LETTER from the ACG Institute Director DEAR COLLEAGUES,

ACG Institute Mission Statement The mission of the American College of Gastroenterology Institute for Clinical Research and Education is to promote digestive health through education of the clinician, the patient and the public, to support clinical research and innovation in gastroenterology and hepatology, and to raise public awareness about prevention of digestive disease.

We learned many lessons as we faced the challenges of the past 12 months. As a community of physicians, we delivered compassionate care to our patients while adapting to an ever-changing landscape with courage, resilience, empathy, and mutual support. We realized we could stretch, pivot and adjust in our professional and personal lives. Similarly, the ACG Institute adjusted to these extraordinary circumstances by doing business differently. Inperson gatherings were transitioned to virtual, interactive formats allowing us to continue our mission uninterrupted. Throughout the pandemic, we promoted digestive health through education of the clinician, patient, and public, while expanding our support of clinical research and focusing public awareness on preventing digestive diseases. In this Annual Report, you will see evidence of the continued success of our signature programs and the exciting growth of new initiatives.

one cohort member remarked: “This program was a godsend during the pandemic, providing muchneeded opportunities to network, meet mentors and develop critical leadership skills.” In the spring, the YPLSP class partnered with ACG Governors to advocate for change in healthcare policy during the virtual fly-in to Capitol Hill. Having spent time with these impressive young physicians, I can attest to the incredible potential of these future College leaders. Our topic-specific schools also adopted a virtual format. Both virtual IBD School and the Functional GI Disorders School proved popular with our membership. New IBS patient tools were developed by the IBS Task Force and an IBS monograph card was disseminated to accompany the release of the latest IBS guideline.

The Edgar Achkar Visiting Professorships (EAVP) and the Young Physician Leadership Scholars Program (YPLSP) were re-imagined without sacrificing quality. Ten GI training programs hosted virtual EAVP visits that featured educational lectures, trainee mentoring, and community partnership. We learned that we could adapt this popular program using technology, which opens new possibilities for expansion beyond the traditional constraints of in-person travel.

At the conclusion of 3 days of a virtual study section, the Research Committee recommended approval of $1.9 million in research funding. Expanded funding for our prestigious Junior Faculty Development Awards ($450,000 over three years), a new Established Investigator Bridge Funding Award, and a new joint ACG/ASGE Epidemiologic Research Award in Gastrointestinal Endoscopy contributed to this remarkable Institute milestone as did our continued support of Clinical Research Awards, Pilot Research Awards, and grants for medical residents and students. I invite you to review the work of our successful applicants.

We welcomed the third and most diverse cohort of the YPLSP. These 36 young physicians met four times virtually in 2020-2021 to develop leadership and communication skills, learn about organizational behavior and healthcare advocacy, and to develop relationships with ACG leadership and peers. As

This past year has demonstrated how digestive diseases disproportionately burden certain minority groups. The pandemic highlighted how healthcare disparities could impact the ability of individuals to achieve their full health potential. So, this year we have introduced a new research opportunity—The

Health Equity Research Award (HERA). The HERA was developed in partnership with the College’s Diversity, Equity, and Inclusion Task Force to support research to reduce disparities in the care and outcomes of racial/ethnic minorities, women, LGBTQ+ populations and those living with a disability. The Institute is excited to support clinical research to address these goals and stimulate clinical, translational, social science, and health services research, accelerating knowledge in this space. At the 2021 Annual Meeting, the ACG Institute is proud to announce the debut of The Center for Leadership, Ethics & Equity (the LE&E Center). This new entity within the ACG Institute will provide resources for programs that promote ethical physician leadership and leadership development; scientific discovery to reduce health disparities and advance health equity to improve patient outcomes, and educational activities that support our commitment to increasing diversity, equity and ethical care in our profession. This year, we will be launching a Visiting Scholar in Equity, Diversity & Ethical Care, and the Health Equity Research Award under the LE&E umbrella. Leadership programs will also be housed in the new Center, including YPLSP and the new Advanced Leadership Development Program, the EAVP and our Prescriptions for Success: Careers in Medicine - ACG High School Visit Program (including, for the first time, three $1,000 college scholarships). Learn more about this exciting new chapter in the future of the Institute in this Annual Report. For the past 26 years, the ACG Institute for Clinical Research and Education has supported our profession and our patients with education

and impactful clinical science. We added a focus on leadership development to strengthen the gastroenterologists’ ability to succeed in their healthcare communities and to expand the pipeline of trained leaders. This year, we begin a new journey to strengthen our College’s commitment to ethical and equitable care. These programs would not be possible without the generous support of our industry partners and members who contribute to the G.U.T. fund. The Institute’s fund to Grow, Uplift, and Transform GI is essential to our continued success. I am deeply grateful for the support of our Pillar Leadership donors and the individual philanthropic gifts that ensure we can continue our transformational work. INTRODUCING THE CENTER FOR LEADERSHIP, ETHICS & EQUITY THE ACG INSTITUTE FOR CLINICAL RESEARCH & EDUCATION IS PROUD TO ANNOUNCE THE LAUNCH OF THE LE&E CENTER, AN ENTITY WITHIN THE INSTITUTE that provides resources and a forum to envision new paradigms for effective clinicianleadership. The catalyst for the launch of The LE&E Center is this time of challenge and crisis during the SARS-CoV-2 pandemic, but it has its roots in the longstanding and significant organizational commitment by the American College of Gastroenterology and by the ACG Institute to strategies, programs, and committee activities that celebrate and increase diversity in the GI profession, cultivate the potential of GI fellows-in-training, junior faculty and early career clinicians, fund promising clinical research, reach high school students, and elevate the leadership skills of GI clinicians at various stages of their career trajectory.

