As its title suggests, one is ikigai, or discovering your life calling or primary source of intrinsic motivation. Raymond suggests thinking of various endeavors that pique interest, then assigning a numeric ranking to each of the following questions: 1. 2. 3. 4.

By scoring possibilities, your ikigai may come to light. “You must remain vigilant in pursuit of your ikigai, persistently scanning the horizon, ever ready to sample new curiosities, new hobbies and career prospects as they avail themselves to you,” Raymond writes. “When there is harmonious alignment with our innate passions and our chosen skill set, that’s where the magic happens.” In reading this, I thought about Pursuits, a magazine series we launched last year featuring Hawai‘i people who are pursuing their passions, whether it’s through work, interests or other endeavors. Since its February 2024 debut, we’ve covered artists, athletes and others, including a former economist who now leads Buddhist talks and group meditation at a Pālolo temple; a musician performing as part of the new Cirque du Soleil ‘Auana show (this month’s feature); and a Stage 4 cancer patient who completed the Kona Ironman in hopes of inspiring others with terminal conditions to keep fighting. I love these stories, and after thinking about it, I realize they’re about people pursuing their ikigai. But the concept extends to articles beyond the series. For this month’s Top Doctors issue, we profile two doctors, Dr. Christina Marzo and Dr. Jerald Garcia, who in interviews relayed their passion for their specific areas of medicine. Marzo’s interest in family medicine was sparked in childhood, after seeing how her grandfather struggled to advocate for her grandmother, who had diabetes. She wants to be not only the medical advocate for her own family, but for her patients, especially those who are powerless. Meanwhile, Garcia reveals how as a 5-year-old he was fascinated with his doctor play set. He says being a doctor feels like his destiny, and he wound up in a specialty that satisfies two big interests—working with his

hands and helping relieve people’s chronic pain, sometimes immediately. Both of them tapped into their ikigai and ended up with successful careers that serve the community. In this early part of the year, as many of us embark on our resolutions, I’ve been thinking about the importance of having purpose. More than ever, in this crazy world we’re living in, we need meaning in our lives, and perhaps one way is to make contributions that are intrinsic to our nature. I hope you’re aligned with your calling or are at least considering how you can be. For me, life is more consequential when I’m invested in whatever I’m doing. And to be honest, being able to share stories in this magazine may very well be my own ikigai.

All Hail the Queen As the drink trends nationally, Kava Queen serves traditional kava in Waialua.

HILE K AVA (OR ‘AWA, AS IT’S KNOWN IN HAWAI‘I) has been around

for thousands of years, it’s having a moment with the rising popularity of hip sober bars across the nation. One of O‘ahu’s latest purveyors of the drink, Kava Queen, serves traditional and nontraditional versions at the Waialua Sugar Mill silos. The North Shore business started selling powdered and prepared kava at Farm Lovers farmers markets in 2021. This past October, owner Ava Taesali opened her bar in the building that also houses Mele Mele Bakery. You’ll find Taesali pouring 8-, 16- and 32-ounce servings of traditional kava, along with flavored varieties when available, daily from 4 to 8 p.m. (9 p.m. on weekends).

Experienced kava connoisseurs can indulge in concentrated shots, while newbies are led to more straightforward preparations, both of which are believed to have medicinal qualities that help relieve anxiety and stress. Taesali loves to explain the history of kava, made from the roots of a pepper plant native to Pacific islands. We try a version from Vanuatu before watching her make ‘awa from Pu‘u O Hoku Ranch on Moloka‘i, which comes in the form of frozen pulp and is never dried. Distinctly less earthy, its taste and smell are reminiscent of fresh-cut grass. And while it yields less than the dried powder, it’s worth trying for comparison. As the late afternoon sun pours through the back door and illuminates us in a soft glow (or was that the kava kicking in?), everything feels right. Wāhine Wayfinders As Hōkūle‘a celebrates 50 years of sailing, female navigators carry a message of cultural resilience and hope to a worldwide community. BY ROBBIE DINGEMAN

Kamalu relies on the sea and sky rather than GPS and satellites. “It’s really hard for anything else actually to penetrate into your focus because you just are constantly looking at the ocean, looking at the waves, looking at the sky, taking every possible clue and cue that’s out there,” she says. “They’re all trying to tell you what’s going on, where you are, and you’re really trying to make sense of it all.” Kamalu, a captain with the Polynesian Voyaging Society, grew up speaking Hawaiian, immersed in cultural learning at Ke Kula Kaiapuni ‘o Pu‘ohala in Kāne‘ohe. Now 38, her first encounter with the canoe was during a field trip as a student there before she transferred to Kamehameha Schools. After receiving her mechanical engineering degree, she landed full time with PVS in 2013 and underwent extensive training with veteran voyagers. For Hōkūle‘a’s worldwide voyage, she coordinated logistics. She believes the widely publicized voyage showcased Native Hawaiian culture and linked Hawai‘i more deeply to the rest of the world. “I think we’ve come to a place of really embracing that much larger global community, allowing Hōkūle‘a to be this connecting point between Hawai‘i, its place and its people and

