in person visits to teletherapy.

— to remain safe during the height of COVID-19.

Additionally, the opportunity for educating caregivers improved, as virtual therapy increased parent engagement.

“During in-person sessions, therapists like myself Tallo have noticed parents seem to draw back and allow the therapist to simply work with their child. They are not developing hands-on skills and their own competence for completing tasks,” Tallo said.

With high caseloads and travel, there was previously not as much time for caregiver training, planning, and explanations of tasks and goals. Telehealth also allows for more flexibility in the case of scheduling conflicts.

There are, however, disadvantages. For those whom Tallo had not interacted with prior to these restrictions, building rapport with new clients has been challenging. “Children need that in-person engagement to build connections, trust and ultimately a companionship with therapists.

If no relationship is formed, then I feel that, in the child’s eyes, I am nothing more than a Youtube video or random person on a phone or laptop screen,” Tallo said.

Thankfully, my daughter built a strong relationship with Tallo prior to the pandemic. As a parent, however, my biggest challenge is getting my

toddler to focus on tasks with someone on a screen, no matter how well she knows that person.

Previously, these sessions were just play time with someone she loved. Now her engagement has dropped.

My job is to follow and demonstrate the therapist’s instructions, and help my daughter understand and engage in the suggested activities.

Although I play the most crucial role in her success, I cannot replace a therapist’s formal training and education.

Kate Doherty, ongoing service coordinator, helps explain this concern. “For more hands-on services like physical therapy and occupational therapy, telehealth can be more of a challenge because [therapists] need to get their hands on the kids in order to guide certain mobilities… Sometimes the parents jump right in and do great with it. Sometimes they’re a bit more hesitant, lack the confidence and don’t want to hurt the child (in the case of infants that need certain manipulations),” she said.

“Most of the parents on my caseload have been pleasantly surprised with their child(ren)’s response to telehealth and their progress with it.

Some children have not responded as well. I’ve had a few families where it just isn’t ideal. The child has no interest in the screen or will not give any attention to it. In those cases, we work as hard as possible to get them some more options,” Doherty added.

Until recently, virtual therapy completely replaced home visits due to COVID-19 restrictions.

The past few months, in-person visits are permitted with special restrictions. On Doherty’s caseload, roughly 50% of children receive ser-

vices strictly online, 20% at approved clinics, 20% at daycare and 10% at libraries or other approved public locations.

“In some cases, children that are getting services at a clinic or public location are also using telehealth once a month or so, as a hybrid Doherty method, in case the clinics or in person options need to be shut down again,” Doherty explained.

Doherty said that while telehealth may not remain the main service delivery option, it has opened doors to more flexibility. It may, “post-pandemic life,” be used as a backup in the case of provider shortages, and transportation or scheduling conflicts, Doherty said.

“I know that many are hoping and likely assuming that it’s just a matter of time before in-home services can resume. That tends to be the most convenient and desirable option for many parents,” Doherty added.

I know my daughter misses oneon-one time with her therapists. Her losing out on this interaction concerns me. I worry about the personal connections she’s formed fading, and the role that may play in her success.

Tallo also misses this personal time with the kids she is used to befriending. Seeing their smiles in person and “getting and giving high fives and hugs” to celebrate accomplishments is one of the things she

A Social Security Perk for Older Parents Dear Savvy Senior, I’ve been told that my two children, ages 14 and 16, may be eligible for Social Security when I file for my retirement benefits. Is this true? What can you tell me?

Dear Viagra, It’s true. If you’re age 62 or older and are still raising young children, there’s a Social Security benefit strategy that can put some extra money in your family coffers.

Here’s how it works.

When you file for Social Security retirement benefits, your minor children can get money on your work record equaling half of what you would receive at full retirement age, now gradually rising from 66 to 67.

Even if you were to take a smaller benefit by claiming earlier, your kids will still get half of your full-retirement age amount.

To qualify, your daughter — whether she’s biological, adopted or a stepdaughter — must be unmarried and under age 18. Kids who are over 18 but still in high school, can collect too until they graduate or turn 19, whichever comes first. (Other rules apply to kids that are disabled.)

But that’s not all.

Because one of your children is only 14, your wife (if you’re married) can collect Social Security benefits on your work record too, and it doesn’t matter if she’s just 40 years old. The minimum age requirements to collect retirement benefits (62) or survivor benefits (60) does not apply when it comes to collecting benefits as the caregiver of a young child. The spouse’s benefit, which is also worth up to half of your benefit, will stop when your daughter turns 16.

But note that there are limits to the amount of money that can be paid to a family. The Social Security “family maximum payment” is determined by a complex formula and can range from 150% to 180% of your full retirement benefit amount. If the total exceeds that, each person’s benefit, except yours, is cut proportionately until it equals the maximum.

Here’s an example of how that’s figured. Let’s say, for example, that your full retirement age benefit is $2,400 per month. That would make your family maximum benefit (according to the Social Security formu-

Subtract your $2,400 benefit from the $4,200 family maximum benefit, which leaves $1,800. That’s the monthly amount that can be split between your two children — $900 each. If your wife wants in on it too, the individual checks are smaller, at $600 a piece, but the family amount is the same.

You should also know that minor children can collect up to half of a disabled parent’s Social Security disability benefit. And if the parent dies, they will get a survivor’s benefit, which is up to 75% of the deceased parent’s basic Social Security benefit.

One Caveat Social Security benefits for your kids may not be available before full retirement age if you are still working. In 2021, you will lose $1 in benefits for every $2 earned over $18,960, except in the year you reach full retirement age. Milich suffers from incurable, terminal kidney cancer that has spread to other parts of her body and causes extreme fatigue, nausea, and pain.