The LE&E Center Imperative The leadership of the College and the ACG Institute believe that effective and ethical leadership in medicine must extend beyond top notch clinical care and inherently recognize, embrace, and promote both equity in our profession and equitable care for patients in ways that foster a healthier and more equitable society. We believe everyone should have a fair and just opportunity to achieve their full health potential – to enjoy health equity. It is imperative to create a strong cohort of well-prepared leaders who will address the critical questions of the future of gastroenterology—leaders who will transform into a reality ACG’s vision of true equity in the GI profession and the highest quality equitable health care for our patients.

Why Leadership, Ethics and Equity Matter A meaningful focus on leadership, ethics, and equity matters because effective clinician-leaders must be well prepared to meet the demands, complexity, and responsibilities of current medical practice, and to navigate future challenges as champions of equitable health care. The times call for great leadership and demand true equity. The LE&E Center aims not only to meet the moment, but also transform the future.

What LE&E Will Do The LE&E Center will educate and cultivate the pipeline of well-prepared leaders in gastroenterology, promote the practical application of ethical leadership, and fund research to produce actionable science that will translate into reducing disparities in health and/or health care, advance health equity, and improve patient outcomes. The LE&E Center will bring together existing initiatives of The ACG Institute focused on leadership development and consolidate new programs and research funding along with transformational tools and experiences. • The LE&E Center Will Develop Leaders The LE&E Center will support GI clinicians in their leadership paths at all stages of the career trajectory by increasing knowledge and improving skills necessary for effective leadership. • The LE&E Center Will Invest in Health Equity Research Through the LE&E Center, the ACG Institute will lead the way in prioritizing scientific discovery that accomplishes meaningful change in the digestive health of communities facing disparities and inequity in healthcare.

Young Physician Leadership Scholars Elevating Great Doctors into Great Leaders The ACG Young Physician Leadership Scholars Program (YPLSP) provides physicians with a comprehensive skill set that will serve as the foundation for the development of leadership and physician advocacy skills. Many young physicians, upon completion of their fellowship, are academically prepared to succeed, but frequently have not received leadership and professional training. As the medical organization serving the needs of clinical gastroenterologists, the ability to manage a business, retain staff, handle work/life balance, and avoid burnout are critical skills that the ACG seeks to provide to young practitioners. The goal of the program is to provide young physicians with a comprehensive skill set that will serve as a foundation for the development of leadership and physician advocacy skills, with a focus on building aptitude in executive decision-making, critical thinking, and understanding organizational behavior. By bringing motivated young physicians together for interactive discussions and a day of advocacy meetings on Capitol Hill, we hope to encourage the development of the next generation of leaders in gastroenterology. ACG Advanced Leadership Development Program

ACG Visiting Scholar in Equity, Diversity & Ethical Care

Building upon the success of the existing Young Physician Leadership Scholars Program, the Advanced Leadership Development Program is designed for physicians who are 10 to 20 years out of fellowship and provides the opportunity to strengthen comprehensive leadership skills. Each cohort consists of a small, select group that meets every other month to explore advanced topics such as conflict and crisis resolution, negotiation, human influence, effective mentorship, and much more. Each participant will conceptualize, develop, and present a College Capstone Project that addresses a critical organizational need.

To create awareness around the issues and challenges related to delivering equitable care, achieving diversity, and instilling ethical decision-making, the Visiting Scholar in Equity, Diversity, and Ethical Care will provide on-site lectures and grand rounds as part of the current ACG Edgar Achkar Visiting Professor program. Visits will be provided to requesting institutions and can be done in conjunction with a disease state topic-driven visit (e.g., IBD / equity, diversity, ethical care).

As part of the LE&E Center’s goal to fund research to produce actionable science that will translate to reducing health and/ or healthcare disparities, this new clinical research category supports up to $50,000 for one year, and is open to faculty or community practitioners in the U.S. and Canada. The goal of the Health Equity Research Award is to support promising research and to advance our knowledge in reducing disparities in the care and outcomes of racial/ethnic minorities, women, LGBTQ+ populations, and/or those living with a disability within the broader context of gastrointestinal and liver disease.

Prescription for Success: Careers in Medicine - ACG High School Visit As part of the ACG Annual Scientific Meeting, the ACG Diversity, Equity, and Inclusion Committee selects a local high school to visit and speak to students about careers in gastroenterology. Selected high schools typically have a STEM magnet program or healthcare focus and have a diverse student population. Students benefit from interacting with ACG physicians and are given the opportunity to use a colonoscope. Participating students are encouraged to apply for a college scholarship connected to the program.

Postpartum Period The Women in GI Committee is spearheading an updated monograph on current best approaches in the treatment of GI diseases and endoscopy during the pregnancy and postpartum period. IBS Pocket Guide Developed in conjunction with the ACG Clinical Guideline for the Management of IBS, this pocket-sized summary of the guideline was produced with the goal of assisting health care providers to better treat patients with Irritable Bowel Syndrome.