everything that it represents to the wider world,” says Kamalu, who was interviewed in Tahiti, where she’s planning future journeys. Master navigator and PVS CEO Nainoa Thompson estimates that about 650 crew members have served on Hōkūle‘a over the past 50 years, including about 60 navigators. He sailed with the first two female crew members, Keani Reiner and Penny Rawlins, from Tahiti in 1976. He now estimates that the number of female crew members has grown to roughly 1 in 3. Thompson says PVS has always functioned as a values-based organization with women playing critical roles. “Women are very focused; they’re disciplined,” he says. “And they come to the training with an open mind, open heart.” Crew member Kayla Kalepa, 19, is among the youngest aboard. She grew up around the canoe because of her father, renowned Maui lifeguard captain and waterman Archie Kalepa. “When I was little, I didn’t understand why my dad was sailing around the world,” she says. “When I turned 14, that’s when I started sailing, because my dad built his own canoe, a 30-foot replica of Hōkūle‘a, and after that, I was hooked.” Meanwhile, having sailed on Hōkūle‘a to Tahiti in 2022, Kai Hoshijo, 27, appreciates the life skills of training at sea. “We’re going to be resourceful and flexible and resilient based on what our voyage might be, whether it be a task or a job, and there’s huge value in that,” she says.

HE BATTLE AGAINST single-use plastic water bottles has gained significant momentum with technology fostering the use of recycled aluminum water bottles at several Hawai‘i hotels and Kamehameha Schools Kapālama. What’s pushed the aluminum bottles into the spotlight? Call it big-time star power. Hawai‘i-born actor Jason Momoa, who plays Aquaman, owns Mananalu Water and has partnered with Boomerang, a North Carolinabased reuse company, to roll out a high-tech machine that can be stationed at sites to sanitize and fill aluminum bottles with his water. Standing racks for guests to place empty bottles have been strategically placed at several hotels, including The Twin Fin, Romer House Waikīkī, ‘Alohilani Resort Waikīkī Beach, Four Seasons Resort Hualālai, Four Seasons Resort Lāna‘i and Four Seasons Resort Ko Olina. Momoa and his business partners expect more hotels, visitor destinations and schools to come aboard soon. Eventually, they want to usher in a new norm, where people return water bottles for reuse instead of disposing of them. “I originally launched Mananalu to challenge the drinking water industry to shift away from single-use plastic bottles that are destroying the health of people and the planet,” Momoa said at a January press event at The Twin Fin. “This technology is a game-changer, and I’m honored to be able to bring it to Hawai‘i first … to help restore the local ecosystem.” If guests in each of the 50,000 or so hotel rooms in Hawai‘i drank from just two water bottles a day, that’s 100,000 bottles a day—a staggering waste, according to Boomerang’s Jason Dibble and Jerrod Freund. And not only do plastic bottles fill up landfills, transporting them is costly to the environment, too. As people become more mindful of their carbon footprints, “You’ll find that people want to do the right thing and return” the bottles, Dibble says. He adds that about 95% of bottles are returned at sites where the machines are stationed around the country.

Tucked into the Fishcake store, the popular community art school offers a breadth of classes, from one-time workshops and weekend series to weekly classes over a few months. The ceramics classes, in particular, are well worth the price as they include all materials and kiln firing. I was fortunate to attend one of 104-year-old artist Esther Nowell’s last ceramic workshops before she passed away in January. The class ($60) focused on maneki-neko or lucky cats, the Japanese figurines often placed in businesses to bring good fortune. Nowell guided us through shaping the cat with clay, from forming the body with pinch pots to attaching arms and a fish or coin for it to hold. In the second session, we glazed our cats, letting our creativity flow through colors and brushstrokes. “Do you know the Dr. Seuss quote?” asked Nowell as she admired our unique creations. “‘Today, you are you, that is truer than true. There is no one alive who is youer than you.’ That’s what we show with our art.”

