What is Holding You Back ? CURRENT SITUATION Utilization of blood products at most hospitals are reported monthly and on a voluntary basis.

Actually Manitoba, Yukon & British Columbia use different systems than other provinces. There are still manual data entries in some reporting systems.
CIHI has difficulty building a reliable and comprehensive database for a global picture of the Canadian health care system.
As we can’t ask every hospital to change its whole IT system (LIS) because it is costly and time consuming, we suggest you standardize the information flows. How to do that? By developing a report template and by enforcing a frequency of data transmission. By doing this, you will homogenize the information you receive and will be able to transmit it to CIHI. They will be able to link the blood transfusion and blood product information with the patient diagnosis and treatment plan to finally identify the rationale for why the patient required blood and eventually make more accurate previsions. What should be on these reports ? Quantity of blood product transfused Patient’s blood group Patient’s hemoglobin At what frequency ? As it is reported monthly now, we suggest you first encourage them to do it weekly and as they get used to it, possibly ask for a daily transmission. On which provinces you should focus your effort? Based on the actual and estimated ED records sent to NACRS, you should focus on Newfoundland and Labrador, Prince Edward Island, New Brunswick, Yukon Territory, Northwest Territories and Nunavut to get a better global portrait. Other provinces should not be dismissed, but they are already doing better then the first ones.

Enhance the Whole Process with Technology What: Implement Radio Frequency Identification (RFID) technology in the ‘’vein-to-vein’’ blood path. How it works: Incorporate a passive RFID tag and a time and temperature indicator labels to each bag used to collect blood. Add an RFID short range reader for every fridge that will be used for the blood storage in hospitals to monitor in and outs. Provide a RFID Tunnel Readers for every hospital, manufacturing sites and testing center to manage upstream and downstream movements of blood bags. Finally, Mobile Readers will be needed for the final step; the transfusion. Advantages: (1) Reduce human error by relaying historical data, improve the monitoring of blood bags by providing real-time tracking and the label provides visual indicator regarding the temperature status of the blood. Thus, you can trace if there was a problem with the temperature along the transportation process. (2) Improve the safety of patients and medical staff by assuring that the right blood is given (3) and improve the efficiency of the overall management of blood bags by reducing the need of manipulations of the product. Since the blood bags don’t need to be in really cold conditions (i.e. under zero degree Celsius), there’s no issue of using these technologies. Also, as you will now be using the VMI method to replenish hospitals, RFID will make your life easier and provide info to make better demand forecast. Scalability: If one day you get to the point where you introduce RFID to also manage the inventory of your medical tools, you will already be familiar with this technology, and probably some facilities can be reused for this purpose. Success Story: Technology that is already used in Australia and that has made its proof to be, so far, the best technology for blood supply tracking. Add-ons: Implantation of Digital Kanban Board in the hospitals storage rooms. How does it works? When an employee takes a bag from the hospital storage, the chip associated with the location of the blood bag is put on the digital board before getting out of the room. The board gets the signal of the chip and sends the information to the IT system that is planning the replenishment.

Pilot Project
For an implementation of this size, we suggest you do a preliminary pilot project in a small scale in order to evaluated feasibility, time, cost, adverse events and improve the process prior to performance of a full-scale project. Even, if we tried to identify all the elements to consider to complete the implementation, they will be more relevant after the pilot project.
Where to start? Considering the number of submitting facilities in each province, we suggest you begin with British Columbia. Why ? Because we judge 30 is a reasonable pool of hospitals to start with in comparison to Alberta or Ontario that have too many or even Manitoba or Yukon that don’t have enough. Risk that the information associated with the blood bag is not accurate. This risk is most likely to happen at the beginning of the whole process since it’s the step that most of the data is entered.
The difficulty to reach new donors is a risk almost impossible to completely eliminate. You need to find a way to pass the first donation barrier by convincing the population.
The pilot project is suggested precisely to identify issues like this one. As the process of collecting data will be standardized, it will be easier for your employees to assure the reliability of the information on the blood bag. The proposed partner, OPTEL, will be the resource at this level since they have expertise and will be able to guide you for specifications like these.
Knowing that only 4% of possible donors actually give blood, there is no shortage of donors to anticipate. However, the challenge is in fact to convince them to give blood! CBS already has several programs in place to reach new donors and retain them, but social medias are under utilized for now. Thus, our proposed marketing plan will make sure to increase the amount of donors in urban areas and in areas where the blood donors level is particularly low.

Compare, year by year, the reduction in the quantity of blood waste because of non-accurate demand forecasting of inventory management. Using the RFID technology will increase the rotation rate and we are convinced that CBS and the hospitals can achieve almost 0% waste in less than 3 years. With the VMI process and RFID technology, it will be possible for you to track the number of times a blood bag is never linked to a patient.
Current situation : 406 000. The objective would be to increase by 30% the number of donations per year in order to, as a final result, reach 800 000 donation in less than 3 years. We could also add to this the objective of increasing by 15% the number of recurrent donors (donates at least twice a year). With the change management program, we want you to achieve a 95% satisfaction rate of employees. We don’t want to let down the employees with this process change. Our mandate was to Strengthen the value proposition the Canadian national blood system can bring to the broader health care systems in which it operates We have presented you different technologies to implement in order to optimize CBS supply chain. We believe that our recommendation is the best solution according to the context and your specific needs. We have showed you a concrete application of the RFID Technology in the whole blood bag supply process, as well as how the use of temperature labels and VMI can improve this process. We have suggested you to create a Supply Chain Committee and have made a proposal for some strong partnerships to start this project as soon as possible. Thereafter, we have detailed for you the steps to do before putting all these technological changes in place across the Canadian blood system. Moreover, we have showed you a synthetized financial plan with the initial investments and the recurrent costs we think this project will imply. We have set up a change management plan that is necessary for the success of a project of this size, and to reassure all stakeholders. Finally, we have presented some risks and the strategies to manage them. Alongside, some KPIs to put in place to measure the impacts of this project.