REPORT: ACG Clinical Research Grants Supporting Clinical Research: Fulfilling the Core Mission of the ACG Institute ESTABLISHED IN 1994, THE ACG INSTITUTE HAS PROVIDED FUNDING TOTALING MORE THAN $24 MILLION to 676 investigators, for research relating to clinical gastrointestinal practice. The College is proud of the achievements of its funded investigators and plans to invest more fully in their leadership and success. The ACG Institute’s continued investment in career development awards and funding for GI fellows-in-training will foster clinical innovations in gastroenterology and hepatology that will improve patient care and strengthen the capabilities of practitioners. Funding for 2021 Clinical Research For 2021, the ACG Institute for Clinical Research and Education approved $1,926,744 in support of outstanding clinical research in gastroenterology. The ACG Institute will support two Junior Faculty Development Awards at an increased yearly rate of $150,000 per year, and an overall level of $900,000. The ACG Institute is proud of the investment in the career development of promising clinical researchers, and the excellent scientific research planned by the Junior Faculty Development Awardees. From among numerous submissions for the 2021 Clinical Research Awards, the Research Committee recommended funding for 16 investigators for a total of $625,494, of which one award supports the new Established Investigator Bridge Funding Award at $300,000, one award supports the new ACG/ASGE Epidemiologic Research Award in Gastrointestinal Endoscopy at a level of $50,000, five are clinical research awards at the $50,000 level, two are pilot awards at the $15,000 level, three are for resident research awards at the $10,000 level, and two are for medical student research awards for a summer project at the $5,000 level.

ACG Institute Invests in GI Career Development The ACG Institute considers the Junior Faculty Development Award the “jewel in the crown” of the College’s research initiatives. This award provides three years of support at the new level of $150,000 annually for a $450,000 total investment in GI clinical investigation. At this level of funding, the Institute is able to attract the best-qualified candidates and provide them with the protected research time when funding is increasingly scarce. Rigorous Review of Clinical Research Applicants by ACG Research Committee In February 2021, the ACG Research Committee, under the leadership of Chair, Millie D. Long, MD, MPH, FACG, and Vice-Chair, Timothy B. Gardner, MD, MS, FACG, convened via Zoom for a three-day review session of the submissions for the ACG 2021 Research Awards. The hard-working committee members encompass a wide range of research expertise in gastroenterology and hepatology. Each year, over 100 grant applications for grant funding are reviewed, spread among the different clinical research categories of Junior Faculty Development Awards, Bridge Funding Award, Clinical Research Awards, and Pilot Programs. This year, a new Research Subcommittee reviewed (via Zoom) the Medical Resident Clinical Research Awards and Medical Student Research Awards available for our junior colleagues. Both of these award types include a travel component for participation in the ACG Annual Scientific Meeting.

The Research Committee members thoroughly review each grant application, with three reviewers assigned to score each grant, utilizing a scoring scale modeled on the NIH scoring system. At the committee’s review meeting in January, applications are discussed in detail with a presentation by each of the three assigned reviewers, followed by an open discussion by the Research Committee and final scoring by all committee members. Once the review process is completed, a recommendation regarding which grants should be funded is made to the ACG Institute’s Director, Neena S. Abraham, MD, MSc(Epi), FACG, who then presents those funding decisions to the ACG Board of Trustees for final approval. The dedicated members of the Research Committee take part in the labor-intensive review process to ensure that it is fair and that the best science is selected for funding. In order to assist applicants in their grantsmanship and future applications, all grant applicants also receive written critiques from the reviewers.

THE ACG INSTITUTE IS EXPANDING CLINICAL RESEARCH AWARD OPPORTUNITIES to produce actionable science that will translate to reducing health and/or healthcare disparities. For 2022, one new research award is available: Health Equity Research Award Up to $50,000 for one-year for faculty at a university, medical school or other healthcare institution or practicing gastroenterologists in a community practice in the U.S. or Canada. The goal of the Health Equity Research Award is to support promising investigators in developing research careers that have a direct bearing on clinical gastrointestinal practice by advancing our knowledge in reducing disparities in the care and outcomes of racial/ethnic minorities, women, LGBTQ+ populations, and/or those living with a disability that is not derivative of a gastrointestinal disease, within the broader context of gastrointestinal and liver diseases. For the purpose of this RFA, a disability is defined as a physical or mental impairment that substantially limits one or more major life activities.

PERSPECTIVE: Incoming Class of Junior Faculty Awardees ACG’s 2021 Junior Faculty Development Awardees Share Thoughts on Their Research and Career Focus Eileen Carpenter, MD, PhD University of Michigan

The Tumor Immune Microenvironment and Pancreatic Cancer Chemoresistance

This year I am starting as faculty at the University of Michigan, specializing in pancreatic diseases. I recently completed my residency and GI fellowship at Michigan as part of their physician-scientist training program, with funding from a T32 research grant and an ACG clinical research award. Pancreatic ductal adenocarcinoma is the third leading cause of cancer-related death in the U.S. Human studies have been challenging due to fresh biospecimen acquisition, patient heterogeneity, and a diverse tumor microenvironment. In my proposal, we address these obstacles by exploiting endoscopic opportunities to obtain specimens during diagnostic biopsy (treatment-naïve) and fiducial placement for radiotherapy (post-treatment). With generous support from the ACG Junior Faculty Development Award, I will test the hypothesis that chemotherapy alters the tumor microenvironment in pancreatic cancer through reprogramming of the local and systemic immune system, which can be exploited to uncover mechanisms of therapy resistance. In Aim 1, we will utilize single cell RNA sequencing and mass cytometry on longitudinally matched patient biospecimens to define immune signatures of tumor response to chemotherapy in patient tumors and peripheral blood with the goal of correlating signatures to disease outcomes. In Aim 2, we will use an in vitro all-human patient derived co-culture

system of tumor organoids and peripheral myeloid cells to dissect the immune-epithelial crosstalk responsible for therapy resistance. Having decided early on that I would devote my career to researching pancreatic cancer, approaching this disease from the perspective of a gastroenterologist has been incredibly rewarding as both a researcher and a physician. I am extremely grateful for the ACG in supporting my proposal, which will not only serve as a launching pad in my path to independence, but also support the development of an invaluable comprehensive dataset of the microenvironment in pancreatic cancer and provide new immunotherapeutic targets in this deadly disease.