NLIKE URBAN HONOLULU, new developments are few

and far between in Central O‘ahu. So, when a fresh build hits the skyline, we take notice. Seated on the 3.45-acre site of the former ‘Aiea Sugar Mill, Aloha Iā Halewilikō is a $62 million project from nonprofit developer EAH Housing, planned for completion in October. An affordable rental community for seniors age 62 and older, the three-story building includes 83 studios and 56 one-bedroom apartments; some of those units will be designated for households at or below 30%, 50% and 60% of the area median income. Along with elevator service, a multipurpose room, tech lounge and fitness room, Aloha Iā Halewilikō will also feature outdoor gathering spaces, gardens and a dog park. An on-site resource coordinator will facilitate gatherings, educational courses, wellness activities and health care services. EAH is also partnering with Lanakila Pacific to provide meal distribution. Taking shape on a 3.11-acre site in lower Hālawa, across the highway from the USS Arizona Memorial, the $168.1 million Hālawa View II development is slated for completion this summer. Born from a partnership between the state of Hawai‘i, the City and County of Honolulu and California-based Pacific Development Group, the 18-story, 302-unit building will offer one-, two- and four-bedroom rental units. The new build is phase two of the Hālawa View Apartments project. In phase one, a 1970s Hālawa high-rise was remodeled in 2014; it’s easily recognizable for its 14-story mural by local artist Kamea Hadar. Along with barbecue and picnic areas, amenities will include a fitness center, sky lounge, study lounge and kids club. Even our newly constructed affordable housing is unaffordable. We need more rental subsidies to keep people housed. My vacant home surcharge bill will raise the needed funds by collecting the general excise tax revenue that would have been collected had that home been rented and occupied by residents. … I’ve also introduced a bill to help kūpuna with home equity stay housed. It’s inspired by an existing federal HUD program. Many retirees cannot keep up with exorbitant cost increases in maintenance fees, assessments and property insurance. My bill would authorize the [Hawai‘i Housing Finance and Development Corp.] to step in to cover those unexpected costs and keep kūpuna housed, ideally in their own home.

CONDO OWNERS HAVE BEEN PASSING DOWN TO RENTERS ASTRONOMICAL COSTS RELATED TO INSURANCE PREMIUM INCREASES. HOW DO YOU PLAN TO KEEP SUCH A CONDOHEAVY DISTRICT AFFORDABLE FOR LOCALS? My previous landlord had to raise my rent for that reason, so I moved into a more affordable building with less amenities. One of my bills directs the insurance commissioner to create standards for three new insurance models: self-insurance, mutual insurance, and what I’m terming building life insurance, or a life insurance policy for a building.

When premiums and deductibles skyrocket, many businesses consider self-insuring, and when they do decide to self-insure, they are now incentivized to invest in practices and policies that minimize risk. Mutual insurance is really a hui of selfinsured buildings offering each other backup. So if you’re in this hui you’re going to pick hui partners that are as financially healthy as your own building. And then finally, there’s building life insurance, which would be ideal for new construction and would be a long-term insurance contract, kind of like life insurance, that would lock in premiums for the life of the building, as long as the preset maintenance schedule and safeguards are followed.

WHAT DO YOU HOPE TO ACCOMPLISH BY THE END OF THE SESSION? I introduced six bills that will increase [government] transparency, accountability and ethics. One is legislative term limits, so not more than 16 years. No. 2 is remove the Legislature’s exemption from the Sunshine Law. No. 3 is to restore the legislators’ compliance with ethics laws that apply to all other state employees. No. 4, candidate campaign funds should not be allowed to carry over indefinitely, resulting in million-dollar war chests. No. 5 prohibits campaign donations from state and county contractors, grantees and lessees, their owners, officers, consultants and subcontractors and anyone sharing a joint account with any one of them. And then finally, increased standards and transparency when leasing public lands. And that mostly applies to the governor, so he’s not allowed to just sign over leases willy-nilly to a really bad tenant from the past. … Even if none of these bills pass, we learn so much about legislative leaders when they kill these kinds of bills.

In the case of the Pedicini Residence, a grand 1979 home on the Kohala Coast, recently reimagined by architect Paige Wilburn and interior designer Marion Philpotts-Miller, those bones included a rare find—an enchanting Japanese Zen garden that the team restored to its former glory. Originally commissioned by a German baron for his mistress, a member of Thai royalty, the home is constructed as a Balinese pod house, featuring pavilions that wrap around and offer views of the garden space. Quiet in the House

