It’s a New Season And with it comes change.

I

T ’ S T I M E to spring into

summer! OK, I know that’s bad. But there’s a reason you may notice fewer (good) puns in this issue than usual. Longtime staffer Stacey Makiya left to pursue another opportunity after 13 years with HONOLULU. I say “staffer” because she wore many hats (and shoes!): stylist, nonprofit leader, fashion editor, events manager. I’m eternally grateful to her for her kindness, the opportunities she gave me when I was still fairly new to the company and, most of all, her heart. It’s impossible to choose a favorite memory with her, but I will say my least favorite: the time she convinced me to go with her to a yoga sculpt class for our fitness blog. My body was not ready. You’ll still find a few stories from her in this issue and the next, and hopefully more down the line. But aloha also means hello, and we’re happy to introduce Andrea Lee, HONOLULU’s new digital editor. In addition to managing the daily flow of stories over on honolulumagazine.com, Andrea, a fellow cheese lover, will lead our content strategy for the web and collaborate with the editorial team on all kinds of exciting projects. Head over to our website to see what she’s been up to, or send an email to say hi: andreal@honolulumagazine.com.

Contributors Navid Baraty is a photographer and visual artist who left behind a career in engineering to pursue his art. He has a deep passion for travel and has lived in multiple cities across the U.S. When he’s not shooting photos for HONOLULU Magazine (you’ll see his shots in ‘Ono, page 87), he’s most likely photographing remote landscapes, cities from above or the wonders of the night sky. Find his work on Instagram: @navidbaraty.

Tracy Chan is a Chinatownbased freelance journalist and photographer who has been covering Hawai‘i events, nightlife and dining for the past 15 years. A journalism graduate of the University of Hawai‘i at Mānoa, she is the former associate editor of Metromix Honolulu and a founding member of Frolic Hawai‘i’s predecessor, Nonstop Honolulu. This month she dives into the history of NextDoor (page 18) as the venue turns 18.

Working on this month’s medical feature (page 26) was a labor of love for Cathy Cruz-George, who recently lost two grandparents and is grateful to the health care workers who cared for them. A former editor of HONOLULU Family and Hawai‘i Business magazines, Cathy and husband Justin George raise an artsy daughter and rescue dog in Honolulu.

Photographer Sean Marrs believes in everyday magic. He’s fascinated by good storytelling, loves meeting people and helping to tell a little of their stories through tiny fractions of seconds. This month, he photographed Onda Pasta’s cooking class at Hana Kitchens (page 20). His favorite magical moments involve adventures with his wife, Candace, and their two wonderful children, Noah and Sadie.

Leave the Door Open After 18 years, NextDoor is still going strong; now a trove of old photos documents the early days of the eclectic Honolulu venue.

W

HEN BUSINESS PARTNERS CHRIS KAHUNAHANA AND SERGIO GOES decided to take over an old Chinatown

venue in 2005 to support Hawai‘i’s underground filmmaking scene, they couldn’t have foreseen what the next 18 years would hold. The nightclub and performance space NextDoor has become a cornerstone of the arts district, hosting DJs such as Steve Aoki and Diplo; rappers Shwayze and Cardi B; Swedish electronic band Little Dragon; as well as up-and-coming local artists, including Goes’ son Gabriel, a solo artist who also plays guitar and sings for the band Backset. The main reason for founding NextDoor was to provide a venue for the Cinema Paradise Independent Film Festival after it lost its space at Restaurant Row, says Andrea Torres, Goes’ widow. “It was their time,

and NextDoor was their artist hui.” Kahunahana and Goes brought in Miguel Innis as a third partner. As a venue at the grassroots level, the new space served as an incubator for local musicians, artists and filmmakers and helped put Honolulu’s Chinatown on the map as an art and culture renaissance swept the neighborhood. Now, thanks to the discovery of a treasure trove of old photos from its earliest days, we can take a step back in time and see how the eclectic venue

got started. Most of the photos were taken by Goes, an award-winning, selftaught local photographer and videographer—and a regular contributor to HONOLULU Magazine from the 1990s until his death in 2008—and archived by Torres, the owner and director of Samadhi Hawai‘i. Find the gallery online at bit.ly/hn-nextdoor. In 2023, a typical week at NextDoor includes live music from 7 to 10 p.m. on at least three days, with DJs playing to the late-night crowd from

10 p.m. until 2 a.m. The club also hosts art events throughout the month, and special events like burlesque shows, punk shows, stand-up comedy and charity fundraisers. Kahunahana sold NextDoor in 2012. “The talent throughout the years has changed, but at its core, NextDoor has always been about giving artists a platform to showcase their skills, and I think we have stuck to that vision,” says Dan Mangum, NextDoor’s marketing director. “It might not be film showcases these days, but Chris and Sergio’s vision was to give creatives a space to create art, and we strive to keep that alive. That’s what separates NextDoor from any other bar.” 43 N. Hotel St., nextdoorhi.com, @nextdoorhi

“I

DON’T LIKE RECIPES. RECIPES ARE OUTLINES.” So begins a gnocchi class at Hana Kitchens, an incubator of culinary microbusinesses at the edge of downtown Honolulu. Tonight’s class is taught by Andrea Onetti, founder of Onda Pasta. “It’s a matter of muscle memory,” he tells 16 apron-clad students who line the counters in the commercial kitchen. They’ve signed up through Instagram, where pasta workshops are joined by classes that include Lebanese cooking and Chinese dumpling making. All are taught by instructors with deep roots in each cuisine, says Hana Kitchens owner Joe Di Condina. Left: Eating freshly made samples is part of the class.