University of Texas Southwestern Sex and Gender Disparities in Hepatocellular Carcinoma Incidence and Prognosis

I am an Assistant Professor of Medicine and transplant hepatologist in the Division of Digestive and Liver Diseases at UT Southwestern. The core focus of my research, as a clinical researcher, is to understand and identify mechanisms contributing to racial/ethnic, sex/ gender, and socioeconomic disparities among patients with chronic liver disease and hepatocellular carcinoma (HCC). I received training in gastroenterology and advanced/transplant hepatology through fellowships at UT Southwestern, and completed a Master of Science in Clinical Science (MSCS) degree in 2020. During fellowship, I completed dedicated research training on the NIH T32 training grant under the mentorship of Dr. Amit Singal and Dr. Jorge Marrero, both internationally recognized experts in the field of HCC early detection, surveillance and biomarkers. As a trainee caring for uninsured and underinsured patients at Parkland Health & Hospital System, the safety-net system of Dallas County, I regularly witnessed the adverse impact of

health disparities on patient outcomes, particularly those with cancer. During that time, I was inspired to pursue a career combining a clinical practice focused on hepatology and disparities-focused research. Men bear a disproportionate burden of HCC, with incidence and mortality rates 2 – 8 times higher than women observed over the past 50 years across all regions of the world and all racial/ethnic groups. Despite this consistent finding, there are limited data identifying the factors underlying this disparity and existing studies have often failed to distinguish the distinct paradigms of sex- (i.e., biologic) and genderrelated (i.e., environmental or behavioral) factors. With the generous support from the ACG Junior Faculty Development Award, I will complete two complementary research projects and training aims that will prepare me to comprehensively evaluate the magnitude and mechanisms of sex and gender disparities in HCC. Guided by a proven multi-level framework for studying cancer disparities, I will leverage unique data and infrastructure from three large cohorts. My first aim is to estimate the association between sexrelated factors and HCC risk in two well-characterized prospective cohorts comprised of >4,000 patients with cirrhosis. The second aim will use identify genderrelated behavioral and healthcare utilization factors associated with HCC survival using patient-reported data from surveys and structured interviews. Data generated from this project will fill important gaps in our understanding of the influence of sex and genderrelated factors on HCC risk and survival and serve as the groundwork for an eventual R01 grant application. It is a great honor to receive this award; the skills I will acquire through completion of the research and training aims will facilitate my transition to an independent research career focused on reducing health disparities in patients with liver disease and HCC.

ACG SCHOLARS: Contributions to the 2021 Annual Scientific Meeting A MAINSTAY OF THE ANNUAL SCIENTIFIC MEETING is the stimulating research submitted by Junior Faculty Development Award recipients, which dominates the oral plenary sessions. This year, at the ACG 2021 Annual Scientific Meeting, the Junior Faculty Development Award recipients will once again be presenting highquality research. As part of their award requirements, grantees are encouraged to submit an abstract to the ACG Annual Scientific Meeting. Submissions are rigorously reviewed and evaluated by the ACG Educational Affairs Committee, which this year selected fewer than 74 papers for oral presentation from the more than 3,700 abstracts submitted. The consistently high percentage of oral papers presented by ACG Scholars at the ACG Annual Scientific Meeting is a testament to the talents of this dynamic group of clinical researchers. Their

work is frequently featured as part of the distinguished President’s Plenary Session, a forum to showcase the best abstract in each organ system category.

PLENARY SESSIONS: 2021 ACG ANNUAL SCIENTIFIC MEETING Patricia P. Bloom, MD University of Michigan Effect of Fecal Microbiota Transplant on Hepatic Encephalopathy Varies by Donor and Recipient Factors

2019 ACG Junior Faculty Development Awardee presents virtually to the Research Committee: THIS YEAR, DUE TO THE VIRTUAL FORMAT of ACG 2020, Dr. Carlson was invited to present an update on the first year of his Junior Faculty Development Award via video, for the Research Committee to review. Dr. Carlson’s project focuses on improving the diagnostic evaluation for patients with dysphagia and an unrevealing endoscopy and manometry, i.e. functional dysphagia. His project aims to do so by evaluating the esophageal response to distension, including esophageal biomechanics, and applying machine learning for predictive analytics. A highlight of the first year of the award was a study that evaluated a cohort of 164 patients with normal motility on high-resolution manometry using functional luminal imaging probe (FLIP) panometry. While the majority (>65%) of the patients had a normal esophageal response to distension, the remainder had abnormal EGJ distensibility and/ or abnormal contractile responses to distension (i.e. abnormal secondary peristalsis). The clinical relevance of abnormal esophageal response to distension was demonstrated through

an association with esophageal retention on barium esophagram among these patients who had normal motility on HRM. Additionally, a novel esophageal motor pattern was described that involved abnormal contraction of the lower esophageal sphincter (LES) in response to distension. Plans for the remainder of the award period are to further explore the physiologic mechanisms related to the esophageal response to distension, such as through mitigation of the distension protocol and pharmacologic challenges. Additionally, predictive models are being developed to help provide clinical decision support for diagnosis of esophageal motility disorders. Dr. Carlson also recently submitted a proposal that applies this preliminary data and aims to expand application of machine learning approaches for the evaluation of esophageal motility disorders to improve classification of inconclusive esophageal motility patterns.