I

F YOU’VE SEEN ‘AUANA, Cirque du Soleil’s new Waikīkī production, you know it’s superlative. World-class acrobatics, circus antics, wondrous artistry: For 80 minutes the show dazzles. But in the quiet before it starts, as the audience fills the small theater, a lone guitarist plucks the notes of “He Mele Lāhui Hawai‘i”—Queen Lili‘uokalani’s anthem for her kingdom—then picks up a Hawaiian nose flute. “People may not be aware of what they’re listening to, but it’s a way of acknowledging and honoring where we are,” says musician John Signor, who’s also the show’s musical director. “It allows me an intimate moment with the audience to welcome them into our space with loving intention.” Born and raised in ‘Ewa Beach, Signor at 65 is the senior among the local cast, which includes hula dancers, singers and other musicians. It’s not his first time in the role—he was also musical director for the visitor show Waikīkī Nei and before that, ‘Ulalena in Lahaina, Maui. A music professor on leave from Leeward Community College, he leads ‘Auana’s musical energy. It’s an all-in role because the music, by turns energizing and lyrical, is an enveloping presence that helps drive the dazzling totality. In the lineup with the band, Signor twists and bobs on his acoustic guitar. You’d never guess his favorite instrument is the nose flute. “It’s as if I’m in the backyard and I’m just playing to relax,” says Signor about the moments before the show. It’s “really very worrisome,” Dr. Jennifer Carney, Hawai‘i Permanente Medical Group’s chief of hematology and oncology, says of the rise in early onset cancers. “Some of these patients aren’t even in the screening age groups yet.” In fact, screening recommendations have changed to address the situation. Colonoscopies are now recommended for people starting at age 45, rather than 50. A woman with an average breast cancer risk should get a mammogram every other year starting at 40, also earlier than previous guidelines. Patients in their 30s and 40s who are grappling with serious cancers often have different concerns than those in their 60s and 70s, Carney says. Younger patients, she says, might have school-age children or be concerned about fertility, and they’re probably still working, whereas older patients are more likely to be retired.

In rankings of the healthiest states, Hawai‘i often comes out on top, or close to it. Year after year, surveys and reports suggest that our state’s residents are more active than their counterparts on the continent, breathe cleaner air, are happier and live longer. But that’s far from the full story. Tucked behind those rosy statistics, doctors say, are rising rates of cancer among patients under 50, genetic links to cancer that affect many Island residents, worrisome increases in obesity and such chronic diseases as diabetes, and persistent gaps in health care for communities that need the most help.

Younger Cancer Patients The state’s medical community is scrambling to understand what’s driving the rise in cancer rates among younger, preretirement-age people. From 2000 to 2021, breast cancer among Asian and Pacific Islander women under 50 soared, from 36 to 55 cases per 100,000 patients, according to the National Cancer Institute. Oncologists also are seeing more people under 50 with colon, pancreatic and lung cancers, and it’s not clear why.

Along with younger oncology patients, doctors say genetic factors are increasingly being linked to malignant growths. For example, liver and gastric cancers tend to be more prevalent among Asians, while Native Hawaiians are at higher risk for several cancers and are more likely to present with more progressed disease. “Part of this is the social determinants of health,” Carney says. “There is also something biological.” The leading cause of cancer deaths nationwide is lung cancer, and in the Islands, Dr. Ayman Abdul-Ghani says he and other doctors see more of it among nonsmokers than the national average, typically among Asian women. Lung cancer rates are also higher among Native Hawaiian women. “Often, the cancer is found when patients go in for something else,” says Abdul-Ghani, a thoracic surgeon at Hawai‘i Pacific Health. “I’ve seen patients who go for shoulder X-rays or computerized tomography scans for their neck or abdomen, and the doctors find lung cancer in the part of the chest covered in the scan.” Native Hawaiians also have a 60% higher chance of developing pancreatic cancer than others, says Queen’s Health Systems oncologist Dr. Jared Acoba. “If you have a Native Hawaiian male who has two risk factors, say who smokes and has one family member with pancreatic cancer, his risk of pancreatic cancer is five times the population.”

Trying to Outsmart Cancer Fortunately, the medical community’s arsenal to fight cancers is growing. In addition to traditional treatments, doctors are using targeted therapy to go after the DNA mutations fueling malignant growths and deploying immunotherapy drugs to stimulate a patient’s own immune system. They’re also using genetic counseling to target higher risk patients. At Kaiser Permanente, for

example, some cancer survivors and patients with family histories of certain variations of the disease qualify for genetic testing to assess their risk. While genetics can’t always accurately predict the future, genetic counseling can encourage patients to seek regular screenings to catch cancers earlier, says Rebecca Yee Bassett, Kaiser Permanente Genetic Counseling section chief. “Family history can be really critical for people to be aware of,” she says, noting that genetic counseling likely will be more readily available in the near-term. “This is precision medicine, where we use our genetic information to help guide doctors and treatments. It’s the ultimate in individualized health care.” Nearly six years ago, Olena Heu, a former local TV news anchor who now runs her own marketing firm, underwent a preventive double mastectomy after genetic testing predicted an increased breast cancer risk. Heu’s grandmother had breast cancer, and her mother and aunt both died of the disease. In 2019, a year after her mother’s passing, the 44-year-old decided to remove both breasts to reduce her risks, and opted out of reconstructive surgery because implants pose risks of their own and would have required additional recuperation. “I think it might have been one of the best medical decisions in my life,” says Heu, who

documented her journey via YouTube videos that reached people around the globe. “It really empowers me to go forward and do all the things I want to do and not have to be worried. Because of science and preventative care, we can make these decisions that will essentially help prolong our lives. We live in a time where we’re given those opportunities.”

