Cardiopulmonary Services are at the Heart of our Growth Strategies “Cardiopulmonary and cardiovascular services are key to growth for Advocate BroMenn Medical Center,” says Bob Miller, vice president of Operations. “We have tremendous opportunity to further develop this service line, particularly our outpatient services.” Industry trends are for hospitals and their outpatient and physician partners to work together in an integrated approach as one seamless service line. “As we continue to work towards better managing our patients’ health, it’s critical that we adopt this integrated approach so we can better care for patients all the way from their early prevention and diagnostic needs through intervention and even later with rehabilitation,” says Bob. BroMenn Medical Center has engaged MedAxiom, a leading cardiopulmonary strategy firm, to assist in developing our Cardiopulmonary Service Line approach. “We have formed a Board that will oversee and direct our efforts for both inpatient and outpatient cardiopulmonary services,” says Bob. Currently, Jim McCriskin, DO, a cardiologist with Advocate Medical Group, is chairing the Board. “Under this Board, we have five Clinical Councils, including: Coronary and Vascular, Electrophysiology and Arrhythmia, Imaging and Heart Failure, Pulmonary
Critical Care, and, perhaps most importantly, Quality.” Leadership of each of the five Clinical Councils includes representatives from BroMenn Medical Center and Advocate Medical Group. Over the course of the next year, each of these Clinical Councils will determine appropriate quality and patient satisfaction improvement metrics, develop plans to grow and expand their clinical programs, and define their financial and cost management goals. Because each Council includes representatives from both BroMenn Medical Center and Advocate Medical Group, they will be able to represent the interests of patients from both the medical center and the outpatient physician offices. “This truly is transformational change for our organization,” says Bob. “This is what will best position us for success in population health management and further distinguish us in the market.”

Living our Mission “About a year ago, one of our chaplain residents was walking through the waiting room in the emergency department and noticed that a patient who had been discharged was still wearing the paper scrubs she had been given in the emergency department,” says Mollie Ward, manager of Spiritual Care and CPE at Advocate BroMenn Medical Center. “The patient did not have appropriate clothing to wear home from the hospital, and did not have anyone nearby who could bring a change of clothes.” Upon further investigation, the Mission and Spiritual Care team learned that this happens about five to seven times per week. “We felt this was an important issue to tackle,” says Mollie. “Every patient who comes to us is a human being worthy of being treated with dignity. It was important to us that patients have access to clean, warm, appropriate clothing when they leave our care.”
The team set out to create the Compassion Closet, which is located in the lower level of the hospital. They reached out to BroMenn Medical Center’s delegate churches, which now take turns restocking the closet one month at a time. Through their members’ donations, each church provides new t-shirts, socks, sweat pants and sweatshirts in a variety of adult sizes. “Associates can also support the Compassion Closet through donations to the Caring Hearts Fund during our annual associate giving campaign,” says Mollie. “Most importantly, if associates see a need, we encourage them to use the closet. Anyone in the hospital—patients or visitors—who have soiled or torn clothing or who are underdressed for the weather can be provided with clean and warm clothes. Simply dial “0” and have the on-call chaplain paged.”