ACG’s Hepatology School HEPATOLOGY SCHOOL continues the legacy

of success of its predecessor program, ACG’s Hepatitis School (2013–2016). Recognizing the need for a more comprehensive course, the ACG Institute implemented a broader curriculum in 2017. The curriculum has continuouisly been improved upon, providing the most comprehensive agenda to this point for attendees. The content is tailored to meet the needs of clinical practitioners and advanced practice providers, with a deep dive into the material and in a smaller, more intimate educational experience that brings the quality clinical education of ACG’s Annual Scientific Meeting and Postgraduate Course closer to where ACG member physicians live and practice.

Topics will continue to evolve, encompassing management, treatment and screening for hepatitis B and hepatitis C, treatment post-cure of HCV, evaluation and treatment of NAFLD and NASH, evaluation and treatment of primary biliary cholangitis, hepatic encephalopathy, and other complications of cirrhosis and hepatocellular carcinoma. ACG’s IBD School ACG’s IBD School was modeled upon the success of ACG’s Hepatitis School. The concept of a specialized course for inflammatory bowel disease gained importance for ACG Institute leaders with the rise of ongoing changes related to IBD. These changes include new FDA-approved agents, new diagnostic tests, and serum drug levels. The IBD School Course Directors, with the help of IBD experts from around the country, felt the need to make ACG members aware of these developments so they could be better prepared to provide the most up-to-date, evidence-based care to their IBD patients. The inaugural IBD School was held on April 7, 2017, in Washington, DC with record attendance. In each subsequent year since the program’s introduction, multiple IBD schools have been offered annually. The faculty is comprised of experts in the field who are not only well-respected researchers, but also busy clinicians. Functional GI disorders are among the most common GI disorders in the general population. Understanding the latest science regarding these disorders and newer treatment modalities is critical to improving clinical outcomes and quality of life for a large group of patients who can be among the most difficult to treat. Drawing upon the findings of the “ACG Monograph on the Management of Irritable Bowel Syndrome” and building upon the success of ACG’s Hepatology School and IBD School, Functional GI Disorders School’s inaugural session was held on September 6, 2019 in Williamsburg, VA. The second program was held in San Diego, CA on January 10, 2020 and the third offering, as a result of COVID-19, took take place virtually on December 5, 2020. The program continues, now building upon the ACG Clinical Guideline on Management of Irritable Bowel Syndrome which was published in January, 2021. Topic areas covered include a broad range of FGIDs with an emphasis on a holistic, integrative management strategy incorporating diet, behavior and evidencebased medical treatments. ACG 2021 IBS Guideline Physician Education “Ready Reference” on Evidence-Based Recommendations for Management of IBS The College issued its first-ever Clinical Guideline on the Management of Irritable Bowel Syndrome (IBS) in the January, 2021 issue of The American Journal of Gastroenterology. The guideline provides clinical recommendations for both diagnostic testing and therapeutic treatments for IBS. The authors identified 25 clinically important questions that clinicians frequently ask and then used GRADE (Grading of Recommendations, Assessment, Development, and Evaluation) methodology to carefully and critically evaluate the data.

These new IBS guidelines aim to inform daily GI practice to help expedite care and to improve symptoms in patients with IBS. The “Ready Reference” was produced as a pocket-sized summary of the guideline with the goal of assisting health care providers to better treat patients with Irritable Bowel Syndrome. The pocket guide includes easy to read point-by-point recommendations as well as a QR code that links directly to the guideline itself.

ACG Young Physician Leadership Scholars Program

The development of future leaders in gastroenterology remains the primary goal of the Young Physician Leadership Scholars Program (YPLSP). The YPLSP cohort in 2020 had 35 participants and, due to the pandemic, was convened in its entirety on a virtual platform. From October 2020 through April 2021, members of the YPLSP class learned leadership strategies, complex communication approaches, and professional advocacy. The virtual format actually provided new benefits, where each Young Leader was afforded the opportunity to create a personal leadership model and to develop this model in advance of the group session. New aspects of the program included Hogan Assessments, where each member of the cohort underwent the Hogan Personality Inventory and was given the opportunity to discuss the results in a private session with a Hogan consultant. Individual leadership personality traits were identified and then analyzed to better inform leadership decision-making and possible leadership barriers. The program began in the fall of 2018 through online modules that guide each participant through various facets of leadership, public policy legislative

advocacy, financial management, and emotional intelligence. Each module is designed to expand a participant’s knowledge base while simultaneously challenging preconceptions. The second YPLSP class gathered face-toface at the 2019 Annual Scientific Meeting in San Antonio. Their full-day session included topics such as how to develop an individual leadership model, strategies to avoid burnout, and negotiation strategies. Due to travel restrictions from COVID-19, the participants gathered virtually in April 2019 as part of the ACG Board of Governors Legislative Fly-In in Washington, DC to advocate for the removal of barriers to CRC screening and Medicare reimbursement for GI procedures. In 2018, the first cohort of YPLSP visited with Members of Congress and their staffs to discuss issues affecting GI practitioners. As part of the Washington, DC visit, the YPLSP scholars participated in sessions on topics including how to handle difficult conversations, discussion of various leadership challenges, and problem-solving exercises. The fourth class of Young Leadership Scholars was selected in the summer of 2021 and will begin their training at the 2021 ACG Annual Scientific Meeting in Las Vegas.