20 HONOLULUMAGAZINE.COM June 2023

Classes are part of the recipe for the incubator, a way for microbusinesses to earn revenue and increase exposure. Onetti, a native of Rome who grew Onda from a farmers market booth into a purveyor for Foodland and about 20 O‘ahu restaurants, is the most frequent teacher. Hana Kitchens’ roster is rich in artisan producers like Banán, Peruvian food truck El Gallo, Tali’s Bagels & Schmear, and spaetzle maker Little Sparrow. Newer ones include Sandra’s Bavarian Delights, which makes soft pretzels and pastries; and Umma’s, which makes Gwangju-style kim chee. Most take turns in the kitchen prepping food for weekend farmers markets, making them unavailable to teach on the days the commercial kitchen sits empty. That could change soon: Di Condina plans chef tastings, pop-ups and more classes later this year, once a bigger show kitchen opens. Tonight, after walking students through the history of gnocchi, having them knead dough and roll it into striated nuggets, Onetti parboils everyone’s batch. Chill your potato dough overnight to reduce moisture, he tells them. If it’s too wet, add more cheese. The gnocchi, fluff y and warm, are tossed in his tomato-mushroom sauce and served with wine, part of a four-course dinner included in the class. The tips, the food, the take-home containers of more gnocchi end up being as experiential as educational—typical of the classes, Di Condina says. Recipes will be emailed later.  55 Merchant St., Suite 20, (917) 854-5069, hanakitchens.com, @hanakitchens_hawaii

Queen Kapi‘olani died in her Waikīkī home on June 24, 1899.

June HONOLULU Magazine emerged from Paradise of the Pacific, a publication commissioned by King Kalākaua that began in 1888, making it the oldest continuously published magazine west of the Mississippi. Each month we take advantage of its enviable archive with a nostalgic dive into the past. For this issue, we flipped back 20 years to see what the city was buzzing about in June 2003. BY J U L I E S TA N L E Y

in 2023. A multipage centerfold highlighted multiple businesses, their owners and the workers who made Waikīkī a must-visit for many people. “There’s a lot of energy here, a lot of people and a lot of activity. You’re always making friends, from everywhere,” says Didi Delania in the original story. She sold tours at Ocean Resort Hotel Waikīkī, now Hyatt Place Waikīkī Beach. Many places mentioned in the article still exist, such as Waikīkī Health Center and St. Augustine Church, both of which are neighborhood staples with active community outreach programs. Others have unfortunately disappeared, like the iconic Wailana Coffee House, which closed its doors in October 2018 after a 48year run, and Wave Waikīkī, a popular nightclub featuring “predominantly local” revelers that closed in 2006.

Waikīkī, for our people Waikīkī, our bustling urban neighborhood and beach

I

n 2003 we celebrated Waikīkī, “where thousands of us live, work, and play.” According to the article, the year prior more than 8.3 million people visited the neighborhood. The total number of visitors to the Islands peaked at 10.4 million in 2019; while COVID-19 caused a drastic dip in tourism, an estimated 9.8 million visitors are expected to return to the Islands

The month’s cover and a picture inside captured local Lindsey Austin holding her surfboard, ready to hit the waves in front of Waikīkī’s Royal Hawaiian hotel. She’s one of the many locals who visited Waikīkī’s shore daily to take advantage of the renowned surf break.

E’VE LONG BEEN FANS of Australia’s effortlessly cool fashions, so we were thrilled to learn that Sydney label Zimmermann, helmed by sisters Nicky and Simone Zimmermann and known for its decidedly light, feminine styles, would be opening its first Hawai‘i outpost this year. The 3,026-square-foot space in Ala Moana Center was crafted with escapism in mind, conveyed through vivid shades of green, curtains patterned with tropical foliage and exuberant abstract paintings by Melbourne artist Jordy van den Nieuwendijk. The store boasts a plethora of runway styles, readyto-wear apparel, swimwear, accessories and keiki pieces from Zimmermann’s Spring ’23 collection, which features playful stripes and Victorian florals inspired by surf town Tamarama’s circa-1900 amusement park Wonderland City, and its Summer Swim ’23 collection, replete with rich jewel hues and sun-kissed gold hardware. Keep an eye out for the shop’s special Hawai‘iexclusive collection, which includes a bikini, sarong, beach towel and more featuring vibrant illustrations of pineapples and monstera leaves.  Zimmermann, 1450 Ala Moana Blvd., Suite 2600, (239) 234-1727, zimmermann.com, @zimmermann

FRESH FINDS | style

Sun Seekers With summer swells and lazy beach days upon us, it’s the perfect time to refresh your swimwear stash with new styles from these three up-and-coming local lines.

LEIMAKANI Empowering women and challenging beauty norms are driving forces behind Leimakani Swimwear. As a young girl growing up in Hilo, designerowner Kali‘a Wasson never felt confident enough to wear a swimsuit in public. Today, her line is putting her on the map—in 2021 she became the first Native Hawaiian to show at Miami Swim Week—thanks to its Hawai‘i-inspired prints and flattering fits made for a wide variety of body types. Determined to create more opportunities for Native Hawaiian and Polynesian talents, Wasson also cofounded the Hawai‘i Swim Show, which debuted on O‘ahu in 2022. During this year’s September showcase, keep an eye out for Leimakani’s new accessories, including fun plumeriapatterned slides and bucket hats, and matching couples’ swimwear. leimakani.com, @leimakani

TAI SWIM For Tyler King, being Native Hawaiian and caring for the environment have always gone hand in hand. Her culture imbued in her a deep respect for the natural world that shines through in her Tai Swim line. As a teen, King upcycled fishing nets into swimwear for classmates at ‘Iolani School. Now, the Kailua-based designer employs sustainable fabrics such as Repreve, a performance fiber made from recycled plastic bottles, and Vita Carvito, a regenerated nylon rated SPF 50plus, as backdrops for her lovely hand-drawn botanical artwork. King intentionally opts for indigenous species, such as wiliwili and kou. Her new Hilo print, created with husband Austin, is inspired by the town’s lush lau‘ae ferns and kukui flowers. taiswim.com, @taiswim

COCONUT AVE In 2020, Mahea Gambill and Kelly Pila, friends from Hilo High School, channeled their shared passion for design and sustainability into Coconut Ave, their swimwear label built around eco-friendly fabrics and manufacturing practices. We’re big fans of its clean-lined silhouettes that while wonderfully unfussy feature playful details, such as half-moon cutouts, twists and lacing. The brand’s tropical prints come courtesy of Gambill, a graphic designer by trade, and range from dreamy sunset shells and palaka to dainty hau flowers and a flecked bronze pattern that mimics the striking coat of the Hawaiian pueo. In 2023 the duo opened a beach-chic boutique in Kona where you’ll also find Coconut Ave’s breezy new resortwear. 69-201 Waikoloa Beach Drive, Waikoloa, coconutave.com, @coconutave

WHAT’S UP, DOC?

