When: May 17-18 Where: Meet at Friendship Baptist church at 5:45PM Return to FBC at 8:30PM on the 18th

Permission Slip ___________________________________ has my permission to attend the “Camp Chautauqua Youth Conference” outing sponsored th th by the Friendship Baptist Church (May 17 & 18 , 2012). I agree not to hold Friendship Baptist Church or its agents liable in the event of an accident or injury to my child. I understand that this outing will consist of transportation to and from Friendship Baptist Church to Camp Chautauqua (~35 miles) in Franklin, OH on busses driven by approved adults. I also understand that there will be all types of camp activities and sports along with Bible-centered messages and singing. Return to Friendship Baptist at 8:30 p.m. on the 18

ATTIRE: Casual Dress clothes for the service on Friday Casual (but respectable) for weekend Shorts must be knee length and loose fitting No spaghetti strap shirts or tank tops for girls No tank tops for boys COST: $40.00

ITEMS TO BRING ALONG (Besides the permission slip and $40) 1.

Bible

2.

DO NOT BRING IPODS or other expensive items. You will have no way of locking them up.

3.

Personal hygiene items (toothbrush, towel, shampoo, razor, etc.)

4.

Wear church clothes for Friday night service (if you have them) Knee length skirts or dresses preferred for ladies (if you have one) Dress pants & shirt (no blue jeans) for young men (if you have them) We will change clothes after the service

5.

Casual clothes for Friday night and all day Saturday Shorts must be knee length and loose fitting Clothing for physical activity (e.g. shorts, pants, t-shirts) No spaghetti strap shirts or tank tops for girls No tank tops for boys

6.

Money if you desire to purchase a conference t-shirt ($10)

7.

A jacket or sweatshirt in case of unseasonably cool weather

8.

Tennis equipment if you would like to use the tennis courts

9.

A flashlight if you are scared of the dark or plan to lose something

10. Sleeping bag or blankets and pillow.