Addressing Inequalities in Care is Key While advances in screening and cutting-edge treatments are giving doctors new tools in fighting disease and giving patients better long-term prognoses, socioeconomic inequalities continue to drive health disparities among Native Hawaiian and Pacific Islander populations. In many rural and remote areas of the state, for instance, there’s a lack of health care providers, and it’s these residents often hit hardest by Hawai‘i’s rising cost of living. “Multiple family members are working two or three jobs to afford the high cost of making ends meet. It’s really hard to exercise, be healthy if you’re always busy and stressed,” says Dr. Jennifer Loh, chief of endocrinology at Hawai‘i Permanente Medical Group. “It’s a lot more expensive to eat healthy as well.” To address inequalities in cancer care, including preventive screenings, the John A. Burns School of Medicine recently partnered with the University of Hawai‘i Cancer Center and Hawai‘i Cancer Consortium members, who represent several top medical groups. The effort, called Ka ‘Umeke Lama, or “Bowl of Enlightenment,” aims to “erase disparities in cancer outcomes” in the Islands by advancing cancer education, deploying oncology providers and early cancer detection units to rural and underserved communiMARCH 2025

ties, launching targeted clinical research trials and developing telehealth systems. The group has its work cut out. Across the state, Native Hawaiians have historically had the highest mortality rates for all types of cancers compared to white residents. Pacific Islanders are also far more likely to be diagnosed with some cancers. Ka ‘Umeke Lama is aimed at developing a “strong and integrated oncology infrastructure” in Hawai‘i and the Pacific, says Dr. Naoto Ueno, director of the UH Cancer Center. He believes cancer care should be equitable and rooted in both innovation and cultural understanding. “Everyone should have access to high-quality cancer care,” he says.

Type 2 Diabetes on the Rise in Hawai‘i When it comes to health disparities, though, cancer isn’t the only concern. Over the past decade, the prevalence of Type 2 diabetes in Hawai‘i has increased by more than 50%. Today, more than 11% of the population—or about 150,000 people—have Type 2 diabetes and 40% are living with pre-diabetes. The most common causes of Type 2 diabetes in adults are a poor diet, a sedentary lifestyle and high stress. When diabetes is untreated or poorly treated, it can lead to a host of other chronic conditions, including heart and kidney disease, nerve damage, mobility problems, and even depression and dementia. Like with other chronic conditions, Native Hawaiians and Pacific Islanders are at higher risk of developing diabetes—2.5 times higher compared to the overall population—the Office of Minority Health reports. They’re also at greater risk of dying from heart disease, the state’s top killer. New research from the American Heart Association shows Native Hawaiians and Pacific Islanders have far higher death rates from heart disease than Asian Americans, and the second highest among all ethnicities, races and other groups behind only Blacks.

Obesity in Local Children a “Significant” Problem Alarmingly, Hawai‘i doctors are also seeing rising rates of obesity among children—and more cases of Type 2 diabetes, too. The Na-

tional Institutes of Health has called childhood obesity in Hawai‘i a “significant” problem, with some communities seeing rates of 40% or more. Overall, a third of all Hawai‘i keiki are overweight or obese. “Before, we would rarely see Type 2 diabetes in children,” Loh says. Now, doctors nationally are finding about 18 cases per 100,000 children, the latest federal data shows, with the highest rates among Blacks, Latinos and Pacific Islanders. Dr. Nadine Tenn Salle, chief of pediatrics for The Queen’s Health Systems, says chronic conditions in childhood must be dealt with holistically. Kids aren’t put on diets, she says, but are encouraged to try healthier foods and activities. Cultural competency is key, too. Dr. Leimomi Kanagusuku, president of the Hawai‘i Academy of Family Physicians, grew up in Wai‘anae and is part Native Hawaiian. She thinks of health as an “intricate web” of factors and tells her patients so. Genes aren’t the only thing that determine health outcomes— daily habits and environment count too, she says. As a family medicine physician with Hawai‘i Pacific Health, Kanagusuku says her patients want to be healthier but need resources and guidance. “Instead of telling patients to eat healthy, you ask them about their day and who cooks.” By tapping into specifics, she offers actionable advice. “So much of health has to do with our surroundings. Native Hawaiians have lower socioeconomic status, more asthma, more cancer, more alcohol use, higher rates of mental health issues.” Untangling it all to help patients is both daunting and rewarding. She recalls one of her younger patients with diabetes and extremely high blood sugar. He didn’t want to go on medication, so Kanagusuku had “some very real conversations” with him about turning his life around. He did, even getting his blood sugar levels below the goal.