From artificial intelligence to supermarkets, here are five trends that show where medicine in the Islands is headed. BY C ATHY CRUZ- GEORGE PHOTOS BY A ARON K. YOSHINO

Dr. Ankur Jain and Dr. Shilpa Jain HONOLULUMAGAZINE.COM June 2023 27

Trend #1: AI HAS ARRIVED.

COLONOSCOPY, ONCE A RITE OF PASSAGE for people

over 50, is now recommended among other colon cancer screening for younger people—because that’s where colon cancer cases are rising. Honolulu gastroenterologists Dr. Ankur Jain and Dr. Shilpa Jain, a husband-wife team, see patients with this cancer as young as their mid-20s. And while fewer people have been dying from colon cancer, many continue to opt out of colonoscopies and other elective procedures, Ankur Jain says. What could help change minds is artificial intelligence. AI is making inroads across medicine in Hawai‘i, including by offering a faster and more accurate way to find potentially cancerous polyps. In February, the Pacific Endoscopy Center in Pearl City introduced GI Genius, which uses the same AI technology that cell phones use to identify faces, to search for polyps in the colon. The polyps appear on a computer screen as green boxes. Use of the GI Genius doesn’t add any more time to the procedure, which still requires an endoscope, says AnnaLyn Ogata, regional vice president for Covenant Physician Partners, the center’s parent company.

28 HONOLULUMAGAZINE.COM June 2023

At the UH Cancer Center’s AI Precision Health Institute, researchers have been using AI to improve cancer treatments and diagnoses since 2018. AI tools can search through records to find text-based medical clues that can lead to diagnosis, says John Shepherd, the institute’s co-founder and principal investigator and a professor at the UH Medical School. It can also help with second opinions on medical cases and lead local scientists to case studies. “We have a fairly low enrollment rate of cancer patients into clinical trials,” Shepherd says. “AI can search all cases in all medical centers in Hawai‘i for specific criteria for enrollment for hundreds of studies simultaneously.” An example of his team’s research is the Hawai‘i Pacific Islands Mammography Registry, which uses AI to identify breast cancer trends, risks and outcomes in outcomes in Asian American, Native Hawaiian and Pacific Islander groups. The registry contains more than 45 million individual images—mammograms, ultrasounds and other procedures performed on 100,000 women over 10 years—connected to breast cancer outcomes and risk factors. Jain says AI’s potential impact on gastroenterology is profound. “We have just recently seen this technology being introduced into clinical practice with AI-assisted colonoscopy. Other applications of AI being studied include risk stratification and early identification of other malignant and nonmalignant lesions of the [gastrointestinal] tract including gastric cancer, GI bleeding, non alcoholic fatty liver disease, and pancreatic cancer,” he says. “I am confident that we will see an explosion of AI in health care over the next few years.”

E’RE ALL PATIENTS AT SOME POINT OR OTHER, so recent trends in medicine matter in fundamental ways. We put local health care on the examination table and come away with a prognosis that signals promise and progress.

THE NEXT TIME YOU HEAD TO A CHAIN GROCERY STORE or drugstore, you might

Trend #2: MILK, EGGS, WELLNESS EXAM? IN-STORE HEALTH CARE OPTIONS ARE GROWING.

want to add a wellness exam to your shopping list. Retailers with in-house pharmacies are beefing up health care offerings far beyond just prescription refills.    Take Longs Drugs CVS Pharmacy, for instance. Its nine O‘ahu in-store MinuteClinics are staffed by nurse practitioners who can provide COVID-19 tests and shots; tuberculosis tests; sports and work physicals; pre-travel health consultations; evaluations of sexually transmitted infections; and routine vaccinations for shingles, hepatitis and more. MinuteClinics recently expanded services to include blood tests; mental health counseling and referrals; chronic disease management for diabetes, high blood pressure and high cholesterol; and screening and treatment for sleep apnea. For common colds, influenza and digestive issues, customers can request virtual appointments at minuteclinic.com.    The nurse practitioners partner with the clinic’s CVS Pharmacy to “close gaps in care and recommend more cost-effective drug therapies,” says regional director Scott Sutton. “Our pharmacists can also evaluate patients with COVID-19 symptoms and prescribe the oral antiviral medication Paxlovid if the patient is eligible. Also, Hawai‘i is one of 10 states where our pharmacists evaluate patients and prescribe birth control.”

THERE’S GROWING PUBLIC INTEREST IN MEDICINAL USE OF CBD AND CANNABIS; LÁ‘AU LAPA‘AU , TRADITIONAL HAWAIIAN HERBAL MEDICINE; AND OTHER ALTERNATIVE APPROACHES.

Supermarket chain Safeway operates 12 in-store pharmacies statewide that provide health screenings, travel vaccinations, mental health medication administration, and routine shots including for COVID-19, influenza and shingles. In February, the chain launched Sincerely Health, a new digital platform that can manage pharmacy orders, schedule vaccine appointments and connect with general practitioners via telehealth. The program starts with an online questionnaire and can track a user’s vital signs, medication regimen and overall health. It’s compatible with Apple Health, Fitbit and Google Fit.

Trend #3: IN-PERSON DOCTOR VISITS ARE THE NEW OLD NORMAL, WHILE TELEHEALTH OPTIONS ARE BOOMING.

AS COVID -19 INFECTION COUNTS SUBSIDE , most patients have returned to in-person doctor visits in the Islands, says Dr. Todd Devere, Kaiser Permanente Hawai‘i’s medical director for informatics and access. Hawai‘i’s largest health care providers report that 75% of doctor visits were held virtually in 2020 and 2021; in 2022, that dropped to 40%. But telehealth looks like it’s here to stay, with many patients and health care providers preferring the convenience and time saved. Kaiser Permanente, the state’s largest health maintenance organization, recently augmented its kp.org portal. Since last year, in addition to emailing their care teams, engaging in video visits and more, members have been able to access on-demand phone and video services 24/7 through Kaiser Permanente’s website and mobile app. Appointments aren’t required, and the round-the-clock virtual service helps health care workers decide if a patient needs in-person urgent care, same-day treatment or an appointment with a physician. “The [virtual] option is still an important one for patients,

30 HONOLULUMAGAZINE.COM June 2023

especially in Hawai‘i where some work two jobs to make a living,” says Dr. Sandra Noon, chief of primary care and internal medicine physician for Hawai‘i Pacific Health Medical Group. “People do not have to take time off from work or find someone to watch their young children. Also, older patients who may have challenges can essentially have their doctor visit their home, virtually.”   Noon recalls a quadriplegic patient who happily shared his home and daily routine with her on-camera during a virtual appointment. “I learned so much as his physician,” she says. “It gave me a valuable perspective into his life and challenges and built upon our relationship.” HMSA was the state’s first health plan to launch a telehealth platform—back in 2009. The state’s largest health insurer is now testing a digital platform that lets patients use their own devices to manage chronic disease and reach health coaches. “The early results have been very promising,” says Matthew Reeves, HMSA senior vice president of clinical integration. “Depending on the success and community receptivity of these pilots, we will look to further expand these offerings.”

