We Welcome Our “Packer Great” Celebrities Gilbert Brown, William Henderson And Doug Evans We are extremely pleased that our 15th Annual Angel Golf Spectacular proudly brings us the opportunity of welcoming three former Packer greats!! It is an honor to have the giant presence of Gilbert Brown (the Grave Digger), the “Spectacular”, William Henderson, and first time Packer celebrity guest-defensive back, Doug Evans, among us. Meet our Green and Gold Packer “Angels” . . . We are blessed to have Gilbert Brown grace us with his presence for the sixth consecutive year. He is one of four true defensive linemen to play 10 seasons for the Packers. Obtained on waivers from Minnesota during the 1993 training camp, he played 125 Packers games (103 starts) with 292 tackles (186 solo) and seven sacks. Gilbert played in 15 Packers playoff games. During the 1996 season, it became known that he would regularly order the “Gilbertburger”, a double Whopper with extra everything and always obtained from the Oneida Street Burger King in Green Bay. Gilbert became a fan favorite, partly because he was easy to spot and partly because of his eccentric grave digger dance. He came up with the “gravedigger” move at Kansas. After a big defensive hit, he would dig an imaginary grave which became his trademark and nickname. Gilbert was a major contributor on strong defenses during the mid 1990’s. His best season was 1996, which concluded with Green Bay winning the Super Bowl XXXI in 1997. He was featured in the Sports Illustrated special, “Where Are They Now?” featuring former professional athletes. In July of 2008, Gilbert was inducted into the Green Bay Packers 38th Hall of Fame. He is a co-owner of the Milwaukee Mile, a racetrack that is often used for NASCAR races. A HUGE and GRATEFUL welcome back to William Henderson . . . The one and only, William Henderson, is honoring us with his fourth appearance at our Golf Spectacular. He not only graciously participates in our Party with the Pack and Breakfast with the Pack bidding process but also honors one of our golf teams by joining them on the course as their celebrity “teammate”!! The best part is that this is all part of our phenomenal bidding opportunities to raise additional monies to benefit those in the cancer-affected community. William Henderson is a former American Football fullback. He was selected as Green Bay’s 2001 “Unsung Hero” in recognition of this efforts both on the field and in the community. He was drafted by the Green Bay Packers in 1995 and played every season with the Packers until his release in 2007. William helped the Packers win their third Super Bowl victory and a year later helped them appear in their fourth Super Bowl. He was durable and effective for the Packers, solidifying the team at fullback when he moved into that starting role in 1996. William proved valuable as pass catcher out of the backfield. He stands tenth all-time on the Packer’s career receptions list with 320 (for 2,409 yards) and first among running backs. He was selected to the 2004 Pro Bowl. William will always be considered a “Packer”. Angel proudly introduces Packer great, Doug Evans . . . Doug Evans was born in Shreveport, Louisiana and attended college at Louisiana Tech. He is a former American football cornerback and safety in the National Football League. Doug was drafted by the Green Bay Packers in the 6th round (141st overall) of the 1993 NFL Draft. Doug played Defensive Back from 1993 for the Green Bay Packers, Jersey #33. Proud career statistics: played 162 games, made 522 tackles, intercepted 28 passes, recovered 9 fumbles and is credited with 6.0 quarterback sacks. Special career highlight: SUPER BOWL CHAMPION (XXXI)!! What an amazing record and what an incredible athlete!! Angel On My Shoulder Scholarship Program Supporting Education… In 2008, Angel On My Shoulder established the Angel Scholarship program. Five $1000 scholarship were offered. Through the generous support of those attending our Golf Auction and your generous contributions, we were able to increase the number of scholarships offered in 2009 to four $1500 Scholarships and nineteen $1000 Scholarships D=;B *D (O .>EKB:;H for a total of $25,000! SCHOLARSHIP For 2011, four $1500 Scholarships and nineteen $1000 Scholarships are again being awarded to graduating High School Seniors of Wisconsin who are survivors of cancer or who have an immediate family member (mother, father, or sibling) affected by cancer. Awards are granted based on qualifications and need. All applications are subject to review by a committee selection process. Students must plan to attend a four year college, specialty or technical school. Scholarship recipients will be notified by mail. Following is the list of all awardees. Angel On My Shoulder 2011 Honoree An Angel among us . . . Mike Derynda… our 2011 Honoree. Mike lives with his wife Tammy, son Jon and daughters, Brittany and Tiffany in Oconomowoc, WI. He enjoys the “small town” feel of living there and is very involved in donating to many local schools and charities through his business – Culvers. He has been a Culver franchisee for 16 years and owns three restaurants in Oconomowoc, Watertown and Fort Atkinson. The businesses are challenging at time but anyone who knows him knows that he is a high energy person.