What About So-Called Wonder Drugs? For Loh, the endocrinologist, “game-changer” drugs like Ozempic are powerful but not the only answer. “It has certainly been impressive” to see the success of Ozempic and similar drugs, she says. “But we don’t know the longterm effects.” The class of drugs that includes Ozempic works by mimicking a hormone to make you feel full faster and likely requires long-term usage. “It’s not a one-sizefits-all,” she says. “But it can play a part in your weightloss journey.” Another part might be monitoring your blood sugar if you have prediabetes. Over-the-counter blood glucose monitors can quickly show patients how food choices, like sweets or rice, impact their blood sugar and how they feel throughout the day. “Some people find it very enlightening,” she says. “That can be empowering.”

No Pain, A Lot to Gain

Dr. Jerald Garcia appeared destined for a career in medicine as a 5-year-old, when his favorite activity was playing with his doctor toys. Fast-forward a few decades, and Garcia’s childhood pastime has resulted in a successful career leading a rapidly growing chronic pain management operation. It’s been 15 years since he first started his practice, a small O‘ahu office where he was the doctor and his wife was his assistant. Today, he operates the Hawai‘i Institute for Pain at three locations—two on O‘ahu and one on Maui—with a team of doctors and medical staff. Garcia’s core work is interventional chronic pain management, which includes high-tech, minimally invasive procedures, including implanting spinal cord stimulators. This year, Garcia is expanding his Hale Pāwa‘a practice to become a “one-stop,” comprehensive center, with cutting-edge surgical, rehab and wellness divisions, along with a new regenerative unit that offers stem cell treatments. As more patients become aware of new treatments, the whole area of medicine will continue to grow and advance—and Garcia wants to be on the cusp of that in Hawai‘i. “This comprehensive approach is the gold standard of treating patients suffering from chronic pain,” he says. “Right now in Hawai‘i, the treatment in this area is very 26

disjointed. You see this doctor to get your injection, then go to another place for therapy, and another place for rehab or behavioral therapy. My goal is to consolidate all those services.” Although trained in anesthesiology, Garcia decided early in his career to devote himself to his sub-specialty of chronic pain management, fueled by the often immediate, dramatic results he was seeing. “You’re using your hands a lot, doing injections, microsurgeries, and you almost immediately see the effect of your interventions—and I love that,” he says. He says patients sometimes come to him in wheelchairs because they’re in such severe distress they can’t walk. After being treated, he says, they walk out the door themselves. He adds that he once implanted a spinal cord stimulator in a debilitated Maui paddleboarder, who was overjoyed to be able to return to the water. “Things like that make it a very gratifying and satisfying specialty,” Garcia says. —Diane Seo

As a young girl,​​ Dr. Christina Marzo’s diabetic grandmother suffered a series of strokes that eventually left her wheelchair-bound and unable to communicate. No one in her family knew much about health care, and it frustrated her to see her grandfather, who immigrated from the Philippines, unable to navigate a complicated medical maze. Now as a family physician at The Queen’s Medical Center, the 39-year-old helps others struggling to advocate for sick loved ones. “I see this exact scenario with many of my patients, and my philosophy with medicine is to be that person for my patients,” she says. “People don’t always understand their disease and how to make the right decisions.” Hawai‘i’s reputation as one of the healthiest states is deceiving, Marzo says. So many people are battling diseases and issues— battles that can be exacerbated by Hawai‘i’s high cost of living. “They need housing and groceries, and there are lots of strains on our health care system,” she says. “I don’t necessarily have the means to address all of these things, but I’m trying to do what I can.” Family medicine is the ideal practice for the Punahou grad, because she says, more than with other medical specialties, she can get to know her patients and their

She often treats multiple generations of families and knows how family dynamics impact their health. “It’s a field of medicine where you’re part of your community and need to understand community issues to provide the best care,” she says. ​​​She also loves having medical students shadow her. She learned so much from her own mentors, recalling how they went out of their way to give her valuable learning experiences. Now, she does the same, motivated to help shape the next generation of Hawai‘i’s family physicians. While family medicine sometimes is cast as less academically rigorous or prestigious than other medical fields, Marzo tells students they don’t need to be specialized surgeons to make positive impacts on people’s lives. “In fact, you can probably have a bigger impact on people’s lives if you are a super solid primary care doctor,” she says. —DS