Trend #4: ALTERNATIVE MEDICINE IS GAINING MORE ACCEPTANCE.

DR. KATHRYN TAKETA-WONG’S PRACTICE —

which falls into the category of complementary and alternative medicine—is booming. As a naturopathic physician and licensed acupunc-

Dr. Kathryn Taketa-Wong HONOLULUMAGAZINE.COM June 2023 31

turist specializing in pediatric autism and attention-deficit/ hyperactivity disorder, Down syndrome and other conditions, she looks at physical health, but also considers how mental and emotional stresses impact overall health. “I then make individualized recommendations primarily focusing on diet, vitamins, herbs and other nutrients to treat those underlying medical conditions,” she says. Taketa-Wong recently hired another naturopathic doctor to accommodate a swell of patients over the past two years. When she opened her practice in 2011, she was met with skepticism from conventional doctors; since then, that hesitation’s faded and she’s seen increasingly more referrals, cooperation and acceptance. There’s growing public interest in medicinal use of CBD and cannabis; lā‘au lapa‘au, traditional Hawaiian herbal medicine; and other alternative approaches. While local insurers have yet to broadly cover these practices, signs point to increasing acceptance in the insurance sector too, she says. Kaiser Permanente Hawai‘i recently expanded its Complementary and Alternative Medicine services, commonly referred to as CAM, says Paula Young, Kaiser Permanente product management director. A supplemental program lets employers bundle acupuncture services with massage therapy and chiropractic services. And Kaiser Permanente members whose plans don’t include CAM can now get reduced rates on acupuncture, massage therapy and chiropractic services, she says. Even smaller providers, such as UHA Health Insurance, now cover some naturopathic visits and some acupuncture, therapeutic massage and chiropractic services. These three are the most common CAM therapies covered by private insurance in Hawai‘i. What’s not covered? Herbal medicines and remedies like yoga, tai chi and meditation. In Hawai‘i, eligible patients can use flex-spending or health savings accounts to cover the costs of some CAM services.    Deciding which therapies qualify for coverage is no easy feat for insurers. “Hawai‘i is a unique cultural melting pot, and the Eastern and alternative medical influences are especially strong here,” says HMSA’s Reeves. But there is “not the same level of scientific evidence and national guidelines for these complementary/alternative treatments that standard medical therapies are measured and vetted by.” HMSA is working with national experts and community providers, he says, to “hear about emerging evidence and guidelines for appropriate coverage with a more holistic view of health.”

32 HONOLULUMAGAZINE.COM June 2023

Trend #5: WE’RE LOSING HEALTH CARE WORKERS FASTER THAN BEFORE. HERE’S WHAT’S BEING DONE.

PHYSICIANS WHO CHOOSE TO PRACTICE —and re-

main—in Hawai‘i are like precious gems. Hawai‘i is in dire need of more physicians, a situation that hasn’t changed since Dr. Kelley Withy and Dr. David Sakamoto published the first Physicians Workforce Report in 2010. The 2022 report pegs the statewide deficit at 776 full-time doctors. “Whenever I talk to groups of people, I remind them to thank their doctors because the ones that are here are working so hard; they’re working for two,” says Withy, a professor at the UH John A. Burns School of Medicine and director of the Hawai‘i/Pacific Basin Area Health Education Center, which works to recruit and retain doctors. What has changed is the breadth and quickening pace of the worker shortage across the health care spectrum. In 2022, the local industry had 3,873 job openings—up 76% from 2019, according to the Hawai‘i Healthcare Workforce Initiative Report and the Healthcare Workforce Initiative published in November 2022. In the same period, the turnover rate for health care workers climbed from 16% to 20%. That includes nurses, nurse aides, medical assistants, phlebotomists, patient service representatives, social workers and others—89 patient-facing roles in total. “When considering Hawai‘i’s health needs, it’s important to look at both these professions and the phy-

sician workforce,” says Hilton Raethel, president and CEO of the Healthcare Association of Hawai‘i. “Despite our hard work in building the workforce pipeline, the pandemic accelerated worker retirements and had people leaving their health care professions faster than they could be replaced.” The local industry is getting creative, with new programs to woo workers. Some examples:

THE UH MEDICAL SCHOOL GRADUATE

Thoracic oncology surgeon Dr. Taryne Imai left her job at Cedars-Sinai Medical Center in Los Angeles and is now pioneering robotic technology as The Queen’s Health System’s thoracic surgery program director. What brought her home was Hawai‘i’s last-place ranking in the nation for early detection of lung cancer. “It was time to serve the community that raised, educated and inspired me to become a physician,” says the JABSOM graduate, whose mission is to increase the rates of lung cancer screenings for Native Hawaiians and nonsmoking women.

• G L I D E PAT H T R A I N I N G . A statewide pilot program began in December 2022 to train employed certified nursing assistants to become licensed practical nurses. The first cohort from O‘ahu, Kaua‘i and Maui is scheduled to graduate and take the licensing exam at the end of this year. Students are employees at ‘Ohana Pacific Health and Kaiser Permanente Hawai‘i. Similar glidepath programs are in the works to train employed phlebotomists to become medical laboratory technicians.

• STUDENT RECRUITMENT. This past academic year, more than 3,000 students in Hawai‘i high schools and colleges participated in the Pre-Health Career Corps, a free program that exposes them to the health care industry. “We meet with students and help them build their résumé, cover letter and practice with mock interviews. Then we help students connect with a facility,” says program director Lark Morin. “We’ve had students shadow pharmacists, pathologists, physician assistants, and so many more that I’ve lost count.”