In the words of his wife, Tammy… “Mike is one of the most generous people you will ever meet. He would give you the shirt off his back if you needed it. He cares about people and loves to live for the moment-usually resulting in everyone around him having a great time. He makes me laugh every single day”. Mike has a soft spot for children and has unlimited patience for teaching kids things. He coached football for many years and is currently trying to teach his youngest daughter to fish. His caring for children led him and his wife Tammy to become active in the Make A Wish Foundation of WI. They sponsored and ran a golf outing for Make A Wish for five consecutive years. Through these outings they raised over $100,000 – enough money to sponsor wishes for 25 children with life threatening illnesses. We salute this remarkable couple!!!! Mike enjoys attending golf outings for charities. Angel On My Shoulder is blessed to witness his caring and above and beyond giving spirit through his active participation in our Angel Golf Spectacular. He is truly a phenomenal “Auction Angel”. Mike is always right up front with his kindness and generosity to make a positive difference to those living with or affected by cancer. Vincent’s Big Pink Bunny Several years ago, the mother of a courageous little boy named Vincent, donated his Big Pink Bunny to Angel On My Shoulder in his memory. This started a special tradition beginning at the first Angel Golf Spectacular in 1997. Each year, the Big Pink Bunny is auctioned, and the money raised is donated in Vincent’s name. The generous bidder has then always donated the Vincent’s Big Pink Bunny back to Angel On My Shoulder to be auctioned again the next year. Our profound thanks go out to those generous contributors who each year help keep Vincent’s spirit alive. Dear Lolly, I cannot tell you how moved I am that Angel On My Shoulder is going to be donating all monies from the auction of the Big Pink Bunny in my son Vincent Kolata’s name. I truly hope that a difference can be made for cancer patients everywhere. You have given me my son’s immortality with this gesture, and I will never be able to thank you enough. I wish only that you would have had the opportunity to meet him, as he was a trooper. If he were alive today, he would be at Camp Angel to encourage all of the children, motivate them, and make them smile. He was infectious that way, and all the children at St. Joseph’s Hospital in Marshfield looked forward to his stay there for his treatments. Voice Auction Rules • The voice auction will take place after the dinner. • All items will be bid for on a competitive basis by hand signaling or bidder number. • All auction items will be sold to the person making the highest bid as announced by the auctioneer. • Disputed items will be settled by a closed bid as deemed necessary by the auction committee.
Silent Auction Rules • Each silent auction item will have a separate bid sheet. You establish a valid bid by writing your bid number and bid amount on the proper bid sheet. Each increase in bid must equal, or exceed, the minimum increase stated on the bid sheet in order to be valid. A minimum bid is required and bids below the minimum will be disqualified. Each new bid must be written on the next open line on the bid sheet. • Items will be sold to the person making the highest value bid. The auction committee may reject any bid not satisfying the conditions. • Illegal bids will not be accepted. • Your bid (oral or written) constitutes a contract to buy. • Silent bidding close time will be announced by auctioneer. • Items may be obtained at the end of the evening at the checkout area.