JONATHAN PELLETT, M.D., is board-certified by the American Board of Orthopedic Surgery and a member of the Pediatric Orthopedic Society of North America. He treats almost every pediatric orthopedic condition and uses a team approach to bring the best subspecialty care at Shriners Children’s Hawai‘i. In addition to providing care for Hawai‘i’s keiki, he is also part of the Shriners Children’s outreach team, providing clinical care in Guam, American Samoa, Fiji and the Marshall Islands. Dr. Pellett earned his medical degree at Washington University School of Medicine and completed his orthopedic surgery residency at the Medical College of Georgia. He completed his fellowship in pediatric orthopedic surgery at Shriners Children’s Hawai‘i and, in 2006, joined the Shriners Children’s team of pediatric surgeons, where he continues to practice today. Dr. Pellett is a recipient of JABSOM’s Allen B. Richardson Award for Teaching Excellence and was named Physician of the Year by the Healthcare Association of Hawai‘i. In 2020, he became president of the Hawai‘i Orthopedic Association, and, in 2021, helped organize and moderate the Hawai‘i Orthopedic Association’s Spring Research Symposium. As part of his desire to help improve the lives of keiki, he also facilitates the Shriners Education and Research (SEARCH) conference, an annual event that brings community physicians and providers together to find comprehensive solutions for pediatric orthopedic conditions.

EARLY IN HIS MEDICAL CAREER, DR. WONG was working in an emergency room when a patient went into cardiac arrest. Dr. Wong graduated from the University of Otago Dunedin Medical School in New Zealand, where he is originally from. He completed his internal medicine and cardiology training at the University of Hawai‘i and the University of Nevada, respectively, with board certifications in Internal Medicine and Cardiovascular Disease. He is also certified by The American Board of Venous and Lymphatic Medicine and has been caring for patients in Hawai‘i since 1994. A LOCAL “HAPA” BOY raised in Kea‘au on the Big Island, Dr. Paul Norio Morton decided to become a doctor when he was a fire and rescue volunteer in college in Ohio. He returned home to attend the John A. Burns School of Medicine (JABSOM) and serve his community. His love of sports, helping people, and technology have come together in his Orthopedic practice in Honolulu, West O‘ahu and Kona. O WE HOLD OUR TONGUES TOO MUCH IN HAWAI‘I?

Does our local culture and its no-make-fuss credo mean we’ll eat an overcooked steak or wait patiently at a hostess stand when a restaurant has open tables? Are we just too shy to ask? Yes, we are. So we put our questions directly to four local restaurateurs. Their responses go a long way toward clarifying the etiquette of restaurant culture.

1 When is it OK to

send food back?

ALEX LE: There needs to be some

kind of restaurant etiquette. People not used to our food will tend to return things because of spice level or it’s just not for them, vague reasonings. And a lot of Hawai‘i people are very polite. But for me, it’s easier if you tell me what is wrong with a dish. I don’t want to be showered with compliments when you don’t like it. I think it’s good when you tell me the risotto was good, but it was really salty. And if you don’t like a dish, say something early. If you eat more than half, we can’t comp that. Also, don’t

over-order and then try to cancel a dish because you’re full while we’re in the middle of making it. I also feel like if a guest wants to modify a dish, like if they ask us to make it without the onions, then that becomes their dish. If you’re going to change a dish from how we meant to serve it, don’t send it back. But, for example, if you get the lamb la lot because we suggested it and described it as barbecued with a low spice level, and you get it and don’t like it, I feel like that’s our responsibility. That’s on us.

KAWEHI HAUG: If there is something wrong with the food, send it back. If you simply don’t like it, don’t send it back. The kitchen is responsible for making the best versions of the dishes presented on its menu. The kitchen is not responsible for your personal likes and dislikes.

Sound reasons to send food back: 1. It’s not cooked to your requested specifications: You ordered a mediumrare steak and it arrives overcooked.

2 Why can’t we sit at that open table? MATTHEW RESICH: I get that a lot. We’ll have a busy night, and we’ll have a reservation coming in, say, 40 minutes, so we’ll tell walk-in customers that unfortunately we won’t have a table until—and we’ll give them a wait time. And they’ll look around and say, “What about that table?” We just explain that table’s reserved, or we’re running a waitlist, and we do have two parties ahead. Then we’ll take their name and number and ask them to wait, or we might suggest having a drink next door at Brew’d while they’re waiting, and we’ll text them when we have a table.

3 Why do some restaurants have a kitchen service fee?

2. You found something in your food—hair, a Band-Aid or worse. 3. The food tastes like it may be spoiled. 4. There is something technically wrong with the dish: It’s cold, burnt, raw. 5. You’re allergic to an ingredient that wasn’t listed as being in the dish. Send it back … but here’s a caveat: Diners with allergies should ask up front if specific allergens are in a dish. But if you order the spring pea pasta assuming it doesn’t have tomatoes and it has tomatoes, send it back anyway. Don’t risk your health on being timid. If it was your error in not clarifying before you ordered, paying for the replacement meal is good form. Restaurants that focus on customer service won’t balk at giving you something else on the house.