A junior at St. Anthony School on Maui, Rob Ryane Gonzales plans to become an anesthesiologist “to see others relieved and free of pain and continue to live happy lives that bring me joy,” he says. The Maui teen has witnessed hospitals and clinics “reduced to skeleton crews where doctors and nurses have to resort to piling more responsibilities on top of what they already do to keep our hospitals running,” says Gonzales, an intern in the Pre-Health Career Corps and chapter president and state vice president for HOSA, a student group for aspiring health care professionals formerly known as Health Occupations Students of America. • L OA N F O R G I V E N E S S . State and federal incentives help doctors repay their student loans. The Hawai‘i State Loan Repayment Program, for instance, pays back loans for primary care and behavioral health doctors who work in nonprofits or areas with shortages of health professionals. The Hawai‘i Dental Association this year donated funds so that more dentists and dental hygienists can receive up to $50,000 a year to repay loans.

THE HIGH SCHOOL STUDENT

THE COLLEGE STUDENT

Princess Jena Santiago’s goal is to become a general and thoracic surgeon. A junior majoring in biochemistry on the pre-med track at UH Mānoa, she hopes to “increase ethnic and racial diversity in the health care field by building and advocating [for] programs supporting and encouraging students to pursue the industry,” she says. As an intern with JABSOM’s Hawai‘i/Pacific Basin Area Health Education Center, she mentors younger students interested in health care. Santiago also is the postsecondary president of HOSA. THE WORKING STUDENT

Ashley Bulosan—a Maui mother of two—last December joined Kaiser Permanente’s first Practical Nurse Trainee Program, which trains Kaiser employees in Hawai‘i to become LPNs. Not only is she enrolled in five courses, she is raising her family while working as a pediatric medical assistant at Kaiser’s Maui Lani Medical Office. “My goal was always to be a nurse, but I never found the opportunity to do so,” says Bulosan, who tried several times in the past to pursue nursing studies. Much of her motivation comes from her co-workers. “The people in my department are very supportive—it’s awesome!”

As Board-Certified and Fellowship-Trained Reproductive Endocrinology and Infertility Specialists, Dr. Frattarelli and Dr. Karmon are a few of the most successful and experienced fertility physicians in the U.S. Approximately 15% of couples have difficulty conceiving. Women should seek medical help if unable to achieve pregnancy following 6 months to a year of regular, unprotected sexual intercourse. Older women and those with a history of pelvic inflammatory disease, endometriosis, miscarriage, irregular or painful menstrual cycles, or men with low sperm count may benefit from consulting with a physician earlier. As the premier Fertility and IVF clinic in Hawaii and the Pacific, the Fertility Institute of Hawaii offers a unique, personalized approach to fertility care while utilizing novel fertility protocols and the most advanced laboratory equipment. Dr. John Frattarelli, Dr. Anatte Karmon, and their team of expertly trained personnel tailor the necessary treatment to individual patients resulting in the best pregnancy rates in the world.

ABOUT US

OUR SERVICES

The Cataract & Vision Center of Hawaii is a full service eye care center with on-site surgical suites and the most up-todate technology to provide patients with premier medical and surgical care. Over 40 years ago, Dr. Worldster Lee, founded the practice and has taken care of generations of local families. Both Dr. Brandon Lee and Dr. Daniel Choi were trained at the nation’s No. 1 eye hospital: Bascom Palmer Eye Institute in Miami, Florida. Combining tradition and technology, the Cataract and Vision Center of Hawaii provides world-class, compassionate care.

TOP OF THEIR CLASS

The UH medical school cultivates some of the nation’s top doctors. Half a century has passed since the John A. Burns School of Medicine welcomed its first class of four-year medical students. Prior to 1973 the school only offered two years of education to aspiring doctors, who would then leave Hawai‘i for further training. Now, JABSOM’s mission is to recruit students and retain them, from medical school and residency to fellowship and faculty. Hundreds of physicians from Hawai‘i land on Castle Connolly Top Doctors each year out of more than 11,000 who are licensed here. Meet the three who grace the cover of this issue—all proud graduates of JABSOM. BY C AT H Y C R U Z- G E O R G E

Kapi‘olani is where Iwamoto completed her pediatric residency and fellowship after graduating from JABSOM in 1988. She is also a faculty member in the school’s pediatrics department. She praises the school’s influence, which extends beyond its classrooms and labs. It provides free health seminars for older adults and mentorship programs for high school students, for example. “That’s what JABSOM is trying to do—help the community continue to thrive and grow.”

R. LYNN M. IWAMOTO, a neonatologist at Kapi‘olani Medical Center for Women and Children, cares for fragile newborns who arrive a wee bit early. “They are so tiny that every treatment, every medication, has to be extremely precise,” says Iwamoto, associate division director of neonatology at Kapi‘olani and a Castle Connolly Top Doctor. “It can be very intense and fast-paced.” Every day, she witnesses strength from the babies’ families who “step up and don’t give up, the ultimate definition of unconditional love,” she says.

R. BRADLEY TOKESHI is a pulmonary and critical-care physician with The Queen’s Health System, specializing in advanced bronchoscopy and pulmonary procedures. Queen’s is the only health care provider in the state that uses robotic navigational bronchoscopy technology to significantly reduce the treatment time for lung cancer. A 2011 graduate of JABSOM, Tokeshi completed his residency in internal medicine in Hawai‘i, then moved to UC Davis Medical Center in California for a fellowship in pulmonary and critical care. He’s one of Castle Connolly’s Top Doctors. Tokeshi’s role model is his father, Dr. Jinichi Tokeshi, a family physician at Kuakini Medical Center and clinical faculty member at JABSOM. Jinichi Tokeshi migrated from Okinawa more than 50 years ago to study biology at UH Mānoa and joined the first four-year class of medical students at JABSOM in 1973. “He is still hands-down, easily the best, most empathetic, committed, professional physician I know,” Bradley Tokeshi says.

LEGACY AWARD

C A R LT O N Y U E N , M . D . & J A S O N T O K U N A G A , M . D . BORN AND RAISED IN HAWAI‘I Locally born, Dr. Carlton Yuen and Dr. Jason Tokunaga are trained ophthalmologists who believe in treating all of their patients like family. As founders of Aloha Vision Consultants, they are committed to serving every patient with outstanding skill, service, integrity and dedication. Because they believe that vision is critical to a happy, independent lifestyle, Drs. Yuen and Tokunaga are committed to providing a full range of quality and advanced vision solutions, including cataract surgery.

