HEALTHY LIVING YMCA’S DIABETES PREVENTION PROGRAM

WHAT IS PREDIABETES?

If you are at high risk for type 2 diabetes, the YMCA’s Diabetes Prevention Program can help you adopt and maintain healthy lifestyles that will help you reduce your chances of developing diabetes.

Prediabetes is a condition in which individuals have blood glucose levels that are higher than normal, but not high enough to be classified as diabetes. People with prediabetes are at high risk of developing type 2 diabetes, heart disease, and stroke.

The YMCA’s Diabetes Prevention Program helps those at high risk of developing type 2 diabetes adopt and maintain healthy lifestyles by eating healthier, increasing physical activity and losing a modest amount of weight in order to reduce their chances of developing the disease. Research by the National Institutes of Health has proven that programs like the YMCA’s Diabetes Prevention Program can reduce the number of cases of type 2 diabetes by 58%.

An estimated one in three U.S. adults has prediabetes, yet just 11% of those with prediabetes know they have it. Prediabetes increases the risk of type 2 diabetes, heart disease and stroke. Risk for developing type 2 diabetes may be reduced or eliminated by weight loss, healthier eating, and increased physical activity.

PROGRAM GOALS:

Reduce body weight by 7%

Increase physical activity to 150 minutes per week

DETAILS: In a classroom setting, a trained lifestyle coach will facilitate a small group of participants in learning about healthier eating, physical activity, and other behavior changes. The year-long program consists of 25 sessions over the course of one year. Program goals include: Reduce body weight by 7% Increase physical activity to 150 minutes per week WHO CAN PARTICIPATE? In order to qualify for the program, you must be at least18 years old, overweight (BMI > 25)* and at high risk for developing type 2 diabetes indicated by a confirmatory blood value†, prediabetes determined by clinical diagnosis of Gestational Diabetes (GDM) during previous pregnancy, or a qualifying risk score. Asian individual(s) BMI > 22. †Individuals who have already been diagnosed with either type 1 or type 2 diabetes do not qualify for this program.

SOCIAL RESPONSIBILITY VOLUNTEERISM & GIVING

SOCIAL SERVICES

VOLUNTEER, DONATE, ADVOCATE Our movement is made up of people of all ages and from every walk of life, all working side-by-side to ensure that everyone, regardless of gender, income, or background, has the opportunity to live life to the fullest. Financial assistance keeps the Y available for kids and families who need us most. We count on the generosity of our members and community to help people of all ages and from all walks of life be more healthy, confident, connected, and secure. When you give to the Y, your gift will have a meaningful, enduring impact right in your own neighborhood. The Y’s Financial Assistance Program is supported by contributions to our Annual Campaign and United Way. And finally, the Y could not survive without the dedication of our volunteers. From youth tutors to committee members, teen mentors to fundraisers, board members to fitness instructors, volunteers lend their time, talents, and energy to help strengthen our community.

Y COMMUNITY

VOLUNTEER, DONATE, ADVOCATE Across the United States, more than 500,000 business leaders, community advocates, parents, teens, and individuals who want to give back and support their neighbors volunteer at the Y. In 10,000 neighborhoods nationwide, including Snohomish County, we have the presence and partnerships to not just promise, but deliver positive personal and social change. Y volunteers give men, women, and children of all ages and from all walks of life the resources and support the need to be healthy, confident, connected and secure.

Y Community provides free or reduced-fee activities to families in partnered emergency and transitional shelters. Activities vary from the YMCA and other locations. Register with your caseworker or the Community Coordinator. Y community programs are made available through community Annual Campaign donations. Contact Member Services if you would like to make a gift to support future programming. YOUTH MEMBERSHIP The Y is for everyone. Youth Memberships are available to teens who are eligible for free or reduced-price lunches at school. Turn in your Youth Membership application with: • School District letter of eligibility for the free/reduced lunch program, or from a school counselor confirming eligibility. • Payment of $17 or $34, based on free or reduced status

Ages 10 and older Additional Information: These Youth Memberships expire August 31. Please reapply each school year. Contact: Kate Rossart, Senior Program Director, 425 493 2415

VOLUNTEERS OF THE Y • Lead our Boards of Directors and branch committees. • Raise funds to ensure the Y is accessible to all members of the community through Annual Campaign. • Motivate and support youth through coaching our sports teams and teaching many of our classes. • Extend a hand to help adults who want to do more, be more or live healthier through healthy living programs. • Participate in opportunities to give back and support neighbors through food drives, community events and more! If you would like to donate some of your time please pick up a volunteer application. Volunteer opportunities and application can be found on our website at ymca-snoco.org - just click on our Social Responsibility link at the top of the page.

SOCIAL RESPONSIBILITY Giving back and supporting our neighbors is part of what makes the Y the Y. We are a community and we lend a hand where we can.

TOGETHER, WE CAN BUILD A BETTER US Every day, the YMCA of Snohomish County gives our community the support it needs to learn, grow, and thrive. From advocacy to food security, disease prevention to athletics, football to dance classes, everything we do helps strengthen our neighborhood and the people who live here. But keeping our neighborhoods great is a full-time job. Every day, our community faces new challenges that create a greater need for the work we do. And we need your help to do it. Your gift can help Snohomish County and everyone in it shine. For a better you. For a better community. The Y.™ For a better us.™ Learn more at ymca-snoco.org/ForABetterUs BLOOD DRIVE Help our community by donating blood at the Mukilteo Family YMCA. Tuesday, August 2 Sunday, September 11 Friday, December 30 2017 ANNUAL CAMPAIGN KICK OFF Tuesday, February 7. Contact the branch if you would like to attend this fundraising event or volunteer to help! DONATE TO THE MUKILTEO FOOD BANK You can drop off non-perishable food donations at any time in our lobby. Thank You! PROVIDING SAFE PLACES FOR YOUTH The Mukilteo YMCA is supporting the Cocoon House Safe Place program, designed to facilitate access to supportive resources for young people in crisis.
