ABOUT US The Arizona Alliance for Community Health Centers (AACHC) has served as the Primary Care Association for the state of Arizona since 1985. Our mission is to promote and facilitate the development and delivery of affordable and accessible community oriented, high-quality, culturally effective primary healthcare for everyone in the state of Arizona. This is accomplished through advocacy, education, and technical assistance. Our central purpose is to support Arizona health centers in leading the provision of exceptional population based healthcare for the communities we serve. The Directory is published as a guide to the community for low-cost primary healthcare options. It is intended to serve as a reference for state, local, public and private health and human service agencies, as well as students and potential healthcare providers searching for practice opportunities in Arizona. Due to the possibility of expansion of sites and services, and changes in personnel in the future, please refer to our website (www.aachc.org) for the most current information. ARIZONA HEALTH CENTERS AT A GLANCE Arizona has 23 Health Center Program Grantees, also known as Health Centers (or Community Health Centers). Health Centers are local, nonprofit, healthcare providers serving people of all income levels in medically underserved communities. Arizona’s Health Centers provide high-quality, affordable, culturally competent primary care and preventive services. Chiricahua Community Health Centers, Inc. Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Jonathan Melk, MD www.cchci.org Chiricahua Community Health Centers, Inc. (CCHCI) is a diverse not-for-profit health center program grantee/federally qualified health center providing a variety of services. It has been developed on a multi-dimensional model that includes medical, dental, and behavioral health carefully integrated with an approach that address every aspect of life (physical, emotional, social, occupational, intellectual, and environmental) for the uninsured, underinsured, veterans, migrant and seasonal farmworkers, and all residents of Cochise County. Currently operating from eight fixed sites, three mobile medical units and two mobile dental units, a full range of primary and preventive health care services are provided for the entire family. CCHCI is the largest primary care provider and offers the only true Sliding Fee Discount Program in Cochise County. Circle the City

Health Care for the Homeless Program Grantee/Federally Qualified Health Center

Chief Executive Officer: Linda Ross, MBA, MPM www.circlethecity.org Circle the City, a 501(c)(3) nonprofit community health organization, exists to create and deliver innovative healthcare solutions that compassionately address the needs of men, women and children facing homelessness. In 2012, Circle the City launched Arizona’s first medical respite center, a 50-bed facility where adults experiencing homelessness could rest and heal following periods of illness and injury. In 2015, Circle the City opened The Parsons Family Health Center, an integrated primary care and behavioral health community health center also serving the homeless of Maricopa County. Circle the City operates a robust outreach program utilizing four mobile medical units each equipped with 2-exam rooms and also utilizes street medicine teams throughout the County. In 2018, Circle the City further expanded to become the sole homeless healthcare provider of primary care services in Maricopa County. This expansion includes a second family health center and medical respite facility on the Human Services Campus as well as multiple additional outreach and afterhours sites of service. Altogether, this creates a seamless continuum of homeless specialty care throughout Maricopa County. Community Health Center of Yavapai Health Center Program Grantee/Federally Qualified Health Center Director: Sharon Rickman, MPA www.chcy.org The Community Health Center of Yavapai (CHCY) has been serving all of Yavapai County since 2001. Since its inception as the Prescott Free Clinic, Inc. in 1993, it has stayed true to its mission to contribute to the well-being of the people of Yavapai County by offering integrated primary health services including adult medical, pediatrics, reproductive health, behavioral health, dental and care coordination. With three full-time locations in Prescott, Prescott Valley, and Cottonwood, CHCY is the medical home for the uninsured, underserved and underinsured in Yavapai County.

Creek Valley Health Clinic Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Hunter Adams, MBA www.creekvalleyhc.com Creek Valley Health Clinic (CVHC) formed to provide comprehensive, quality healthcare to the rural valley surrounding Colorado City, Arizona. This nonprofit organization is solely composed of community members, and the clinic is opening to patients on December 16,2019. There has not been primary care in this region for over 10 years, with the nearest medical organization in Hurricane, UT 25 miles away. Creek Valley Health Clinic delivers general primary medical care, diagnostic laboratory, chronic disease management, well-child services, voluntary family planning, immunizations, preventative dental, gynecological care, prenatal and postpartum care, behavioral health, as well as enabling services, such as case management, eligibility assistance, health education, outreach, transportation, and translation. Horizon Health and Wellness Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Laura Larson-Huffaker, MC, LPC www.hhwaz.org Horizon Health and Wellness has developed an innovative practice model that embodies our philosophy of whole person care while promoting wellness. This approach to health care addresses the whole person by integrating behavioral health with primary health care while promoting wellness and prevention. In this model, our medical and behavioral health staff can work with patients in the same exam rooms to ensure integrated care. Both are involved in assessments, brief interventions and follow-up with patients. Where appropriate, patients may also be referred for wellness activities (e.g. smoking cessation, nutritional education, physical activities, etc.), as well as traditional psychotherapy, and psychiatric services. Horizon Health and Wellness is excited to implement this innovative practice model at a time when health care is undergoing transformation nationally. It is our hope that our integrated model will improve outcomes for people and reduce health care costs for society. Mariposa is the largest primary care provider of medical, dental and health education services in Santa Cruz County. As a patient centered health care home, MCHC’s purpose is to ensure access to culturally appropriate primary care and community-based health education and assistance, regardless of an individual’s ability to pay. North Country HealthCare Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Anne Newland, MD, MPH www.northcountryhealthcare.org North Country HealthCare’s roots reach back to the Flagstaff Community Free Clinic, which was founded in 1991 by a group of local health care providers and administrators. The founders were concerned about the large number of uninsured people who were presenting at the Flagstaff Medical Center Emergency Room because they had no medical home. In 1996, North Country Community Health Center became a federally qualified health center (FQHC) and moved into an old roller skating rink in Flagstaff’s Sunnyside neighborhood. Today, North Country HealthCare serves 12 communities across northern Arizona. Sun Life’s mission began in 1976, as West Pinal Family Health Center. Sun Life was an outgrowth of the Arizona Job College, which was developed to provide job training for migrant and seasonal farmworkers. From these early and humble beginnings, Sun Life has grown to become Pinal County’s largest primary health care provider. What makes Sun Life different is their unfailing concern for the well-being of their patients and willingness to provide the best possible experience for every person that walks through their doors. Sunset Health Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: David Rogers, MBA www.mysunsethealth.org Sunset Health provides a comprehensive array of primary care services such as medical, dental, behavioral health, pharmacy, obstetric and gynecologist services. The emphasis in the provision of primary care services is upon prevention and health maintenance of patients with chronic conditions. Sunset also provides health education and disease self- management education. The center employs approximately 285 health care professionals, of whom 35 are physicians, dentists, pharmacists, or mid-level practitioners serving 29,000 users (inclusive of 6,000 migrant workers).