WHAT ARE CATARACTS? Cataracts are the progressive clouding of the natural lens in the eye that can make simple daily activities, such as looking at street signs, a burden. When cataracts develop, glasses and contacts are no longer able to provide visual relief. Solutions such as cataract surgery can make a tremendous impact in restoring vision.

1029 Kapahulu Ave #502, Honolulu, HI 96816 (808) 782-1861

PERSONALIZED SOLUTIONS FOR YOUR CATARACTS Drs. Yuen and Tokunaga are at the forefront of technological advances in eye surgery. They were selected as the first doctors in Hawaii and among the first in the nation to implant the next generation Symfony with Optiblue Extended Depth of Focus Intraocular Lens (EDOF IOL), allowing patients with active lifestyles a wider range of vision with less glare. Drs. Yuen and Tokunaga were also selected as the first doctors in the nation to perform a pilot study on advanced computerized guided correction of astigmatism with laser assisted cataract surgery. Drs. Yuen and Tokunaga offer personalized solutions to enhance your vision over all ranges. They are experienced in multiple platforms including laserassisted cataract surgery, astigmatism correction lenses and multifocal lenses including trifocal and EDOF lenses. Drs. Yuen and Tokunaga feel so

99-128 Aiea Heights Drive #304 Aiea, HI 96701 (808) 688-6898

fortunate to provide these solutions to allow you to perform most activities without contacts or glasses.

WHAT IS CHRONIC DRY EYE? Most patients believe dry eyes are a result of inadequate tear production despite having tried artificial tears and prescription drops without relief. This is because chronic dry eyes are often caused by obstructed oil glands, otherwise known as Meibomian Gland Dysfunction (MGD). Symptoms include: burning, dryness, tearing, fluctuating vision, scratchiness, and grittiness. If you notice fluctuating vision with driving, watching TV, using your cell phone, or reading, you may be suffering from MGD. Drs. Yuen and Tokunaga are excited to offer LIPIFlow, a simple 12-minute office treatment that opens your obstructed oil glands to improve your symptoms and protect you from further progression of gland dysfunction.

DR. HISAMI OBA

R. HISAMI OBA, a primary-care physician at Kaiser Permanente’s Māpunapuna clinic, vividly remembers how she felt as a medical student giving a speech at JABSOM’s “willed-body ceremony”—a memorial service honoring those who donated their bodies to the school for research. The love, respect and appreciation for the silent teachers and their grieving families left her “in awe of the privilege that we as physicians are given to enter some of the most sacred places within a patient’s life,” she says. The medical school was Oba’s own sacred place, a haven where she felt “cared for, seen, heard and truly valued” as a person and future physician. She graduated in 2017 and completed a residency at Kaiser Permanente Hawai‘i. Today, the Castle Connolly Rising Star serves as the associate program director of Kaiser Permanente’s internal medicine residency program. “It brings me true joy to encourage and train the new generation of physicians,” Oba says.

WHEN DR. DARRELL LEE CO-FOUNDED PACIFIC ENDOSCOPY CENTER TO BRING QUALITY GASTROINTESTINAL CARE TO WEST O‘AHU, he had no idea it would lead to the creation of Hawai‘i’s largest group of GI centers. Starting with PEC in Pearl City, that group now encompasses three other centers in Honolulu and Hilo. “We are the largest provider of GI services, and together we provide the best and most advanced GI care in Hawai‘i,” Lee says. Lee’s vision started years ago when he realized there was no full-time GI specialist on the West Side. Worse, the wait time for a routine colonoscopy—the most important tool in detecting and preventing colon cancer—was up to six months. Enter PEC. All its gastroenterologists are board-certified and use state-of-the-art techniques and equipment, including the state’s only colonoscope with a 330-degree field of view, nearly double the traditional 170-degree view. Now patients can schedule exams usually within two weeks. Lee’s commitment to serving the community goes beyond his work. Knowing the importance of selflessness and compassion, Lee and his wife, Cathy, taught their daughter those same values from a young age. Their guidance led to the creation and development of the nonprofit organization Kidz For A Cause, which has raised over $760,000 for various charities on O‘ahu. Additionally, Lee is paving the way for the next generation of local medical professionals: As the first person from our islands to be elected to Creighton University’s Board of Trustees, he is paying it forward with an endowment to help send Hawai‘i students to medical school at the university, ensuring quality care for all on O‘ahu for years to come.

WE ARE THE LARGEST PROVIDER OF GI SERVICES, AND TOGETHER WE PROVIDE THE BEST AND MOST ADVANCED GI CARE IN HAWAI‘I. 72 HONOLULUMAGAZINE.COM June 2023

SONNY J. H. WONG, M.D., FACC, RPHS

CARDIOVASCULAR DISEASE/VEIN DISORDERS

EARLY ON IN HIS MEDICAL CAREER, DR. WONG WAS WORKING IN AN EMERGENCY ROOM when a patient went into cardiac arrest. He watched in awe as the attending doctor remained calm and logical faced with a life or death situation. This experience led Dr. Wong to not only specialize in cardiology but emulate how this physician practiced medicine. In his nearly 30 years of experience in cardiology, Dr. Wong has done just that. His patients note that he is caring, thorough, and an excellent listener and communicator. As a cardiologist, his goal is to make a difference in every patient’s life. Recently, his practice, the state’s most comprehensive clinic for treating vein disorders, expanded to five locations. There are three offices on Oahu in Honolulu, Kailua, and Waipahu and two on Maui in Kihei and Kulamalu. The growth prompted a name change for the clinic. The same state-ofthe-art care patients came to known and love at Windward Heart, Vein and MediSpa is now Heart + Vein Hawaii. Heart and Vein Hawaii specializes in the evaluation and treatment of vein disorders. Treatment options include endovascular radiofrequency ablation, VenaSeal, Clarivein, compression stockings, ultrasound examinations, microphlebectomy, sclerotherapy, and Excel V laser treatment. Dr. Wong graduated from University of Otago Dunedin Medical School in New Zealand where he is originally from. He completed his internal medicine and cardiology training at the University of Hawai‘i and University of Nevada respectively, with board certifications in Internal Medicine and Cardiovascular Disease. He is also certified by The American Board of Venous and Lymphatic Medicine and has been caring for patients in Hawai‘i since 1994. Heart and Vein Hawaii is excited to welcome new patients to the expanded practice this year!