DUSTY GRABLE: The kitchen service fee has been sweeping the nation. This should be a topic of national conversation: If major cities want good cooks, what can they do? The most common thought is why don’t you just raise your prices and pay your people better? I wish it were that simple. People don’t think of the cooks when you get a $30 bowl of ramen. The responsibility of keeping prices low with a full-service restaurant with rent payments and staffing and a functioning bathroom and sound system and everything else, people struggle to understand the price when you can get similar food at a food truck. That’s a common misunderstanding. At our restaurants, 100% of our dessert sales goes to the kitchen team. There is a financial commitment from a business standpoint, but there are a lot of wings to it. People are stoked on it. People who weren’t going to get dessert order dessert. I would say 90% of guests get dessert. And most guests get both desserts, so that’s $22 for every table.

LE: Some restaurants charge a 6% kitchen service fee. I think that’s a lot. We charge 3.5% and that goes to our cooks so they can have a fraction of what the servers make. Our restaurant culture, everyone is on the same team and down to help each other out. We’re trying to make sure we’re in this together as far as service and the rewards that come with it. Customers can defer. If they don’t want to [pay the kitchen service fee], then we take it off the bill. It’s a very, very small percentage that defer.

4 Can I bring

my emotional support pet to your restaurant?

GRABLE: This is a really big one and

it’s incredibly frustrating to try to answer. The Department of Health says you can’t have animals in a dining area unless they’re a service animal. For a little while they were cracking down. The Department of Health doesn’t recognize emotional support animals as service animals. Service animals are really limited to certain services such as seeing eye dogs. An emotional support animal is not the same thing. That includes outdoor dining—it’s just people are less inclined to complain about that. None of my servers enjoys having a conversation with a guest about whether an animal is a service animal. That’s getting tougher to navigate. I would love for the community to understand that restaurants in general don’t want to regulate this and are doing it out of fear. I hope it also sends the message that we ourselves have nothing against dogs.

5 Why is splitting checks such a big deal at some restaurants?

HAUG: In my experience as a restaurant owner, most diners in Hawai‘i are happy to order on one check and work out the math on their own. But some parties don’t want to hui together for a bill, and that’s OK. At least, it should be. If a restaurant makes an issue of this, it’s probably because it really can be humbug to go back into the system to divide up the orders after everyone’s already ordered on a single check. I worked at a restaurant where the point-of-sale system could not split one check more than six ways. A good server will anticipate that guests may want to split the bill and ask before they start taking orders. A conscientious guest will inform the server before they order that they want to split the bill.

GRABLE: In general, POS systems

now are so user-friendly that it shouldn’t be an issue anymore. But if a restaurant has been around for 10 years-plus, it’s likely their POS system doesn’t allow for splitting a lot of checks easily. In some cases, the system will crash if you try to do too many checks. And it’s very difficult to reinvest $40,000 for a new system. Other times it’s an issue of service speed and efficiency. Teams need to train how to organize and take an order in a fashion that allows the server to be ready when a guest asks for separate checks. Getting 10 people to get their money and their cards out at the same time when you’ve got other guests waiting for attention is not easy. When I worked at Stage Restaurant and Amuse Wine Bar, a lot of people would be going to a concert or show at Blaisdell, and you would have your entire bar get up and leave at once. Everybody’s waiting until the last minute because they want to enjoy. If they’re all doing separate checks, you could be in a pickle.

HAUG: There’s simply no way to guarantee a zero-wait-time reservation. Even at a well-run restaurant, gazillions of things can go off the rails during a typical service. But the thing that goes the most wrong with reservations is other people with reservations. If a party with an earlier reservation is staying longer than the restaurant anticipated, is it fair to you? No. Neither is it fair to ask the other customer to vacate their table. Both of you are spending your time and money at the restaurant. This does require patience and grace on your part. If the staff is not communicating about the status of your reservation, if they are not gracious about your frustrations, if they are not trying to improve your experience, then it’s more than fair to feel that you wasted your time making a reservation. And to let the restaurant know that.

RESICH: That does happen often.

We don’t impose a time limit. As much as we want to, we can’t force people out of their table. We think if we’re doing our job properly, we’re greeting the table on time, spieling about our menu on time, getting their drink order on time, the food order on time. But you do get groups of friends who haven’t seen each other in a long time, and we don’t force them out. So on a busy night, there might be a wait even with your reservation, but we do our best to mitigate that. And if someone’s been waiting for their table for 15 minutes, we will send out complimentary garlic bread.