Strategic Initiative 1 – Growth
Assessing Our Community’s Needs Advocate BroMenn Medical Center recently completed a community health needs assessment in McLean County. A team of community and hospital leaders worked together to determine what health conditions were most prevalent in the community, and which of them could be most positively impacted by the hospital. After reviewing primary and secondary research, the group recommended its improvement efforts focus on mental health services and obesity. According to their findings, some of the most common diagnoses for patients in BroMenn Medical Center’s emergency department were psychiatric in nature. In addition, suicide rates in McLean County have increased from 8.0/100,000 in 2006 to 10.6/100,000 in 2010. This is higher than the state rate of 9.3/100,000 (2008). And the number of phone calls to the PATH (Providing Access to Help) 2-1-1 Crisis Hotline number have increased by 22.9% between 2010 and 2012.
Plans to Help BroMenn Medical Center has longstanding interest and expertise in the area of mental health. In addition to our inpatient mental health unit, we have assisted with development of the Recovery Court Program for McLean County and enjoy a successful partnership with Proctor Hospital to offer addiction recovery services. The McLean County Wellness Coalition works to promote healthy eating and active living, both keys elements in the fight against obesity. Hospital leaders and associates have been engaged in this coalition since its inception in 2011. BroMenn Medical Center will continue to partner closely with the coalition, the McLean County Health Department and other area organizations such as the American Heart Association and the YMCA on programs to decrease the prevalence of obesity.
Strategic Initiative 2 – Regional Destination Benefits of Our Magnet Journey As shared in the last issue of Strategic Directions, Advocate BroMenn Medical Center is in the very early stages of our Magnet Journey. “This journey will help us further develop and sustain our culture of excellence,” says Laurie Round, vice president of Patient Services and chief nursing executive. “It requires the support and participation of every area—not just nursing—in the hospital.” Research indicates that achieving Magnet status helps organizations: • Attract and retain top talent • Improve patient care, safety and satisfaction • Foster a culture of collaboration • Improved financial success
An Orthopedic Destination By the end of December, you will see quite a few changes to orthopedics on 6 West at Advocate BroMenn Medical Center. “As we continue working to make BroMenn Medical Center a Regional Destination, we realized that a few physical changes to the orthopedics unit would enhance our patient experience,” says John Hesse, vice president of Business Development. “Working in partnership with our physicians, we have experienced great outcomes and wonderful patient satisfaction with our orthopedic services. With a few cosmetic updates—new flooring and paint, new furniture and signage, we believe our patients and their families will enjoy a more comfortable stay as well.”
In addition to the aesthetic upgrades, the remodeled floor will also deliver additional services to our patients. “One of the larger lounges on the unit is being converted to a therapy room with new equipment to provide enhanced physical and occupational therapy.” says John. “This approach is more patient-centric by providing a more comprehensive therapy experience delivered directly to the patient.” “This has been a great collaboration between many disciplines along with our nursing team,” says Stephanie Moore, Director of Nursing. “Between the remodeling changes and new therapy space, as well as some IT upgrades in each of the patient rooms, we are really excited about the enhanced service this will provide to the patients we serve.”
Strategic Initiative 3 – Operational Excellence PerfectServe PerfectServe is a new physician communications system that will go live at Advocate BroMenn Medical Center and Advocate Eureka Hospital on Wednesday, December 4. It allows staff to quickly and easily contact members of the medical staff, and for physicians to control how they receive communication from the hospital. Through PerfectServe, physicians can designate a preferred method of contact (pager, mobile phone, text message, etc.) depending on the time of day, the caller, and/or the reason for the call (consult, new admissions, etc.). Additionally, the PerfectServe Clinician mobile app allows physicians to remotely make call schedule changes, as well as change where their calls should route and how their messages are received.
PerfectServe also offers: • A secure, HIPAA compliant platform • The ability for the physician to hide his or her phone number when calling patients – only the physician’s main office number will display on the patient’s caller ID “I think this is going to be a great physician satisfier,” says James Nevin, MD, vice president of Medical Management at BroMenn Medical Center. “The doctors will receive fewer unnecessary interruptions and fewer lost or delayed messages. Most importantly, it allows our hospitals to communicate better with physicians and improve quality and safety for our patients.”
CPOE Increases Safety, Quality and Efficiency Beginning next March, computerized physician order entry (CPOE) at Advocate BroMenn Medical Center and Advocate Eureka Hospital will allow doctors to enter their orders and patient care instructions directly into the computer. The orders will then be sent electronically to the appropriate medical staff member or department responsible for fulfilling the order, such as pharmacy, radiology or laboratory. CPOE offers a number of benefits, including: • Reduced medication errors and adverse drug events • Decreased delays in order completion • Reduced errors related to handwriting or transcriptions • Order entry at either the point-of-care or remotely at an offsite location • Error checking for duplicate or incorrect doses or tests
CPOE has been proven to help reduce medical errors and improve quality and efficiency. Recent research shows that use of CPOE in all urban hospitals in the US could prevent as many as 3 million serious medication errors each year. Studies have also shown that CPOE reduces length of stay; reduces repeat tests; reduces turnaround times for laboratory, pharmacy and radiology requests; and delivers cost savings to the hospital.
Advocate Eureka Updates
Quality Excellence Achievement Award Recently, the Illinois Hospital Association (IHA) Institute for Innovations in Care and Quality recognized Advocate Eureka Hospital with a Quality Excellence Achievement Award for innovative quality interventions and best practices designed to transform health care.
“This award is a wonderful example of the amazing care every one of you provide each day,” says Anna Laible, administrator of Eureka Hospital. “It took everyone working together to prevent falls for us to earn this Quality Achievement Award from the Illinois Hospital Association. Thank you for all you do to help keep our patients safe!”
Eureka Hospital was selected as the Rural/Critical Access Hospital winner for their project “’No One Falls’ is Achievable: Patient Fall Reduction in a Critical Access Hospital.” This initiative reduced the incidence of inpatient falls by implementing several fall prevention initiatives with a focus on a sustainable change in culture and improved patient safety. The hospital’s inpatient unit has not had a patient fall in more than 15 months.
Meeting the needs of Woodford County Advocate Eureka Hospital recently completed a health needs assessment to determine what health conditions were most prevalent in the community and which of those needs its team could most impact. The team of community leaders and Eureka Hospital associates determined that the highest need was for mental health services. Mental health diagnoses are among the top 10 (in volume) diagnoses that are seen in the Eureka Hospital emergency department and psychosis is the third most frequent inpatient diagnosis of a hospitalized Woodford County resident. Additionally, the number of mental health services provided to Woodford County residents has decreased substantially since the closure of the Tazwood Mental Health Center satellite facility in Eureka.
Based on these findings, Eureka Hospital is collaborating with the Woodford County Health Department and the Tazwood Mental Health Center to expand access to mental health care to residents of Woodford County. This includes availability of a psychiatrist and mental health counselors on a regular basis at the health department and continuing behavioral health education to staff at Eureka Hospital and local Advocate Medical Group physicians. Other issues identified by the assessment include obesity and oral health, and substance abuse by adults.

Rodney Davis Visit
Representative Keith Sommer, Representative Bill Mitchell, Senator Jason Barickman, Senator Bill Brady and Representative Dan Brady joined Advocate BroMenn Medical Center associates on October 10 for our third annual Legislative Forum.
United States Congressman Rodney Davis visited the Advocate BroMenn Medical Center campus on November 6.