AS A SURGEON AT SHRINERS CHILDREN’S HAWAII, Dr. Craig M. Ono enjoys caring for wonderful children and their families throughout Hawai‘i and the south and western Pacific Islands. He and his team provide the highest quality of care for a broad range of musculoskeletal problems. Dr. Ono earned his medical degree at Northwestern University Medical School and completed his internship and orthopedic surgery residency at Tripler Army Medical Center. After completing his pediatric orthopedics fellowship at the Shriners Children’s Hawai‘i, he travelled between Hawai‘i, South Korea, Washington and Macedonia as an active duty member of the United States Army. He returned to Shriners Children’s in 2000 and felt fortunate to have the opportunity to have two parallel careers as a pediatric orthopedic surgeon and as an officer in the United States Army Reserve. Dr. Ono was attracted to orthopedic surgery first because of the physical nature of the practice. He stayed because of the patients, families, practice partners and mentors. Today, he aims to help develop the next generations of physicians and surgeons, both as a teacher at UH John A. Burns School of Medicine and Tripler Army Medical Center. He is part of a team that is academically rigorous with cutting-edge research and presentations for continuing medical education courses.

JONATHAN PELLETT, M.D., is board certified by the American Board of Orthopedic Surgery and a member of the Pediatric Orthopedic Society of North America. He treats almost every pediatric orthopedic condition and uses a team approach to bring the best subspecialty care at Shriners Children’s Hawai‘i. In addition to providing care for Island keiki, he is also part of the Shriners outreach team, providing clinical care in Guam, American Samoa, Fiji and the Marshall Islands. Dr. Pellett earned his medical degree at Washington University School of Medicine and completed his orthopedic surgery residency at the Medical College of Georgia. He completed his fellowship in pediatric orthopedic surgery at Shriners Children’s Hawai‘i and, in 2006, joined the Shriners team of pediatric surgeons, where he continues to practice today. Dr. Pellett is a recipient of JABSOM’s Allen B. Richardson Award for Teaching Excellence and was named Physician of the Year by the Healthcare Association of Hawai‘i. In 2020, he became president of the Hawai‘i Orthopedic Association, and, in 2021, helped organize and moderate the Hawai‘i Orthopedic Association’s Spring Research Symposium. As part of his desire to help improve the lives of keiki, he also facilitates the Shriners Education and Research (SEARCH) conference, an annual event that brings community physicians and providers together to find comprehensive solutions for pediatric orthopedic conditions.

DR. MURARIU, ALONG WITH HIS PARTNER, DR. ROBERT PETERSON, RUNS ATHENA CLINIC, a boutique-style practice that offers more than 40 years of combined world-class expertise in plastic surgery. Athena Clinic provides a comprehensive range of aesthetic services, from medical grade skin care to noninvasive treatments for skin tightening and fat reduction, as well as in-office plastic surgery procedures. Dr. Murariu specializes in aesthetic surgery, with a special expertise in breast aesthetics and high-definition liposculpting, as well as head-to-toe reconstructive surgery, which can help restore both function and aesthetics after trauma or oncologic resections. One of Dr. Murariu’s unique strengths is his expertise in performing robotic-assisted reconstructions, a technique only a handful of plastic surgeons in the nation offer. Robotic-assisted reconstructions have the benefit of smaller incisions, less pain and faster recovery times. During his time as the Director of Microsurgery and Lymphatic Surgery in Pittsburgh, he likely performed the most robotic reconstructions in the US for breast, pelvic, and head and neck. He also helped co-found the first robotic task force for his main society and has published his techniques and outcomes to inspire more plastic surgeons to offer it to their patients. He shares his research and techniques internationally at plastic surgery conferences and, last year, gave more than 50 talks at the local, national and international level. In the next year, Dr. Murariu aims to expand Athena Clinic and offer medispa services at a separate location to complement its surgical practice. His partner, Dr. Peterson, is also building a new clinic in Hainan, China, making Athena Clinic an international presence with surgeons based in multiple locations. Dr. Murariu’s motivation to be the best doctor he can be comes from his commitment to charitable activities, as demonstrated by his involvement in Rotary Club and recognition for his local and international charitable efforts. Dr. Murariu attributes his success to perseverance, hard work and always putting his patients’ best interests first. He spent nine years after medical school, furthering his training in plastic surgery to deliver the best care possible to the people of Hawai‘i. Patients can trust Dr. Murariu’s expertise, dedication and commitment to excellence in providing world-class plastic surgery services at Athena Clinic.

He specializes in state-of-the-art cataract surgery and, as a fellowship-trained glaucoma surgeon, the latest treatments for glaucoma that are minimally invasive, safer and have faster recovery times. As an assistant clinical professor at JABSOM, he enjoys sharing his expertise with current and future physicians of Hawai‘i. “I strive to have an impact not only on my patients, but the community as a whole,” says Dr. Yim, who enjoys spending his time off with his wife and two young sons. Through JABSOM, he continues to research and publish articles ranging from the evaluation of certain types of eye cancers to potential new treatments, with his most recent article on the treatment of neovascular glaucoma published just last year. He also serves on advisory boards, offers consultation for pharmaceutical and medical device companies, gives talks across the nation and is involved in a national prospective clinical trial focusing on minimally invasive glaucoma procedures. Dr. Yim believes in treating the patient, not just the disease. “One of the reasons I love my job is because of the special relationships I’m able to develop,” says the surgeon, who performed life-changing ocular surgeries in Ghana as part of the Himalayan Cataract Project. “Having the ability to connect with people on a personal level—not just as a physician, but as a friend—allows me to have an impact on their health and life as a whole.” He currently serves on the board of directors for the Homeless Outreach Medical Education Project, through which he helped establish the first free ophthalmology clinic for the underserved community, and as a board examiner for the American Board of Ophthalmology.

Delicious by Design We explore the aesthetics at some of Honolulu’s most beautiful eateries, from the elegantly minimal and midcentury-meetstropical to the delightfully daring and oh-so-luxe. HONOLULUMAGAZINE.COM June 2023 87

Call it ambience, call it a vibe. If the food is intended to dazzle your taste buds, then the hundreds of details that go into a restaurant’s interior design are meant to be a feast for the eyes. From cool patterned tiles and original wallpaper to chic lighting and painstakingly handcrafted trims—taking it all in is a huge part of the dining experience.