Levy, III, MD organized Tune It Up: A Concert to Raise Awareness of Colorectal Cancer. This free livestream event took place on March 31, 2021 and featured dynamic performances by a talented collective of musicians. ACG EDUCATION UNIVERSE EMOTIONAL INTELLIGENCE MODULE Building upon her experiences in the YPLSP program, Sara E. Ancello, DO, spearheaded an effort to create the ACG Education Universe Emotional Intelligence Module. Dr. Ancello notes that, “Emotional intelligence is the ability to identify our own emotions and the emotions of others, and use them to guide our thinking and behavior to better manage relationships. In practice, however, EI is much more than just a simple sentence... High-proficiency EI has been linked to improved job satisfaction, physician wellness, and may even reduce burnout. It can improve communication, peer and patient relationships, and encourage successful professional interactions. EI is an essential component for organizational and leadership development, negotiation, and conflict resolution– skills that are necessary for medical directors, division chiefs, department chairs, and other leadership roles. However, even as EI has continued to gain a foothold in popular and business culture, the professional development of EI in medicine continues to be lacking. This is why the American College of Gastroenterology has decided to shine a spotlight on this important topic.”

THE ACG INSTITUTE’S EDGAR ACHKAR VISITING PROFESSORSHIP PROGRAM represents the exemplary educational offerings of the ACG Institute and the ongoing mission to provide quality educational opportunities for members. As fewer resources are available for visiting speakers, the ACG Institute continues to create opportunities for speakers to serve as faculty for medical grand rounds presentations and to enhance the educational experience for GI fellowsin-training while providing objective, timely, and clinically relevant presentations to ACG member physicians in the community. The purpose of these visits is to reach a diverse number of programs while aiming to maximize the impact of each visit by targeting GI training programs, which could also leverage an additional community event with local ACG member physicians, sometimes in conjunction with a local gut club. The program offers expertise for a specific area of need at each institution. 2021 marks the eighth year of the ACG Edgar Achkar Visiting Professorship program’s offerings, with 12 visits for GI fellows-in-training and faculty.

The Edgar Achkar Visiting Professorship Program—Virtual Visit Format THE EDGAR ACHKAR VISITING PROFESSORSHIP PROGRAM TRANSITIONED TO A VIRTUAL FORMAT IN THE SUMMER OF 2020. This change, like so many other adjustments that occurred around the world, became a necessity as a result of COVID-19. The transition, we are thrilled to report, has been a success—offering new opportunities while at the same time, presenting unique challenges. The new virtual format includes grand rounds, lectures, and one-onone time for faculty and trainees to connect with visiting professors.

visited Providence-Providence Park Hospital on September 9-10, 2020 by invitation from Dr. Issam Turk to present on esophageal science. Dr. Pandolfino attended live cases on the first day and was able to provide the endoscopist with live feedback and tips during the case. During this first session, the group had two interesting achalasia cases that they explored and they both ended up with excellent outcomes. For the second session of the day, Dr. Pandolfino gave a GI dedicated talk on motility to in-house GI fellows, attendings and some GI staff from outside institutions. On the last day, Dr. Pandolfino attended the Grand Rounds with Internal Medicine in-house to discuss GERD and Barrett’s. The lecture on GERD was well received and viewed as very impactful. Dr. Pandolfino was fortunate enough to be there in person which made the visit that more engaging for the 90 attendees and those that joined virtually.

visited University of California, Irvine on September 16, 2020 to present on hepatology and transplant hepatology, by invitation from Dr. Nimisha Parekh. The visit started with a talk on acute liver failure. Following this talk, GI fellows delivered their liver case presentations to Dr. Kwo. The fellows were able to engage with Dr. Kwo and get firsthand feedback from a respected member in the hepatology field. For

DAVID A. JOHNSON, MD, MACG visited Robert Wood Johnson Rutgers on November 13, 2020, by invitation from Dr. Vinod Rustgi. The first session was a lecture on the effects that sleep has on health and diseases and how that provides translational opportunities for intervention. Dr. Johnson presented on the “Mechanisms of Sleep Dysfunction Leading to Cytokine Upregulation, Implications in Carcinogenesis and Cancer Survival,” which many of the fellows found to be captivating. In addition, Dr. Johnson was able to give insight on the societal and

During this lecture, Dr. Kwo discussed hepatitis C diagnosis, treatment, and insight into the evolution of the hepatologists’ role in treatment. GI fellows also benefited from Dr. Kwo’s advice on pursuing an academic career in gastroenterology as a clinical investigator. The visit ended

Dr. Johnson was engaging, and the fellows particularly appreciated the portion of the presentation on the impact of ammonia, sarcopenia and frailty in cirrhosis and which included significant clinical pearls. Dr. Johnson’s presentation emphasized that the future of translational GI is now and that a more “multi-disciplinary, highly collaborative” approach will help “expedite the discovery of new diagnostic tools and treatments.”

Dr. Johnson: “The ability to connect through the overwhelming restrictions was awesome. ACG resiliently effective ability to provide this connection to continue education.. priceless!”

with a community lecture to gastroenterologists on “Where We are [with NAFLD] in 202[1] and What Does the Future Hold?” Dr. Kwo was able to dive into the different causes (including alcohol consumption and obesity), fibrosis assessment for patients with NAFLD, and the approaches to treatment of NASH and the lifestyle modifications needed to treat NAFLD.

Dr. Kwo: “Virtual visits can be as rewarding as in-person visits.”

Dr. Abhishek Bhurwal, Robert Wood Johnson: “Dr. David Johnson’s presentation on 'Effect of Sleep on GI Health and Disease' really opened the eyes of the fellows to the impact of sleep on diseases. It was fascinating to understand how circadian rhythm disruption leads to increased intestinal permeability. Virtual didactics render physical distance nearly irrelevant and offer a convenient live didactic session. The fellows and faculty enjoyed the sessions and asked questions.”

visited Washington University School of Medicine on November 17-18, 2020 by invitation from Dr. Parakaal Deepak. The first evening, Dr. Kane presented on women in GI, which included 18 attendees, comprised of female GI faculty as well as female fellows and residents in the GI division. This event allowed them to network and engage with a female leader in her field of expertise. Dr. Kane spent the next day meeting one-on-one with fellows to provide mentoring and an opportunity for them to ask any specific case-related questions. That evening, Dr. Kane discussed IBD, pregnancy, and parenthood. In particular, Dr. Kane explained the risks of adverse pregnancy-related outcomes and the importance of the use of biologics to treat pregnant women with IBD.