“I’m dying. I’m in my bed alone; this is how I spend my day. This isn’t surviving! This isn’t even existing!

This is suffering!” she said on compassionandchoices.org.

“I’m not afraid to be a cancer patient; I’m afraid to be a cancer patient who’s suffering, who’s in horrific pain that can’t be controlled, who

possibly ends up in a coma. I want to spend time with my family. But having the option to die peacefully at the end of my life, when I’m suffering, is so important.”

Milich does not have the option of dying peacefully without suffering. New Yorkers who are dying from a terminal illness do not have the option of death with dignity.

She has been one of the many advocates demanding that legislators pass the Medical Aid in Dying Act. “It’s not just Jennifer [Milich],” said Corinne Carey, New York campaign director for Compassion &

“Medical Aid in Dying Act (MAID) is one option a terminally ill, mentally capable adult, with the ability to self-administer medication — when and if they choose — to bring about a peaceful and humane death, should their end-of-life suffering become too great to bear,” said Gene Hughes of Utica. Hughes is an advocate for the rights of those living with disabilities who believes that personal autonomy should also apply to end-of-life decisions.

The MAID Act, which has the overwhelming support of most New Yorkers and doctors, is drafted after the Oregon Death with Dignity. It allows a mentally competent adult who is terminally ill, with six months or less to live, request prescription medication from their doctor which they can decide to take if their suffering becomes too great to bear, so they can die peacefully.

“It’s a good bill, long overdue,” Hughes added. “It gives people a choice. It’s not forcing anybody to do anything. You don’t want it, don’t ask for it.”

The bill includes more than a dozen safeguards. Some of the safeguards include: the patient must be able to take the medication themselves; only people with an incurable and irreversible terminal illness and six-month prognosis, confirmed by two doctors, are eligible for medical aid in dying. A terminally ill person can withdraw their request for medication, not take the medication once they have it, or otherwise change their mind at any point in time.

Currently, the bill is under consideration by the Senate and Assembly Health Committees, David Levine, consultant to End of Life Choices, advised: “We are pleased by the number of sponsors it has, and it’s supported by Gov. Cuomo.”

After it’s approved by both committees, it must be approved by another set of committees before it goes to the floor for a vote, he added.

“We’re hopeful it will pass this legislative session.”

New York voters and doctors overwhelmingly support medical aid in dying.

“Lawmakers have the power to stop needless suffering now, in 2021,” Carey added. Now is the time. The evidence is there, we have years of evidence showing that these laws work as intended: few people use them, but countless others gain the peace of mind of knowing that should suffering become too great, there is an option for a peaceful death.”

The reasons people want the option of medical aid in dying are as varied as people themselves, but at the core is a desire to avoid suffering and leaving loved ones with the enduring memory of their suffering.

How we die, comfortably with family, or amidst a host of beeping machines, strangers and intense pain, should be our choice.

As Barbara Coombs Lee of Compassion & Choices says in Diane Rehm’s book “When My Time Comes:” “The technology that medicine wields is not necessarily in an individual’s best interests. Only individuals can decide what is best for them. It’s as though medicine has gotten ahead of human desire.

There are so many ways to keep us alive and the incredibly sophisticated means of keeping people alive don’t always take into account what people themselves want.”

“Everyone brings their own beliefs and values ​​into the equation, reaching the end of life in a different way,” said Carey. “For most New Yorkers, medical aid in dying is not a final decision in life that they will face. However, for Jennifer Millich and thousands of others who are living with a terminal diagnosis and pain and suffering, the medical aid in dying may actually light up at the end of the tunnel.”

H ealth News ECMC Family Health & Internal Medicine Centers Earn National Recognition Erie County Medical Center (ECMC) Corporation announced that the National Committee for Quality Assurance (NCQA) has again recognized ECMC’s Family Health Center and its Internal Medicine Center for achieving sustaining practices through the New York State Patient-Centered Medical Home (PCMH) program. Both primary care services had been previously designated PCMH programs: Family Health Center in 2016 and the Internal Medicine Center in 2017.

“ECMC’s Family Health Center and our Internal Medicine Center have consistently provided high quality primary care services to their patients, which is re-affirmed by this updated national recognition for these centers. New Complex Contraception and Family Planning Care Launches, Increase Access to Care

A broader range of reproductive and gynecological services will be available in Western New York, now that UBMD Obstetrics & Gynecology (OBGYN) is instituting a new clinical initiative, addressing concerns around complex contraception and family planning.

The program has launched in an effort to help patients who may have medical considerations complicating their ability to select and receive birth control.

“Patients with these concerns may benefit from seeing a physician who has additional training and expertise in complex contraception,” said physician Sarah Berga, president of UBMD OBGYN. “With a complex contraception specialist, they can find an option that fosters health and works for their lifestyle.”

Gift bags of nonperishables were delivered last spring; craft supplies and activity books along with iPads were provided to low-income housing communities; Hero Boxes were delivered to deserving veterans; Christmas presents were arranged for many isolated seniors with little or no family; and recently, stuffed animals and goody bags were delivered to residents at Mary Agnes Manor in Buffalo for Valentine’s Day. Applications for future wish recipients are being accepted on an ongoing basis, whether grand or small. Wish recipients must meet the following requirements: 65+ and a resident of WNY; U.S. citizen; annual income $38,000 or less for a household of one or $44,000 or less for a household of two; cognitively, emotionally and physically capable of communicating and experiencing the wish; and unable to fulfill the wish on their own.
