On the basis of academic merit, Viva Voce performance and keeping in view the Local Criteria of hiring governed by letter No SHS/NRHM/J&K/K/4126-37 dated 11-09-2013 received from Mission Director NRHM J&K(State Health Society J&K and as approved by the Selection committee constituted for the purpose, the following candidates have been found eligible for Hiring under National Rural Health Mission initially for a period of current financial year(20132014) , extendable on the basis of performance and guidelines of National Rural Health Mission. In case of deficiency with reference to eligibility etc proved at any time of the Hiring process, the candidate shall forfeit his /her Hiring. 8. In case, at any time it is found that any candidate has used any fraudulent means/ certificate (PRC, Marital Status) to avail local criteria, his/her candidature /Hiring will be cancelled. The bidders need to submit the proof for purchase of e-Bid document as stated in the above table through Demand Draft of any nationalized/scheduled Bank pledged to Chief Accounts Officer with Chief Engineer, EM&RE Wing, Jammu Payable at Jammu. The scanned copy of the Demand Draft must be enclosed along with the e-Bids but the original Demand Draft should reach the office of Chief Engineer, EM&RE Wing, Jammu at least one day before the last date of Online E-Bid submission date and time. 3. All the e-Bids must be accompanied by the scanned copy of EMD in the form of CDR/FDR/Bank Guarantee issued by a nationalized/scheduled Bank only, pledged to the Chief Accounts Officer with Chief Engineer, EM&RE Wing, Jammu. No Interest would be payable on Earnest Money deposited with the Department. a. Hard Copy of original instrument of EMD shall be submitted to the office of the Chief Engineer, EM&RE Wing, Canal Road Bhagwati Nagar Jammu-180016 at least one day before the last date of Online E-Bid submission date and time. b. The date and time of opening of Financial-Bids shall be notified on Web Site http://jktenders.gov.in.This is conveyed to the qualified bidders automatically through an e-mail message on their e-mail address. The Financial-bids shall be opened accordingly online on the same Web Site at the Office of Chief Engineer, EM&RE Wing, Canal Road Bhagwati Nagar Jammu-180016. 4. Intending bidders are requested to submit their offers along with latest type test reports from CPRI / NABL accredited laboratories as per terms, conditions, technical specifications and annexures forming part of the tender document. 5. The e-Bids will be electronically opened in the presence of bidder’s representatives, who choose to attend at the venue, date and time mentioned in the above table or any subsequent day to the convenience of the Tender Opening Committee. An authority letter of bidder will be required to be produced. 6. In the event of date specified for e-Bids opening being declared a holiday for department’s office then the due date for opening of e-Bids shall be the following working day at the appointed time and place. 7. If any bidder/ tenderer withdraws his bid/ tender or makes any modifications in the terms and conditions of the bid, the earnest money of such bidder shall be forfeited. 8. Quantities indicated in the BOQ (Bill of Quantity) are liable to increase or decrease by 50% as per Department requirement. 9. All the required documents excluding Price Schedule/BOQ should be uploaded by the e-Bidder electronically in the PDF format, whereas Price Schedule/BOQ should be uploaded electronically in the same BOQ sheet provided with the SBD(Standard Bidding Document). 10. To participate in e-bidding process, bidders have to get ‘Digital Signature Certificate (DSC)’ class III(b) as per Information Technology Act-2000, to participate in online bidding. 11. Hard copy of Technical and Commercial bid documents (complete) -3 SETS must be delivered to the office of Chief Engineer, EM&RE, Wing, Canal Road Bhagwati Nagar Jammu-180016 in two sealed covers, at least one day before the last date of Online E-Bid submission date and time. One sealed cover containing 3 Sets of Commercial Bid Documents shall be super scribed as “PART-I COMMERCIAL BID AGAINST NIT NO: CEMREJMU/09 of 2013 Dated 06.12.2013” And another sealed cover containing 3 Sets of TECHNICAL Bid Documents shall be superscribed as “PART-II TECHNICAL BID AGAINST NIT NO: CEMREJMU/09 of 2013 Dated 06.12.2013” The name of the bidder and NIT No. shall be clearly written on the cover. It may be noted that the hard copy should be an exact replica of the uploaded offer documents. For evaluation purposes the uploaded offer documents will be treated as authentic and final. The hard copy shall be used only for reference purpose. Any documents submitted in hard copy but not uploaded on the website shall be treated as irrelevant. 12.