Dr. Kane: “I think the biggest impact was getting to spend time with fellows and faculty in a manner that was as personal as one could get in the virtual environment. We could see each other’s faces and that was helpful. I was nervous about how this would go, but it was seamless.”

visited Atrium Health on March 11, 2021 to present on CRC screening and genetics, by invitation from Dr. Steven Zacks. Dr. Burke’s first talk was a discussion. The topic of the discussion was “Enhancing the Efficacy and Quality of Colonoscopy,” which was largely a lecture about understanding quality indicators for colonoscopy, recognition of new guidance on polyp detection, resection and post polypectomy surveillance, and the identification of opportunities to enhance effective colonoscopy. Dr. Burke emphasized the use of measurement to report quality and that doing so can improve indicators in the future. In addition, Dr. Burke explored the factors impacting colonoscopy efficacy, including technique of operator incomplete removal of polyps, incomplete removal of a polyp, missed adenoma/ cancer, biologic variation, and inadequate bowel preparation. A large part of the lecture was on the importance of understanding the impact and quality of bowel preparation. In addition, Dr. Burke also explored the approach to serrated polyposis syndrome, including diagnosis, endoscopic management, surveillance phase, and the need to screen first degree relatives. GI fellows and faculty had an opportunity to interact with Dr. Burke virtually and ask questions on topics such as interval CRC and CRC screening.

Dr. Burke: “The lectureship is an honor and provides a great opportunity to network and mingle with the current and next generation of gastroenterologists. I truly miss the personal contact with trainees and faculty which is the reason this lectureship exists.” Dr. Steven Zacks, Atrium Health: “The whole experience of having a nationally recognized expert update the fellows on colorectal cancer screening was engaging.”

Dr. Parakkal Deepak: “Given the CDC guidelines in place due to COVID-19, we believe the experience was absolutely wonderful given the circumstances.”

MOHAMED O. OTHMAN, MD visited the University of Miami on March 24, 2021, to present on third space endoscopy and the road to robotic endoscopy, by invitation from Dr. David Kerman. During the visit, Dr. Othman met with faculty, fellows, and endoscopy leadership to share and learn about their program. In the morning, Dr. Othman gave a presentation on the landscape and future of endoscopy and Dr. Othman discussed the successes and challenges of endoscopic submucosal dissection (ESD) for lesion removal. In addition, he explained the advantages of peroral endoscopic myotomy (POEM) as a treatment option for patients with achalasia. As the field of robotic endoscopy continues to rapidly change, Dr. Othman highlighted that the use of “hybrid systems are on the rise and they will enable more complex endoscopic procedures,” leading to better results for patients. Dr. Othman had the opportunity to meet one-onone with three of the faculty to discuss specific cases and share his expertise on endoscopic procedures. According to Dr. Kerman, Dr. Othman’s presentation on

third space endoscopy was “tremendous and [attendees] were thrilled to have him chat with our faculty on tips for successful endoscopy station.” These case presentations included cases that explored dysplasia, intestinal adenocarcinoma in the background of intestinal metaplasia, endoscopic resection with ESD, and EMR. Dr. Othman was able to share his recommendations on how patients with early esophageal adenocarcinoma should be evaluated for ESD.

Dr. Othman: “ACG’s visiting professor program promotes collaborations among institutions and fosters information exchange. This progam is a great success and a testimony how ACG is supporting its trainee members.” Dr. David Kerman, University of Miami: “We were grateful that the format allowed for successful integration of our community partners.” DOUGLAS K. REX, MD, MACG visited Westchester Medical Center on May 19, 2021, to present on "CRC Screening and Modern Management of Colorectal Polyps" by invitation from Dr. Zahava Farkas. Dr. Rex presented a discussion on advanced endoscopic techniques at Westchester Medical Center’s gastroenterology and surgery Grand Rounds. In particular, Dr. Rex addressed colon polyp management (detection, resection and surveillance), advanced endoscopic techniques (EMR, ESD, underwater,

etc.), and the recent changes in the recommended interval guidelines for surveillance colonoscopy. That afternoon, Dr. Rex gave a broad review of colon cancer screening with a focus on the recently lowered age target for initiating screening and how a PCP can approach patient questions regarding alternate screening methods without detracting from colonoscopy as the gold standard. Following this session, Dr. Rex met with GI fellows and attendings to discuss the future of colon cancer screening.

SATISH S.C. RAO, MD, PhD, FACG Dr. Rao was invited to University of Rochester Medical Center by Dr. Danielle Marino on April 27, 2021 to present on difficult to treat anorectal problems. Dr. Rao’s visit began with a presentation to fellows where he shared his famous digital rectal exam technique during case presentations, as well as giving his unique perspective on some of their challenging cases. Dr. Rao then met either one-on-one or in small groups with a number of the University of Rochester faculty to offer his advice on cases and to offer his expertise on how to treat difficult anorectal problems. During his informative Gut Club presentation, Dr. Rao spoke about the advances in evaluation, diagnostic tests, and treatment of common anorectal disorders. Dr. Rao presented a case of a patient with chronic and severe anorectal disorders and explained the toll that

it has taken on their personal life and the extensive tests and interventions they have had to manage the pain. In addition, Dr. Rao shared the advantages of using translumbosacral anorectal magnetic stimulation (TAMS) tests for anorectal disease diagnosis, and translumbosacral neuromodulation therapy (TNT) as a novel treatment for levator ani syndrome (LAS). On the final day, Dr. Rao delivered a Grand Rounds Lecture on gas and bloating, which also explored SIBO and brain fogginess. In addition to lecturing on the predisposing factors of SIBO/SIFO, Dr. Rao explained the patient history information, culture tests, and different stool and breath tests that are necessary for investigating SIBO.