Istanbul Hawai‘i

STANBUL, SHE IS GORGEOUS. But what sets it apart is the narrative behind its warm, inviting Mediterranean aesthetic. Owner-chef Ahu Hettema and her family designed and made by hand most of the dazzling décor themselves. Hettema imagined the pieces she wanted, blueprinted her ideas, then handed them over to her husband and father, both engineers. The result? Distressed, cerulean-stained doors that double as bench backs; tiered chandeliers inspired by those in Istanbul’s grand Hagia Sophia; a stunning arched bar made of oak, and rustic wood tables and floors that allow the earthy, rich tones of the space’s accessories, including ceramics and a 100-year-old rug from Turkey, to shine. Hettema notes, “This is my version of a modern Turkish home.” —SM

Sushi Sho’s cavernous interior

Sushi Sho

Y

OU ENTER AT THE MIDPOINT OF A TUNNEL-LIKE ROOM, your

gaze immediately drawn to the right. Here is Sushi Sho’s center stage, where long dark-wood slats of a semicircular ceiling meet the blond arc of a cypress sushi bar. Behind the counter, two carved panels featuring depictions of Hanaya Yohei, the 19th century creator of the Edomae sushi style, and a Hawaiian moi, hint at Sho’s uniquely Hawai‘i take on the technique. All the lights shine down on this space, illuminating Edomae sushi masters as they slice precisely cured seafood, shape it onto rice and place the morsels of nigiri sushi before diners ringing the stage. The effect is like that of an oyster, shining and opalescent, revealing its pearl—the result of a collaboration between chef-proprietor Keiji Nakazawa and Japanese design firm Nomura Co., with Brooklyn’s Guerin Glass Architects. —MT

perfect sense that this Foodland-owned eatery’s name is derived from the Latin phrase meaning “and others.” Local design firm The Vanguard Theory combined signatures of Mediterranean architecture, such as stucco and dramatic arches, with elements of Hawaiian, Japanese and Portuguese culture to stylish effect. Look out for seating upholstered in palaka, a plaid fabric made popular in Hawai‘i’s plantation days; a concrete bar top finished with kintsugi-inspired gold inlay; and our favorite touch, chairs that look as if they’re draped with colorful lei. —BT

ELIVERING YOURSELF FROM DOWNTOWN HONOLULU’S BUSTLING concrete-laden

business district directly into Podmore can be a shock to the senses. The effect is akin to stepping straight into a luxe jewel box. Inside the brunch and cocktail destination, you’re met with lush velvet-clad banquettes and bar stools in moody emerald and amethyst hues, brass trims and a lustrous porcelainfinished bar. Owners Katherine Nomura and chef Anthony Rush tasked architecture firm WRNS Studio with giving the space a bit of British flair, a nod to Rush’s home country and the duo’s affinity for London’s grand hotel bars and private members clubs. Even the elegant wisteria wall mural has a tie to England, mimicking a corner of Rush’s parents’ garden in Devon. For Nomura’s connection to the bloom, keep an eye out for a treasured framed needlepoint crafted by her grandmother.—BT

fresher, hipper Kaimana Beach Hotel, its Hau Tree eatery (named for its beloved 100-plus-year-old trees) was reimagined as well. Its resulting “beachside casual-luxury” aesthetic, dreamt up by Henderson Design Group, starts with custom-designed teak and rope-trimmed chairs and a relaxed coastal palette in the open-air dining room and carries over to woven light fixtures and artisanal accent tiles at the bar. And, we would be remiss if we didn’t mention the view of the nearby lounge area’s showstopping gallery wall featuring tropical art in a punchy mix of turquoise, pink and orange. —BT

Plan On It

I

RECENTLY DECIDED TO GET A WILL. That might seem like a bizarre thing to do for a 33-year-old, but I’ve been thinking about it for a while. A few years ago, a friend of mine was riding his motorcycle on the H-1 when a driver swerved into his lane. My buddy was thrown off his bike and spent a few days in the hospital. Fortunately his injuries weren’t lifethreatening. But if they had been, at least his affairs were in order because he had a will. “You just never know,” he’d told me several months prior to the accident, when I asked him why a 20-something who sat at a computer all day needed to have a last will and testament ready to go. Makes sense. But of course, like most big decisions in my life, I put off getting one of my own. Setting up a will always took a back seat to work and my daily life. According to Gallup, less than half of adults in the U.S. have a will in place. That population not only includes me but also my mother, who, despite being a home health nurse who specializes in senior care, has avoided getting a will for herself. It’s something that only old people do, she reasoned. She’s healthy and “not old at all.” Over the course of her 20-year career, she’s seen the havoc that not having a will can cause for families when a person passes away, but she’s also seen the arguments and anger that sometimes emerge in conversations about advance health care directives, estate planning and inheritances. Maybe because of this, my mother instead opted for years to scribble her various bank account numbers, email addresses and other critical info onto Post-its that she would give me while I was in the middle of doing something else. “If anything happens to me, you’ll be sorry if you don’t know this stuff!” she’d 96 HONOLULUMAGAZINE.COM June 2023

say. I’d absentmindedly stick the notes in a binder or between the pages of a book for safekeeping. Then I’d rediscover them months or years later with no clue what the numbers meant. It wasn’t a great system. Technically, Hawai‘i accepts holographic wills, or wills that are handwritten and signed without a lawyer and a notary. But a more formal, notarized will seems like a safer way to go. I reached out to a local estate attorney for his take on the process so I could look into getting one for both my mother and myself. I figured it might be easier to convince my mom to get a will if she went through the process with me. We both need one because, well, we all need one. Plus— in case anyone forgot—we’re all still technically experiencing a pandemic that has claimed millions of lives. As my friend would say: “You just never know.” The lawyer’s advice was to begin with a conversation: What do I want to happen if I get seriously injured and can’t make decisions on my own? What do I want done with my remains when I die? And, of course, who am I leaving all my billions of dollars and nifty trinkets to? For mom and me, having this conversation was the most critical part. All those years she spent caring for other people’s elders, I thought—and maybe she did too—that it somehow insulated her from getting old. Meanwhile in her mind, I was probably always going to remain a little kid. Like a lot of families, the two of us were content with never broaching the topic of our final wishes—or at least, never being in a situation where we needed to. We’re still currently in the process of finalizing our wills. I’m glad we talked about it, even though I knew it’d be an awkward conversation. Luckily, I broke the ice by offering her my prized collection of gently used Post-its.