Special Appeal - Angel Adventures Angel On My Shoulder is pleased to offer Angel Adventures Camp for 16 to 18-year-olds affected by cancer through a parent, sibling or grandparent, or who have recently lost a loved one through cancer. Like Camp Angel and Camp Teen Angel, Angel Adventures hopes to address the needs of healthy young people in a family affected by cancer. This camp will intermix fun, physical activities with sharing session chats that will allow the campers to share their experiences and feelings and learn empowerment skills. Our inaugural session was held February 2010 and our second camp was held in February 2011. Enrollment is limited to 12 campers. Campers must be attending high school. Coach bus transportation to and from camp will be provided from designated pickup points throughout the state. Campers arrive Friday evening, Saturday and Sunday are filled with activities, and Campers begin their trip home Monday morning. This camp is scheduled to coincide with President’s Day, a Monday holiday in many school districts. Some of the activities at camp include broom ball, tubing, cross country skiing, snowshoeing, a horse-drawn sleigh ride with a campfire and hot chocolate, a martial arts class and enough down time for the campers to bond. Sharing sessions are facilitated by Kathy Mathie and Pastor Bill Trosien. Our goals is to provide them with the opportunity to have fun, share experiences, and perhaps gain some insight and coping skills, while bonding with other people in situations similar to their own. Sixteen campers have attended our first two camps. We anticipate having full camps in the future as we are getting the word out to high school guidance counselors and oncology units. Serve You Custom Prescription Management sponsored our first camp and our second camp was subsidized by a NAILBA grant. The grant proposal was graciously written by Michelle Bansemer of Serve You Custom Prescription Management. Lodging, meals, transportation and activities average about $6000 per camp. 23

Healing Angels
Healing Angels is a unique retreat that is custom designed for cancer survivors. It is available to those who have finished their oncology treatments (i.e. chemotherapy and/or
radiation), procedures and/or surgeries and are at the gateway of physical and emotional healing. The heart of these three-day retreats is a healing atmosphere of love, hope, understanding, learning and pampering of cancer survivors.
Uplifting Angels Uplifting Angels, the very first program created by Angel On My Shoulder was implemented in 1995. In the fall and spring of the year, very special “thinking of you” deliveries are planned, prepared and coordinated by Mary Long, our Uplifting Angels Director. These uplifting packages are delivered to over 77 hospitals and cancer clinics throughout the state. Approximately 5,000 cancer patients (young and old alike) are recipients of these right from the heart gifts that are designed to brighten their day and let them know they are tenderly thought of and cared about. A wonderful team of “Uplifting Angels” graciously and generously offer their time and talents to deliver these gifts that carry messages of hope and support to those living with cancer.
Angel Care Angel Care offers cancer caregivers the opportunity to gather together in a safe, non-judgmental, nurturing and understanding environment to share their stories. These special “Guardian Angels” learn techniques which help them reduce fatigue associated with the physical, mental and emotional stress brought on by the cancer crisis.
Guardian Angel Angel On My Shoulder proudly recognizes and honors those amazing people that are a part of the care giving community through its “Guardian Angel” Program. Unselfishly giving of themselves, these “above and beyond” individuals provide unwavering support, comfort and care from many dimensions to those living with cancer. These outstanding Guardian Angels receive a “just for you” gift package and a well-deserved certificate of commendation in recognition of their tireless and incredible efforts.
Angel Wings Angel Wings is designed to grant special requests for support to individuals and/or families affected by cancer. It offers those experiencing cancer broader opportunities to receive help and uplift their spirits through different vehicles of support without duplicating services available through other foundations and organizations. Camp Angel/Camp Teen Angel/Angel Adventures These one of a kind weekend retreats for young people affected by cancer through loved ones (parent, sibling or grandparent) or who have experienced the loss of a loved one through cancer are cost-free to the campers. Camp Angel and Camp Teen Angel are presented through the partnership of Angel On My Shoulder and Marshfield Clinic. Angel Adventures is presented through the generous financial support of Serve-You Custom Prescription Management, Inc. Our mission is to provide young people the opportunity to meet others like themselves in a well-supervised atmosphere of love and acceptance knowing that their pain is understood. There is a wonderful spirit of bonding and camaraderie as campers create special friendships and participate in a variety of fun-filled activities.
The philosophy of Camp Angel, Camp Teen Angel, and Angel Adventures is to help young people realize: •
It’s okay to smile, laugh and have fun even though a loved one is hurting.

It’s okay to be sad, angry and frustrated about what they are going through due to a loved one’s illness.