Dr. Danielle Marino of the University of Rochester Medical Center: “We really enjoyed the time that Dr. Rao was able to spend [with us virtually] going through case presentations with the fellows. The fellows were able to see a unique perspective from a master clinician within the field of neurogastroenterology and motility. We only wish we had more time together!” “Dr. Rao walking [fellows] through his famous digital rectal exam during case presentations, as well as getting his unique perspective on some of our challenging cases [is what fellows appreciated the most].”

Dr. Rex: “The faculty and fellows had a real interest in colonoscopy and CRC screening. I really enjoyed the Q&A sessions and the small group interaction with the fellows. “The Westchester program is an outstanding clinical program and I was very honored to spend time with the group. I appreciate the invitation and wish we could have gotten together in person. Thanks to ACG for this great opportunity!”

BRIAN E. LACY, MD, PhD, FACG visited Allegheny General Hospital on May 27, 2021, to present on esophageal motility disorders and a diagnostic approach to reading high resolution esophageal manometry. Thursday evening Dr. Lacy presented GI Grand Rounds on "Esophageal Motility Disorders" to Allegeheny General Hospital GI physicians, program alumni, and local GI physicians. The session was met with a lot of questions and then followed up by a lecture and teaching session on Friday on, "Interpretation of Esophageal Manometry." In addition, there was a discussion about gastroparesis and functional dyspepsia and the overlap in these two important sensorimotor disorders of the upper GI tract. Dr. Lacy had a nice one-on-one chat with one of the faculty members to discuss research projects and career development. He believes that he was able to provide a new perspective on several different topics.

Dr. Brian Lacy: “This continues to be an exceptional opportunity for training programs to request national and international experts to discuss educational topics and areas of interest that might not be available locally. This program highlights the American College of Gastroenterology’s dedication to education.” Dr. Michael Babich: “It was exciting to hear experts give their views on current management. The topic was extremely pertinent to daily practice yet is not well understood; the speaker is a leader in the field of motility who provided an exceptional look at the current data and future prospects of the practice. Dr Lacy’s teaching style/skill was conducive to learning and made this a high-impact session. His practical, step-by-step approach to interpreting esophageal manometry facilitated learning.”

COREY A. SIEGEL, MD, MS visited William Beaumont Hospital on June 2, 2021 “Refocusing IBD Patient management: Personalized, Proactive, and Patient-Centered Care.” There were over 100 participants, including specialists in primary care, infectious disease, endocrinology, pediatric gastroenterology and of course adults gastroenterologists. On July 20th, Dr Siegel visited virtually again to present at the GI Grand Rounds. He presented on “Hospital Care of the Acute Severe Ulcerative Colitis Flare” and “Outside the Box Therapeutic Options for IBD." Dr. Siegel answered questions from the faculty, including case presentations. At the later session, Dr Seigel entertained questions from GI fellows, including career advice and presenting some IBD cases. Overall, “the biggest impact was Dr. Siegel’s comments/ advice regarding practical details of everyday care: how to address patient’s concerns and engage them in their care, how to navigate obstacles in patient care due to insurance restrictions.”

Dr. Corey Siegel: “The ability to get to know other gastroenterologists and fellows and share ideas is always a highlight for me as part of these visiting professorships.”

Dr. Laith Jamil: “Dr. Siegel was chosen by our GI fellowship program as the ACG Edgar Achkar Visiting Professor for his outstanding reputation as a researcher and educator. We asked him to give a virtual presentation at our Internal Medicine Grand Rounds, which he graciously agreed to. We then had over 10 questions that were answered by Dr Siegel. Some of the comments on Microsoft Teams; “Very interesting lecture…There are a lot of parallels to primary care/endocrine world… I found his GI Grand Rounds on management of severe ulcerative colitis very good. I have not seen such a well-presented talk on a fairly obscure but clinically relevant topic. Clearly distinguished between clinical opinion, science, and speculation. I actually took notes (unusual for me).” Another physician commented “I am grateful for the opportunity to refresh my knowledge in IBD and learn about practical aspects of daily patient care.”

Robert E. Kravetz, MD, MACG has written a new book, A Look Back: Reflections on Medical History & Artifacts from the Pages of The American Journal of Gastroenterology. Supported by the ACG Institute, Archives Committee, and the leadership of the College, this recently published volume fulfills the vision of Dr. Kravetz “to preserve the individual articles that had been printed in The American Journal of Gastroenterology over a 10 year period. The articles would be lost in the future if not preserved and published in a single volume. Each article provides an insight into the history of medicine as practiced in the past. The emphasis is on items used by gastroenterologists but also covers broad aspects of medicine in general. I wanted to publish a “coffee table” type book that could also be enjoyed by the general public as well as individuals in the medical field who would like to learn about the history of medicine.”

About the Institute The ACG Institute for Clinical Research and Education promotes the College’s commitment to clinical research and physician and patient education. The ACG Institute’s record to date is a source of pride, particularly its significant and growing support of young clinical investigators. The ACG Institute has grown since its introduction in 1994, both in terms of the breadth of its investment in clinical GI investigators and the scope of its activities, which now include the development of evidence-based systematic reviews and related clinical education for physicians and patients.
